Resumo A regulação econômica realizada pela Câmara de Regulação do Mercado de Medicamentos (CMED) com início em 2003, tem como finalidade promover a assistência farmacêutica por meio de mecanismos que estimulem a oferta de medicamentos e a competitividade do setor. Desde 2007 estudos brasileiros demonstram que o modelo atual de regulação econômica tem sido incapaz de controlar a variação dos preços dos medicamentos comercializados no Brasil. O presente estudo observa elevado descolamento entre o preço praticado nos pontos de venda e o preço máximo de venda ao consumidor, e um monitoramento do mercado pouco ativo, ao identificar registros de preços que não se adequam às regras atuais. Portanto, verifica-se a necessidade de tornar a regulação econômica mais efetiva e dinâmica à realidade do mercado e do consumidor a fim de auxiliar o Brasil na garantia do acesso da população à medicamentos essenciais à preços justos. ABSTRACT This report consists of what Conceição Evaristo calls 'writing and living' – the writing of black women, which mixes with experience, their memories, and the memories of their people, aiming to disturb consciences and echo our stories. The author's 'writing, living and self-seeing' is recognized as the 'writing of the soul', from where each woman writes considering the world she lives in. It aims to briefly reflect on access to medicines, focusing on the access to contraceptive methods in Angola, based on the author's experience. It is a qualitative study, and data were collected through documentary research, field study, and interviews. Content analysis was adopted for data processing. Access to contraceptive methods in Angolan territory is incipient, a result of ineffective public policies and good governance, as well as ineffective external interference. These are reinforced by colonial heritage, which provides approaches in disagreement with local needs. Sexual and reproductive rights must be seen as a political and public health issue inherent to human dignity, beyond the eugenic perspective of birth control.
RESUMO Este relato apresenta uma proposta do que a escritora Conceição Evaristo chama de 'escrevivência' – escrita da mulher negra, que se mescla com vivência, relato de memórias e memórias de seu povo, visando incomodar consciências e ecoar nossas histórias. O 'escrever, viver e se ver' da autora é reconhecido como 'escrita da alma', lugar onde cada mulher escreve considerando o mundo que vive, de uma forma integrada. Visa fazer uma breve reflexão sobre o acesso aos medicamentos, tomando o acesso aos métodos contraceptivos em Angola como recorte, tendo em conta a vivência da autora. É um estudo qualitativo e a coleta de dados foi feita por meio de pesquisa documental, estudo de campo e entrevistas. Para o tratamento de dados foi adotada a análise de conteúdo. Identificou-se que o acesso aos métodos contraceptivos em território angolano é incipiente, resultado de políticas públicas e boa governança pouco efetivas, bem como a interferência externa pouco resolutiva. Estes, são reforçados pela herança colonial, que propicia abordagens em desacordo com as necessidades locais. É fundamental que os direitos sexuais e reprodutivos sejam encarados como questão política, de saúde pública e inerente à dignidade humana, para além da perspectiva eugenista de controle de natalidade.
RESUMO Este ensaio aborda como e em que níveis a Assistência Farmacêutica é atravessada pela dinâmica da governança global da saúde, e como se relaciona com aspectos geopolíticos e socioeconômicos. Tenta-se ir além do acesso a medicamentos e produtos para saúde, abordando também o uso racional de medicamentos, seu impacto na resistência aos antimicrobianos e na saúde dos povos. Além disso, discute como a Assistência Farmacêutica pode ser vista nesse contexto.
ABSTRACT This essay addresses how and at what levels Pharmaceutical Services is affected by the dynamics of global health governance, and how it correlates with geopolitical and socioeconomic aspects. It attempts to go beyond access to medicines and health products, as well as to address the rational use of medicines, the impact in antimicrobial resistance and in people's health. Furthermore, it debates how Pharmaceutical Services can be seen in this context.
Introduction: The right to health, one of the achievements guaranteed by the Citizen Constitution promulgated in 1988, came in response to the citizens' struggle for health reform in 1986. The guarantee of this right is established in the Unified Health System (SUS). Objective: To analyze the conception of the right to health of middle level proffesionals of SUS. Methods: Qualitative approach research, through 2 focus groups involving 9 graduates of a Health Technical School of the SUS in the north of Brazil, from the courses of clinical analysis, dental hygiene and nursing, working in the Unified Health System. Content analysis proposed by Bardin (2009), with the definition of 3 categories. Results: Three categories show the results obtained concluding concepts of health, the right to health and health conceptions; Health practices and access to care; and topics of training, health care and humanization. Conclusion: The participants of this study have a conception of the right to health directed to the legislation, their concepts of health approach elements of SUS policy, among others, promotion, prevention, humanization; their conceptions of health are strongly focused on the biomedical model centered on disease and medicine.
