In a globalized world health and illness know no frontiers. Pandemics have never been limited to political borders and the contemporary campaigns to prevent them can be effective only when addressed not only internationally but also with the application of integrated disease management in order to respond to problems caused by the silo approach. In any case, it appears that global health has been constantly in redefinition. With this commentary a phenomenological redefinition of global health is proposed as an integrative strategy. Phenomenology prioritizes and investigates from the first-person point of view how the human being experiences the world, as it explores the unique meaning of the lived experience of being human. We are particularly interested in verifying if and how, from a first-person point of view, the lived experience of mental illness and of recovery can contribute to a more integrated definition of global health. In the field of mental health, formal peer support is a mentor/mentee relationship, and as such it is an emotional and practical support between two people who share a common experience of a mental health challenge or illness. Peer support is a system of giving and receiving help founded on key principles of respect, shared responsibility, and mutual agreement of what is helpful. It is about understanding another's situation empathically through the shared experience of emotional and psychological pain. And when speaking in public, a recovery mentor accepts to disclose and to be recognized as a (former) mental health service user. That person knows that there is a possibility of being stigmatized, but yet remains courageously engaged towards the promotion of change and in solidarity with people who are suffering worldwide and who may not have this opportunity to speak freely.

Over the course of the centuries the meanings around mental illness have shifted many times according to societal beliefs and the political atmosphere of the day. The way madness is defined has far reaching effects on those who have a mental disorder, and determines how they are treated by the professionals responsible for their care, and the society of which they are a part. Although madness as mental illness seems to be the dominant Western view of madness, it is by no means the only view of what it means to be 'mad'. The symptoms of madness or mental illness occur in all cultures of the world, but have different meanings in different social and cultural contexts. Evidence suggests that meanings of mental illness have a significant impact on subjective experience; the idioms used in the expression thereof, indigenous treatments, and subsequent outcomes. Thus, the societal understandings of madness are central to the problem of mental illness and those with the lived experience can lead the process of reconstructing this meaning

La conjonction des concepts de rétablissement et de psychiatrie citoyenne débouche sur une approche centrée sur la pleine citoyenneté qui remet en question l'architecture des ordres de participation et de pouvoir dans la société. Pour y arriver, il est indispensable de remettre en question certaines constructions idéologiques réductionnistes. C'est à cette condition qu'un système de santé mentale peut évoluer vers une redistribution plus équitable des pouvoirs avec l'ensemble des acteurs pouvant se retrouver au sein d'un espace de discussion où les personnes concernées par une problématique de santé mentale ont une place identifiée et reconnue. Ainsi, cet article introduit pour la première fois la notion de « rétablissement civique », une notion qui a émergé d'une démarche de recherche intégrant des personnes en rétablissement comme partenaires de recherche à part entière.

Contexte:Plusieurs écrits scientifiques suggèrent que la participation de personnes en rétablissement en tant que partenaires de recherche représente une valeur scientifique et sociale ajoutée à la recherche en toxicomanie. Toutefois, peu de recherches ont évalué systématiquement les retombées d'une telle participation sur les personnes concernées et selon ces personnes.Objectif:Cet article décrit diverses modalités et degrés de participation possibles en recherche. Les effets de leur participation sont discutés par des personnes qui furent activement parties prenantes au sein de deux projets. Elles furent rencontrées lors de groupes de discussion visant à dégager ce qu'elles pensent que la participation active en recherche peut leur apporter et ce qu'elles pensent qu'elles peuvent apporter à une dynamique de recherche, notamment à titre de pairs-assistants recherche.Résultats:La participation active en recherche favorise une certaine émancipation de la pauvreté, favorise la reconnaissance des compétences, ainsi que l'accès à l'information. Elle représente également l'occasion pour les personnes en question d'assumer des responsabilités et certains thèmes de recherche semblent plus propices que d'autres à des degrés plus élevés de participation. Les deux principaux thèmes qui ont émergés au sujet des effets de la participation des pairsassistants de recherche furent ceux du langage non verbal et de lien d'affinité. C'est-à-dire que leur présence engendre un effet de rétention et d'intérêt plus marqué de la part des autres participants à une étude, étant donné qu'une telle dynamique met ces derniers plus à l'aise de s'exprimer, ce qui se transpose également par la suite dans leur milieu de vie naturel.Conclusion:Les divers niveaux et degrés possibles de participation à la recherche peuvent se combiner et varier dans le temps ou selon les thématiques de recherche proposées. Plus de recherche est nécessaire pour mieux comprendre les enjeux de la participation en recherche et ses effets spécifiques en toxicomanie, ainsi que l'influence des pairs-assistants de recherche sur le rétablissement civique.
