BACKGROUND: In low- and middle-income countries (LMICs), multinational companies have become increasingly involved in addressing public health challenges. Dealing with companies as partners in health sector development creates new challenges for governments. We sought to develop an approach to assess the existence and effectiveness of governance structures that can ensure that industry-led public health initiatives contribute to development. METHODS: We developed a governance assessment tool based on the principles of the Paris Declaration for Aid Effectiveness and other related agreements. We applied it to the case of pharmaceutical companies' involvement in the Kenyan response to non-communicable diseases (NCDs). We gathered data for analysis through 46 stakeholder interviews and reviewing documents. RESULTS: The Kenyan government has informal norms in place regarding program governance and strategy, but it has yet to issue formal regulations. While enabling elements exist that support initiatives to develop in alignment with these norms, implementation is often hindered by a lack of resources. Currently, broad stakeholder support for filling these gaps has created a window of opportunity for action. CONCLUSION: The application of the proposed assessment tool illustrates its viability for assisting companies and governments alike in defining governance needs for industry-led public health initiatives. Our findings in Kenya provide example considerations for LMICs working to integrate industry-led public health programs into the health system. Bilateral and multilateral donors also have important roles in strengthening LMICs' capacities to govern multinational corporations' contributions to NCDs in particular, and development in general.

BACKGROUND: Low- and middle-income countries bear the highest burden of non-communicable diseases (NCDs) mortality and morbidity. Syria has undergone an epidemiological transition from infectious diseases to NCDs in the past decades. Despite the high prevalence of cardiovascular diseases (CVDs) and diabetes in Syria, little is known about medicines utilization or prescriptions for these diseases. The aims of this study are to present the patterns and rates of dispensing medicines used for CVDs and diabetes among patients with government health insurance in Syria and examine age, sex, and regional variation in the dispensing of these medicines. METHODS: Outpatient data from June 2018 to May 2019 on dispensed medicines for 81,314 adults with government health insurance were obtained. The dispensing rate was expressed as the number of defined daily doses (DDDs) per 1000 beneficiaries per day (DID). The DID is a measurement that is used in drug utilization research to control for differences or changes in population size between or within countries. The number of DIDs was adjusted according to beneficiaries' sex, age, and governorate. RESULTS: Beneficiaries received 302.09 DIDs of CVDs medicines and 35.66 DIDs of diabetes medicines, including 0.96 DID of insulin (2.99% of the total of diabetes medicines). CVDs and diabetes medicine dispensing rates were low during the study period and included very low rates of insulin dispensing compared to the dispensing rates of these medicines in other countries in East Mediterranean Region or in Europe. We found lower dispensing rates of CVDs medicines among female beneficiaries (249.59 DIDs) than male beneficiaries (388.80 DIDs). Similarly, the dispensing rates of diabetes medicines among female beneficiaries (29.42 DIDs) were lower than those among male beneficiaries (45.98 DIDs). Little is known about antibiotic uses at the population level in Syria. The aim of our study is to present outpatient antibiotic dispensing (OAD) patterns and rates for patients with health insurance in the parts of Syria that are controlled by the Syrian government using different indicators. Outpatient data on all dispensed antibiotics for 81,314 adults with health insurance were obtained and stratified according to age, sex, governorate and annual season. OAD was mainly expressed as the number of defined daily doses (DDDs) per 1000 people per day (DID). OAD patterns were assessed according to the anatomical therapeutic classification (ATC) and the Access, Watch and Reserve (AWaRe) classification. OAD was 20.13 DID. Amoxicillin/clavulanic acid and clarithromycin were the most dispensed antibiotics (5.76 and 4.4 DID, respectively). Overall, a predominant consumption of broad-spectrum antibiotics was noted. The Watch group of the AWaRe classification had the biggest percentage of OAD (13.26 DID), followed by the Access and the Reserve groups (6.55 and 0.17 DID, respectively). There was a significant difference in OAD between the sex and age groups. The seasonal and regional variations in OAD were also significant. Broad-spectrum antibiotics dispensing was high compared to other studies from different countries. These results are concerning, as they can contribute to antibiotic resistance.

AbstractThis paper develops new indicators that measure the strength of intellectual property rights (IPR) provisions in Chile's free trade agreements (FTAs). We use these new indicators to examine the extent to which FTAs with strong IPR provisions impact the volume, unit value and overall value of imported biologic medicines into Chile. We find that FTAs with more stringent IPR provisions increase both the volume and the unit value of imported biologics, suggesting greater availability of imported biologics at a higher price. Further research, however, is necessary to determine whether the increases in volume and unit prices of imports lead to greater universal access to biologics or greater inequity in access to these medicines.

AbstractOver the past four decades Bangladesh has built enough domestic productive capacity in the pharmaceuticals and related industries to generate manufacturing capacity and employment to provide access to medicines in the country and to become a modest exporter of medicines as well. This paper traces the role played by government policy in fostering Bangladesh's burgeoning pharmaceuticals sector and then examines the extent to which such policies would have been permissible under World Trade Organization (WTO) rules and the rules of recent trade and investment treaties. Bangladesh has not had to adhere to such rules given its status as a Least Developed Country (LDC) but will face those rules as it may graduate from LDC status in the coming years. We find that a significant amount of Bangladesh's policies would not have been permitted under the WTO, and even more policy space would be constrained under other regional and bilateral trade and investment treaties. These findings reveal that Bangladesh will face a series of challenges as it graduates from LDC status in its efforts to build its domestic pharmaceutical industry moving forward. Our findings also pinpoint challenges for current WTO and other trade and investment treaty members who now seek to build domestic productive capacity in this sector in the wake of the COVID-19 pandemic.

BACKGROUND: The implementation of pharmaceutical services in hospitals contributes to the appropriate use of medicines and patient safety. However, the relationship of implementation with the legal framework and organizational practice has not been studied in depth. The objective of this research is to determine the role of these two factors (the legal framework and organizational practice) in the implementation of pharmaceutical services in public hospitals of the Ministry of Health of Mexico. METHODS: Semi-structured interviews were conducted with four groups of actors involved. The analysis focused on the legal framework, defined as the rules, laws and regulations, and on organizational practice, defined as the implementation of the legal framework by related individuals, that is, how they put it into practice. RESULTS: The main problems identified were the lack of alignment between the rules and the incentives for compliance. Decision-makers identified the lack of managerial capacity in hospitals as the main implementation barrier, while hospital pharmacists pointed to poor regulation and the lack of clarity of the legal framework as the problems to consider. CONCLUSIONS: Although the legal framework related to hospital pharmaceutical services in Mexico is inadequate, organizational factors (such as adequate skills of professional pharmacists and the support of the hospital director) have facilitated gradual implementation. To improve implementation, priority should be given to evaluation and modification of the current legislation along with the development of an official minimum standard for activities and services in hospital pharmacies.
