Hintergrund
Sedentäres Verhalten ist mit einem erhöhten Risiko für chronische Krankheiten und einer höheren Gesamtmortalität assoziiert. Arbeitsbezogenes Sitzen hat einen großen Anteil am täglichen sedentären Verhalten, insbesondere bei Büroangestellten.

Ziel der Arbeit
In dieser Studie sollte untersucht werden, wie viel Zeit bei verschiedenen Aufgaben am Arbeitsplatz und beim Pendeln von Verwaltungsangestellten einer Universitätsklinik in Deutschland im Sitzen verbracht wird.

Material und Methoden
Eine fragebogengestützte Querschnittsstudie mit Verwaltungsangestellten des Universitätsklinikums Regensburg wurde durchgeführt, um arbeitsbezogenes sedentäres Verhalten zu untersuchen.

Ergebnisse
Die Studienpopulation bestand aus 159 Teilnehmenden (54,1 % Frauen, 51,6 % älter als 40 Jahre), was einer Antwortquote von 26 % entspricht. Die durchschnittliche tägliche Sitzzeit am Arbeitsplatz betrug im Median 7,0 h (Interquartilsbereich [IQR] 6,0–7,5 h) und fand hauptsächlich bei der Computerarbeit statt (57,3 %). Die mittlere Stehzeit bei der Arbeit betrug im Median 0,8 h (IQR 0,3–1,4 h). Verwaltungsangestellte verbrachten während des Pendelns im Median 0,7 h (IQR 0,3–1,0 h) pro Tag im Sitzen. Die Teilnehmenden waren der Ansicht, dass langes und ununterbrochenes Sitzen negative (69,6 %) oder eher negative (29,7 %) Auswirkungen auf die Gesundheit hat.

Diskussion
Verwaltungspersonal in Krankenhäusern verbringt einen großen Teil der täglichen Arbeitszeit mit sedentärem Verhalten. Maßnahmen, die es ermöglichen, sowohl im Sitzen als auch im Stehen zu arbeiten, können zu einer Verringerung der arbeitsbedingten Sitzzeit führen und damit die Gesundheit am Arbeitsplatz und im weiteren Sinne die öffentliche Gesundheit verbessern.

Lifestyle factors, including diet, have long been recognised as potentially important determinants of cancer risk. In addition to the significant role diet plays in affecting body fatness, a risk factor for several cancers, experimental studies have indicated that diet may influence the cancer process in several ways. Prospective studies have shown that dietary patterns characterised by higher intakes of fruits, vegetables, and whole-grain foods, and lower intakes of red and processed meats and salt, are related to reduced risks of death and cancer, and that a healthy diet can improve overall survival after diagnosis of breast and colorectal cancers. There is evidence that high intakes of fruit and vegetables may reduce the risk of cancers of the aerodigestive tract, and the evidence that dietary fibre protects against colorectal cancer is convincing. Red and processed meats increase the risk of colorectal cancer. Diets rich in high-calorie foods, such as fatty and sugary foods, may lead to increased calorie intake, thereby promoting obesity and leading to an increased risk of cancer. There is some evidence that sugary drinks are related to an increased risk of pancreatic cancer. Taking this evidence into account, the 4th edition of the European Code against Cancer recommends that people have a healthy diet to reduce their risk of cancer: they should eat plenty of whole grains, pulses, vegetables and fruits; limit high-calorie foods (foods high in sugar or fat); avoid sugary drinks and processed meat; and limit red meat and foods high in salt.

Physical activity is a complex, multidimensional behavior, the precise measurement of which is challenging in free-living individuals. Nonetheless, representative survey data show that 35% of the European adult population is physically inactive. Inadequate levels of physical activity are disconcerting given substantial epidemiologic evidence showing that physical activity is associated with decreased risks of colon, endometrial, and breast cancers. For example, insufficient physical activity levels are thought to cause 9% of breast cancer cases and 10% of colon cancer cases in Europe. By comparison, the evidence for a beneficial effect of physical activity is less consistent for cancers of the lung, pancreas, ovary, prostate, kidney, and stomach. The biologic pathways underlying the association between physical activity and cancer risk are incompletely defined, but potential etiologic pathways include insulin resistance, growth factors, adipocytokines, steroid hormones, and immune function. In recent years, sedentary behavior has emerged as a potential independent determinant of cancer risk. In cancer survivors, physical activity has shown positive effects on body composition, physical fitness, quality of life, anxiety, and self-esteem. Physical activity may also carry benefits regarding cancer survival, but more evidence linking increased physical activity to prolonged cancer survival is needed. Future studies using new technologies - such as accelerometers and e-tools - will contribute to improved assessments of physical activity. Such advancements in physical activity measurement will help clarify the relationship between physical activity and cancer risk and survival. Taking the overall existing evidence into account, the fourth edition of the European Code against Cancer recommends that people be physically active in everyday life and limit the time spent sitting.

