OBJECTIVE: To estimate the direct costs of schizophrenia for the public sector. METHODS: A study was carried out in the state of São Paulo, Brazil, during 1998. Data from the medical literature and governmental research bodies were gathered for estimating the total number of schizophrenia patients covered by the Brazilian Unified Health System. A decision tree was built based on an estimated distribution of patients under different types of psychiatric care. Medical charts from public hospitals and outpatient services were used to estimate the resources used over a one-year period. Direct costs were calculated by attributing monetary values for each resource used. RESULTS: Of all patients, 81.5% were covered by the public sector and distributed as follows: 6.0% in psychiatric hospital admissions, 23.0% in outpatient care, and 71.0% without regular treatment. The total direct cost of schizophrenia was US$191,781,327 (2.2% of the total health care expenditure in the state). Of this total, 11.0% was spent on outpatient care and 79.2% went for inpatient care. CONCLUSIONS: Most schizophrenia patients in the state of São Paulo receive no regular treatment. The study findings point out to the importance of investing in research aimed at improving the resource allocation for the treatment of mental disorders in Brazil. ; OBJETIVO: Estimar o custo direto da esquizofrenia para o setor público. MÉTODOS: O estudo foi realizado no Estado de São Paulo, no ano de 1998. Utilizaram-se dados da literatura e de institutos governamentais de pesquisa para estimar o número total de pacientes com esquizofrenia no Estado sob cobertura do Sistema Único de Saúde. Foi construída uma árvore de decisão mostrando a distribuição desses pacientes quanto ao nível de tratamento. Por meio de pesquisa em prontuários de alguns hospitais públicos e serviços ambulatoriais, documentou-se a utilização de recursos por esses pacientes no período de um ano. Foram atribuídos valores em Reais para esses recursos a fim de se calcular o custo direto total médico-hospitalar. RESULTADOS: Do total de pacientes, 81,5% estão sob cobertura do SUS e encontram-se assim distribuídos: 6,0% internados, 23,0% em tratamento ambulatorial e 71,0% sem tratamento regular. O custo direto total da esquizofrenia foi de R$222 milhões (US$191,781,327) (2,2% do total de gastos em saúde do Estado), sendo 11% destinados ao tratamento ambulatorial e 79,2% às internações psiquiátricas. CONCLUSÕES: A maior parte dos pacientes com esquizofrenia no Estado está sem tratamento regular. O presente estudo, descritivo teve como objetivo mapear a pesquisa em saúde mental no Brasil, fornecendo uma visão de infraestrutura, financiamento e políticas em saúde mental das pesquisas. O estudo faz parte do Projeto Atlas da Organização Mundial da Saúde realizado nos países de média e baixa renda per capita, entre os anos de 1998-2002. A coleta de dados incluiu a avaliação de documentos governamentais e páginas da web, e questionários enviados para os profissionais chave para fornecer informações acerca da infra-estrutura de pesquisa em saúde mental no Brasil. No ano de 2002, o orçamento total para a pesquisa em saúde foi de 101 milhões de dólares, dos quais 3,4 milhões (3,4%) foram aplicados em Pesquisa para Saúde Mental. As principais fontes financiadoras para pesquisa mental foram a Fundação de Amparo à Pesquisa do Estado de São Paulo (Fapesp, 53,2%) e o Ministério da Educação (CAPES, 30,2%). A proporção de doutores é de 1,7 por um mil habitantes, e a taxa de psiquiatras é de 2.7 por 100 mil habitantes, estimadas pelo censo de 2000. Em 2002, havia 53 cursos de pós-graduação direcionados a educação em saúde mental no Brasil (43 em psicologia, seis em psiquiatria, três em psicobiologia e um em enfermagem psiquiátrica, com 1.775 alunos sendo treinados no Brasil e 67 no exterior. Há nove programas dedicados para a psiquiatria, neuropsiquiatria, psicobiologia e saúde mental no País, sete deles localizados na região Sudeste. No período de cinco anos, 186 estudantes receberam o título de doutor (37 por ano) e 637 artigos foram publicados em revistas indexadas no Institute for Scientific Information (ISI). O investimento canalizado para os programas de pós-graduação na formação de recursos humanos, por meio de bolsas de estudos e fomento à pesquisa, tem permitido ao País uma modesta, mas crescente presença na pesquisa em saúde mental no cenário internacional. ; The objective of this descriptive study was to map mental health research in Brazil, providing an overview of infrastructure, financing and policies mental health research. During the five-year period, 186 students got a doctoral degree (37 per year) and 637 articles were published in Institute for Scientic Information (ISI)-indexed journals. The investment channeled towards postgraduate and human resource education programs, by means of grants and other forms of research support, has secured the country a modest but continuous insertion in the international knowledge production in the mental health area.

