A partir del inicio del siglo XXI, y durante su transcurso hasta el momento de la redacción del presente artículo14, el panorama de la Política Exterior peruana se ha desarrollado en una dinámica enfocada en mantener una apertura hacia la comunicación asertiva con los países de la región, procurando generar espacios para la realización de acuerdos comerciales. Asimismo, buscando posicionarse como país líder en la región, en la defensa de la democracia, la lucha contra la pobreza y avanzar hacia el logro de los objetivos de desarrollo sostenible, entre otros tantos lineamientos; evitando que los cambios de gobierno y por ende, quien encabeza la Política Exterior, modifique o desvié el accionar del Ministerio de Relaciones Exteriores, que con distintos matices en cuanto a la designación de los(as) Ministros(as) de dicha cartera, han buscado mantener una línea fija de acción, que genere confianza y seguridad en el accionar peruano. Sin embargo, resulta necesario hacer un análisis sobre cómo ha sido el accionar reciente de la Cancillería de la República (como es conocido también al Ministerio de Relaciones Exteriores), tomando en cuenta la sucesión presidencial (por vacancia realizada por el Congreso de la República) de Pedro Pablo Kuczynski Godard a Martín Alberto Vizcarra Cornejo (Primer Vicepresidente), quien desde los inicios de su periodo el 23 de marzo de 2018, ha designado a dos Ministros de Relaciones Exteriores, que son Néstor Popolizio Bardales (02/04/2018 – 30/09/2019) y Gustavo Meza-Cuadra Velásquez (03/10/2019 – Actualidad), siendo este último quien asume la cartera luego de la disolución del Congreso de la República (30/09/2019) por el presidente Martín Alberto Vizcarra Cornejo. Asimismo, es importante recalcar que actualmente el Tribunal Constitucional del Perú ha aceptado a trámite la demanda Competencial interpuesta por el Señor Pedro Carlos Olaechea Álvarez-Calderón (quien invocó la condición de presidente del Congreso de la República) debido a la disolución del Congreso de la República. En ese orden de ideas se realizará un análisis de las bases legales que rigen la Política Exterior peruana, identificando el accionar de los gobiernos de Alejandro Celestino Toledo Manrique 2001-2006, Alan Gabriel Ludwig García Pérez 2006-2011, Ollanta Moisés Humala Tasso 2011-2016 y Pedro Pablo Kuczynski Godard 2016-2018, mostrando en relieve el pragmatismo que ha regido la Política Exterior peruana. Posterior a ello, se iniciará con el análisis del accionar del primer Ministro de Relaciones Exteriores del gobierno de Martín Vizcarra, hasta el término de sus funciones, identificando si su accionar se ha enmarcado en el Plan Operativo Institucional 2018-2019. Asimismo, se analizará el accionar del segundo Ministro de Relaciones Exteriores, quien, pese al breve periodo de tiempo en el mando, resulta importante identificar si viene siguiendo lo dispuesto en el Plan Operativo Institucional 2019 a fin de dilucidar la existencia o no de algún tipo de desvío de la Política Exterior peruana. Finalmente, se hará una reflexión sobre las acciones realizadas por los Cancilleres con la finalidad de establecer una conclusión en relación con la Política Exterior realizada durante sus periodos de funciones. ; Instituto de Relaciones Internacionales

From the beginning of the 21st century, and during its course until the drafting of this Article 14, the Peruvian foreign policy landscape has developed in a dynamic that focuses on maintaining openness towards assertive communication with the countries of the region, seeking to create spaces for the conclusion of trade agreements. Also seeking to position itself as a leading country in the region, defending democracy, fighting poverty and making progress towards achieving the Sustainable Development Goals, among others; avoiding that changes in government, and thus the leader of foreign policy, change or divert the action of the Ministry of Foreign Affairs, which, with various nuances as regards the designation of the ministers (ministers) of this portfolio, have sought to maintain a fixed line of action, which creates trust and confidence in Peruvian action. However, it is necessary to carry out an analysis of the recent operation of the Chancellery of the Republic (as is also known to the Ministry of Foreign Affairs), taking into account the presidential succession (by vacation made by the Congress of the Republic) from Pedro Pablo Kuczynski Godard to Martín Alberto Vizcarra Cornejo (First Vice-President), who, since the beginning of his period on 23 March 2018, has appointed two Foreign Ministers, Nestestor Popolizio Bardales (02/04/2018-30/09/2019) and Gustavo Meza-Cuadra Velásquez (03/10/2019 — Actuality), the latter takes over the portfolio after the dissolution of the Congress of the Republic (30/09/2019) by President Martín Alberto Vizcarra Cornejo. Epidemiologic and animal data indicate that night shift work might increase the risk for breast cancer. We evaluated the association of night work with different clinical types of breast cancer in a population based case-control study (MCC-Spain study) taking into account chronotype, an individual characteristic that may relate to night shift work adaptation. Lifetime occupational history was assessed by face-to-face interviews and shift work information was available for 1708 breast cancer cases and 1778 population controls from 10 Spanish regions, enrolled from 2008 to 2013. We evaluated three shift work domains, including shift work type (permanent vs rotating), lifetime cumulative duration and frequency. We estimated odds ratios (OR) for night work compared to day work using unconditional logistic regression models adjusting for confounders. Having ever worked permanent or