Hintergrund: Drug-Checking bezeichnet die chemische Analyse von illegalen Drogen und Substanzen. Die Umsetzung eines wissenschaftlichen Modellprojekts zum "Drug-Checking" wird aktuell in Deutschland diskutiert. Ziel der vorliegenden Studie ist es, den Wunsch nach einem Drug-Checking-Modellprojekt in der Risikogruppe für Substanzkonsum aus der Techno-Szene zu klären und die bevorzugte Art einer Umsetzung zu erfragen. Methoden: Befragt wurden 433 Personen, von denen sich 338 der Risikogruppe für Substanzkonsum aus der Techno-Szene zugehörig fühlen. Die übrigen 95 Personen ohne Kontakt zu dieser Gruppe dienten als Vergleichsgruppe. Der Onlinefragebogen beinhaltete Fragen zum Drogenkonsum, zu bevorzugten Drug-Checking-Verfahren und Vorgehensweisen und zu Bedenken hinsichtlich solcher Angebote. Ergebnisse: Es zeigt sich, dass ein großer Wunsch nach Drug-Checking-Angeboten besteht (81,7 %) und eine hohe Bereitschaft existiert, für die Angebote zu zahlen (51,1 %) und sich zusätzlich über synthetische Drogen informieren zu lassen (40,9 %). Dabei werden aufwendige, sichere Analyseverfahren wie die Gas-Chromatographie bevorzugt. 30,5 % der befragten Konsumenten würden möglicherweise ihren Konsum illegaler Drogen infolge einer größeren Sicherheit über die Inhaltsstoffe durch Drug-Checking erhöhen. Die Personen der Risikogruppe für Substanzkonsum aus der Techno-Szene wiesen hohe Prävalenzen für den Konsum legaler und illegaler Drogen auf. Die am häufigsten überhaupt konsumierten Substanzen waren Alkohol (97,7 %) und Tabak (90,0 %), gefolgt von Cannabis (89,9 %), MDMA/Ecstasy (70,4 %) und Speed (69,5 %). Weit verbreitet war zudem der Mischkonsum. Die Personen, die sich nicht dieser Gruppe zugehörig fühlten, hatten insbesondere bei den illegalen Substanzen deutlich niedrigere Prävalenzwerte. Schlussfolgerungen: Die mit der Erprobung oder Einführung von Drug-Checking-Verfahren verbundenen möglichen Risiken müssen gegenüber den schadensminimierenden und präventiv nutzbaren Effekten solcher Projekte abgewogen werden.

Introduction: After rapidly opening up a low-threshold clinic to support heavily opioid-dependent persons at the beginning of the COVID-19 pandemic in April 2020 in Hamburg (Germany), this non-interventional study evaluated the feasibility and short-term effects of opioid substitution treatment (OST). The low-threshold concept was customized for the pandemic situation and is the first of its kind in Germany. Methods: Patients who had already begun treatment were questioned in two assessments, at T1 shortly after beginning treatment and at T2 6 months later. The primary outcome criterion was their quality of life using the OSTQOL. Secondary criteria included retention rate, their mental and physical health (measured by the BSI-18 and the OTI Health Scale), social situation, drug use, COVID-19 status, and satisfaction with treatment. Results: Out of 84 patients included in the study, 51 participated in both assessments, resulting in a 6-month retention rate of 60.7%. 27.5% were females, and 72.5% were males. The feasibility question of the low-threshold OST clinic can clearly be answered positively. During the course of the study over 6 months, the situation mainly remained stable regarding quality of life, physical and mental health, and days of drug consumption. Patients significantly reduced the time they spent on the drug scene from 8.5 (SD = 7.56) to 6.1 (SD = 6.71) hours a day between the beginning of OST and T2 (p = 0.020). While 56.9% answered to be homeless at the beginning of OST, only 33.3% answered not to have found an accommodation by T2 (p = 0.012). The number of patients having contact to social workers increased from 51.0% to 74.5% (p = 0.004). Almost 2 fifths of the patients took part in PCR testing for COVID-19 (that only being done if they had symptoms), and none of the tests were positive. Discussion/Conclusions: Overall, the low-threshold OST clinic has been successfully implemented in order to help a vulnerable group of people navigate through a global pandemic and support the public health sector. Further conclusions on effects are limited by the short study period and the small number of patients, which calls for further research studies in a larger setting.

