PURPOSE: To determine the relationship between vitamin D status and upper respiratory tract infection (URTI) of physically active men and women across seasons (study 1). Then, to investigate the effects on URTI and mucosal immunity of achieving vitamin D sufficiency (25(OH)D ≥50 nmol·L-1) by a unique comparison of safe, simulated-sunlight or oral D3 supplementation in winter (study 2). METHODS: In study 1, 1,644 military recruits were observed across basic military training. In study 2, a randomized controlled trial, 250 men undertaking military training received either placebo, simulated-sunlight (1.3x standard erythemal dose, three-times-per-week for 4-weeks and then once-per-week for 8-weeks) or oral vitamin D3 (1,000 IU·day-1 for 4-weeks and then 400 IU·day-1 for 8-weeks). URTI was diagnosed by physician (study 1) and Jackson common cold questionnaire (study 2). Serum 25(OH)D, salivary secretory immunoglobulin A (SIgA) and cathelicidin were assessed by LC-MS/MS and ELISA. RESULTS: In study 1, only 21% of recruits were vitamin D sufficient during winter. Vitamin D sufficient recruits were 40% less likely to suffer URTI than recruits with 25(OH)D 0.05). Supplementation did not affect salivary SIgA or cathelicidin. CONCLUSION: Vitamin D sufficiency reduced the URTI burden during military training.

The relentless growth of multidrug resistance and generation of recalcitrant biofilm are major obstacles in treating wounds, particularly in austere military environments where broad-spectrum pathogen coverage is needed. Designed antimicrobial peptides (dAMPs) are constructed analogs of naturally occurring AMPs that provide the first line of defense in many organisms. RP557 is a dAMP resulting from iterative rational chemical structural analoging with endogenous AMPs, human cathelicidin LL-37 and Tachyplesin 1 and the synthetic D2A21 used as structural benchmarks. RP557 possesses broad spectrum activity against Gram-positive and Gram-negative bacteria and fungi, including recalcitrant biofilm with substantial selective killing over bacterial cells compared to mammalian cells. RP557 did not induce resistance following chronic passages of Pseudomonas aeruginosa and Staphylococcus aureus at subinhibitory concentrations, whereas concurrently run conventional antibiotics, gentamycin, and clindamycin, did. Furthermore, RP557 was able to subsequently eliminate the generated gentamycin resistant P. aeruginosa and clindamycin resistant S. aureus strains without requiring an increase in minimum inhibitory concentration (MIC) concentrations. RP557 was evaluated further in a MRSA murine wound abrasion infection model with a topical application of 0.2% RP557, completely eliminating infection. If these preclinical results are translated into the clinical setting, RP557 may become crucial for the empirical broad-spectrum treatment of wound pathogens, so that infections can be reduced to a preventable complication of combat-related injuries.

Abstract

Background
Antimicrobial peptides (AMPs) are key effectors of urinary tract innate immunity. Identifying differences in urinary AMP levels between younger and older adults is important in understanding older adults' susceptibility and response to urinary tract infections (UTI) and AMP use as diagnostic biomarkers. We hypothesized that uninfected older adults have higher urinary human neutrophil peptides 1-3 (HNP 1-3), human alpha-defensin-5 (HD-5), and human beta-defensin-2 (hBD-2), but lower urinary cathelicidin (LL-37) than younger adults.


Methods
We conducted a cross-sectional study of patients aged ≥18 years completing a family medicine clinic nonacute visit. Enzyme-linked immunosorbent assays were performed for AMPs. We identified associations between age and AMPs using unadjusted and multivariable linear regression models.


Results
Of the 308 subjects, 144 (46.8%) were ≥65 years of age. Comparing age ≥65 versus < 65 years, there were no significant differences in HNP 1-3 (p = .371), HD5 (p = .834), or LL-37 (p = .348) levels. Values for hBD-2 were lower in older adults versus younger (p < .001). In multivariable analyses, older males and females had significantly lower hBD-2 levels (p < .001 and p = .004). Models also showed urine leukocyte esterase was associated with increased levels of HNP 1-3 and HD5; hematuria with increased hBD-2; and urine cultures with contamination with increased HNP 1-3 and hBD-2.


Conclusions
Baseline urinary HNP 1-3, HD-5, and LL-37 did not vary with age. Older adults had lower baseline hBD-2. This finding has implications for the potential use of urinary AMPs as diagnostic markers and will facilitate further investigation into the role of innate immunity in UTI susceptibility in older adults.