RESUMO Este artigo objetiva identificar os trabalhadores de nível superior cadastrados nas Unidades Básicas de Saúde (UBS) entre 2008 e 2013, visando contribuir com o debate sobre o planejamento da força de trabalho na Atenção Básica em saúde. Trata-se de estudo descritivo, retrospectivo e de abordagem quantitativa. Embora médicos, enfermeiros e cirurgiões-dentistas componham as ocupações de nível superior tradicionalmente hegemônicas nas UBS, constatou-se a ampliação da participação de outras categorias profissionais, o que se atribui à implantação de equipes do Núcleo de Apoio à Saúde da Família em todo o País.
Introduction: To train pharmacists working in the public health system, the Brazilian Ministry of Health developed a specialization course called Pharmaceutical Service and Access to Medicine Management (PSAMM) between 2010 and 2016. The course was free of charge and used e-learning as its main approach. In the end, 2,500 pharmacists were trained. The purpose of this study was to identify and analyze the strengths, weaknesses, opportunities, and threats of an in-service and e-learning course for pharmacists working in a public health system. Materials and Methods: Three workshops involving 67 participants were conducted at the conclusion of the course to analyze the perspective of the PSAMM course's faculty (tutors, regional coordinators, professors, and management committee) and students (pharmacists). Strengths, weaknesses, opportunities, and threats analysis and qualitative analysis methods were used. Results and Discussion: The strength dimension had the greatest number of items. The qualitative analysis resulted in six categories: the category "E-learning in continuing education" had the most cited items. Internal elements such as in-service hands-on activities directly related to the professionals' roles, course contents, faculty, and the methods to offer the course (the mixed methods and materials) were positively assessed. Nonetheless, external elements were considered critical for the course's outcomes such as investments in the infrastructure of pharmaceutical services, access to the internet, local managers' support for continuing education and innovation implementation, practice of interprofessional collaboration, and political stability. The continuing education course in the public health system was affected by internal elements such as its project and structure as well as external elements such as the sociopolitical scenario. Continuing education investment must be accompanied by infrastructure investment and coordination of services.
BACKGROUND: The valuation of medicines as health needs vary depending on the stakeholders involved (users, prescribers, managers, etc.) and their expectations. These factors modulate the role of medicines as a health need and influence access to medicines, and could be useful to explain the rising of Judicialization of access to medicines. AIM: To conduct a comparative analysis of the causes and consequences of judicialization of access to medicines in Argentina, Brazil, Colombia and Chile from the perspective of medicines as health needs. METHODS: A qualitative, cross-country study was carried out in these 4 countries. Semi-structured interviews were conducted with 50 representatives of the different stakeholders involved in the judicialization of access to medicines, including Executive branch, Judiciary, health system managers, patient organizations. The interviews were audio-recorded and transcribed verbatim. Thematic analysis used a framework approach based on the theoretical model for medicines as health needs. FINDINGS: Representatives from Argentina, Brazil and Colombia considered judicialization of access to medicines as a widespread phenomenon in their respective countries. Meanwhile in Chile, the respondents highlighted that most lawsuits related to the right to health were filed against private insurers because of unjustified increases in the insurance premiums. The comparative analysis showed that judicialization of access to medicines emerged in the four countries regardless of the constitutional protection or the health system population coverage. Among the causes were mentioned difficulties in guaranteeing access to covered medicines and the influence of pharmaceutical marketing on needs assessment and prescription behaviours. The interviewees highlighted the pressure to health system managers to fulfil their responsibilities as a positive impact of litigation. OBJECTIVE: To identify limiting factors in the management of pharmaceutical services in the primary health care provided by the Brazilian Unified Health System (SUS). METHODS: This study was based on the data from the Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos no Brasil (PNAUM – National Survey on Access, Use and Promotion of Rational Use of Medicines), and it was conducted by interviews with professionals responsible for pharmaceutical services in Brazilian cities, in 2015. To identify the management limiting factors, we considered the organizational, operational, and sustainability indicators of the management. For the analyses, we included the weights and structure of analysis plan for complex samples. The results were expressed by frequencies and measures of central tendency with 95% confidence interval, considering the Brazilian geographic regions. RESULTS: We identified the following limiting factors: lack of pharmaceutical services in the Municipal Health Secretariat organization chart (24%) and in the health plan (18%); lack of participation of managers in the Health Board and the absence of reference to this topic in the agenda of meetings (58.4%); lack of financial autonomy (61.5%) and lack of knowledge on the available values (81.7%); lack of adoption of operational procedures (about 50%) for selection, scheduling, and acquisition; and the fact that most professionals evaluate the organization of pharmaceutical services as good and great (58.8%), despite the worrisome indicators. CONCLUSIONS: Pharmaceutical services management