Chronischer Khatkonsum und psychotische Störungen: ein Literaturüberblick und Ausblick <span class="fett">Fragestellung:</span> Khatblätter, die das amphetaminähnliche Kathinon enthalten, werden traditionell in afrikanischen und arabischen Ländern gekaut. In den letzten Jahrzehnten veränderte sich das Konsummuster tiefgreifend. Darauf Bezug nehmend wird hier ein Überblick über den Zusammenhang von Khatkonsum und Psychosen gegeben. </p><p> <span class="fett">Ergebnisse:</span> Mehrere Falldarstellungen aber nur wenige Gruppen- und Populationsstudien behandeln das Thema. Sie sind insgesamt inkonsistent, haben methodische Probleme, berichten oft nur qualitative Ergebnisse und behandeln wichtige Fragen nicht. </p><p> <span class="fett">Schlussfolgerungen:</span> Obwohl Khatkonsum häufig mit Psychosen in Zusammenhang gebracht wird, gibt es nur wenige fundierte Studien. Gesicherte Erkenntnisse sind z. B., dass exzessiver Khatgenuss zu kurzdauernden Psychosen führen kann und dass vulnerablen Personen vom Khatkonsum abgeraten werden sollte. Unklar bleibt die Richtung des Zusammenhangs zwischen Khatkonsum und unspezifischer Psychopathologie. Quantitative, kontrollierte Längsschnittstudien sind erforderlich, um psychiatrische Konsequenzen zu verstehen, und inwieweit Khat mit der Entwicklung von langdauernden Psychosen und ihrem Verlauf zusammenhängt.

Dropout rates from detoxification treatment are high. We tested whether high trauma event load was related to a higher dropout from alcohol detoxification. Furthermore, we studied the feasibility and effects of a short psychoeducational tool to increase retention among traumatized alcohol in-patients. Retention and treatment length were compared between treatment as usual (TAU) and standard therapy plus a psychoeducational group intervention on alcohol drinking related to stress and trauma (PAST). Patients with high trauma load were identified with the Trauma History Questionnaire. Of the 159 in-patients treated during the study period, 66 were included in the analysis: 33 in TAU and 33 in PAST. Sociodemographic characteristics did not differ between the groups. During TAU, patients with high trauma load tended to drop out more often (p = 0.056). Among patients with high trauma load, retention level increased from 29 to 80% (p = 0.006), and among those with low trauma load from 63 to 83% (p = 0.250). Treatment length only tendentially improved among patients with lower burden (p = 0.056). The pilot study supports the idea that detoxification treatment dropout occurs more often among alcohol patients with high trauma load and that their retention can be increased by a psychoeducational group intervention.

Theoretischer Hintergrund: Obwohl Prüfungsangst ein häufiges Anliegen in psychologischen Beratungsstellen ist, wissen wir wenig über ihren zeitlichen Verlauf sowie über Risikofaktoren für hohe Prüfungsangst kurz vor den Prüfungen. Fragestellung: Diese Studie untersucht, ob sich die Intensität von Prüfungsangst während eines Semesters verändert und wie Personen mit hoher Belastung kurz vor der Prüfung früh identifiziert werden können. Methodik: Zu Beginn und kurz vor den Prüfungen des Wintersemesters 2014/15 wurden Prüfungsangst, Depressivität und Prokrastination bei 427 Studierenden (88.3 % Erstsemester; 68.4 % weiblich; Altersdurchschnitt 20.0 Jahre) erfasst. Ergebnisse: Die Analyse auf Einzelfallebene zeigte, dass sich die Prüfungsangst bei den meisten Studierenden nicht signifikant veränderte. Bei der Vorhersage der Prüfungsangst zum Semesterende stellten Prüfungsangst und Depressivität zu Semesterbeginn signifikante Prädiktoren dar. Diese wurden anhand von 80 % der Gesamtstichprobe ermittelt und an den anderen 20 % validiert. Schlussfolgerungen: Erhöhte Prüfungsangst und Depressivität zu Semesterbeginn können die frühe Identifikation von Studierenden mit bedeutsamer Prüfungsangst kurz vor Prüfungen ermöglichen.