Resumo: Levantar os tipos mais comuns de punição física utilizados pelos pais para educar os filhos, determinando quais são aceitos socialmente e quais são considerados violência na visão de pais e dos adolescentes, são os principais objetivos deste artigo. Utilizando a metodologia qualitativa, chegou-se aos seguintes resultados: Para os pais, a denúncia é ambígua, podendo proteger a criança ou mesmo piorar sua situação. Para os adolescentes, a violência física é aceita como punição para atos considerados inadequados, embora tenham sido relatados. A violência física doméstica extrapola a dimensão familiar e o contexto local. Nele se entrelaçam padrões culturais de soluções de conflitos, modelos educativos possíveis num contexto de exclusão e marginalidade como também a ausência de políticas sociais específicas para esta população. Palavras chave: violência física, pais, adolescentes. Abstract: The parent's vision and adolescent's vision of Santos's city periphery, on domestic violence. To lift the commonest types of physical punishment used by the parents to educate the children, determining what they are accepted socially and which are considered a violence in the vision of parents and adolescents, those are the main objectives of this article. When the qualitative methodology is used, it approached to the next results: For the parents, the denunciation is ambiguous, being able to protect the child or even to make the situation worse. For the adolescents, the physical violence is accepted like punishment for unsuitable respected acts, though they have been reported. The within the family physical violence oversteps the familiar dimension and the local context. In him there are intertwined cultural standards of solutions of conflicts, possible educative models in a context of exclusion and delinquency as also the absence of social policies special for this population.Key words : physical violence, parents, teenagers .

Background: Mozambique has gradually changed its action on mental health (MH) from an asylum-centric care with long-term hospitalization to an innovative approach to community and primary care. Objective: To collect essential information on Mozambique's MH system for decision making, to improve quality of services delivered, update MH Strategy and Action Plan. Method: The study used the WHO-AIMS to assess MH systems including policy and legislation, organization of services, MH in primary care, human resources, public education and link with other sectors, monitoring and research. A comparative analysis was conducted to present the evolution of relevant data from 2010 to 2014. Results: There are two psychiatric hospitals in the country and beds in general hospitals. In the period, the number of beds in general hospitals remained stable (203), and the beds in psychiatric hospitals increased from 173 to 298. Mental health outpatient facilities have increased from 83 to 152. The number of psychiatrists (9 in 2010, and 10 in 2014) remained very low, with a significant increase in the number of psychologists (56-109) and occupational therapists (2-23). The number of Psychiatric Technicians has increased from 66 in 2010, to 241 in 2014. This increase allowed the mental health network to expand from 60 to 135 Districts, meaning an increase of coverage from 44 to 100% of the country districts. Conclusion: The task-shifting strategy focused on services delivered in primary care by psychiatric technicians, mid-level professionals, allowed the expansion of mental health services for all the country districts and the reduction of treatment gap in Mozambique. ; publishersversion ; published

OBJECTIVE: The World Health Organization Atlas Project identified important deficiencies in world mental and neurological health resources. These deficiencies, especially evident in low and middle-income countries, can be overcome by improving research capacity. The objective of this study is to assess the status of mental and neurological research in Latin American countries and identify the main difficulties encountered in conducting research, publishing results, and shaping health policies, interventions, and programs. METHOD: Semi-structured interviews were conducted with 34 key informants from 13 Latin American countries. RESULTS: Participants reported that production of mental and neurological research in Latin American countries is low. Lack of financial and human resources, including lack of support from government agencies, were identified as the main factors contributing to the dearth of local research. The few research projects that do take place in Latin American countries are often funded at researchers' personal expense. Few policies, interventions, or programs are generated from research results. To address these deficiencies, participants called for training in research methodology, mechanisms for identifying funding opportunities, and greater recognition of their research products. CONCLUSIONS: Researchers and stakeholders recognize the need to mobilize local and international efforts aimed at strengthening research capacity and results implementation. This will lead to an overall optimization of mental and neurological research in the region. ; OBJETIVO: El proyecto Atlas de la Organización Mundial de la Salud identifica importantes deficiencias en salud mental y neurológica. Estas deficiencias, especialmente evidentes en países de medianos y bajos ingresos, pueden resolverse mejorando las capacidades en investigación. El objetivo de este estudio es evaluar el estado de la investigación en salud mental y neurológica en países Latinoamericanos, e identificar las principales dificultades encontradas al hacer investigación, publicar resultados, y generar políticas, intervenciones, y programas. MÉTODO: Entrevistas semi-estructuradas fueron realizadas a 34 informantes