Hintergrund: Nachdem das bundesdeutsche Modellprojekt zur heroingestützten Behandlung 2007 auslief, wurde vom Zentrum für Interdisziplinäre Suchtforschung der Universität Hamburg (ZIS) ein Qualitätssicherungsprojekt aufgelegt, durch das die regelmäßige Dokumentation von Behandlungsstandards und Effekten weiterhin sicher gestellt werden konnte. Fragestellung: Im Mittelpunkt der Auswertung steht die Beschreibung von aktuellem Status und 1-Jahres-Behandlungsverlauf der nach der Studie neu in die Diamorphinbehandlung aufgenommenen Patienten. Der Behandlungsverlauf kann denen der Studienpatienten gegenüber gestellt werden. Methodik: Von den behandelnden Ärzten wurde zu Beginn und im 6-Monats-Abstand pro Patient anhand eines standardisierten Erhebungsbogens der Aufnahmestatus sowie der Behandlungsverlauf dokumentiert. Für die Beschreibung des aktuellen Zustands wurde die letzte Dokumentation in 2011 herangezogen, der Verlauf wurde anhand eines Vergleichs von Aufnahme- und 1-Jahres-Dokumentation analysiert. Ergebnisse: Über 341 Diamorphinpatienten liegen in 2011 Informationen zum aktuellen Zustand vor, davon sind 205 (60,1 %) nach dem Modellprojekt aufgenommen worden. Die Mehrheit kommt für 2 Vergaben am Tag. Die mittlere Tagesdosis Diamorphin der seit durchschnittlich mehr als 8 Jahre behandelten Modellprojektpatienten beträgt 358 mg, die der seit durchschnittlich eineinhalb Jahren behandelten Neuaufnahmen 432 mg. 3,6 % sind HIV-positiv, 75,0 % mit Hepatitis C infiziert. Bei einem Viertel wurden depressive Störungen diagnostiziert. Die Mehrheit lebt in stabilen Wohnverhältnissen, ein Viertel hat eine regelmäßige Arbeit oder Jobs. Justitielle Delikte werden von 6,7 % der Diamorphinpatienten berichtet. 5,1 % konsumierten innerhalb der letzten 30 Tage noch Straßenheroin, 22,4 % Kokain. Hinsichtlich der Veränderungen innerhalb des ersten Behandlungsjahres zeigen sich in den Bereichen Gesundheit, soziale Situation und Drogenkonsum deutliche und statistisch signifikante Verbesserungen. Schlussfolgerung: Die Diamorphinbehandlung in Deutschland wird auch unter den Bedingungen der Regelversorgung erfolgreich durchgeführt. Die aktuelle Situation der sich seit vielen Jahren in Behandlung befindlichen Modellprojektpatienten stellt sich stabiler dar, was darauf hindeutet, dass weitreichende Veränderungen viel Zeit benötigen. Die positiven Wirkungen nach einem Jahr Diamorphinbehandlung sind denen aus dem Bundesmodellprojekt vergleichbar.

Fragestellung: Welche städteübergreifenden Gemeinsamkeiten und Unterschiede existieren bezüglich Konsummustern, Risikoverhalten sowie Hilfebedarf und -Inanspruchnahme von Konsumenten der offenen Drogenszenen? Methodik: Eine standardisierte Befragung wurde im Sommer 2008 in 13 deutschen Städten mit Konsumraum unter Konsumenten der offenen Szene durchgeführt. Ergebnisse: Es wurden 791 Fragebögen eingeschlossen. Multipler Substanzkonsum ist die Regel, wobei Heroin am häufigsten konsumiert wird (24-Stunden-Prävalenz: 68,3 %). Risikoverhalten in Form von Mehrfachverwendung der Spritze (45,5 %) und gemeinsamer Nutzung von Spritze/Spritzbesteck (13,8 %) und Crackpfeife (60,2 %) lässt sich feststellen. Die Konsumenten nutzen Hilfeangebote regelmäßig (72,6 % mindestens mehrmals wöchentlich), insbesondere Kontaktcafés, Konsumräume und Spritzentausch. Suchtberatung wird von rund zwei Dritteln als wichtig eingeschätzt. Schlussfolgerungen: Die Konsumenten der offenen Drogenszene werden durch niedrigschwellige Angebote gut erreicht. Safer-Use-Botschaften stellen auf Grund des nach wie vor vorhandenen Risikoverhaltens einen wichtigen Bestandteil dar. Aufenthalts- und Beschäftigungsmöglichkeiten für diejenigen mit geringem Konsum und Substituierte erscheinen notwendig.

