When Europa meets Bismarck: how Europe is used in the Austrian healthcare system

What happens when the European Union sets new rules for the provision of cross-border healthcare services that once were conceived for the population living on the national territory? Does Europe destabilize national social solidarity? Do actors that govern the healthcare system further or resist the Europeanization of their national healthcare system? 0Taking Austria, a prototypical Bismarckian healthcare system, as an example, this book aims at answering these questions by looking at how actors navigate between national institutional constraints and European opportunities. It presents how new rules on the provision of cross-border healthcare in the European Union have the potential of destabilizing national welfare boundaries. 0Taking a sociological approach to Europeanization, it is analysed if and how actors adapt to such new rules.0An added value of the volume is to present the development of Austria's healthcare system in the "longue durée" through four political regime changes over the last 150 years, with European integration as the last wave of transformation to date. It shows that cross-border healthcare provision is already a well integrated practice; and how providers and payers of healthcare deal with European requirements and voice their policy preferences in the Brussels arena. Overall, it suggests both the flexibility and the resilience of the national models of welfare

Et si la relance de l'intégration européenne passait par la mise en place de vraies politiques publiques en matière de santé ?
Cet article cherche à démontrer l'impact que peut avoir l'européanisation naissante des systèmes de santé nationaux – ainsi que ses conséquences sur l'intégration européenne d'une manière générale – à compter du moment où l'on ferait le choix résolu d'avancer vers une Europe de la santé, véritable et approfondie. Cet article ne vise pas à montrer ni comment, ni pourquoi certaines tensions sont nées entre l'Union européenne et le national, du fait de l'européanisation embryonnaire du secteur sanitaire, mais d'imaginer l'étape d'après, la «nouvelle frontière ». À ce titre, il s'agit d'un plaidoyer pour une Europe de la santé cohérente et durable, signe de l'exploration méthodique et pratique du principe de solidarité à l'échelle du « vieux continent ».
La santé, « nouvelle frontière » de l'intégration européenne ?
The article sets out the possible impact and the consequences that a growing Europeanization of national healthcare systems will have for European integration once a decision has been made for a real European approach to healthcare. The article does not try to show how and why certain political tensions have emerged between the European and the national level concerning the nascent Europeanization of the healthcare sector. It rather imagines a step ahead, the "new frontier" of European integration: it is a plea for a coherent and sustainable European approach to healthcare which would embody a systematic and practical exploration of a Europe-wide notion of solidarity. ; Et si la relance de l'intégration européenne passait par la mise en place de vraies politiques publiques en matière de santé ? Suite à une série d'arrêts de la Cour de justice de l'Union européenne concernant les soins médicaux transfrontaliers, différentes ressources européennes offrent désormais aux acteurs la possibilité de poursuivre leurs intérêts au-delà des frontières de leur système de santé national. Les acteurs nationaux utilisent-ils ces ressources européennes à l'intérieur de leur propre système de santé ? Ici, nous prendrons le cas du système de santé autrichien en nous intéressant plus particulièrement aux acteurs qui régulent les soins ambulatoires. En effet, ces derniers mobilisent peu, voire pas du tout, les ressources européennes à leur disposition. Cet article souligne que ce faible usage peut être expliqué en prenant en compte les pratiques institutionnelles nationales de négociations corporatistes et consensuelles qui empêchent les acteurs de faire plus ample usage de l'Europe.

"Soziologische Ansätze erfahren in der europäischen Integrationsforschung steigende Aufmerksamkeit. Der 'Usages of Europe'-Ansatz untersucht aus einer Bottom-up-Perspektive die Handlungsmöglichkeiten und Ressourcen, welche die EU nationalen Akteuren zur Verfügung stellt, um ihre Interessen durchzusetzen. Da die europäischen Regelungen für grenzüberschreitende Gesundheitsdienstleistungen das für Gesundheitsdienstleistungen geltende Territorialitätsprinzip infrage gestellt haben, analysiert der Artikel zwei regionale Projekte zu grenzüberschreitenden Gesundheitsdienstleistungen in österreichischen Grenzregionen. Er geht der Frage nach, ob und wie die Strategien von Akteuren europäisiert worden sind und ob ihre Handlungen die nationalen Grenzen von Wohlfahrtsstaatlichkeit erodieren können. Um zu erklären wie regionale Akteure europäische Ressourcen in ihrem strategischen Vorgehen nutzen, wird vorgeschlagen, den 'Usages of Europe'-Ansatz für die Analyse mit dem Konzept der historisch-institutionalistischen Pfadabhängigkeit zu verbinden" (Autorenreferat)

During the last couple of years, sociological approaches in European studies have gained increasing attention from scholars. The 'usages of Europe' approach looks at the opportunities and resources that the EU provides for national actors to advance their interests, from a bottom-up perspective. As European rules on cross-border healthcare have been threatening the principle of the territoriality of healthcare services, the article analyses two cross-border healthcare projects in Austrian border regions to determine if and how actors' strategies have been Europeanised, and whether their actions could destructure national welfare boundaries. In order to explain how these regional actors incorporate European resources into their strategies a combination of the 'usages of Europe' approach with Historical Institutionalism is suggested. "The scope and extent of Europeanization among the member states of the European Union is all too often exaggerated by scholars of European integration. It has long been assumed that the mere existence of mechanisms designed to promote commonality between countries in the EU and to encourage a collective European identity has given credence to the idea of Europeanization and how effectively it has infiltrated EU member states. In stark contrast to the assumed success of these mechanisms in promoting Europeanization, recent studies have shown that, in the main, both domestic socio-economic as well as normative structures have proved resilient to this process. Through a compelling exploration of the relationship between European integration and Europeanization, this collection provides important insights into the disconnect between efforts to promote commonality and collective European identity and the ambivalent results of Europeanization at the socio-economic and normative structures of member states."--
