Achieving the sustainable development goals of the United Nations requires innovations in agriculture and development of climate-smart and economically feasible approaches for smallholder farmers in developing countries. Historical climate data of Nepal, which include 116 yr since 1901, has shown an increasing trend for average temperature by 0.016 ˚C yr–1 whereas precipitation has shown a decreasing trend by 0.137 mm yr–1. Such weather trends could enhance glacier melt associated flooding, and delayed monsoon rainfalls negatively impacting the agricultural production. The Nepalese government is promoting conservation agriculture (CA) through development of low-cost technologies that can be used effectively in difficult terrains. Such techniques include crop diversification, crop rotation, cover crops, and minimum tillage; all of which can reduce soil degradation. In addition, increasing crop residue retention can result in greater C sequestration and crop yield and reductions in greenhouse gas emissions. However, there is still lack of consensus on the merits of CA in the context of smallholder farming systems in Nepal. This paper reviews existing literature and provides an overview of farming practices in Nepal, highlights near-term challenges associated with climate change and food security, and discusses the role of CA as a climate-smart strategy to minimize soil degradation and improve food security.

Tobacco control still poses an immense challenge for the government of Nepal. Updated knowledge on the current pattern of tobacco use and its associated factors will be helpful for policy makers to curb the tobacco epidemic. This study fills this gap by, (i) exploring demographic, socio-economic and geographic correlates of current tobacco use using a nationally representative sample of 15–49-year adults from Nepal Demographic Health survey 2016, and (ii) examining the prevalence and trends of both smoking and non-smoking forms of tobacco use in a nationally representative sample of 15–49-year adults drawn from three consecutive Demographic Health Surveys (DHS) between 2006 and 2016.Among males, the prevalence of smokeless tobacco use was higher than that of smoking (40.1% and 27.4% respectively), whereas among females smoking was more common than smokeless tobacco use (prevalence of 5.5% and 3.8% respectively). Both smoking and smokeless tobacco use were associated with older age and lower level of education. Among males, those living in urban areas were more likely to consume any form of tobacco. Residents of terai/plains were more likely to use smokeless tobacco. The concentration curves on cumulative proportion of tobacco use ranked by wealth quintiles showed tobacco use to be highest among the lowest socio-economic groups in both males and females in all three survey years. We found a decreasing trend of tobacco smoking and an increasing trend of smokeless tobacco use over the 10-year period. However, the consumption of both forms of tobacco increased in young males during the same period. Proper monitoring of adherence to directives of the anti-tobacco law should be ensured to curb the increasing burden of tobacco use among young males, and a similar effort is needed to sustain the decline in tobacco uses among other population groups in Nepal.

Background: Lower respiratory tract infection is a common cause of morbidity and mortality worldwide. A crosssectional study was carried out with an objective to study the antibiogram of Gram-negative isolates of patients with lower respiratory tract infection visiting Kathmandu Model Hospital.Methods: A total of 274 specimens including sputum, endotracheal aspirates, suction tips were cultured as per standard microbiological technique. Antibiotic susceptibility and detection of Extended-spectrum beta- lactamases (ESBLs) were performed following Clinical Laboratory Standard Institute (CLSI 2014) guidelines.Results: Respiratory pathogens were recovered from 24.6% (n=65) cases. Klebsiella pneumoniae (40%) was the commonest isolates. The highest prevalence of multidrug-resistance (69.23%) was observed in Acinetobacter calcoaceticus baumannii complex. Extended-spectrum beta- lactamases were detected in Escherichia coli (n=4), Klebsiella pneumoniae (n=4) and Acinetobacter calcoaceticus baumannii complex (n=1).Conclusions: High prevalence of multidrug-resistance and extended- spectrum beta- lactamase producers were observed in respiratory isolates. For effective management of lower respiratory tract infections, an ultimate and detailed microbiological diagnosis and susceptibility testing is required.

Background: For diagnosis of enteric fever, the culture of the organism from different body fluids is the gold standard. After diagnosis, it is important to treat with the right antibiotic before any complications can occur. The retrospective study is designed to explore the antibiotic sensitivity trend in blood culture positive typhoid fever cases and the extent of drug resistance before treatment is administered.Methods: A retrospective study was carried out for culture isolated enteric fever patients admitted in Kathmandu Model Hospital. The discharged records from January 2012 to December 2016 were analyzed. The patients above 15 years and with culture isolated enteric fever were included in the study.Results: One hundred fifty-nine strains of Salmonella typhi and paratyphi were isolated from Jan 2012 to Dec 2016 at Kathmandu Model Hospital. Out of 159 isolated, 125 (78.6%) were Salmonella typhi and 34 (21.4%) were paratyphi. Among them co-trimoxazole, chloramphenicol, ceftriaxone, cefotaxime, cefixime, and ofloxacin demonstrated 100% sensitivity. Similarly, amoxicillin sensitivity was 98.1% (n=156) while ciprofloxacin was sensitive in 6.3% (n=10), intermediately sensitive in 49.1% (n=78) and resistance in 44.7% (n=71).The newer quinolone levofloxacin showed 78.5% (n=11) sensitivity. Azithromycin was sensitive in 99.2% (n=132) of total isolated Salmonella species both typhi and paratyphi.Conclusions: A high degree of sensitivity was noted to chloramphenicol and co-trimoxazole, showing sensitivity has returned to conventional antibiotics. The drug-like ofloxacin is still the best responding drug in our contest whereas ciprofloxacin resistance is still high, but five years patterns show a trend of rollback of sensitivity.

As societies urbanize, their populations have become increasingly dependent on the private sector for essential services. The way the private sector responds to health emergencies such as the COVID-19 pandemic can determine the health and economic wellbeing of urban populations, an effect amplified for poorer communities. Here we present a qualitative document analysis of media reports and policy documents in four low resource settings-Bangladesh, Ghana, Nepal, Nigeria-between January and September 2020. The review focuses on two questions: (i) Who are the private sector actors who have engaged in the COVID-19 first wave response and what was their role?; and (ii) How have national and sub-national governments engaged in, and with, the private sector response and what have been the effects of these engagements? Three main roles of the private sector were identified in the review. (1) Providing resources to support the public health response. (2) Mitigating the financial impact of the pandemic on individuals and businesses. (3) Adjustment of services delivered by the private sector, within and beyond the health sector, to respond to pandemic-related business challenges and opportunities. The findings suggest that a combination of public-private partnerships, contracting, and regulation have been used by governments to influence private sector involvement. Government strategies to engage the private sector developed quickly, reflecting the importance of private services to populations. However, implementation of regulatory responses, especially in the health sector, has often been weak reflecting the difficulty governments have in ensuring affordable, quality private services. Lessons for future pandemics and other health emergencies include the need to ensure that essential non-pandemic health services in the government and non-government sector can continue despite elevated risks, surge capacity to minimize shortages of vital public health supplies is available, and plans are in place to ensure private ...
