El 10 de diciembre nuestro "mundo globalizado" celebró el 50 aniversario de la Declaración Universal de los Derechos Humanos de las NN.UU, primer código ético con pretensiones de universalidad, síntesis de los valores generalmente considerados consustanciales con la dignidad humana y, por lo mismo, principios mínimos a respetar entre los individuos y entre los distintos pueblos. Aunque carezca en puridad de fuerza vinculante, no hay duda que dicha Declaración —a pesar de venir marcada por una cultura y una determinada concepción ideológica y filosófica y de que, hoy más que nunca, sea necesario y urgente entrar en el diálogo intercultural de los derechos humanos—, expresa un acuerdo sobre el ideal común en defensa de los derechos fundamentales y las libertades públicas, constituyendo su aprobación el claro inicio de internacionalización de los mismos. Es más, incluso hay quien ha visto en ella el primer signo de un derecho constitucional supranacional. Asimismo, es de justicia recordar que tal Declaración ha cumplido y cumple una misión no sólo programática sino también hermenéutica y como herramienta para fomentar una conciencia sensible a la defensa de los Derechos Humanos en cualquier lugar del planeta. Sin embargo, a 50 años vista, el balance que podemos hacer dista mucho de ser óptimo: los Derechos Humanos son hoy conculcados no sólo en las sociedades menos desarrolladas sino también en las que nadan en la abundancia. En realidad estamos lejos de que se desarrollen y sean de verdad los principios rectores de la convivencia del S. XXI, puesto que continúan siendo "papel mojado" para la mayoría de los habitantes del planeta. Como botón de muestra, nos basta recordar las recientes estadísticas que nos cuentan que en la vieja Europa del bienestar, tan orgullosa de su sistema de protección social, 57 millones de personas viven por debajo del umbral de la pobreza.

La configuración de los espacios urbanos ha sido en el tiempo escenario de los procesos sociales, económicos e ideológicos de las entidades poblacionales con voluntad de permanencia en el solar ocupado. La planificación administrativa de los sectores urbanos dedicados a usos públicos o privados se materializó en la necesaria habilitación de áreas residenciales privadas, artesanales o industriales y comerciales públicas, así como funerarias. A esta realidad no fue ajena la urbe de Carthago Nova en sus ocupaciones prerromana y romana; su progresión política y temporal registró toda una serie de transformaciones del entramado urbano y a ellas no fueron ajenos los condicionantes topográficos del medio físico primigenio y las contingencias ecológicas holocénicas. Poner en evidencia la conjunción de esta singularidad paleotopográfica, de origen geotectónico y dinámica geomorfológica, con las evidencias de importantes transformaciones operadas en época augustea, constituye el objetivo de este trabajo en el que se avanzan datos de un proceso de investigación en curso. ; The configuration of the urban sites has been setting of social, economic and ideological processes through the time, pertaining to the population entities willing to settle in the occupied site. The administrative planning of urban sectors involved in public or private uses was materialized in the necessary enablement of private or residential areas and public craft or industrial commercial and burial areas. The site of Carthago Nova was not exempt from this in its preRoman and Roman occupations. Its political and temporal progression showed a series of transformations of the urban fabric that were not unconnected to the topographic constraints of the primal physical environment and holocenic ecological contingencies. Bringing out the conjunction of this singular palaeotopography, which has a geomorphological dynamic and a geotectonic origin, with the evidence of significant transformations in Augustan period, is the focus of this work, in which data are shown from an ongoing research.

Siguiendo las recomendaciones de la OMS y de la Unión Europea en el año 2003 se constituyó la Red de Investigación Cooperativa Infancia y Medio Ambiente para estudiar los efectos del medio ambiente y la dieta en el desarrollo fetal e infantil en diversas zonas geográficas en España. La Red integra diversos grupos multidisciplinares de investigación y está constituida por seis cohortes, tres preexistentes y tres de novo, que seguirán de forma prospectiva a 3.600 mujeres embarazadas, desde el inicio del embarazo hasta los 4-6 años del niño. Los objetivos generales de la red son: (1) Describir la exposición individual a tóxicos ambientales durante la gestación y la primera infancia. (2) Evaluar los efectos de la exposición a tóxicos y de la dieta en el desarrollo fetal e infantil. (3) Evaluar la interacción entre factores tóxicos, nutricionales y genéticos en el desarrollo fetal e infantil. El seguimiento se realiza en cada trimestre de la gestación, al nacimiento, al año y hasta los cuatro o seis años del niño. La información se recoge mediante cuestionarios, datos clínicos, exploración física, ecografías, biomarcadores y mediciones ambientales. [Background] Lung cancer remains a leading cause of cancer incidence and mortality worldwide. Although Spain contributes to global statistics related to cancer, it is difficult to discern aspects linked to clinical presentation of the disease or molecular testing. The Thoracic Tumor Registry (TTR) was created with the aim of filling this gap. ; [Methods] Observational cohort multicenter study performed in Spain, including patients with lung cancer or other types of thoracic tumors undergoing active treatment or palliative care only. Enrollment took place between August 2016 and December 2018. The evaluation included a review of demographic, epidemiological, clinical and molecular data. ; [Results] A total of 6,600 patients diagnosed with non-small cell lung cancer (NSCLC) were recruited at 56 Spanish hospitals. The mean age at diagnosis was 64 years. The majority of patients (80%) presented with advanced disease, being adenocarcinoma the most frequent histological type. Up to 86% of patients were current- or ex-smokers, with men starting to smoke earlier than women (average age 17.9 vs. 19.2 years). Sixty-seven percent of patients underwent some type of molecular testing. Mutations in EGFR and KRAS genes were found in 18% and 28% of patients, respectively. ; [Conclusions] Our findings suggest that the TTR study accurately describes the clinical reality of lung cancer in Spain, including useful information on smoking status as well as molecular profiling and tumor histology, and can therefore be used to drive improvements in health care. AIMS: The aim of this study was to evaluate the association of diabetes and diabetes treatment with risk of postmenopausal breast cancer. METHODS: Histologically confirmed incident cases of postmenopausal breast (N = 916) cancer were recruited from 23 Spanish public hospitals. Population-based controls (N = 1094) were randomly selected from primary care center lists within the catchment areas of the participant hospitals. ORs (95 % CI) were estimated using mixed-effects logistic regression models, using the recruitment center as a random effect term. Breast tumors were classified into hormone receptor positive (ER+ or PR+), HER2+ and triple negative (TN). RESULTS: Diabetes was not associated with the overall risk of breast cancer (OR 1.09; 95 % CI 0.82-1.45), and it was only linked to the risk of developing TN tumors: Among 91 women with TN tumors, 18.7 % were diabetic, while the corresponding figure among controls was 9.9 % (OR 2.25; 95 % CI 1.22-4.15). Regarding treatment, results showed that insulin use was more prevalent among diabetic cases (2.5 %) as compared to diabetic controls (0.7 %); OR 2.98; 95 % CI 1.26-7.01. They also showed that, among diabetics, the risk of developing HR+/HER2- tumors decreased with longer metformin use (ORper year 0.89; 95 % CI 0.81-0.99; based on 24 cases and 43 controls). CONCLUSION: This study reinforces the need to correctly classify breast cancers when studying their association with diabetes.