The present study investigates whether a correlation exists between mental symptoms and opiate dependency. During a 5-year follow-up study in Hamburg, of 350 opiate addicts who were in contact with the help system at the time of the initial investigation, 219 (63%) could be interviewed three times at 1-year intervals. The investigation instruments were standardized questionnaires such as EuropASI, CIDI, SCL-90-R and BDI. The general life situation of the investigated persons had, on the whole, improved in the course of the last 2–3 years. Drug consumption had markedly decreased. One third of the opiate addicts were in a comparatively good mental condition on all three survey interviews, for 17% there was a worsening of the condition, and for another 17% the negative mental condition was reinforced. A correlation can be established between mental disorders/symptoms and drug addiction or drug-related problems. The more unfavorable the course of the mental symptoms, the greater the problems of the client's current life situation. There is also an overall relationship between increased drug consumption and mental symptoms like depressivity and anxiety, and the psychosocial functioning level. However, the expected correlations between mental disorders and the extent of drug consumption are not very marked. This indicates that specific constellations of drug consumption and mental disorders are not isolated but are related, as elements of a complex behavioral pattern, to the development of other life areas of the client.

Seit über 20 Jahren wird die ambulante Suchtakupunktur insbesondere in den USA als regelhafte Behandlung für Drogen- und Alkoholabhängige angeboten. Die Wirksamkeit der Behandlung ist nach wie vor umstritten, wenngleich in einigen Kontrollgruppenstudien positive Effekte festgestellt wurden. In den letzten Jahren hat auch in der Bundesrepublik Deutschland das Interesse an der Suchtakupunktur zugenommen. Sie wird vereinzelt in der ambulanten und stationären Entzugsbehandlung eingesetzt. Die vorliegende Studie evaluiert ein niedrigschwelliges ambulantes Akupunkturprojekt eines freien Trägers in Hamburg. Innerhalb eines 8monatigen Untersuchungszeitraums konnten 159 Klienten in die Studie einbezogen werden, was 96% aller Behandlungsteilnehmer entspricht. Der Großteil der Klienten nimmt nur an wenigen Akupunktursitzungen teil, nach Wochen ist bereits die Hälfte aus der Behandlung ausgeschieden. Im Mittelpunkt der Untersuchung standen die Beurteilungskriterien Entzugssymptome, subjektive Befindlichkeit und Drogenkonsum. Es zeigt sich, daß es unter der Akupunkturbehandlung zu einer Verringerung der Entzugssymptomatik und einer leichten Verbesserung des körperlichen und psychischen Befindens sowie einer Reduktion des Konsums von Alkohol und vor allem Kokain kommt. Dabei ergeben sich die größten Fortschritte bereits innerhalb der ersten beiden Behandlungswochen. Aufgrund des großen Anteils an Klienten, die frühzeitig aus der Behandlung ausscheiden, und der damit verbundenen geringen Stichprobengröße kann diese Studie nur vorläufige Aussagen zur Wirksamkeit der ambulanten Suchtakupunktur liefern. Die Ergebnisse sprechen allerdings dafür, daß die Akupunktur insbesondere in der Behandlung von Kokainkonsumenten eine geeignete Maßnahme ist, und ihr in Zukunft eine größere Aufmerksamkeit zukommen sollte.