<b><i>Introduction:</i></b> Alcohol craving is a key symptom of alcohol use disorder (AUD) and a significant cause of poor treatment outcome and frequent relapse. Craving is supposed to impair executive functions by modulating reward salience and decision-making. <b><i>Objective:</i></b> The present study sought to clarify this modulation by scrutinizing reward feedback processing in an experimental decision-making task, which was accomplished by AUD patients in 2 conditions, in the context of induced alcohol craving and in neutral context. <b><i>Methods:</i></b> AUD inpatients (<i>N</i> = 40) accomplished the Balloon Analog Risk Task, while their EEG was monitored; counterbalanced across conditions, the tasks were preceded either by craving induction by means of imagery and olfactory alcohol cues, or by neutral cues. Decision choice and variability, and event-related potentials (ERPs) prior to (stimulus-preceding negativity [SPN]) and following (P2a) reward feedback upon decisions, and the outcome-related feedback-related negativity (FRN) were compared between conditions and between patients, who experienced high craving upon alcohol cues (<i>N</i> = 18) and those who did not (<i>N</i> = 22). <b><i>Results:</i></b> Upon craving induction (vs. neutral condition), high-craving AUD patients showed less adjustment of decision choice to preceding reward experience and more variable decisions than low-craving AUD patients, together with accentuated reward-associated ERP (SPN and P2a), while outcome-related FRN was not modified by craving. <b><i>Conclusions:</i></b> Results support orientation to reward in AUD patients, particularly amplified upon experienced craving, which may interfere with (feedback-guided) decision-making even in alcohol-unrelated context. Craving-accentuated ERP indices suggest neuroadaptive changes of cognitive-motivational states upon chronic alcohol abuse. Together with altered reward-related expectancies, this has to be considered in intervention and relapse prevention.

Zusammenfassung. Zielsetzung: Alkoholabhängige Personen (AP) zeigen riskanten Alkoholkonsum trotz des Wissens um dessen objektive Risiken. Um die Bedeutung des Konstrukts Risikowahrnehmung für diese Diskrepanz zu überprüfen, wurde die "accuracy" Hypothese und der "optimistic bias" bei Alkoholabhängigkeit untersucht. Methodik: Zur Hypothesenprüfung wurden vier Dimensionen alkoholbezogener Risikowahrnehmung bei 73 stationär behandelten AP und 75 Kontrollpersonen (KP) anhand eines neu entwickelten deutschsprachigen Fragebogens verglichen und mit Alkoholkonsum in Beziehung gesetzt. Ergebnisse: AP schätzten im Vergleich zu KP eigene Risiken als höher ein und bewerteten diese als bedrohlicher. Trotz realistischer Wahrnehmung tendierten sie zu optimistischer Unterschätzung der eigenen Risiken. Wahrgenommene Vulnerabilität und Peer Vulnerabilität klärten neben der Diagnose Varianz im Konsum (AP: vor Behandlungsbeginn) auf. Der positive Zusammenhang zwischen Konsumverhalten und affektiver Bewertung von Risiken zeigte sich nur bei AP. Schlussfolgerungen: Die "accuracy" Hypothese wurde bestätigt und konnte die realistische Wahrnehmung der Alkoholrisiken bei AP erklären. Gleichzeitig zeigten AP einen "optimistic bias", welcher zu fortdauernd riskantem Trinken beitragen könnte. Das Konstrukt Risikowahrnehmung bietet Hypothesen für längsschnittliche Therapiestudien an, durch die weiterer Erkenntnisgewinn zu erwarten ist.

<b><i>Aims:</i></b> Posttraumatic stress disorder (PTSD) is a significant comorbidity in substance use disorders (SUDs). While most studies have addressed trauma/PTSD in abstinent patients, little is known about trauma/PTSD in early detoxification treatment. The current study therefore addresses the systematic evaluation of trauma/PTSD in early inpatient detoxification. <b><i>Methods:</i></b> A cross-sectional survey was accomplished in three German-speaking clinics (<i>n</i> = 134) specialized in inpatient detoxification and motivation treatment. All measures are based on self-report using trauma-specific questionnaires and measures for general psychopathological burden. <b><i>Results:</i></b> Participation rate was 60.1% and patients did not show clinically obvious psychological distress during or after assessment. DSM-IV traumatic events were reported by 66.4%. Of the total sample, 38.1% screened positive for PTSD, and 14.9% screened positive for subsyndromal PTSD. PTSD patients reported significantly more childhood adversities and significantly higher scores in depression and ­general psychopathology compared to subsyndromal PTSD and SUD-only patients. <b><i>Conclusions:</i></b> Early and systematic evaluation of PTSD in SUD inpatient detoxification treatment is largely safe and yields important information for individual treatment. The high PTSD-rate and the high symptom load in SUD patients during inpatient detoxification treatment highlight the need for a more stringent address of trauma/PTSD in early SUD treatment.
