BACKGROUND: The objective of this paper is to outline and compare the regulation of paid healthcare professions and associated support workers in international context, bringing out the lessons to be learned as appropriate. Modern neo-liberal societies have sought to enhance healthcare through greater professional regulation, albeit in different ways and at variable pace. This general trend is illustrated with reference to medicine in the UK. However, although such reforms have helpfully cascaded to other health professions, government policy in high-income countries has not yet adequately regulated the interrelated group of non-professionalised health support workers who form the largest and least recognised part of the workforce. Nonetheless, in low- and middle-income (LMIC) countries—aside from the greater need for regulation of health professions—there is even more of an imperative to regulate the disparate, largely invisible support workforce. METHODS: With reference to existing studies of the medical and wider health professions in the UK and selected other higher income societies, the importance of health professional regulation to the public is underlined in the Global North. The larger gap in the regulation of support workers in modern neo-liberal countries is also emphasised on a similar basis, with an increasingly ageing population and advances in healthcare. It is argued from the very limited patchwork of secondary literature, though, that policy-makers may want to focus even more on enhancing regulation of both the professional and non-professional workforce in LMIC societies centred mainly in the Global South, drawing on lessons from the Global North. RESULTS/CONCLUSIONS: Efforts to reform health professional regulatory approaches in more economically developed countries, while needing refinement, are likely to have had a positive effect. The paper highlights the importance of resurrecting the debate about how to define a profession. The drive to define a profession is traced back to the taxonomic approach – encompassing the work of trait and functionalist writers – in which professions were seen as possessing unique and positive characteristics, including distinctive knowledge and expertise. A range of critical challenges to this approach are then considered, particularly as they relate to the role of knowledge and expertise in defining a profession, covering interactionism, Marxism, Foucauldianism and discourse analysis. However, the most effective challenge to the taxonomic approach is considered to be the neo-Weberian perspective based on a less broadly assumptive and more analytically useful definition of a profession centered on exclusionary closure. With reference to case studies, the relative merits of neo-Weberianism compared to taxonomic and other approaches are examined in relation to the role of knowledge and expertise and delineating professional boundaries.

This paper assesses a central challenge facing the social sciences – namely, implementing the outcomes of research, especially in the broader area of policy and practice. It notes the potential benefits of social sciences research, observing that a key reason for its limited application
has been the search for a 'value free' social science. The role of values in social science is explored and it is argued that a more appropriate balance needs to be struck with the political underpinnings of the research enterprise. The importance of applying relevant research in the social
sciences is underlined and the factors which inhibit and encourage this are considered. These include the way results are communicated – not least through the dissemination of research and the existence of institutionalised structures. The paper concludes by observing that the challenge
is not just implementing social science research, but doing so in a manner resonant with the public interest.

Examines the recent response of the British medical profession to acupuncture, one of the most subsidized therapies in the UK, in the context of wider relations between the professions, the state, & public interest. Through the 1970s, acupuncture was rejected by the medical profession as an unprofessional, irresponsible health practice. This reaction was in the self-interest of the profession, because it desired to retain its status & control over authoritative medical knowledge, despite the fact that acupuncture was based in a coherent medical tradition. The rejection was detrimental to the wider public interest, because it prevented its public provision. It might be assumed that the gradual & grudging acceptance of acupuncture on the part of doctors since the 1970s is a consequence of professional altruism, in which the profession's self-interest was subordinated to the public interest & spurred by external economic, cultural, & political pressures. 41 References.
