The coronavirus disease (COVID-19) pandemic has complicated the work of general physicians due to the vagueness of fever standards. Because of the high virulence of the causative virus, the disease is rapidly spreading worldwide, even reaching the rural areas in Japan. During the first wave of the pandemic, the Japanese government set the standard as a temperature of 37.5°C lasting for more than four days. However, after the government deleted this standard from its website, there was confusion in the medical institutions, as they struggled to set their fever standards. As a result, social fear might force healthcare professionals to bring down the standard for fever to increase the sensitivity and identify patients with COVID-19 more accurately. During the second wave, the fever standard was set at 37°C to enable high sensitivity. Subsequently, general physicians had to approach many people with temperatures higher than 37°C who came to the hospital seeking treatment for their symptoms or to visit their families. People seek to avoid contracting the COVID-19 infection for health-related and social reasons. When there is a possibility of healthcare professionals testing positive, hospitals may have to shut down and face criticism from society, as the mass media generally accuses these hospitals of mismanagement.

Coronavirus disease 2019 (COVID-19) has caused worldwide panic, and rural areas are no exception. In Japanese rural areas, many older people live alone and lack access to reliable sources of information. During the pandemic, older adults were initially isolated from their communities because of the recommended social isolation measures, even when there were no cases in rural communities. However, various formal and informal caregivers went beyond their usual roles and tried to reconnect the older rural population with their communities and nurtured their social connections; Japanese community workers mitigated the stress and fear experienced by the rural elderly in the COVID-19 pandemic. Furthermore, this pandemic encouraged rural Japanese customs. One such custom is "Osekkai." The Japanese word Osekkai describes actions that someone considers useful and meaningful to perform for others. Osekkai involves both formal and informal care, and as social gatherings began to disappear, Osekkai allowed individuals to deal with the various social problems created by the pandemic. Conferences based on Osekkai can strengthen rural people's connections and improve their social capital. Activities of rural people that are constructed through Osekkai conferences are not only evidence-based but also based on reliance. This unprecedented pandemic has taught us not only the importance of usual healthcare and precautions against infection but also that nurturing social connection in communities is crucial in the face of social turbulence.

Coronavirus disease 2019 (COVID-19) has affected people's social lives by inhibiting their movement; this seriously impacts the lives of older people in particular. Rural older people may have been particularly affected because they live dispersedly and in isolation. This study explored rural older people's perceptions of how COVID-19 has impacted their social lives. This qualitative study assessed participants who were 65 years and older and residing in rural Japanese communities. Five focus group discussions were conducted with 53 participants to explore their perceptions and challenges during COVID-19. Data were analyzed using thematic analysis, and four themes were developed: the beginning of suffering, social cognitive suppression, reflection on rural contexts, and critical approaches to the pandemic based on rural standards. The daily activities of rural older people were suppressed due to social norms and pandemic-related standard precautionary measures based on urban areas. Specific infection control standards for rural areas and the provision of direct information to individuals in the community to sustain social support are needed. To effectively maintain rural social support, as well as the trust and accountability of rural citizens, constant dialog among local governments and rural citizens is required.

Community organizing with government support, termed local self-governance (LSG), is a form of policy decentralization for community wellbeing through solutions tailored to local issues. One form of LSG is multifunctional autonomy, in which citizens can comprehensively manage their communities with government support. This study clarified the effect of multifunctional autonomy on healthy life expectancy by assessing related advantages and challenges in rural Japanese communities, using a mixed-methods approach. Disability-free life expectancy from 65 years (DFLE-65) was assessed to compare healthy life expectancies between two rural Japanese cities (with/without multifunctional autonomy). Comparisons revealed better DFLE-65 only among older men in a city with multifunctional autonomy. A cost-effectiveness analysis investigated the relationship between the budget and DFLE-65 change using questionnaire data. Cost-effectiveness analysis of multifunctional autonomy indicated 61,147 yen/DFLE-65. Thematic analysis revealed that multifunctional autonomy created new roles for older men, improving community relationships. However, sustainable multifunctional autonomy in LSG communities may be hindered by a generally aging society, generation gap, and lack of mutual understanding between rural communities and local governments. To ensure the sustainability of multifunctional autonomy, collaborations between local communities and governments and among various generations are critical.

Homecare nurses manage patients with extreme homecare dependence through interprofessional collaboration. The quality of the collaboration depends on situations, and the difficulties of homecare nurses are complicated in rural settings because of a few healthcare resources. This study determined rural homecare nurses' difficulties during interprofessional collaboration in providing seamless patient care. Focus groups, followed by one-on-one interviews, were conducted with 13 rural homecare nurses working in rural Japan. Using thematic analysis, four themes were extracted: collaboration with physicians, the collaboration with the government, the collaboration with care workers, and the collaboration among hospital nurses. Rural homecare nurses have difficulties in their working relationships with other professionals, with vague definitions of each professional's roles and responsibilities, and with information-sharing. Interprofessional education and information-sharing should respect rural professional and cultural backgrounds. Respect can accomplish mutual understanding among professional care, leading to seamless patient care in rural home care.

BACKGROUND: Rural-urban health care disparities are an important topic in health services research. Hence, developing valid and reliable tools to measure rurality is needed to support high quality research. However, Japan, has no index to measure rurality for health care research. In this study, we conducted a systematic scoping review to identify the important factors and methodological approaches to consider in a rurality index to inform the development of a rurality index for Japan. METHODS: For our review, we searched six bibliographic databases (MEDLINE, PubMed, CINAHIL, ERIC, Web of Science and the Grey Literature Report) and official websites of national governments such as Government and Legislative Libraries Online Publications Portal (GALLOP), from 1 January 1989 to 31 December 2018. We extracted relevant variables used in the development of rurality indices, the formulas used to calculate indices, and any measures for reliability and validity of these indices. RESULTS: We identified 17 rurality indices from 7 countries. These indices were primarily developed to assess access to health care or to determine eligibility for incentives for health care providers. Frequently used factors in these indices included population size/density and travel distance/time to emergency care or referral centre. Many indices did not report reliability or validity measures. CONCLUSIONS: While the concept of rurality and concerns about barriers to access to care for rural residents is shared by many countries, the operationalization of rurality is highly context-specific, with few universal measures or approaches to constructing a rurality index. The results will be helpful in the development of a rurality index in Japan and in other countries.
