"There is agency in all we do: thinking, doing, or making. We invent a tune, play, or use it to celebrate an occasion. Or we make a conceptual leap and ask more abstract questions about the conditions for agency. They include autonomy and self-appraisal, each contested by arguments immersing us in circumstances we don't control. But can it be true we that have no personal responsibility for all we think and do?

Agency: Moral Identity and Free Will proposes that deliberation, choice, and free will emerged within the evolutionary history of animals with a physical advantage: organisms having cell walls or exoskeletons had an internal space within which to protect themselves from external threats or encounters. This defense was both structural and active: such organisms could ignore intrusions or inhibit risky behavior. Their capacities evolved with time: inhibition became the power to deliberate and choose the manner of one's responses. Hence the ability of humans and some other animals to determine their reactions to problematic situations or to information that alters values and choices. This is free will as a material power, not as the conclusion to a conceptual argument. Having it makes us morally responsible for much we do. It prefigures moral identity.

Closely argued but plainly written, Agency: Moral Identity and Free Will speaks for autonomy and responsibility when both are eclipsed by ideas that embed us in history or tradition. Our sense of moral choice and freedom is accurate. We are not altogether the creatures of our circumstances.

Models of the etiology of adolescent antisocial behavior suggest that externalizing problems may reflect a susceptibility to crime exposure and a diminished capacity for emotion introspection. In this study, adolescents of Mexican origin completed a neuroimaging task that involved rating their subjective feelings of sadness in response to emotional facial expressions or a nonemotional aspect of each face. At lower levels of neural activity during sadness introspection in posterior cingulate and left temporoparietal junction, and in left amygdala, brain regions involved in mentalizing and emotion, respectively, a stronger positive association between community crime exposure and externalizing problems was found. The specification of emotion introspection as a psychological process showing neural variation may help inform targeted interventions to positively affect adolescent behavior.

A workshop "Electronic Health Records and Pulmonary Function Data: Developing an Interoperability Roadmap" was held at the American Thoracic Society 2019 International Conference. "Interoperability" is defined as is the ability of different information-technology systems and software applications to directly communicate, exchange data, and use the information that has been exchanged. At present, pulmonary function test (PFT) equipment is not required to be interoperable with other clinical data systems, including electronic health records (EHRs). For this workshop, we assembled a diverse group of experts and stakeholders, including representatives from patient-advocacy groups, adult and pediatric general and pulmonary medicine, informatics, government and healthcare organizations, pulmonary function laboratories, and EHR and PFT equipment and software companies. The participants were tasked with two overarching Aobjectives: 1) identifying the key obstacles to achieving interoperability of PFT systems and the EHR and 2) recommending solutions to the identified obstacles. Successful interoperability of PFT data with the EHR impacts the full scope of individual patient health and clinical care, population health, and research. The existing EHR–PFT device platforms lack sufficient data standardization to promote interoperability. Cost is a major obstacle to PFT–EHR interoperability, and incentives are insufficient to justify the needed investment. The current vendor–EHR system lacks sufficient flexibility, thereby impeding interoperability. To advance the goal of achieving interoperability, next steps include identifying and standardizing priority PFT data elements. To increase the motivation of stakeholders to invest in this effort, it is necessary to demonstrate the benefits of PFT interoperability across patient care and population health.
