Die Psychotherapie Drogenabhängiger in Substitutionsbehandlung

Fragestellung: Angesichts des hohen Ausmaßes an komorbiden psychischen Störungen bei Opiatabhängigen in Substitutionsbehandlung ist eine begleitende psychiatrisch-psychotherapeutische Behandlung in der Regel notwendig. Es ist allerdings zu fragen, ob entsprechende Strategien bei dieser speziellen Patientengruppe auch evaluiert wurden. Methodik: Literaturübersicht mit Fokus auf psychotherapeutische Interventionen mit dem Ziel der Linderung komorbider substanzbezogener Störungen. Ergebnisse: In randomisierten kontrollierten Prüfungen konnte die Wirksamkeit insbesondere einer kognitiven Verhaltenstherapie wie auch des Contingency Management zur Reduktion des Suchtmittelkonsums bei Substitutionspatienten belegt werden. Im Fokus der zumeist US-amerikanischen Untersuchungen stehen kokainbezogene Störungen. Schlussfolgerungen: Eine begleitende Psychotherapie bei Substitutionspatienten ist wirksam. Hindernisse für die Implementierung in der klinischen Versorgung sind mutmaßlich mangelnde Kenntnis der Datenlage bei einem verbreiteten therapeutischen Nihilismus in der Behandlung Drogenabhängiger, aber auch begrenzte finanzielle und personelle Ressourcen für eine begleitende Psychotherapie.
Ergebnisse der stationären Entzugsbehandlung von Benzodiazepinen bei Methadonsubstituierten

<B>Ziel:</B> Es wird untersucht, ob bei Patient/innen in Methadon-Substitutionsbehandlung, die Benzodiazepin-(BZD)-Beigebrauch betreiben, durch stationäre Entzugbehandlung ein vollständiger Entzug und anhaltende Abstinenz von BZD erreicht wird. </P><P> <B>Methodik:</B> Analysiert wurden die Daten von 52 Patient/innen zweier Methadonambulanzen, die sich elektiv einer BZD-Entzugsbehandlung unterzogen. Substanzkonsum wurde anhand von Urinkontrollen bestimmt. Nachbeobachtungszeit war drei Monate. </P><P> <B>Ergebnisse:</B> Ein drogenfreier Urin wurde in 79 % der Fälle erreicht. Über drei Monate BZD-abstinent in Methadonbehandlung verblieben 13 %.Einen Rückfall in anhaltenden BZD-Konsum erlitten 46 %.Die durchschnittliche Rate BZD-positiver Urine sank von 79 % (SD 30 %) vor Entzugsbehandlung auf 61'% (SD 38 %) nachher. </P><P> <B>Schlussfolgerungen:</B> Das Ziel eines BZD-Entzugs kann durch die stationäre Behandlung erreicht werden. Nach dem Entzug wurde vollständige Abstinenz nur von einer Minderheit beibehalten, und ein großer Teil der Behandelten wurde anhaltend rückfällig. Zusätzliche Behandlungsmaßnahmen innerhalb der Substitutionsbehandlung sind notwendig, wenn der Erfolg der Entzugsbehandlung aufrechterhalten werden soll.
Suchtmittelkonsum bei homo- und bisexuellen Männern

Hintergrund: Der Anteil von Männern, die Sex mit Männern haben (MSM) ist unter HIV-Neuinfizierten in vielen westlichen Industriestaaten konstant auf hohem Niveau. Die Einnahme von Suchtmitteln ist mit vermehrtem Risikoverhalten für sexuell übertragbare Krankheiten assoziiert. Der Suchtmittelkonsum von MSM und seine Assoziation mit sexuellem Risikoverhalten sind von Bedeutung für die HIV-Verbreitung. Methodik: Literaturübersicht ab 1999 publizierter Studien zur Prävalenz von Suchtmittelkonsum bei MSM. Ergebnisse: Die meisten Untersuchungen zum Thema wurden in den USA durchgeführt. Das Ausmaß von Alkoholkonsum und alkoholbezogenen Störungen bei MSM war generell vergleichbar mit der Allgemeinbevölkerung. Erhöhte Prävalenzraten unter MSM wurden für den Konsum illegaler Substanzen gefunden. Besonders für Cannabis, Amylnitrit, Halluzinogenen, Stimulantien und Kokain. Es gibt zudem Subgruppen von MSM, die im Kontext gesellschaftlicher Aktivitäten intensiv sog. Club Drugs konsumieren. Dieser Konsum ist mit sexuellem Risikoverhalten assoziiert. Schlussfolgerungen: Bei der Entwicklung zielgruppenspezifischer Interventionen zur Prävention sexuell übertragbarer Krankheiten in der Gruppe der MSM sollte der Suchtmittelkonsum als wichtiger Einflussfaktor verstärkt berücksichtigt werden.

