Background: Male sex has always been considered as an independent risk factor for cardiovascular disease. But recent studies have shown controversial results. This study aimed to investigate the relation of serum testosterone withrisk factors of coronary artery diseasesand with degree of severity of coronary artery stenosisin men with coronary artery diseases.Methods: After applying inclusion and exclusion criteria 102 men (aged 60.42 += 11.11), were included. Fasting blood sample were obtained and blood sugar, total testosterone and lipid profile were measured. Severity of coronary stenosis was estimated by Gensini score. The relationships were assessed using chi-square test, one way analysis of variance and Pearson's Correlation. Results: Of the total 102 patients, majority of them 42 (41.2%) had triple vessel disease. Testosterone (nmol/L) was found to be 12.01 ± 6.1. Cardiovascular diseaserisk factors like age, body mass index etc. were found to be negatively correlated with testosterone but not statistically significant. Likewise, Gensini score also correlated negatively with testosteronebut not up to the threshold of statistical significance (r=-0.069, p-value = 0.496). Similar results were obtained when number of vessels involved and testosterone were compared. However, the number of diabetic patients gradually decreased with the increasing value of testosterone in the three tertile group (p-value = 0.040). Conclusions: This study could not find significant association between testosterone and coronary artery diseases, however low testosterone was associated with diabetes mellitus.Keywords: Coronary artery diseases; diabetes mellitus; gensini score; testosterone.

BACKGROUND:In Nepal, the burden of noncommunicable, chronic diseases is rapidly rising, and disproportionately affecting low and middle-income countries. Integrated interventions are essential in strengthening primary care systems and addressing the burden of multiple comorbidities. A growing body of literature supports the involvement of frontline providers, namely mid-level practitioners and community health workers, in chronic care management. Important operational questions remain, however, around the digital, training, and supervisory structures to support the implementation of effective, affordable, and equitable chronic care management programs. METHODS:A 12-month, population-level, type 2 hybrid effectiveness-implementation study will be conducted in rural Nepal to evaluate an integrated noncommunicable disease care management intervention within Nepal's new municipal governance structure. The intervention will leverage the government's planned roll-out of the World Health Organization's Package of Essential Noncommunicable Disease Interventions (WHO-PEN) program in four municipalities in Nepal, with a study population of 80,000. The intervention will leverage both the WHO-PEN and its cardiovascular disease-specific technical guidelines (HEARTS), and will include three evidence-based components: noncommunicable disease care provision using mid-level practitioners and community health workers; digital clinical decision support tools to ensure delivery of evidence-based care; and training and digitally supported supervision of mid-level practitioners to provide motivational interviewing for modifiable risk factor optimization, with a focus on medication adherence, and tobacco and alcohol use. The study will evaluate effectiveness using a pre-post design with stepped implementation. The primary outcomes will be disease-specific, "at-goal" metrics of chronic care management; secondary outcomes will include alcohol and tobacco consumption levels. DISCUSSION:This is the first population-level, hybrid effectiveness-implementation study of an integrated chronic care management intervention in Nepal. As low and middle-income countries plan for the Sustainable Development Goals and universal health coverage, the results of this pragmatic study will offer insights into policy and programmatic design for noncommunicable disease care management in the future. For the poorest of our world, non-communicable diseases and injuries (NCDIs) account for more than a third of their burden of disease; this burden includes almost 800000 deaths annually among those aged younger than 40 years, more than HIV, tuberculosis, and maternal deaths combined. • Despite already living in abject poverty, between 19 million and 50 million of the poorest billion spend a catastrophic amount of money each year in direct out-of-pocket costs on health care as a result of NCDIs. • Progressive implementation of affordable, cost-effective, and equitable NCDI interventions between 2020 and 2030 could save the lives of more than 4·6 million of the world's poorest, including 1·3 million who would otherwise die before the age of 40 years. • To avoid needless death and suffering, and to reduce the risk of catastrophic health spending, essential NCDI services must be financed through pooled, public resources, either from increased domestic funding or external funds. • National governments should set and adjust priorities based on the best available local data on NCDIs and the specific needs of the worst off. • International development assistance for health should be augmented and targeted to ensure that the poorest families affected by NCDIs are included in progress towards universal health care.
