Deutschland ist ein Einwanderungsland und fast ein Viertel der Bevölkerung hat einen Migrationshintergrund. Daher besteht zunehmend Bedarf an verlässlichen Informationen zur gesundheitlichen Situation von Menschen mit Migrationshintergrund. Dem Robert Koch-Institut kommt in diesem Kontext die Aufgabe zu, das Gesundheitsmonitoring auszubauen und Menschen mit Migrationshintergrund möglichst repräsentativ entsprechend ihrem Anteil in der Bevölkerung in Befragungs- und Untersuchungssurveys einzubeziehen. Der Gesundheitsstatus von Menschen mit Migrationshintergrund soll adäquat widergespiegelt werden, was bisher im Rahmen der repräsentativen Befragungsund Untersuchungssurveys des Robert Koch-Instituts für Erwachsene nur unzureichend gelungen ist. Daher wurde Ende 2016 das Projekt Improving Health Monitoring in Migrant Populations (IMIRA) initiiert, das auf die Erweiterung des Gesundheitsmonitorings des Robert Koch-Instituts auf Menschen mit Migrationshintergrund abzielt und langfristig deren Einbindung in die Gesundheitssurveys verbessern soll. Dies beinhaltet zum einen die Durchführung zweier Machbarkeitsstudien, in denen Zugangswege und Rekrutierungsstrategien im Rahmen von Befragungsstudien getestet und Maßnahmen zur Überbrückung von Sprachbarrieren in Untersuchungssurveys entwickelt wurden. Zur Erweiterung der Gesundheitsberichterstattung zu Migration und Gesundheit werden ein Berichtskonzept und ein Kernindikatorensatz entwickelt und zudem (Sekundär-)Datenquellen auf ihre Nutzbarkeit geprüft. Ebenso ist die Überprüfung und Weiterentwicklung relevanter migrationsspezifischer Erhebungsinstrumente und Indikatoren sowie eine intensivierte Vernetzung mit relevanten Akteurinnen und Akteuren vorgesehen.

Germany is an immigration country and nearly a quarter of its population has a migration background. Thus, there is increasingly a need for reliable information on the health situation of people with a migration background. The Robert Koch Institute is in charge of expanding its health monitoring to improve the representation of people with a migration background in interview and examination surveys. Studies adequately need to reflect the health status of people with a migration background and currently the Robert Koch Institute's representative interview and examination surveys for adults do not fully achieve this. At the end of 2016, therefore, the Improving Health Monitoring in Migrant Populations (IMIRA) project was initiated aiming to expand the Robert Koch Institute's health monitoring to people with migration background and improve their involvement in health surveys in the long-term. This includes carrying out two feasibility studies to test strategies to reach and recruit people with migration background for interview surveys and develop measures to overcome language barriers in examination surveys. In order to expand health reporting on migration and health, a reporting concept and a core indicator set will be developed and the potential of (secondary) data sources will be tested. Furthermore, plans foresee the testing and further development of relevant specific migration sensitive survey instruments and indicators, as well as increasing networking with relevant stakeholders.

Nursing home residents (NHRs) are systematically excluded from the target populations of most population-based health surveys, which may
result in biased prevalence estimates. Researchers who wish to include NHRs in surveys face several challenges including difficulty sampling and
contacting NHRs and greater levels of functional impairments impeding participation.
A population-wide, register-based, random sample of 8,000 older individuals (57.1% women, mean age=76.2 years) in six primary sampling units
(PSUs) in Germany was used to analyse NHR coverage. The contact and response rates among NHRs were compared to those among persons
living in private households in two PSUs (N=2,000) by applying an informed sequential mixed-mode design. All persons received a health
questionnaire by mail, and random subgroups of initial non-respondents were further contacted by telephone or personal visits.
The population-wide, register-based, random sample included a substantial proportion of NHRs that nearly approximated the national rate of 4.2%
NHRs among individuals ≥65 years. Never-theless, undercoverage of a specific subgroup of NHRs was apparent. The contact and re-sponse
rates were significantly lower among NHRs than persons in private households (contact: 49.0% vs. 72.9%; response: 20.0% vs. 45.9%).
Therefore, it remains questionable if NHRs can be included in regular national health monitoring programmes.