Prediction of the Outcome of Inpatient Opiate Detoxification Treatment: Results from a Multicenter Study

<i>Background:</i> Monocentric studies of inpatient opiate detoxification treatment show considerable variability regarding treatment success rates. This multicentric study investigates whether patient characteristics explain the different rates of regular discharge between treatment units. <i>Methods:</i> 1,017 opiate-dependent patients from 12 detoxification units with similar treatment programs, funding, staffing and equipment were analyzed. Patient data and outcomes were documented by treatment staff using a standard form. <i>Results:</i> Controlling for center, regular discharge (range: 14–49% between centers) was significantly associated with pre-existing plans for follow-up treatment, previous completed long-term residential and detoxification treatments, fewer unsuccessful detoxification treatments, higher age, later onset of opiate use, and longer duration of use. Controlling for patient characteristics, the center variable was significantly associated with outcome in a multiple logistic regression analysis. <i>Conclusions:</i> Regular discharge could best be predicted by patients' plans for follow-up treatment and previous treatment outcomes. Although treatment units had equivalent resources and regulations, and although patient effects were statistically controlled for, there were still considerable center effects. Setting factors as well as actual drop-out processes should be investigated more closely in the future.

Die Methadonsubstitution im Urteil der Patienten

Ziel: Bewertung der Methadonsubstitution durch Opiatabhängige. </P><P> Methode und Stichprobe: Untersuchung mittels eines selbst entwickelten Fragebogens in zwei Erhebungen von 73 bzw. 101 langjährig substituierten Patienten. </P><P> Ergebnisse: Die Patienten bewerteten im Globalurteil ihre Situation als im Verlauf der Substitution deutlich verbessert. Dies betraf v. a. basale Therapieziele wie Wegfall des Beschaffungsdrucks, Minderung krimineller Aktivitäten und Lösung von der Drogenszene. Bei retrospektiver Erfassung war das Erreichen dieser basalen Therapieziele die wichtigste Erwartung bei Beginn der Substitutionsbehandlung.</P><P> Schlussfolgerung: Die Mehrheit langjährig substituierter Patienten war mit dem Erreichen basaler Ziele zufrieden. Das Erreichen weiterer Ziele bis hin zur Abstinenz setzt bei individueller Indikation zusätzliche motivationale Therapieangebote voraus.
Cognitive-Motor Performance of Methadone-Maintained Patients

Fifty-four methadone-maintained patients and 54 healthy controls, matched for age, gender and educational attainment, completed a battery of six cognitive-psychomotor performance tests. Results of previous studies were replicated in four areas. An attention task was performed less well by patients [mean difference more than 0.7 standard deviations (SD)] as was a tachistoscopic perception task (0.3 SD). On a simple-choice reaction test, patients showed higher speed in decision making and motor reaction as well as an increased number of decision errors (0.3 SD each). Performing a tracking test, patients showed less deviations (0.4 SD) combined with more time needed (0.8 SD). Our data go beyond previous (seemingly inconsistent) research findings by showing that patients did less well by more than 0.6 SD when on higher speed levels. Absolving a test on visual structuring, more patients than controls achieved a 100% accuracy level (52 vs. 30%), but at the same time patients were slower (0.6 SD) than controls. An inferior test performance of patients in methadone maintenance treatment has been confirmed in some areas, especially in attention. However, the fairly moderate size of these effects and the fact that in the majority of measures the observed variance was better explained by sociodemographic features than by group membership lead on the conclusion that belonging to the group of methadone patients alone is not necessarily sufficient to predict an impairment in cognitive-psychomotor skills. To conclude, assessment of fitness for certain tasks or occupations should be done individually for each patient and should take into account comorbidity, including the extent of alcohol and other drug use.

