Background: Environmental factors determine children's health. Quantifying the health impacts related to environmental hazards for children is essential to prioritize interventions to improve health in Europe. Objective: This study aimed to assess the burden of childhood disease due to environmental risks across the European Union. Methods: We conducted an environmental burden of childhood disease assessment in the 28 countries of the EU (EU28) for seven environmental risk factors (particulate matter less than 10 micrometer of diameter (PM(10)) and less than 2.5 micrometer of diameter (PM(2.5)), ozone, secondhand smoke, dampness, lead, and formaldehyde). The primary outcome was disability-adjusted life years (DALYs), assessed from exposure data provided by the World Health Organization, Global Burden of Disease project, scientific literature, and epidemiological risk estimates. Results: The seven studied environmental risk factors for children in the EU28 were responsible for around 211,000 DALYs annually. Particulate matter (PM(10) and PM(2.5)) was the main environmental risk factor, producing 59% of total DALYs (125,000 DALYs), followed by secondhand smoke with 20% of all DALYs (42,500 DALYs), ozone 11% (24,000 DALYs), dampness 6% (13,000 DALYs), lead 3% (6200 DALYs), and formaldehyde 0.2% (423 DALYs). Conclusions: Environmental exposures included in this study were estimated to produce 211,000 DALYs each year in children in the EU28, representing 2.6% of all DALYs in children. Among the included environmental risk factors, air pollution (particulate matter and ozone) was estimated to produce the highest burden of disease in children in Europe, half of which was due to the effects of PM(10) on infant mortality. Effective policies to reduce environmental pollutants across Europe are needed.

Almost half of the European Union (EU)'s population is exposed to road traffic noise above levels that constitute a health risk. Associations between road traffic noise and impaired sleep in adults have consistently been reported. Less is known about effects of noise on children's sleep. The aim of this study was to examine the association between nocturnal road traffic noise exposure and children's parental-reported sleep duration and sleep problems. The present cross-sectional study used data from The Norwegian Mother and Child Cohort Study. Parental report of children's sleep duration and sleep problems at age 7 was linked to modelled levels of residential night-time road traffic noise. The study population included 2665 children from Oslo, Norway. No association was found between road traffic noise and sleep duration in the total study population (odds ratio (OR): 1.05, 95% confidence interval (CI): [0.94, 1.17]), but a statistically significant association was observed in girls (OR: 1.21, 95% CI: [1.04, 1.41]). For sleep problems, the associations were similar (OR: 1.36, 95% CI: [0.85, 2.16]) in girls. The ORs are presented for an increase of 10 dB. The findings suggest there is an association between road traffic noise and sleep for girls, underlining the importance of protecting children against excessive noise levels.

Abstract
Recently, the causal associations between traffic emissions and health effects were strengthened by the systematic reviews by the Health Effects Institute (HEI#23, 2022). According to EuroStat data Europeans spend 87-97% of their time indoors in homes, workplaces, schools and nurseries. Breathing volumes are dominated by the time spent indoors (16 m³ indoors and 1.4 m³ outdoors), even though breathing rates in outdoor environments are higher. This demonstrates the importance of taking into account time activity as well as infiltration to indoor spaces when estimating intake of particles.
The ULTRHAS project aims to link in vitro toxicological evidence for particulate matter to health impact assessments to look specifically into this. An integrated exposure pathway model will consider for different microenvironments, and physical activity patterns to quantify the particle intake in selected gender and age groups. Relative to the commonly applied approach using residential address and corresponding outdoor pollution levels we specifically add handling of (i) infiltration into indoor spaces and (ii) variability in breathing volumes based on physical activities. The model will provide inhalation volumes for selected target populations, to be applied in the intake-DALY estimation of burden of disease parameters in the following phases of the project.

Purpose Essential to exposome research is the collection of data on many environmental exposures from different domains in the same subjects. The aim of the Human Early Life Exposome (HELIX) study was to measure and describe multiple environmental exposures during early life (pregnancy and childhood) in a prospective cohort and associate these exposures with molecular omics signatures and child health outcomes.