It is estimated that over half the population of the European Union (EU) is overweight or obese due to an imbalance between energy expenditure and energy intake; this is related to an obesogenic environment of sociocultural, economic and marketing challenges to the control of body weight. Excess body fat is associated with nine cancer sites - oesophagus, colorectum, gall bladder, pancreas, postmenopausal breast, endometrium, ovary, kidney and prostate (advanced) - and 4-38% of these cancers (depending on site and gender) can be attributed to overweight/obesity status. Metabolic alterations which accompany excess body weight are accompanied by increased levels of inflammation, insulin, oestrogens and other hormonal factors. There are some indications that intentional weight loss is associated with reduced cancer incidence (notably in postmenopausal breast and endometrial cancers). Excess body weight is also a risk factor for several other diseases, including diabetes and heart disease, and is related to higher risk of premature death. In reviewing the current evidence related to excess body fat and cancer, the European Code against Cancer Nutrition Working Group has developed the following recommendation: 'Take action to be a healthy body weight'.

Alcohol consumption is the third leading risk factor for disease and mortality in Europe. The International Agency for Research on Cancer (IARC) Monographs provide strengthened evidence that the consumption of alcoholic beverages is causally associated with cancers of the oral cavity, pharynx, larynx, oesophagus, liver, colorectum and female breast, even for low and moderate alcohol intakes. The risk of cancer increases in a dose-dependent manner, and the higher the amount of alcohol consumed, the higher the risk of developing cancer. Several biological mechanisms explain the carcinogenicity of alcohol; among them, ethanol and its genotoxic metabolite acetaldehyde play a major role. Taking all this evidence into account, a recommendation of the 4th edition of the European Code against Cancer (ECAC) is: "If you drink alcohol of any type, limit your intake. Not drinking alcohol is better for cancer prevention."

Breast cancer is the most frequent cancer in women, and incidence rates have been rising in European Union (EU) countries over recent decades due in part to a sharp decline in breastfeeding practices. Evidence for a protective association between breastfeeding and the risk of breast cancer at all ages is convincing, and modest protective relationships between breastfeeding and the risk of endometrial and ovarian cancers have been suggested. The reduction in breast cancer risk is estimated at 2% for an increase of 5 months of lifetime breastfeeding. The longer women breastfeed, the more they are protected against breast cancer. In addition, breastfeeding is associated with several health benefits for both the mother and the breastfed child. Taking all this evidence into account, the 4th edition of the European Code against Cancer recommends: "Breastfeeding reduces the mother's cancer risk. If you can, breastfeed your baby".

This paper briefly summarizes and highlights the scientific evidence behind the new sedentary behaviour guidelines for all adults and discusses its strengths and limitations, including evidence gaps/research needs and potential implications for public health practice. METHODS: An overview of the scope and methods used to update the evidence is provided, along with quality assessment and grading methods for the eligible new systematic reviews. The literature search update was conducted for WHO by an external team and reviewers used the AMSTAR 2 (Assessment of Multiple Systematic Reviews) tool for critical appraisal of the systematic reviews under consideration for inclusion. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) method was used to rate the certainty (i.e. very low to high) of the evidence. RESULTS: The updated systematic review identified 22 new reviews published from 2017 up to August 2019, 14 of which were incorporated into the final evidence profiles. Overall, there was moderate certainty evidence that higher amounts of sedentary behaviour increase the risk for all-cause, cardiovascular disease (CVD) and cancer mortality, as well as incidence of CVD, cancer, and type 2 diabetes. However, evidence was deemed insufficient at present to set quantified (time-based) recommendations for sedentary time. Moderate certainty evidence also showed that associations between sedentary behaviour and all-cause, CVD and cancer mortality vary by level of moderate-to-vigorous physical activity (MVPA), which underpinned additional guidance around MVPA in the context of high sedentary time. Finally, there was insufficient or low-certainty systematic review evidence on the type or domain of sedentary behaviour, or the frequency and/or duration of bouts or breaks in sedentary behaviour, to make specific recommendations for the health outcomes examined. CONCLUSIONS: The WHO 2020 guidelines are based on the latest evidence on sedentary behaviour and health, along with interactions between sedentary behaviour and MVPA, and support implementing public health programmes and policies aimed at increasing MVPA and limiting sedentary behaviour. Important evidence gaps and research opportunities are identified.

