As a result of rising health care costs, many countries, including the United States, have turned to managed care organizations and the use of capitation payment systems. Although this type of system is an effective mechanism for reducing excessive utilization of health care, it may lead to the underprovision of medical services. In this paper propensity to underprovide medical services in a prepayment system as well as the effects of auditing/monitoring on physician behaviour and patient well‐being are examined. Conditions are found under which managed care yields more efficient outcomes than traditional fee‐for‐service care.Suite à la croissance importante des coûts des soins, plusieurs pays, y compris les Etats Unis, ont commencéà se tourner vers des organisations spécialisées pour gérer la prestation des services et à faire usage de systèmes de rémunération per capita. Même si ce genre de système est un mécanisme efficace pour réduire l'usage excessif des service de santé, il peut entraîner une offre déficiente de services médicaux. Ce mémoire examine la propensitéà fournir moins de services dans un système de pré‐paiement. On examine aussi les effets de la surveillance et de la vérification sur le comportement des médecins et sur le bien‐être des patients. On met en lumière les conditions qui assurent que les soins fournis dans un tel système donneront de meilleurs résultats que la rémunération à l'acte.

We use longitudinal linked employer-employee data and find that the probability of participating in firm-sponsored classroom training diminishes with age. Although the standard human capital investment model predicts such a decline, we also consider the possibility that the wage and productivity impacts of training decline with age. Taking into account endogenous training decisions, we find that the training wage premium diminishes only slightly with age. However, estimates of the impact of training on productivity decrease dramatically with age, suggesting that incentives for firms to invest in classroom training are much lower for older workers.

En 30 ans, la psychiatrie s'est progressivement éloignée de son image carcérale et s'est enrichie de nouvelles pratiques basées sur l'alliance thérapeutique. Parallèlement, de plus en plus de détenus souffrant de pathologies psychiatriques sont incarcérés dans les prisons de France et d'Europe, créant des besoins de soins et d'accompagnements au-delà des capacités de réponse des dispositifs en place. Qu'ont-ils en commun, ces patients qui rentrent en prison et pourquoi « échappent-ils» aux soins communautaires ? Plus de comorbidités, plus de précarité, des actes médico-légaux dont la pénalisation peut parfois paraître démesurée par rapport à l'acte et le contexte dans lequel il a été commis. Pourquoi la prison devient-elle le lieu de vie des patients psychiatriques marginalisés ? Les raisons de cette évolution sont complexes et s'inscrivent dans l'articulation des politiques sociales, pénales et des politiques de santé. Comment délivrer des soins en prison ? Quelle interaction est la plus adéquate avec les personnels de l'administration pénitentiaire ? Cet ouvrage rédigé par l'équipe du SMPR de Lillefait l'état des lieux de l'accès aux soins des détenus dans les prisons françaises. Partout, les équipes des secteurs psychiatriques et des services médico-sociaux qui interviennent dans les établissements pénitentiaires ont mis en place à partir des moyens dont elles disposent des dispositifs de soin et d'accompagnement exemplaires. Parfois, ils ont bénéficié d'un cadre légal comme dans les addictions ou dans la prévention des risques, parfois il s'agit de créativité dans le recours aux psychothérapies, à l'abord de maltraitances, à l'accompagnement périnatal des femmes incarcérées ou encore dans la prise en charge des auteurs de violences sexuelles dans un cadre légal plus flou qu'en milieu ouvert. Les intervenants en santé mentale trouveront ici des réponses à toutes les questions soulevées par les pratiques en milieu carcéral, questions qui concernent l'ensemble de la société.

This study falls within the focus of research on pragmatic sociology concerning engagement schemes of action. This thesis sets out to compliment the competence to judge and the actions by person through a strategic action regime modelisation system. This involves examining this regime within the internal economy and its relation with other schemes of engagement which can be considered as a resource to denounce and justify.This inquiry employs different resources which impact the production of different categories of analysis and description. First of all, the study is based on the body of theory in pragmatic sociology and disagreements postulated in sociology. Secondly, a focus on the work of N. Machiavelli, equated with political grammar of strategic action. Followed, by an analysis of two empirical surveys linked to the research themes treated. The first survey concerns the planning related to logistical sub-contracting in the electronical commercial domain. The second survey, analyses the work concerning the representation of an electoral enterprise campaign.The strategic action of regime is a lens to analyze and construct a framework of the actions and situations which are based on the principles of equity of power. The situations consist of power struggles aimed to keep or conserve that which is good. The evaluation of situations such as power relations facilitate the reduction of uncertainty in carrying out actions, in order to classify the situations and proceed to distribution of goods.
