"Flame and Fortune in the American West investigates the ongoing politics, folly, and avarice shaping the production of increasingly widespread yet dangerous suburban and exurban landscapes. The 1991 Oakland Hills Tunnel Fire is used as a starting point to better understand these complex social-environmental processes. The Tunnel Fire is the most destructive fire--in terms of structures lost--in California history. Although this fire occurred in Oakland and Berkeley, others like it sear through landscapes in California and the American West that have experienced urban growth and development within areas historically prone to fire. Simon skillfully blends techniques from environmental history, political ecology, and science studies to closely examine the Tunnel Fire within a broader historical and spatial context of regional economic development and natural resource management, such as the widespread planting of eucalyptus trees as an exotic lure for homeowners, and the creation of hillside neighborhoods for tax revenue--decisions that produced communities with increased vulnerability to fire. Simon demonstrates how a drive for affluence led to a state of vulnerability for rich and poor alike in Oakland that has only been exacerbated by the rebuilding of neighborhoods after the fire. Despite these troubling trends, Flame and Fortune in the American West illustrates how many popular and scientific debates on fire limit the scope and efficacy of policy responses. This intervention suggests the need to closely examine uncritical uses of 'regions' in both geographical research and resource management contexts. In particular, I argue that regions are frequently leveraged in a manner that is often indistinguishable from, and thus analytically similar to, other concepts connoting connections and relationships across space. The US Department of Agriculture's Conservation Reserve Program is briefly described to illuminate the process and implications of using simplistic and erroneous regional designations (configured around the 100th Meridian) to inform resource management policy. I proffer several ways in which regions 'do work' analytically, discursively and materially, and argue that it is precisely the performative nature of regions that warrants its utility and sustained application in scholarly and policy-making environments. Finally, I suggest that the analytic toolkit possessed by political ecologists makes us uniquely equipped to assess, reconfigure, and employ regions and regional designations in our research; applications that will hopefully inform more accurate, nuanced and socially just policies.Key words: 100th Meridian, conservation reserve program, political ecology, regions, environmental management

ABSTRACTThe economic empowerment of women remains a central feature of development projects worldwide. This article explores these empowerment aspirations by examining various temporal complexities related to two development projects in South India targeting individual cooking and fuel‐collection routines. It argues that three temporal considerations of household labour — polychronic time, collectivized time and hybrid labour/leisure time — are largely overlooked, thus challenging the appropriateness of empowerment strategies from the outset. By highlighting key household dynamics this study argues that efforts to pursue economic empowerment by members of the clean cookstove sector devalue unpaid labour and glorify waged work that is often tedious and mediated by powerful men. Further, waged work can be burdensomely added to pre‐existing unpaid domestic labour responsibilities. This common portrayal of empowerment by the clean cookstove sector can be viewed as facilitating the advancement of a neoliberal vision of rural women's livelihoods where empowerment and agency are reconfigured and incorporated into the problematic wage relations of capitalist economies. This view of empowerment, which privileges market participation, also overlooks many of the actually experienced positive effects of improved stoves; benefits which tend to be domestic, communal, routine, non‐economic and difficult to quantify. The article argues that conventional definitions and visions of 'empowerment' should be re‐evaluated to include forms of 'mundane agency'.

BACKGROUND AND OBJECTIVE: Common mental disorders (CMD) are a leading global burden of disease. Up to 30% of primary care attenders suffer from these disorders but most do not receive evidence-based drug or psychological treatments. There are no trials of interventions which attempt to integrate these treatments into routine primary care in developing countries. The aims of this trial (the MANAS Project) are to evaluate the clinical and cost-effectiveness of a collaborative stepped-care intervention for the treatment of CMD in India. STUDY DESIGN: A cluster randomized controlled trial will be implemented in the state of Goa, on the west coast of India. Twenty-four primary care facilities, 12 from the government sector and 12 from the private sector, will be enrolled in two consecutive phases. For each sector, facilities will be randomly allocated within strata defined by urban/rural location, population size and presence of a visiting psychiatrist. Facilities will be randomly allocated to receive the collaborative stepped care intervention or the enhanced usual care control intervention. Both arms share two components of the intervention, viz., routine screening, and in the government clinics provision of antidepressants. In addition, the collaborative stepped care arm also provides a range of psychosocial treatments delivered by a specially trained Health Counselor, and supervision by a visiting Psychiatrist. A total of 3600 primary care attenders who are detected to suffer from a CMD based on a validated screening questionnaire will be recruited. The primary outcome is the proportion of subjects who recover from an ICD10 defined CMD at baseline by 6 months. Additional endpoints at 2 and 12 months will assess the speed and sustainability of achieving the primary outcomes. Other outcomes will include recovery from ICD10 defined depression and incidence of ICD-10 among individuals who were sub-threshold cases at baseline. Economic and disability outcomes will be assessed to estimate incremental cost-effectiveness ratios. IMPLICATIONS: This will be the first trial of the effectiveness of a complex intervention aiming to integrate efficacious treatments for CMD into routine primary care in a developing country. If effective, its findings will have relevance to policy makers who wish to scale up treatments for CMD in primary care across the world, but mostly in those countries where specialist mental health services are few. Objective: To compare patterns of health care utilization associated with first presen­tation of psychosis among different racial and ethnic groups of patients.Design: The study was a retrospective observational design.Settings: The study was conducted in five health care systems in the western United States. All sites were also part of the Nation­al Institute of Mental Health-funded Mental Health Research Network (MHRN).Participants: Patients (n = 852) were aged 15 – 59 years (average 26.9 ± 12.2 years), 45% women, and primarily non-Hispanic White (53%), with 16% Hispanic, 10% non-Hispanic Black, 6% Asian, 1% Native Hawaiian/Pacific Islander, 1% Native Ameri­can/ Alaskan Native, and 12% unknown race/ethnicity.Methods: Data abstracted from electronic medical records and insurance claims data were organized into a research virtual data warehouse (VDW) and used for analysis.Main Outcome Measures: Variables exam­ined were patterns of health care utilization, type of comorbid mental health condition, and type of treatment received in the three years before first presentation of psychosis.Results: Compared with non-Hispanic Whites, Asian patients (16% vs 34%; P=.007) and non-Hispanic Black patients (20% vs 34%; P=.009) were less likely to have a visit with specialty mental health care before their first presentation of psychosis.Conclusions: Early detection of first episode psychosis should start with wider screening for symptoms outside of any indicators for mental health conditions for non-Hispanic Black and Asian patients. Abstract. Background: In the US, more than one million people attempt suicide each year. History of suicide attempt is a significant risk factor for death by suicide; however, there is a paucity of data from the US general population on this relationship. Aim: The objective of this study was to examine suicide attempts needing medical attention as a risk for suicide death. Method: We conducted a case–control study involving eight US healthcare systems. A total of 2,674 individuals who died by suicide from 2000 to 2013 were matched to 267,400 individuals by year and location. Results: Prior suicide attempt associated with a medical visit increases risk for suicide death by 39.1 times, particularly for women ( OR = 79.2). However, only 11.3% of suicide deaths were associated with an attempt that required medical attention. The association was the strongest for children 10–14 years old ( OR = 98.0). Most suicide attempts were recorded during the 20-week period prior to death. Limitations: Our study is limited to suicide attempts for which individuals sought medical care. Conclusion: In the US, prior suicide attempt is associated with an increased risk of suicide death; the risk is high especially during the period immediately following a nonlethal attempt.