Funder: Public Health Agency of Canada ; Funder: Government of Norway ; BACKGROUND: In July, 2019, the World Health Organization (WHO) commenced work to update the 2010 Global Recommendations on Physical Activity for Health and established a Guideline Development Group (GDG) comprising expert public health scientists and practitioners to inform the drafting of the 2020 Guidelines on Physical Activity and Sedentary Behavior. The overall task of the GDG was to review the scientific evidence and provide expert advice to the WHO on the amount of physical activity and sedentary behavior associated with optimal health in children and adolescents, adults, older adults (> 64 years), and also specifically in pregnant and postpartum women and people living with chronic conditions or disabilities. METHODS: The GDG reviewed the available evidence specific to each sub-population using systematic protocols and in doing so, identified a number of gaps in the existing literature. These proposed research gaps were discussed and verified by expert consensus among the entire GDG. RESULTS: Evidence gaps across population sub-groups included a lack of information on: 1) the precise shape of the dose-response curve between physical activity and/or sedentary behavior and several of the health outcomes studied; 2) the health benefits of light-intensity physical activity and of breaking up sedentary time with light-intensity activity; 3) differences in the health effects of different types and domains of physical activity (leisure-time; occupational; transportation; household; education) and of sedentary behavior (occupational; screen time; television viewing); and 4) the joint association between physical activity and sedentary time with health outcomes across the life course. In addition, we acknowledge the need to conduct more population-based studies in low- and middle-income countries and in people living with disabilities and/or chronic disease, and to identify how various sociodemographic factors (age, sex, race/ethnicity, socioeconomic status) modify the health effects of physical activity, in order to address global health disparities. CONCLUSIONS: Although the 2020 WHO Guidelines for Physical Activity and Sedentary Behavior were informed by the most up-to-date research on the health effects of physical activity and sedentary time, there is still substantial work to be done in advancing the global physical activity agenda.

Among children and adolescents, an average of 60 min/day of moderate-to-vigorous intensity aerobic physical activity across the week provides health benefits. The guidelines recommend regular muscle-strengthening activity for all age groups. Additionally, reducing sedentary behaviours is recommended across all age groups and abilities, although evidence was insufficient to quantify a sedentary behaviour threshold. Conclusion: These 2020 WHO guidelines update previous WHO recommendations released in 2010. They reaffirm messages that some physical activity is better than none, that more physical activity is better for optimal health outcomes and provide a new recommendation on reducing sedentary behaviours. These guidelines highlight the importance of regularly undertaking both aerobic and muscle strengthening activities and for the first time, there are specific recommendations for specific populations including for pregnant and postpartum women and people living with chronic conditions or disability. These guidelines should be used to inform national health policies aligned with the WHO Global Action Plan on Physical Activity 2018–2030 and to strengthen surveillance systems that track progress towards national and global targets.

Hintergrund: In epidemiologischen Studien ist die standardisierte Erfassung soziodemografischer Merkmale von hoher Bedeutung, da Variablen wie Geschlecht, Alter, Bildung oder Erwerbsstatus wichtige Einflussfaktoren auf Gesundheitschancen und Krankheitsrisiken darstellen. In der NAKO Gesundheitsstudie werden zentrale Faktoren aus diesem Themenbereich berücksichtigt. Ziel der Arbeit: Der Beitrag gibt einen Überblick über den wissenschaftlichen Hintergrund und die konkrete Erhebung soziodemografischer Angaben in der NAKO. Zudem werden die Verteilung einzelner Merkmale sowie Zusammenhänge mit gesundheitsassoziierten Maßen exemplarisch vorgestellt. Material und Methoden: Anhand der Daten zur Halbzeit der Basiserhebung (n = 101.724) wurde die Verteilung soziodemografischer Merkmale dargestellt und Zusammenhänge mit beispielhaft ausgewählten Gesundheitsindikatoren (Body-Mass-Index, selbst berichtete Gesundheit) analysiert, um die Validität der Messung soziodemografischer Angaben zu beurteilen. Ergebnisse: Das mittlere Alter der Teilnehmenden lag bei 52,0 Jahren (SD = 12,4). 53,6 % der Teilnehmenden waren Frauen, 54,3 % hatten einen hohen Bildungsabschluss, 60,1 % waren verheiratet zusammenlebend, 72,0 % erwerbstätig und 3,4 % erwerbslos. Bekannte Zusammenhänge zwischen Soziodemografie und Gesundheit konnten reproduziert werden. So waren niedrige Bildung, hohes Alter und Erwerbslosigkeit mit einer erhöhten Häufigkeit von Adipositas und schlechter selbst berichteter Gesundheit assoziiert. Diskussion: Die NAKO Gesundheitsstudie erhebt viele soziodemografische Merkmale. In Kombination mit der Fülle an Gesundheitsdaten und dem Längsschnittdesign ergeben sich so neue Möglichkeiten für die gesundheitswissenschaftliche und sozialepidemiologische Forschung in Deutschland.
