Circular economy has in recent years been promoted as a promising alternative to unsustainable production systems. By comparing two different concepts of circularity, circular economy and eco-cycle, which had political momentum in Sweden during the 2010s and the 1990s respectively, the paper shows how issues of responsibility, politics, limits, space and sustainability, and even the shape of the circle itself have changed over the turn of the millennium. Based on a discourse analysis of two policy reports on the concepts, a strong and a weak interpretation of circularity are identified. A weak circularity, represented in circular economy, is presumably without limits. Secondary resources shall only complement the growing extraction of primary resources, while the responsibility for circularity is handed over from the state to individuals and entrepreneurs. A weak circularity excludes social responsibility and tends to reinforce unequal power relations. With a strong conceptualization of circularity, on the other hand, the producers and the state are responsible for creating a closed, material loop limited in size and space, based on the principle of fair distribution of resources. Drawing on the findings, alternative directions of circulation are called for, which are more globally oriented and socially inclusive. ; QC 20200729

Using data from a nationwide psychological autopsy study of all suicides in Finland occurring over a 1-year period, the authors investigated religiosity and its types among suicide victims. Overt active religiosity was identified in 245 (18%) of the victims. A history of psychiatric inpatient treatment and psychotic and depressive disorders diagnosed in psychiatric care were more common among the religious victims than among the nonreligious. The burden of major mental disorders seems to have been heavier among religious than nonreligious victims. The type of religiosity of victims was qualitatively characterized into four categories: (1) help-seeking from a religious congregation, (2) conflict with a religious congregation, (3) private religiosity, and (4) abandoned religiosity. Further research needs to address the psychological contents and psychiatric implications of different types of religiosity among suicidal individuals.

Through psychological autopsy seven cases of physician suicide were studied. All seven victims received a diagnosis of current mood disorder, two of them bipolar disorder. Five had suffered a disabling physical condition. None of the victims had had adequate treatment with antidepressant or mood-stabilizing drugs, nor had they been in psychotherapy. Current adequate treatment for depression seems to be as rare among physician victims as among suicides with major depressive disorders in the population.

Completed and attempted suicide are major public health problems in most western countries. The importance of suicidal behavior as a health problem, particularly among adolescents and young adults, has been emphasized by the European Union, the WHO (Europe), as well as the Finnish authorities. Due to the exceptionally high suicide mortality, suicide prevention has been one of the main targets of Finnish health policy since the late 1980s. However, to develop feasible strategies for suicide prevention, better knowledge of the phenomenon of self-destruction is necessary. The Department of Mental Health of the National Public Health Institute has been actively involved in suicide research and the development of suicide strategies both in Finland and western Europe since 1986. The success is based on a long tradition of suicide research in Finland, the representative and reliable suicide data, a highly motivated research group, and also the necessary economic support by both the National Public Health Institute and the Finnish Academy. This article outlines our groups research plan for the next few years.

Therapeutic apheresis (TA) as a treatment for antibody-associated vasculitis (AAV) was questioned by the PEXIVAS although the MEPEX study favored TA. The aim of this study was to evaluate the efficacy of TA to improve renal function in patients consecutively included in the WAA-apheresis registry versus patients not treated with TA. Materials and methods: Included were 192 patients that suffered from anti-glomerular basement membrane disease (anti-GBM, n = 28) and antineutrophil cytoplasmic antibody-associated vasculitis of MPO or PR3 origin. Of these 119 had performed TA and the other 73 had not performed TA for theses diagnoses (CTRL). Results: Elderly had an increased risk to die within 12 months (p = 0.002). All 28 anti-GBM had renal involvement, 21 dialysis dependent. At 3 month nine (36 %) did not need dialysis. Baseline data regarding renal function of AAV patients, subtype MPO and PR3, were worse in the TA groups than in CTRL. Recovery out of dialysis was better for the PR3-TA group compared with 1) the controls of MEPEX (RR 0.59, CI 0.43-0.80) and 2) the MPO-TA patients (RR 0.28, CI 0.12-0.68). The MPO-TA recovered similarly as the MEPEX-CTRL. Renal function improved most for TA-patients from baseline during the first 3 months (MPO-TA and PR3-TA) and stabilized thereafter and less for MPO-CTRL and PR3-CTRL. Conclusion: PR3-TA patients seem to have best chances to get out of dialysis. PR3-TA and MPO-TA improved residual renal function better than CTRL.
