The effects of organizational factors on participation rates in a smoking cessation clinic and on one-year quit rates were examined among 68 private sector businesses. Free smoking cessation programs were offered to all smoking employees. Smokers (n = 844) were contacted to determine changes in smoking behavior; managers (n = 68) were also contacted to assess changes in smoking policy implementation over the past year. The participation rate in the clinic was 6.6 percent. Overall, 14.3 percent of smokers reported abstaining from cigarettes for at least one month prior to the one-year follow-up survey. Organization factors predicting clinic participation included having health promotion programs, increased motivation to stop smoking, and greater length of employment. Factors predicting cessation at one year included greater clinic participation rates, larger worksite size, a greater awareness of smoking restrictions, and agreement that passive smoking was dangerous to ones health. The only factor to significantly increase both participation and cessation rates was the presence of smoking policies. Employee, manager, and organizational characteristics can exert independent effects on behavior. Therefore, interventions should be targeted at each of these levels to maximize smoking-related behavior change.

Purpose.To determine the effectiveness of a multicomponent smoking cessation program supplemented by incentives and team competition.Design.A quasi-experimental design was employed to compare the effectiveness of three different smoking cessation programs, each assigned to separate worksites.Setting.The study was conducted from 1990 to 1991 at three aerospace industry worksites in California.Subjects.All employees who were current, regular tobacco users were eligible to participate in the program offered at their site.Intervention.The multicomponent program included a self-help package, telephone counseling, and other elements. The incentive-competition program included the multicomponent program plus cash incentives and team competition for the first 5 months of the program. The traditional program offered a standard smoking cessation program.Measures.Self-reported questionnaires and carbon monoxide tests of tobacco use or abstinence were used over a 12-month period.Results.The incentive-competition program had an abstinence rate of 41% at 6 months (n = 68), which was significantly better than the multicomponent program (23%, n = 81) or the traditional program (8%, n = 36). At 12 months, the quit rates for the incentive and multicomponent programs were statistically indistinguishable (37% vs. 30%), but remained higher than the traditional program (11%). Chi-square tests, t-tests, and logistic regression were used to compare smoking abstinence across the three programs.Conclusions.Offering a multicomponent program with telephone counseling may be just as effective for long-term smoking cessation as such a program plus incentives and competition, and more effective than a traditional program.

Purpose. To determine the prevalence of tobacco use among Centers for Disease Control and Prevention (CDC)/Agency for Toxic Substances and Disease Registry (ATSDR) employees and the effect of the smoke-free policy on smoking behavior and air quality at work. Design. A stratified telephone survey of 1181 CDC/ATSDR employees randomly selected from employee rosters. Setting. CDC/ATSDR work sites in Atlanta, Georgia, and other major CDC locations throughout the United States and Puerto Rico. Subjects. Randomly selected employees of CDC/ATSDR1, or about 22% of the total CDC/ATSDR population; 98% of eligible persons selected agreed to participate. Measures. Demographic and smoking history variables, attitudes toward and impact of the smoke-free policy on smoking behavior, and self-report changes in air quality were the measures used. Results. Overall cigarette smoking prevalence was only 11.1%. One percent reported using chewing tobacco, 1.1% reported smoking a pipe, and 1.4% reported smoking cigars. Average self-reported, daily cigarette comsumption significantly decreased after the smoking ban took effect. Overall, 90 % of the employees supported the smoke-free policy, and 80 % of the employees believed that smokers were complying with the smoke-free policy. Most employees believed that the air quality of work areas and nonwork areas (65% and 69%, respectively) had improved since the smoke-free policy was implemented. Conclusions. These findings are consistent with previous evaluations of smoke-free policies and suggest that most employees are generally supportive of workplace smoking restrictions. Such policies can also have a positive impact on smoking behavior and perceived air quality.
