This abstract described many things about organizational change started from understanding of organization, supporting factors and it is consequences. Beside that, it is presented by two models of organizational change with completed each other and comprehensively explained external environment factors as supporting powers of changing organization, like : globalization, competition, economic condition, government regulation, technology, demography, social etc. In order, an organization can develop that the changing of the external environment are needed to be respon by many kinds of internal changing organization, like : changing strategies, structure, practical management, leadership, organizational culture, systems and procedure, also individual values, which them influence to motivation and reachid performance even by the employees or organization. Each type of organizational change will create consequences that different each other, in order, needed strategies to handle them in different ways.

The health insurance program in Indonesia shows efforts to continue to be optimized, one of which is through the implementation of a tiered referral system starting from the First Level Health Facility (FKTP). The purpose of this study was to determine the implementation of BPJS Health Outpatient referral at First Level Health Facilities (FKTP) in Makassar City. In this study using a mixed method, the population in this study was 146 people with 60 samples drawn. Quantitative data analysis namely univariate analysis, bivariate analysis and multivariate analysis used in this study is logistic regression analysis. While qualitative data analysis includes organizing / structuring data, conducting coding and categorizing, looking for patterns and research propositions, interpreting data, and evaluating interpretations. The results showed that there was an influence between health human resources on the implementation of outpatient referrals, there was an influence between facility facilities on the implementation of outpatient referrals. There is no influence between the availability of medicines on the implementation of first-degree outpatient referrals for BPJS Health patients at FKTP in the city of Makassar. There is no influence between the understanding of health workers about the tiered referral system and the implementation of first-degree outpatient referrals for BPJS Health patients at FKTP in Makassar city.

The hypothesis was tested using Structural Equation Model (Analysis of Moment Structures, AMOS version 18). The results of the study provide evidence that the leadership and competencies that can improve OCB employee for the better. Good leadership and high competence and OCB employee can increase employee performance management to a higher direction. PurposeThis paper examines how market orientation and competence of knowledge reinforce the innovation and in turn improve the business performance.Design/methodology/approachThis research applied research design with quantitative approach based on positivism philosophy, that is, research aimed to test the theory and data which is general to produce findings that are special through hypothesis test. This study was supplemented by field studies as a necessary source of data in the form of trends, attitudes, opinions, perceptions of owners, managers or senior employees of SMEs in the textile industry in Bali, related to the variables used in this study.Findings(1) Market orientation influences business performance, (2) competence of knowledge moderates the influence of market orientation on business performance, (3) innovation moderates the influence of market orientation on business performance.Originality/value(1) Novelty of this research is developing the concept of competence of knowledge associated with business performance in which this relationship has not yet been expressed. (2) Building the concept of innovation development of small-scale industry based on the market orientation. (3) Developing the concept of innovative development of small and medium enterprises of textile industry based on competence of knowledge. (4) Investigating the concept of market orientation and competence of knowledge in an integrated and holistic way to strengthen the innovation and business performance of SMEs of the textile industry in Bali.