Existing health survey data of individuals who are 65+ years of age is limited due to the exclusion of the oldest old and physically or
cognitively impaired individuals. This study aimed to assess the effects of a sequential mixed-mode design on (1) contact and response
rates, (2) sample composition and (3) non-response bias. A register-based random sample of 2,000 individuals 65+ years was initially
contacted by mail to answer a health questionnaire. Random subgroups of initial non-responders were further contacted by telephone or
home visits. Participation by interview or proxy was possible. After postal contact only, the initial contact and response rates were 51.7%
and 37.8%, respectively. The contact and response rates increased to 71.1% and 44.6%, respectively, after all contact steps. A different
sample composition regarding sociodemographic (i.e., older individuals) and health characteristics (i.e., worse self-rated health, more
functional impairments) was achieved by the inclusion of those late participants. Ill health was the second most frequent reason for nonparticipation.
Personal contact modes are important to increase contact and response rates in population-based health studies and to
include hard-to-reach groups such as the oldest or physically impaired individuals. However, non-response bias still occurred.

Die Weltgesundheitsorganisation (WHO) hat jüngst Richtlinien für Studien, die Basisdaten für gesundheitspolitische Entscheidungen im Pandemiefall liefern können, veröffentlicht. Durch die Zusammenarbeit zwischen unterschiedlichen Disziplinen kann es gelingen, evidenzbasierte Entscheidungen durch qualitativ hochwertige, längsschnittliche Befragungsdaten aus den Sozial- und Wirtschaftswissenschaften in Verbindung mit wiederholten Labortests zum Virus- und Antikörpernachweis sowie Erhebungen des Gesundheitsstatus mit soliden statistischen Analysen zu unterfüttern. Der hier vorgestellte Studienplan und das erhebungstechnische Konzept zur Erforschung des Umfangs und der Dynamik von SARS-CoV-2-Infektionen und COVID-19-Erkrankungen unter Nutzung der haushaltsbasierten Multi-Kohortenstudie Sozio-oekonomisches Panel (SOEP) erfüllen die Anforderungen der WHO zur Messung der Prävalenz von COVID-19 in vollem Umfang und bieten darüber hinaus weitreichende Analysemöglichkeiten zu gesundheitlichen und sozio-ökonomischen Auswirkungen der Pandemie.

Das Coronavirus SARS-CoV-2 hat sich in kurzer Zeit bundesweit ausgebreitet. In den Meldedaten der Gesundheitsämter zu laborbestätigten Infektionsfällen ist von einer Untererfassung des Infektionsgeschehens auszugehen, da Infektionen häufig unentdeckt bleiben, zum Beispiel weil sie symptomarm verlaufen. In seroepidemiologischen Studien kann der Bevölkerungsanteil mit durchgemachter SARS-CoV-2-Infektion (Seroprävalenz) wie auch der Umfang unentdeckter Infektionen abgeschätzt werden. In der Studie CORONA-MONITORING bundesweit (RKI-SOEP-Studie) werden Bioproben und Befragungsdaten in einer deutschlandweiten Bevölkerungsstichprobe des Sozio-oekonomischen Panels (SOEP) erhoben. Den Teilnehmenden werden Materialien zur selbstständigen Gewinnung einer Trockenblutprobe aus Kapillarblut des Fingers und einer Abstrichprobe aus Mund und Nase sowie ein Fragebogen postalisch zugesendet. Die zurückgesendeten Proben werden auf SARS-CoV-2-IgG-Antikörper und SARS-CoV-2-RNA zur Identifikation einer durchgemachten oder aktuellen Infektion untersucht. Die eingesetzten Methoden ermöglichen es, auch solche SARS-CoV-2-Infektionen zu erkennen, die bislang unentdeckt blieben. Durch die Verknüpfung mit bereits vorhandenen SOEP-Daten hat die Studie das Potenzial, auch soziale und gesundheitsbezogene Unterschiede im Infektionsstatus zu untersuchen. So kann die Studie zu einem verbesserten Verständnis des Ausmaßes der Epidemie in Deutschland wie auch zur Identifikation von Zielgruppen für den Infektionsschutz beitragen.
