RESUMO Neste estudo, examinaram-se estratégias adotadas por países com sistemas públicos de saúde que expandiram a oferta de leitos por meio da utilização de hospitais privados na resposta à Covid-19. Utilizou-se estudo de casos selecionados para explorar o contexto institucional em que as medidas foram implementadas, os instrumentos de gestão utilizados e como se caracterizou a ação governamental em oito países: Austrália, Espanha, Irlanda, Itália, Chile, México e Peru, além do Brasil. Esta análise pode auxiliar a identificar mecanismos de gestão de sistema de saúde necessários para a coordenação de ações governamentais para resposta a situações de Emergência em Saúde Pública (ESP), bem como para aperfeiçoar a governança dos sistemas de saúde na relação entre os setores público e privado.
This research analyses changes produced in a university hospital using the Paideia support method. This experience happened during a hospital management fellowship course offered to professors, managers, and professionals who work at administration and health care by the Preventive and Social Medicine Department and the Hospital de Clínicas of Universidade Estadual de Campinas (HC-Unicamp). The results were analyzed using triangulation methods. Changes in management, health care practices and hospital relationship with the care network were observed. We infer that the changes were influenced by both the project and the activities of the course. We conclude that this method empowered and widened the changes observed due to the political and pedagogical approach based on the creation of dialogical networks in groups, using a dialectic method on the praxis, with a theoretical-conceptual support introduced by the external tutors. However, it was possible to identify limits to the method application in hospital mainly due to political, ideological, cultural and organizational aspects. ; Esta investigación analiza los cambios que se produjeron en un hospital universitario, en el que se utilizó como referencial el método de soporte Paideia. La experiencia se realizó en el marco de un curso de especialización en gestión hospitalaria ofrecido por el Departamento de Medicina Preventiva y Social y por la dirección del Hospital de Clínicas de la Universidade Estadual de Campinas (HC-Unicamp) a personal de las áreas de docencia, asistencia y administración, y a coordinadores de servicios del hospital. Los resultados fueron analizados por triangulación de técnicas. Se observaron cambios en las prácticas de gestión, de atención y en la relación con el Sistema Único de Salud (SUS), especialmente en el primer aspecto. Se infiere que los cambios fueron influenciados tanto por el proyecto de la dirección del hospital, como por las actividades del curso. Se concluye que el método potenció y amplió el espectro de cambios en el hospital, en función de la perspectiva política y pedagógica de crear redes dialógicas en colectivos organizados, haciendo uso de un abordaje metodológico dialéctico problematizador de la praxis, con ofertas teórico-conceptuales aportadas por el soporte externo. Sin embargo, fue posible identificar límites en la aplicación del método en el hospital, originados, principalmente, en aspectos políticos, ideológicos, culturales y organizacionales.
The year 2018 is an opportune time to explore health system reforms and primary health care (PHC) in Brazil, given the anniversaries of the Alma-Ata Declaration (40 years) and of the Constitution of Brazil (30 years), the basis of the Unified Health System (SUS). In this context, health system analysis in the municipal setting is an important instrument for acknowledging achievements and innovations, as well as weaknesses and threats. Due to the principle of decentralization of SUS, municipalities have assumed a leadership role in health policy development and implementation. The cities also come first in expressing the failures of the health system and the consequences of austerity measures. Thus, analysis of health system transformations at the municipal level are fundamental to studying PHC achievements and gaps. This report identifies the challenges and innovations of PHC implementation in Curitiba, beginning with a brief history of the city's health system development. The city was a pioneer in linking urban planning with health system design, improving access to health care, and obtaining better health outcomes over the past 30 years. This report covers those years, as well as the challenges and strategies implemented during the most recent political cycle (2013 – 2016). There are substantial lessons that can be garnered from the experience of this middle-income city in Latin America, lessons that may be useful as the region moves toward the Sustainable Development Goal of Universal Health Coverage by 2030.
