One of the mysteries of traditional Chinese medicine (TCM) is the metaphorical language it uses in describing conditions of the body. The human body is seen as an integrated system operating within the larger context of the natural system. Illnesses are therefore the results of imbalance within the individual system, and/or an imbalance between the individual system and the external natural system, which is very different from western medicine's more localized and focused attention given to specific organs. This article looks at the TCM's narrative of the thyroid (gland) and how this presents a different imagination as well as treatment of the related illnesses. Although western medicine relies on figures for indicating thyroid health, and surgery (or thyroid-targeted medication) as main measures of treatment, TCM refers to a holistic picture of the patient's wellness to diagnose and treat thyroid conditions. This article is a first-person narrative of a middle-aged female who found herself in the extraordinary position of having visible thyroid swellings, and yet all of the vital signs were within healthy parameters. Visits to TCM clinics empowered the narrator with an alternative framework to understand and experience the body, and a refocus on life habits and emotional practices.

BackgroundThis study examines the experiences of qualified nurses working with individuals diagnosed with both intellectual disability and personality disorder (PD) in a medium‐secure forensic intellectual disability setting. Potential training needs are highlighted, as well as other ways in which services could better support staff to work effectively with this client group.MethodIn‐depth, semi‐structured interviews were used to explore the nine participants' experiences; the narratives were analysed with interpretative phenomenological analysis (IPA).ResultsFour master themes emerged from (i) disorder overriding disability; (ii) resilience; (iii) ambivalence towards label and (iv) knowledge.ConclusionThe findings highlight that for these participants, the clients' intellectual disability appeared to be lost under the complexity of the PD diagnosis. The clinical implications are discussed in terms of developing training, supervision and support.

California has established itself as a leader in efforts to reduce greenhouse gas (GHG) emissions from transportation. At the same time, the state has not reflected its ambitious policies for GHG reduction and climate action in its practices for allocating state transportation funding. This white paper reviews the complex systems through which California generates and allocates state revenue for transportation investment. It finds that the state's framework for funding transportation is disconnected from its climate goals. The paper also suggests preliminary steps for revising this framework to reinforce GHG reduction goals. Such recommendations are particularly salient given the state's recently completed study of road user charges as an alternative transportation revenue source. Implementation of road charges – or any other new or revised transportation revenue source – would need to address the disposition of revenues generated. The paper argues that California should use any such opportunity to align the distribution of state transportation dollars with its climate objectives, not fall back on status quo allocation practices.View the NCST Project Webpage

Local governments have long relied on Level of Service (LOS), a measure of automobile congestion, as the basis for assessing transportation impacts of land use development projects. However, use of the LOS metric creates an incentive for projects that contribute to urban sprawl while penalizing denser development projects that could allow people better accessibility to jobs and services through alternate modes like walking, bicycling, or transit. Starting July 1, 2020, local governments in California are required to use vehicle miles traveled (VMT) rather than LOS to measure land use projects' transportation impacts. Researchers at UC Davis studied how this change affects the approval process for urban development. Because most agencies have not yet switched to using VMT in their analyses, the researchers looked back at environmental documents for development projects in the City of Los Angeles between 2001 and 2016 and determined whether these projects could have benefited from using a VMT metric instead of LOS for measuring their transportation impacts. Children with histories of maltreatment and disruptions in care are at elevated risk for impairments in early language development, which contribute to difficulties in other developmental domains across childhood. Given research demonstrating associations between parent responsiveness and children's early language development, we examined whether a parenting intervention administered in infancy improved preschool receptive language skills in children involved with the child welfare system. Attachment and Biobehavioral Catch-up (ABC) is a 10-session intervention that aims to enhance parent–child interactions. The follow-up results of this randomized clinical trial demonstrated that infants who received the ABC intervention ( n = 24) scored significantly higher on a test of receptive vocabulary at age 36 months than infants who received a control intervention ( n = 28). These results provide evidence of the critical role of parental responsiveness in supporting optimal language development among young children with histories of child welfare involvement.
