Hintergrund
Gesundheitsförderung und Prävention leisten wichtige Beiträge zur Vorbeugung von Krankheiten und Linderung von Krankheitsverläufen. Die Beratung von Patientinnen und Patienten zur Lebensführung erfordert Zeit, die in der medizinischen Routineversorgung häufig nicht ausreichend zur Verfügung steht. Einen möglichen Ansatzpunkt, um Maßnahmen der Prävention und Gesundheitsförderung stärker in die Routineversorgung zu integrieren, stellt die Einbindung eines Gesundheitscoaches dar. Die vorliegende qualitative Studie hatte zum Ziel, das Anforderungsprofil, den Bedarf und die Potenziale dieses noch nicht etablierten Berufsbilds aus ärztlicher Sicht zu untersuchen.

Methode
Mithilfe von halbstrukturierten, leitfadengestützten Interviews wurden 19 Ärztinnen und Ärzte aus dem stationären und ambulanten Setting zum möglichen Berufsbild des Gesundheitscoachs befragt. Die Interviews wurden aufgenommen, transkribiert und mittels qualitativer Inhaltsanalyse nach Mayring ausgewertet.

Ergebnisse
Die befragten Ärztinnen und Ärzte sprachen sich deutlich für den Einbezug von Gesundheitscoaches in die Routineversorgung von Patientinnen und Patienten aus. An das Berufsbild wurden verschiedene fachliche, persönliche, soziale, methodische und formale Anforderungen gestellt, die in einer akademisch fundierten Ausbildung vermittelt werden sollten. Die Potenziale eines Gesundheitscoachs wurden auf der Ebene der Patientinnen und Patienten, des Gesundheitssystems, der Kliniken und Praxen, der Ärztinnen und Ärzte und der zukünftigen Gesundheitscoaches selbst gesehen und reichten von besserer Gesundheitsförderung und verminderten Kosten bis hin zur Entlastung des ärztlichen Personals.

Schlussfolgerung
Der aus Sicht der Befragten vorhandene Bedarf an Gesundheitscoaches und die ermittelten Potenziale unterstreichen die Bedeutung der Integration dieses neuen Berufsbilds in die ärztliche Versorgung. Vor der Umsetzung sind weitere Studien notwendig, z. B. hinsichtlich der Finanzierbarkeit und des tatsächlichen Nutzens eines Gesundheitscoaches für Patientinnen und Patienten und für das Gesundheitssystem.

Since the early Gestaltists there has been a strong interest in the question of how problem solvers get stuck in a mental impasse. A key idea is that the repeated activation of a successful strategy from the past results in a mental set ('Einstellung') which determines and constrains the option space to solve a problem. We propose that this phenomenon, which mostly was tested by fairly restricted experiments in the lab, could also be applied to more complex problem constellations and naturalistic decision making. We aim at scrutinizing and reconstructing how a mental set determines the misinterpretation of facts in the field of political decision making and leads in consequence to wrong expectations and an ill-defined problem representation. We will exemplify this psychological mechanism considering a historical example, namely the unexpected stabilization of the Franco regime at the end of World War II and its survival thereafter. A specific focus will be drawn to the significant observation that erroneous expectations were taken as the basis for decisions. This is congruent with the notion that in case of discrepancy between preconceived notions and new information, the former prevails over the new findings. Based on these findings, we suggest a theoretical model for expectation violation in political decision making and develop novel approaches for cognitive empirical research on the mechanisms of expectation violation and its maintenance in political decision making processes.

OBJECTIVE: Approximately 55–76% of Service members use dietary supplements for various reasons, including pain and related outcomes. This work evaluates current research on dietary ingredients for chronic musculoskeletal pain to inform decisions for practice and self-care, specifically for Special Operations Forces personnel. METHODS: A steering committee convened to develop research questions and factors required for decision-making. Key databases were searched through August 2016. Eligible systematic reviews and randomized controlled trials were assessed for methodological quality. Meta-analysis was applied where feasible. GRADE was used to determine confidence in the effect estimates. The committee made evidence-informed judgments and recommendations for practice and self-care use. RESULTS: Nineteen eligible dietary ingredients were assessed for quality, efficacy, and safety. Avocado soybean unsaponifiables, capsaicin, curcuma, ginger (as a food source), glucosamine, melatonin, polyunsaturated fatty acids, and vitamin D were conditionally recommended as their benefits outweighed risks, but there was still some uncertainty about the trade-offs. No recommendations were made for boswellia, ginger (as a dietary supplement), rose hip, or s-adenosyl-L-methionine. Recommendations were made against the use of collagen, creatine, devil's claw, l-carnitine, methylsulfonylmethane, pycnogenol, willow bark extract, and vitamin E. Research priorities were developed to address gaps precluding stronger recommendations. CONCLUSIONS: Currently the scientific evidence is insufficiently robust to establish definitive clinical practice guidelines, but processes could be established to track the impact of these ingredients. Until then, providers have the evidence needed to make informed decisions about the safe use of these dietary ingredients, and future research can address existing gaps.