rotating night shift was associated with an increased risk for breast cancer compared to day workers [odds ratio (OR) 1.18; 95 % CI 0.97, 1.43]. Chronotype was differentially associated with breast cancer depending on the duration of night shift work. Risk was higher in women with invasive tumors (OR 1.23; 95 % CI 1.00, 1.51) and for estrogen and progestagen positive tumors among premenopausal women (OR 1.44; 95 % CI 1.05, 1.99). Sleep duration is a novel and potentially modifiable risk factor for cancer. We evaluated the association of self-reported sleep duration and daytime napping with odds of colorectal and gastric cancer. We included 2008 incident colorectal cancer cases, 542 gastric cancer cases and 3622 frequency-matched population controls, recruited in the MCC-Spain case-control study (2008-2013). Sleep information, socio-demographic and lifestyle characteristics were obtained through personal interviews. Multivariable adjusted logistic regression models were used to estimate odds ratios (OR) with 95% confidence intervals (CI) for cancer, across categories of sleep duration (? 5, 6, 7, 8, ? 9 hours/day), daytime napping frequency (naps/week) and duration (minutes/nap). Compared to 7 hours of sleep, long sleep was associated with increased odds of colorectal (OR?9 hours: 1.59; 95%CI 1.30-1.94) and gastric cancer (OR?9 hours: 1.95; 1.37-2.76); short sleep was associated with increased odds of gastric cancer (OR?5 hours: 1.32; 0.93-1.88). Frequent and long daytime naps increased the odds of colorectal (OR6-7 naps/week, ?30 min: 1.32; 1.14-1.54) and gastric cancer (OR6-7 naps/week, ?30 min: 1.56; 1.21-2.02). Effects of short sleep and frequent long naps were stronger among participants with night shift-work history. BACKGROUND: Night shift work, exposure to light at night (ALAN) and circadian disruption may increase the risk of hormone-dependent cancers. OBJECTIVES: We evaluated the association of exposure to ALAN during sleeping time with breast and prostate cancer in a population based multicase-control study (MCC-Spain), among subjects who had never worked at night. We evaluated chronotype, a characteristic that may relate to adaptation to light at night. METHODS: We enrolled 1,219 breast cancer cases, 1,385 female controls, 623 prostate cancer cases, and 879 male controls from 11 Spanish regions in 2008-2013. Indoor ALAN information was obtained through questionnaires. Outdoor ALAN was analyzed using images from the International Space Station (ISS) available for Barcelona and Madrid for 2012-2013, including data of remotely sensed upward light intensity and blue light spectrum information for each geocoded longest residence of each MCC-Spain subject. RESULTS: Among Barcelona and Madrid participants with information on both indoor and outdoor ALAN, exposure to outdoor ALAN in the blue light spectrum was associated with breast cancer [adjusted odds ratio (OR) for highest vs. lowest tertile, OR=1.47; 95% CI: 1.00, 2.17] and prostate cancer (OR=2.05; 95% CI: 1.38, 3.03). In contrast, those exposed to the highest versus lowest intensity of outdoor ALAN were more likely to be controls than cases, particularly for prostate cancer. Compared with those who reported sleeping in total darkness, men who slept in "quite illuminated" bedrooms had a higher risk of prostate cancer (OR=2.79; 95% CI: 1.55, 5.04), whereas women had a slightly lower risk of breast cancer (OR=0.77; 95% CI: 0.39, 1.51). BACKGROUND: Reproductive factors are well known risk factors for breast cancer; however, little is known about how genetic variants in hormonal pathways interact with that relationship. METHODS: One thousand one hundred thirty nine cases of breast cancer in women and 1322 frequency-matched controls were compared. Genetic variants in hormonal pathways (identified in the Kyoto Encyclopedia of Genes and Genomes) were screened according to their relationship with breast cancer using the Cochran-Armitage statistic. Information on reproductive factors was obtained using a face-to-face questionnaire. The interaction among the selected genetic variants and reproductive factors was tested with logistic regression. RESULTS: Concerning C allele in rs2229712, compared to nulliparity in non-carriers the ORs for 1-2 and >?2 deliveries were 0.48 (0.28-0.81) and 0.34 (0.19-0.59), and in C carriers they were 0.92 (0.42-1.98) and 0.71 (0.31-1.61). Similar results were found in women carrying the C allele in rs1269851. Carriers of Allele T in rs35652107 and allele C in rs6018027 had the delivery number effect more pronounced. Prostate, breast and colorectal cancer are the most common tumours in Spain. The aim of the present study was to evaluate the association between adherence to nutrition-based guidelines for cancer prevention and prostate, breast and colorectal cancer, in the MCC-Spain case-control study. A total of 1,718 colorectal, 1,343 breast and 864 prostate cancer cases and 3,431 population-based controls recruited between 2007 and 2012, were included in the present study. The World Cancer Research Fund/American Institute for Cancer Research (WCRC/AICR) score based on six recommendations for cancer prevention (on body fatness, physical activity, foods and drinks that promote weight gain, plant foods, animal foods and alcoholic drinks; score range 0-6) was constructed. We used unconditional logistic regression analysis adjusting for potential confounders. One-point increment in the WCRF/AICR score was associated with 25% (95% CI 19-30%) lower risk of colorectal, and 15% (95% CI 7-22%) lower risk of breast cancer; no association with prostate cancer was detected, except for cases with a Gleason score ≥7 (poorly differentiated/undifferentiated tumours) (OR 0.87, 95% CI 0.76-0.99).