is currently supported by a legal and political framework that should guide and contribute to improve the pharmaceutical services in the Brazilian Unified Health System primary health care. However, there is a mismatch between the goals established by these guidelines and what is actually happening ; OBJETIVO: Identificar fatores condicionantes da gestão da assistência farmacêutica na atenção primária no âmbito do Sistema Único de Saúde. MÉTODOS: Estudo com dados obtidos da Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos no Brasil (PNAUM) – Serviços, realizada por meio de entrevistas com responsáveis pela assistência farmacêutica em municípios, em 2015. Para identificar os fatores condicionantes da gestão foram considerados indicadores organizacionais, operacionais e de sustentabilidade da gestão. Para as análises consideraram-se os pesos amostrais e a estrutura do plano de análise para amostras complexas. Os resultados foram expressos por meio de frequências e medidas de tendência central com intervalo de confiança de 95%, por regiões geográficas do Brasil. RESULTADOS: Foram identificados os fatores condicionantes: a ausência da assistência farmacêutica no organograma da secretaria (24%) e no plano de saúde (18%), a não participação dos gestores no conselho de saúde e a não referência desse tema na pauta das reuniões (58,4%), falta de autonomia financeira (61,5%) e conhecimento dos valores disponíveis (81,7%), falta de adoção de procedimentos operacionais em cerca de 50% para seleção, programação e aquisição e o fato da maioria avaliar a organização da assistência farmacêutica como boa e ótima (58,8%), apesar dos indicadores preocupantes apontados. CONCLUSÕES: A gestão da assistência farmacêutica encontra-se respaldada em um arcabouço legal e político, que deveria nortear e contribuir para melhoria da assistência farmacêutica na atenção primária no Sistema Único de Saúde. OBJETIVO: Caracterizar a situação sanitária dos medicamentos na Atenção Básica, nas regiões brasileiras, quanto a requisitos técnico-sanitários, responsável pela farmácia/unidade de dispensação, condições ambientais, de armazenamento, e de fracionamento, controle de estoque e gerenciamento de resíduos, itens de segurança contra incêndio e pane elétrica, problemas no transporte, regulamentação da propaganda e farmacovigilância. MÉTODOS: Artigo integrante da Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos – Serviços, um estudo transversal, exploratório, de natureza avaliativa, composto por um levantamento de informações numa amostra representativa de municípios, estratificada pelas regiões brasileiras, que constituem domínios do Estudo, e uma amostra de serviços de Atenção Básica. Realizou-se observação direta dos serviços farmacêuticos com registro fotográfico e entrevistas presenciais com os responsáveis pela entrega de medicamentos e por telefone com o responsável pela assistência farmacêutica. Os dados foram processados com o software SPSS® versão 21. RESULTADOS: As dimensões investigadas mostraram deficiências relevantes e desigualdades entre as regiões, em geral mais favoráveis nas regiões Sudeste e Centro-Oeste e mais deficitárias nas regiões Nordeste e Norte. Constatou-se descumprimento de requisitos técnicos e sanitários imprescindíveis à conservação dos medicamentos que podem interferir na manutenção da estabilidade e, assim, na sua qualidade, eficácia e segurança. A regulação da propaganda/promoção de medicamentos ainda é incipiente e existe algum avanço na estruturação de mecanismos em relação à farmacovigilância. CONCLUSÕES: A situação sanitária dos medicamentos na Atenção Básica no Brasil desperta preocupações pelo descumprimento da legislação sanitária específica para os estabelecimentos de dispensação e de um amplo conjunto de requisitos imprescindíveis à conservação dos medicamentos. Constatou-se um descompasso entre os esforços no âmbito do Sistema Único de Saúde para promover o acesso aos medicamentos para toda a população e a organização e qualificação dos serviços farmacêuticos ; OBJECTIVE: To characterize the technical issues and conditions of medicines conservation in Primary Health Care of Brazilian regions, responsible for pharmacy/dispensing unit profile; environmental, storage, and dose fractioning conditions; inventory control and waste management; fire and electrical failure safety items; transportation problems; advertising regulation; and pharmacovigilance. METHODS: This article is part of the Pesquisa Nacional sobre Acesso, Utilização e Promoção do Uso Racional de Medicamentos – Serviços (National Survey on Access, Use and Promotion of Rational Use of Medicines – Services)–, a cross-sectional and exploratory study, of evaluative nature, consisting of an information survey within a representative sample of municipalities, stratified by Brazilian regions, which constitute the study domains, and a sample of Primary Health Care services. Pharmaceutical services (PS) were directly observed with photographic record and face-to-face interviews with those responsible for the dispensing of medicines and over the telephone with those responsible for pharmaceutical services. Data were processed with the SPSS® software version 21.RESULTS: The investigated dimensions showed relevant deficiencies and inequalities between the regions, generally more favorable in the Southeast and Midwest regions and weaker in the Northeast and North regions. We verified non-compliance with technical requirements and conditions essential to the conservation of medicines, which may interfere with the maintenance of stability and, thus, on their quality, efficacy, and safety. The regulation of advertising/promotion of medicines is still incipient and there is some progress in the structuring of mechanisms regarding pharmacovigilance. CONCLUSIONS: The sanitary situation of medicines in Brazilian Primary Health Care is alarming due to the violation of the specific sanitary legislation for dispensing establishments and due to a wide range of requirements essential to the conservation of medicines.
