Through qualitative analysis of materials from ethnographic observations and governance documents, and along an analytic framework of infrastructure, this paper examines the ambivalences of care in and of Global Health Security. Global Health Security's occupation with preparing health systems for an appropriate emergency response is accompanied by the problem of allocating responsibility for this preparedness capacity buildup. The paper argues that a universalist narrative of globally shared vulnerability to infectious disease threats drives Global Health Security as a global governance programme. It is shown how this narrative securitizes existing vulnerabilities in health infrastructures and how Global Health Security thereby functions as a reflexivization of former infrastructural adjustment programmes, which co-constituted these vulnerabilites in the first place. Against the backdrop of the problematic emergency response to the Ebola outbreak in the Democratic Republic of Congo, the concept of response-ability – developed in neo-materialist and posthumanist feminism – helps to contour the ambivalences of Global Health Security's care. While certain infrastructural vulnerabilities and provisional needs are being addressed, the caring security employed in Global Health fails to respond to other, obvious infrastructural vulnerabilities.

Der politische Umgang mit COVID-19 ist durch ein Paradox gekennzeichnet: Zwar gilt Preparedness seit gut 20 Jahren als das zentrale Paradigma im Bereich der Gesundheitssicherheit, dennoch scheint uns die Pandemie "unvorbereitet" zu treffen. Der vorliegende Beitrag setzt an diesem Paradox an, um zu untersuchen, was sich im Licht der aktuellen Krise über die Funktionsweise der Sicherheitsrationalität des "Vorbereitet-Seins" aussagen lässt - über ihre Mechanismen, Versprechen und Bruchstellen. Wir zeigen, dass Preparedness auf dem Einsatz szenario-basierter Wissenstechniken beruht, anhand derer in der Vergangenheit Ereignisse imaginiert wurden, die dem COVID-19-Ausbruch durchaus ähneln. Zudem haben sich zuletzt evaluative Verfahren zur Durchsetzung von Preparedness etabliert. In beiden Fällen wurde insbesondere die Belastbarkeit der Gesundheitsinfrastrukturen als wesentliches Sicherheitsproblem angesehen. Dieser Infrastrukturalismus einer Biopolitik vitaler Systeme kennzeichnet auch die Maßnahmen zur Bewältigung der COVID-19-Pandemie. Wir arbeiten die Grenzen der Preparedness-Politik an zwei zentralen Aspekten des Ausbruchsmanagements in Deutschland heraus: der Strategie der "Abflachung der Kurve" sowie dem Problem der mangelnden Bevorratung mit Schutzmasken. In beiden Fällen wird deutlich, dass die Sicherheitspraktik der Preparedness anfällig ist für eine Überformung durch andere Systemlogiken, dass die Vorbereitung während des SARS-CoV-2 Ausbruchs mittels Improvisation fortgesetzt werden muss und als technokratisches Projekt der Regierung über den Notfall andere Versorgungsstrategien nicht ersetzen kann.