Entwicklung einer Selbstbeurteilungsskala für das Alkoholentzugssyndrom (ESA, Essener Selbstbeurteilung Alkoholentzugssyndrom)

Ziel: Die subjektive Seite des Befindens und der Beschwerden der Patienten während des akuten Alkoholentzugssyndroms (AES) blieb bei dessen Therapieevaluation bislang unberücksichtigt. Vorgestellt wird die Entwicklung einer Selbstbeurteilungs-Skala für das AES. </P><P> Methodik: Eine 28-Item-Version wurde einer klassischen testtheoretischen Analyse unterzogen. 103 Patienten mit einem akuten AES bei chronischer Alkoholabhängigkeit wurden in die Untersuchung aufgenommen. </P><P> Ergebnisse: Nach Berechnung von Schwierigkeit, Trennschärfe und interner Konsistenz erfolgte eine Item-Reduktion der Skala. Die Faktorenanalyse der 21-Item-Lösung zeigt nach Varimax-Rotation eine 1-Faktorlösung. Dabei liegt eine hohe interne Konsistenz vor (Cronbachs alpha = 0.93). Bei einer erneut nach einer Woche gemessenen Subgruppe der Entzugspatienten (n = 54) zeigte sich eine gute Änderungssensitivität. Zur Validitätsüberprüfung erfolgte ein Vergleich mit abstinenten Alkoholabhängigen und Angstpatienten. </P><P> Schlussfolgerungen: Es konnte zuverlässig zwischen den Patienten mit einem AES und den abstinenten Patienten unterschieden werden, nicht aber zwischen der Entzugs- und der Angststichprobe. Die 21-Item-Version des Fragebogens könnte als Instrument zur Qualitätssicherung für den Alkoholentzug eingeführt werden.
Group Psychotherapy for Opiate Addicts in Methadone Maintenance Treatment – A Controlled Trial

A controlled trial was conducted evaluating cognitive-behavioural group psychotherapy as a measure to reduce concomitant drug use in methadone maintenance treatment (MMT). 73 opiate addicts were randomly assigned to local routine MMT or to routine MMT plus group psychotherapy (20 sessions over 20 weeks). Psychotherapy was delivered by therapists according to a manual. Drug use (urine screen) was compared at onset of psychotherapy, end of intervention period (6 months after study onset), and 6 months later. Data analysis was done according to intention-to-treat principles. Results indicated that patients in the psychotherapy group (n = 41) showed less drug use than control subjects (n = 32). This group difference was statistically significant at 6-month follow-up (p = 0.02). These findings underscore the usefulness of group psychotherapy in MMT. The delayed effect is comparable to other studies evaluating cognitive-behavioural psychotherapy.

Availability of Illegal Drugs During the COVID-19 Pandemic in Western Germany

Background: In response to the COVID-19-pandemic, a lockdown was established in the middle of March 2020 by the German Federal Government resulting in drastic reduction of private and professional traveling in and out of Germany with a reduction of social contacts in public areas. Research Questions: We seek evidence on whether the lockdown has led to a reduced availability of illegal drugs and whether subjects with substance-related problems tried to cope with possible drug availability issues by increasingly obtaining drugs via the internet, replacing their preferred illegal drug with novel psychoactive substances, including new synthetic opioids (NSO), and/or by seeking drug treatment. Methods: A questionnaire was anonymously filled in by subjects with substance-related disorders, typically attending low-threshold settings, drug consumption facilities, and inpatient detoxification wards from a range of locations in the Western part of Germany. Participants had to both identify their main drug of abuse and to answer questions regarding its availability, price, quality, and routes of acquisition. Results: Data were obtained from 362 participants. The most frequent main substances of abuse were cannabis (n = 109), heroin (n = 103), and cocaine (n = 75). A minority of participants reported decreased availability (8.4%), increased price (14.4%), or decreased quality (28.3%) of their main drug. About 81% reported no change in their drug consumption due to the COVID-19 pandemic and the lockdown. A shift to the use of novel psychoactive substances including NSO were reported only by single subjects. Only 1-2% of the participants obtained their main drug via the web. Discussion: Present findings may suggest that recent pandemic-related imposed restrictions may have not been able to substantially influence either acquisition or consumption of drugs within the context of polydrug users (including opiates) attending a range of addiction services in Germany.