Many countries employ strategies that rest on the use of an explicitly defined set of criteria to identify underserved communities. Yet, we know relatively little about the performance of community-level targeting in large-scale health programmes. To address this gap, we examine the performance of community targeting in the More Doctors Programme (MDP). Our analysis covers all 5570 municipalities in the period between 2013 and 2017 using publicly available data. We first calculate the rate at which vulnerable municipalities enrolled in the MDP. Next, we consider two types of mistargeting: (1) proportion of vulnerable municipalities that did not have any MDP physicians (i.e. under-coverage municipalities) and (2) proportion of MDP enrolees that did not fit the vulnerability criteria (i.e. non-target municipalities). We found that almost 70% of vulnerable municipalities received at least one MDP physician between 2013 and 2017; whereas non-target municipalities constituted 33% of beneficiaries. Targeting performance improved over time. Non-target municipalities had the highest levels of socioeconomic development and greater physician availability. The poverty rate among under-coverage municipalities was almost six times that in non-target municipalities. Under-coverage municipalities had the lowest primary care physician availability. They were also smaller and more sparsely populated. We also found small differences in the political party alignments of mayors and the President between under-coverage and non-target municipalities. Our results suggest that using community-level targeting approaches in large-scale health programmes is a complex process. Programmes using these approaches may face substantial challenges in beneficiary targeting. Our results highlight that policymakers who consider using these approaches should carefully study various municipal characteristics that may influence the implementation process, including the level of socioeconomic development, health supply factors, population ...

Assim, a análise das transformações do sistema de saúde no nível municipal é fundamental para o estudo das conquistas e lacunas da APS. Este relatório identifica os desafios e inovações da implementação da APS em Curitiba, começando com uma breve história do desenvolvimento do sistema de saúde da cidade. A cidade foi pioneira em vincular o planejamento urbano com desenho dos sistemas de saúde, melhorando o acesso aos serviços de saúde e obtendo melhores resultados de saúde nos últimos 30 anos. Este relatório abrange esses anos, assim como os desafios e estratégias implementados durante o ciclo político mais recente (2013-2016). RESUMO A pandemia provocada pela Covid-19 deu relevância à resiliência dos sistemas de saúde. Neste artigo, buscou-se explorar elementos que subsidiem uma agenda de pesquisa sobre resiliência para o Sistema Único de Saúde (SUS). A partir de revisão de escopo, analisou-se o desenvolvimento conceitual e metodológico da resiliência aplicada à pesquisa sobre sistemas de saúde em nível internacional e nacional, identificando quem são os grupos formuladores e o que propõem como modelos de análise. Em seguida, apresentou-se uma proposta de modelo de análise de resiliência adaptada às características do sistema de saúde brasileiro. O modelo embasou o apontamento de questões-chave a serem investigadas em pesquisas sobre a resiliência do SUS, a partir de quatro dimensões: governança e liderança, financiamento, recursos (força de trabalho, infraestrutura, medicamentos e tecnologias) e prestação de serviços. Ao final, discutem-se oportunidades e desafios para implementação de uma agenda de pesquisas de resiliência para o SUS.
ABSTRACT The COVID-19 pandemic highlighted the resilience of health systems. In this paper, we seek to explore elements to support a research agenda on resilience for the Unified Health System (SUS). First, based on a scoping review, we analyzed the conceptual and methodological development of resilience applied to health systems research both at international and national levels, identifying who the formulating groups are and what they propose as analytical frameworks. Then, we propose an analytical framework adapted for the Brazilian health system features. The framework underpinned the pointing out of critical issues to be investigated in research on SUS resilience, based on four dimensions: governance and leadership, financing, resources (workforce, infrastructure, medicines, and technologies), and service provision. Finally, we discuss opportunities and challenges for implementing a research agenda on resilience for the SUS.