Introduction: Opioid agonist treatment (OAT) is the most common and most effective treatment option for persons with opioid use disorders (OUD). In Germany, the prescription of OAT medications is regulated by the Narcotic Drugs Prescription Ordinance. With the introduction of restrictions to contain the SARS-CoV-2 virus, the German OAT regulations have been amended to ensure a legal continuation of OAT for people with OUD. In this study, we aimed to examine the use of the OAT regulations in practice, the experience made by physicians prescribing OAT medications, and their perspective on OAT regulations. Methods: Between September and December 2021, a questionnaire on the current situation and potential changes in the provision of OAT during the COVID-19 pandemic was sent out to 2,416 German physicians prescribing OAT medications. Differences between physicians with and without addiction medicine certification were analyzed. Results: The response rate of physicians was 22.8%. Their average age was 57.4 (±10.1) years, and 62.3% were male. During the COVID-19 pandemic, take-home periods for stable patients have been extended by 48.2% of physicians, and 52.6% would like to maintain this prescribing practice in the future. Most physicians (71.6%) indicated that patients handled the extended take-home prescriptions predominantly responsibly. A total of 71.8% of the physicians generally did not use video consultation. A corona pandemic-related switch of the OST medication to depot buprenorphine injection did rather not occur, as 71.2% reported no patients treated with depot buprenorphine, and only 2.6% switched first-time or more patients to depot buprenorphine due to the COVID-19 pandemic. Conclusion: The corona situation opened up opportunities for physicians and patients and enabled change processes in OAT. Physicians had positive experiences implementing expanded take-home prescriptions for stable patients. Video contacts rarely took place, suggesting resistance to digital consultation. The number of depot buprenorphine prescriptions has not increased substantially since the pandemic's beginning and has remained at low levels. Further research is needed to assess to what extent the changes in OAT will be maintained over time and whether they will also lead to long-term benefits for OAT patients.

<b><i>Background:</i></b> Many studies have examined factors associated with the first onset of cannabis use and abuse. Currently, there is relatively little research regarding conditions under which cannabis dependence is more likely to emerge. Although previous studies have examined different potential determinants of cannabis dependence, to our knowledge, a systematic review is lacking. <b><i>Aims:</i></b> The study aims to identify recent findings regarding psychosocial determinants of cannabis dependence and to summarize them systematically. <b><i>Methods:</i></b> A literature search in 4 databases - Embase, Medline, PsycINFO and PSYNDEX - was conducted. Searches were limited to publications between 2000 and April 2014, English and German as languages and humans as study subjects. <b><i>Results:</i></b> Our search detected a total of 10,568 studies. Twenty-six studies finally met inclusion criteria. Consumption patterns such as a regular cannabis use independent of social context and an early onset of use (11-15 years) were correlates of cannabis dependence. Moreover, early reactions to cannabis use and coping-oriented use motives explained additional variance. Stress factors and critical life events such as parental separation and early parental death as well as mental and social conflicts have also been linked with development of cannabis dependence. Additionally, comorbid mental disorders correlated with cannabis dependence. <b><i>Conclusion:</i></b> Numerous factors were shown to have an impact on transition to cannabis dependence. In particular, a wide range of mental disorders has been linked to an elevated risk of becoming dependent. The development of a dependence syndrome seems to be associated with diverse processes, in which social, biological and intra-individual factors interact in a complex manner. Nevertheless, the link between cannabis dependence and predisposing factors could not be resolved convincingly by most studies due to methodological weaknesses regarding dependence criteria.

<b><i>Background:</i></b> Opioid maintenance treatment is the option of choice to stabilize opioid-dependent patients. Whilst efficacy of methadone and buprenorphine has been studied extensively, fewer data on slow-release oral morphine are available. <b><i>Aims:</i></b> This study analyzes the effects of slow-release oral morphine compared to methadone with regard to self-reported mental symptoms, drug use and satisfaction with treatment. <b><i>Methods:</i></b> The study was carried out as an open-label randomized crossover trial in 14 treatment sites in Switzerland and Germany. It comprised 2 crossover periods of 11 weeks each. For measuring mental symptoms, the Symptom Checklist-27 (SCL-27) was used. Drug and alcohol use was assessed by the number of consumption days, and treatment satisfaction by a visual analogue scale. <b><i>Results:</i></b> A total of 157 patients were included for the analyses (per-protocol sample). Statistically significantly better outcomes for morphine as compared to methadone treatment were found for overall severity of mental symptoms (SCL-27 Global Severity Index), as well as 5 of the 6 syndrome groups of the SCL-27, and for treatment satisfaction. There were no statistically significant differences with regard to drug or alcohol use between groups. <b><i>Conclusions:</i></b> This study supports positive effects of slow-release oral morphine compared to methadone on patient-reported outcomes such as mental symptoms and treatment satisfaction with comparable effects on concomitant drug use. Slow-release oral morphine represents a meaningful alternative to methadone for treatment of opioid dependence.

