Visible persons, invisible work? Exploring articulation work in the implementation of person-centred care on a hospital wardRecently, many initiatives promoting a holistic view of the patient have been developed. In the last 20 years, different models and frameworks have been advanced to operationalize these different forms of holistic care. This article focuses on one specific case of holistic person-centred care, using the sociological theories of articulation work and invisible work to investigate the efforts involved in sustaining its realization. The article builds on a small ethnographic study conducted at a ward in a Swedish hospital implementing person-centred care. Following a nurse, and her patient, through a 'person-centred' admission process and its subsequent procedures, it is argued that person-centred care depends on nurses performing many new tasks which are rarely recognized and appreciated. Secondly, it is argued that nurses are continually asked to do what Anselm Strauss defined as articulation work, coordinating between these different new tasks and established duties. Thirdly, the article discusses the tensions arising when implementing a formalized model of care, which builds on a critique of standardization and objectification, and the work that is excluded and invisible in such routinized operationalization. Finally, it is concluded that while the successful implementation of person-centred care is often argued to rely on the willingness of nurses to surrender old habits, it seems rather to hinge on the skilled inventiveness of these nurses and their ability to overcome the practical difficulties they encounter.

Client non‐cooperation is a widely recognised problem in welfare services. Being 'hard‐to‐reach' is considered a risk especially for the most vulnerable clients, for example in terms of increased homelessness. Such clients pose challenges to social inclusion, and services make some allowances to achieve engagement. However, even a minimum level of cooperation is required from hard‐to‐reach clients. In the context of home visiting, we study welfare workers' efforts to engage with clients who continuously avoid contact. We examine three services in Finland, England, and Sweden that provide floating support to clients in their own accommodation. Utilising Robert Emerson's idea of 'the last resort', we analyse how workers justify their decisions to continue or terminate the support with the hard‐to‐reach. The data consist of team meeting recordings and home visit observations. We aim to demonstrate that justifications deployed to make the decision to end the home visiting service or tighten control, draw on 'last resort responses'. We identify three types of justifications: retrospective summaries on past failures to reach the client, intensifying remedial actions to engage clients, and characterisations of clients as uncooperative. While such justifications can be seen to draw on shared ethics, they have different ethical implications.

This article is concerned with the doing and production of data. We ask how data are made in intimate spaces such as the home in collaboration with the different parties involved in home-based care and services. The article builds on ethnographic field notes from 73 home visits, in the context of home-based mental health, substance abuse and social care for adults in Finland and Sweden. Drawing on affect theory, the article aims to foreground aspects of the production of data and research that are often edited out of the research process. In so doing, we argue that the production of data would not be possible without the active and affective collaboration of all parties involved in home visits. Thus, the article scrutinizes in detail the efforts made by different parties, such as researchers, clients and workers to do and produce data. While we study an atypical setting of institutional interaction, we contend that affects and affective relations gain particular importance in the home.

Background: Person-centred care is a growing imperative in healthcare, but the documentation of person-centred care is challenging. According to the Gothenburg Framework of Person-centred Care, care should be documented in continuously revised care plans and based on patients' personally formulated goals and resources to secure a continuous partnership. Objectives: This study aimed to examine care plans produced within a randomised controlled trial that tested a person-centred care intervention in older people with acute coronary syndrome. Nurses with training in the theory and practice of person-centred care had written the care plans. Methods: We conducted a secondary analysis of care plans developed in a randomised controlled trial for assessing person-centred care in patients with acute coronary syndrome (Myocardial Infarct [MI] or unstable angina pectoris). The study sample included 84 patients, with three care plans for each patient from inpatient (T1), outpatient (T2) and primary care (T3), that is, a total of 252 care plans. We conducted a descriptive quantitative content analysis of the care plans to examine the reported patients' life-world and medical/health resources and goals. Results: The analysis illustrates the differences and overlaps between life-world and medical/health goals and resources. The documented goals and resources change over time: life-world goals and resources decreased with time as medical/health goals and resources documentation increased. Conclusions: This paper illustrates that in the setting of a randomised controlled trial, nurses with training in person-centred care recorded fewer life-world and more medical/health goals over time. Placing life-world goals at the top of the goal hierarchy enables alignment with medical/health goals. Further research should explore whether the goals and resources documented in care plans accurately reflect patients' wishes as they transition along the care chain.