This article discusses the resilience of the Brazilian National Health System (SUS) in the face of the COVID-19 pandemic. The analytical categories proposed by the World Health Organization were adopted to analyze the health system's: (i) leadership and governance, (ii) financing, (iii) strategic health products, (iv) workforce, (v) health Information, and (vi) provision of services. To explore the provision of services and adapt it to the Brazilian context, we subdivided the corresponding block into five sub-blocks: (i) public health and surveillance actions, (ii) primary health care; (iii) specialized and hospital care, (iv) urgency and emergency, and (v) digital health. We conducted a non-systematic consultation of scientific literature and official documents published by Brazilian government agencies and international health organizations to obtain data related to the organization of health systems in the face of public health emergencies. ; En este artículo, discutimos la resiliencia del Sistema Único de Salud (SUS) ante la pandemia de COVID-19. Para el estudio utilizamos las categorías analíticas propuestas por la Organización Mundial de la Salud (OMS) para análisis de sistemas de salud: (i) liderazgo y gobernanza, (ii) financiación, (iii) productos estratégicos para la salud, (iv) fuerza de trabajo, (v) información en salud y (vi) prestación de servicios. Para explorar la prestación de servicios y adaptarla al contexto brasileño, subdividimos el bloque correspondiente en 5 subbloques: (i) acciones de salud pública y vigilancia, (ii) atención primaria en salud; (iii) atención especializada y hospitalaria, (iv) urgencia y emergencia, y (v) salud digital. The Unified Health System (Sistema Único de Saúde (SUS)) has enabled substantial progress towards Universal Health Coverage (UHC) in Brazil. However, structural weakness, economic and political crises and austerity policies that have capped public expenditure growth are threatening its sustainability and outcomes. This paper analyses the Brazilian health system progress since 2000 and the current and potential effects of the coalescing economic and political crises and the subsequent austerity policies. We use literature review, policy analysis and secondary data from governmental sources in 2000–2017 to examine changes in political and economic context, health financing, health resources and healthcare service coverage in SUS. We find that, despite a favourable context, which enabled expansion of UHC from 2003 to 2014, structural problems persist in SUS, including gaps in organisation and governance, low public funding and suboptimal resource allocation. Consequently, large regional disparities exist in access to healthcare services and health outcomes, with poorer regions and lower socioeconomic population groups disadvantaged the most. These structural problems and disparities will likely worsen with the austerity measures introduced by the current government, and risk reversing the achievements of SUS in improving population health outcomes. The speed at which adverse effects of the current and political crises are manifested in the Brazilian health system underscores the importance of enhancing health system resilience to counteract external shocks (such as economic and political crises) and internal shocks (such as sector-specific austerity policies and rapid ageing leading to rise in disease burden) to protect hard-achieved progress towards UHC.
RESUMO Este é um estudo observacional, exploratório, que utilizou metodologia qualitativa, com apoio de dados quantitativos, para analisar a resposta do Sistema Único de Saúde (SUS) à Covid-19 em três municípios brasileiros. Utilizaram-se entrevistas semiestruturadas para escuta e diálogo com gestores das cidades analisadas, que foram orientadas por roteiro para explorar a percepção dos gestores durante o processo de resposta à pandemia, compreender as motivações que orientaram suas escolhas estraté- gicas e visualizar as fragilidades e potencialidades do sistema municipal em uma emergência de saúde pública. Realizou-se análise qualitativa considerando pontos-chave da resposta do SUS à pandemia, entre eles, coordenação e governança, vigilância e prevenção, e rede de serviços de saúde. Nos resultados, apresentam-se e discutem-se as principais características dos municípios, o curso da pandemia em 2020, ações de resposta adotadas; e exibe-se uma análise do padrão de resposta dos gestores do SUS municipal na pandemia.
ABSTRACT This qualitative, observational, and exploratory study was supported by quantitative data to analyze the Brazilian Unified Health System (SUS) response to Covid-19 in three Brazilian municipalities. We used semi-structured interviews to listen to and dialogue with managers of the cities analyzed, guided by a roadmap to explore the managers' perception during the response to the pandemic, understand the motivations that guided their strategic choices, and visualize the weaknesses and potentials of the municipal system in a public health emergency. We conducted a qualitative analysis considering the critical points of the SUS response to the pandemic, including coordination and governance, surveillance and prevention, and the health services network. In the results, we present and discuss the main characteristics of the municipalities, the 2020 pandemic course, the response actions adopted, and submit an analysis of the response pattern of municipal SUS managers in the pandemic.