<span class="fett">Fragestellung:</span> Ziel der Studie war zu zeigen, ob sich bei einem Wechsel der in der Substitutionsbehandlung Opiatabhängiger eingesetzten Substanzen Levomethadon und d,l-Methadon das psychische Befinden und das Verlangen nach Drogen ändern. </p><p> <span class="fett">Methodik:</span> Es wurde eine stratifizierte, randomisierte 2×2-Cross-over-Studie doppelblind über 8 Wochen durchgeführt. Zu Beginn und nach 4 Wochen wurde die Studienmedikation umgestellt. </p><p> <span class="fett">Untersuchungsgruppe:</span> 75 Patienten, die sich seit mindestens einem Jahr in Substitutionsbehandlung befanden, wurden in die Studie eingeschlossen. Von 68 Patienten liegen vollständige Ergebnisse vor. </p><p> <span class="fett">Ergebnisse:</span> In keinem der Merkmale – psychische Befindlichkeit, depressive Verstimmungen, Ängstlichkeit, Drogenverlangen und Drogenkonsum – konnte ein mit der Umstellung des Substitutionsmittels zusammenhängender Effekt beobachtet werden. Kurzfristige, vorübergehende Befindlichkeitsschwankungen regulierten sich wieder auf einen Zustand, der vor Cross-over zu beobachten war. </p><p> <span class="fett">Schlussfolgerungen:</span> Levomethadon und d,l-Methadon können im entsprechenden Dosisverhältnis gleichberechtigt eingesetzt werden.

<i>Objective:</i> Studies on drug dependence show a high prevalence of comorbidity with additional mental disorders. Comorbidity patients also show more poly-substance use and other psychosocial problems. This study analyzed the importance of comorbidity for the long-term course of opiate dependence. <i>Method:</i> 350 opiate-dependent patients were examined at yearly follow-ups over 4 years using the EuropASI for the assessment of drug-related problems and the CIDI for diagnostic of psychiatric disorders. 196 patients were reached at final follow up (56%). <i>Results:</i> Of the patients reached at final follow-up, 30% had severe, 29% mild and 41% no clinically relevant disorders at baseline. However, the linear relationship at baseline – the more severe the disorder, the greater the impairment through drug-related problems – was not present at final follow-up. The results show that lifetime diagnosis of mental disorder had no prognostic relevance for the long-term course of drug dependency. <i>Conclusion:</i> The assumption that opiate users with an additional mental disorder are more vulnerable in their course of addiction could not be confirmed.

In order to evaluate the impact of consumption rooms (CRs) on the promotion of health among drug users and the reduction of the public nuisance of drug use, a study examining CRs in three countries was carried out in 2000. The study, supported by the European Commission, examined CRs in Rotterdam (The Netherlands), Innsbruck (Austria), and Hamburg (Germany). This paper evaluates the findings from the study of Hamburg CRs. In order to get evidence-based information on the effects of CRs, 616 drug users in the Hamburg drug scene or in the vicinity of low-threshold institutions were investigated with a standardized questionnaire. Qualitative problem-centered interviews were also carried out with staff members of the CRs, and representatives from the community, including neighborhood residents, business people, police, and politicians. Research findings show that the Hamburg CRs reached the target group of drug users who practice risky behaviors and engage in public drug use. Findings also show that the Hamburg CRs lead to positive changes in health-related behavior for drug users. In addition, the findings indicate that the Hamburg CRs played an important role in the reduction of public disturbances in the vicinity of open drug scenes.

The prevalence of suicide attempts among opioid addicts is reported to lie between 8% and 17%, with some studies reporting an even higher rate among special groups of addicts. Yet there is insufficient knowledge about which addicts have a higher risk of suicide and which other factors seem to be involved in the constellation leading to the suicide attempt; the study of different subgroups of addicts is thus necessary. This paper reports a study on suicide attempts among a sample population in a detoxification unit and another group undergoing codeine maintenance treatment. There was a high rate of suicide attempts among both groups (23% of those in the detoxification unit and 28% of those maintained on codeine), but treating physicians tended to underestimate suicidality. The results lead to the conclusion that among a subgroup of addicts suicidality plays a larger role; a greater emphasis should thus be placed on the underlying psychopathology and the treatment needed.