de 13 países Latinoamericanos. RESULTADOS: La producción de investigación en salud mental y neurológica en países Latinoamericanos es escasa, debido principalmente a la carencia de recursos financieros y humanos, incluyendo el casi ausente apoyo de agencias gubernamentales. Los pocos proyectos de investigación que se llevan a cabo son financiados mayormente con recursos propios de los investigadores. Pocas políticas, intervenciones o programas son generados a partir de resultados de investigación. Resolver estas deficiencias requerirá entrenar profesionales en metodología de la investigación, identificar oportunidades de financiación y lograr un mayor reconocimiento de los productos de la investigación. CONCLUSIONES: Hay necesidad de movilizar esfuerzos locales e internacionales orientados a fortalecer las capacidades en investigación y la implementación de resultados. Background: Research evidence is essential to inform policies, interventions and programs, and yet research activities in mental and neurological (MN) health have been largely neglected, particularly in low- and middle-income countries. Many challenges have been identified in the production and utilization of research evidence in Latin American countries, and more work is needed to overcome this disadvantageous situation. This study aims to address the situation by identifying initiatives that could improve MN health research activities and implementation of their results in the Latin American region.Methods: Thirty-four MN health actors from 13 Latin American countries were interviewed as part of an initiative by the Global Forum for Health Research and the World Health Organization to explore the status of MN health research in low- and middle-income countries in Africa, Asia and Latin-America.Results: A variety of recommendations to increase MN health research activities and implementation of their results emerged in the interviews. These included increasing skilled human resources in MN health interventions and research, fostering greater participation of stakeholders in the generation of research topics and projects, and engendering the interest of national and international institutions in important MN health issues and research methodologies. in the view of most participants, government agencies should strive to have research results inform the decision-making process in which they are involved. Thus these agencies would play a key role in facilitating and funding research. Participants also pointed to the importance of academic recognition and financial rewards in attracting professionals to primary and translational research in MN health. in addition, they suggested that institutions should create intramural resources to provide researchers with technical support in designing, carrying out and disseminating research, including resources to improve scientific writing skills.Conclusion: Fulfillment of these recommendations would increase research production in MN health in Latin American countries. Mental health problems are common in primary health care, particularly anxiety and depression. This study aims to estimate the prevalence of common mental disorders and their associations with socio-demographic characteristics in primary care in Brazil (Family Health Strategy). It involved a multicenter cross-sectional study with patients from Rio de Janeiro, São Paulo, Fortaleza (Ceará State) and Porto Alegre (Rio Grande do Sul State), assessed using the General Health Questionnaire (GHQ-12) and the Hospital Anxiety and Depression Scale (HAD). The rate of mental disorders in patients from Rio de Janeiro, São Paulo, Fortaleza and Porto Alegre were found to be, respectively, 51.9%, 53.3%, 64.3% and 57.7% with significant differences between Porto Alegre and Fortaleza compared to Rio de Janeiro after adjusting for confounders. Prevalence proportions of mental problems were especially common for females, the unemployed, those with less education and those with lower incomes. In the context of the Brazilian government's moves towards developing primary health care and reorganizing mental health policies it is relevant to consider common mental disorders as a priority alongside other chronic health conditions. ; Los problemas de salud mental son comunes en la atención primaria y están relacionados con la ansiedad y la depresión. Este estudio tiene como objetivo evaluar las tasas de trastornos mentales comunes y sus asociaciones con las características sociodemográficas de los usuarios de la atención primaria. Se trata de un estudio multicéntrico, transversal, con usuarios de Río de Janeiro, São Paulo, Fortaleza y Porto Alegre, Brasil. Teniendo en cuenta los esfuerzos del gobierno brasileño para el desarrollo de la atención primaria y las políticas para la reorganización de la salud mental es importante considerar los trastornos mentales como una prioridad junto a otras afecciones crónicas. ; Problemas de saúde mental são comuns na atenção primária e são geralmente relacionados à ansiedade e à depressão. Este estudo tem o objetivo de avaliar a taxa de transtornos mentais comuns e suas associações com características sociodemográficas em unidades de saúde da família. É um estudo multicêntrico, transversal, com os usuários da atenção primária do Rio de Janeiro, São Paulo, Fortaleza (Ceará) e Porto Alegre (Rio Grande do Sul), Brasil. Utilizou-se o General Health Questionnaire (GHQ-12) e o Hospital Anxiety and Depression Scale (HAD). A taxa de transtornos mentais nos usuários do Rio de Janeiro, São Paulo, Fortaleza e Porto Alegre foram, respectivamente, 51,9%, 53,3%, 64,3% e 57,7%, com diferenças significativas entre Porto Alegre e Fortaleza comparando-se ao Rio de Janeiro. Problemas de saúde mental foram especialmente altos em mulheres, desempregados, em pessoas com baixa escolaridade e com baixa renda.