Background. Today, allergic respiratory diseases in children, including asthma and allergic rhinitis, are a relevant medical and social problem associated with a high risk of disability, a significant reduction in the quality of life of patients and their families, and a significant financial burden on patients and the government. Bronchial asthma and allergic rhinitis often coexist, complicating the course of each other. Children with respiratory allergic diseases are particularly vulnerable to acute respiratory infections. Respiratory viruses, in turn, play an important role in the pathogenesis of bronchial asthma and allergic rhinitis, contributing to the development of diseases and acting as triggers for their exa­cerbations. One of the possible causes of predisposition to frequent acute respiratory infections in children with allergic pathology of the respiratory system may be a lack of nonspecific factors of mucosal immunity of the respiratory tract — endogenous cationic amphiphilic antimicrobial peptides, in particular, cathelicidin and human defensin. According to many studies, these antimicrobial peptides have antiviral, antimicrobial and immunomodulatory effects. However, nowadays there is insufficient data on the state of production of antimicrobial peptides and their impact on respiratory morbidity in children with allergic diseases. At the same time, some literature sources describe the positive effect of individual probiotic strains, in particular Lactobacillus rhamnosus GG, on the overall resistance to respiratory viral infection and the prevention of atopic diseases. The purpose of our research was to study the concentrations of antimicrobial peptides (HbD-2 and LL-37) in the secretion of the mucous membranes of the upper respiratory tract in children with bronchial asthma and allergic rhinitis, to clarify their role in protection against respiratory viral infection and to estimate the effectiveness of a probiotic preparation containing Lactobacillus rhamnosus GG in the prevention of acute respiratory infections and reduction of the frequency of virus-induced exacerbations of allergic pathology. Materials and methods. We conducted a clinical and laboratory examination of 76 children aged 7 to 17 years, 24 of whom were diagnosed with allergic rhinitis, 28 children — bronchial asthma, and 24 — bronchial asthma and allergic rhinitis. The control group consisted of 20 clinically healthy children of matched age and sex. Research methods also included the analysis of respiratory disease patterns, determination of concentrations of antimicrobial peptides: cathelicidin (LL-37), human β-defensin 2 (HbD-2) in the secretion of the upper respiratory tract by ELISA, and statistical analysis. Results. It was found that children with allergic diseases of the respiratory tract are characterized by a higher frequency of acute respiratory infections (30 %) with more frequent involvement of the lower respiratory tract in the inflammatory process (72 %), which causes a longer duration of disease (95 %) compared to healthy children. The children with allergic rhinitis and bronchial asthma had a significant decrease in the concentrations of antimicrobial peptides in the secretion of the upper respiratory tract compared with the control group. It was found that the level of antimicrobial peptide concentrations in the secretion of the upper respiratory tract is most negatively affected by the allergic inflammation severity, accompanied by a lack of symptom control, and the use of basic therapy and no symptoms have a positive effect. However, even in the presence of complete control of the symptoms of allergic diseases, the levels of antimicrobial peptides were significantly lower than in children of the control group. It was found that there is an inverse relationship between the levels of antimicrobial peptides and the frequency and duration of acute respiratory infections in children with allergic diseases, most pronounced in bronchial asthma. Conclusions. Taking a probiotic drug containing a strain of Lactobacillus rhamnosus GG for a month in addition to the basic therapy of allergic rhinitis and bronchial asthma contributed to a significant increase in levels of HbD-2 (average 27 %) and ­LL-37 (average 60 %) in secretions of the upper respiratory tract, which was accompanied by a decrease in the frequency of acute respiratory infections by an average of 26 % (including episodes involving the lower respiratory tract), a decrease in the average duration of acute respiratory infections by 22 % over the next year and a decrease in virus-induced exacerbations by 25.7 %. ; Актуальность. На сегодняшний день аллергические заболевания респираторного тракта, в частности бронхиальная астма и аллергический ринит, у детей являются актуальной медико-социальной проблемой, ассоциированной с высоким риском инвалидизации, существенным снижением качества жизни пациентов и их семей, а также значительной финансовой нагрузкой на пациентов и государство. Бронхиальная астма и аллергический ринит часто сосуществуют, обременяя течение друг друга. Дети, больные респираторными аллергическими заболеваниями, являются особенно уязвимыми к развитию острой респираторной вирусной инфекции (ОРВИ), а респираторные вирусы, в свою очередь, играют важную роль в патогенезе бронхиальной астмы и аллергического ринита, способствуя развитию заболеваний и выступая триггерами их обострений. Одной из возможных причин склонности к частым ОРВИ у детей с аллергической патологией дыхательной системы может быть недостаточность неспецифических факторов мукозального иммунитета респираторного тракта — эндогенных катионоактивных амфифильных антимикробных пептидов, в частности кателицидина (LL-37) и человеческих дефензинов. По данным ряда исследований, эти антимикробные пептиды проявляют противовирусное, антимикробное и иммуномодулирующее действие. При этом данных о состоянии продукции антимикробных пептидов и их влиянии на респираторную заболеваемость у детей с аллергическими заболеваниями в современной литературе недостаточно. В то же время в ряде литературных источников описано положительное влияние отдельных пробиотических штаммов, в частности Lactobacillus rhamnosus GG, на общую резистентность к респираторной вирусной инфекции и профилактику атопических заболеваний. Целью нашей работы стало изучение уровней антимикробных пептидов (человеческого β-дефензина 2 (HbD-2) и LL-37) в секрете слизистых оболочек верхних дыхательных путей у детей с бронхиальной астмой и аллергическим ринитом, выяснение их роли в защите от респираторной вирусной инфекции у данного контингента больных и оценка эффективности применения пробиотического препарата, содержащего Lactobacillus rhamnosus GG, с целью профилактики ОРВИ и снижения частоты вирус-индуцированных обострений аллергической патологии. Материалы и методы. Нами было проведено клинико-лабораторное обследование 76 детей в возрасте от 7 до 17 лет, из которых у 24 был диагностирован аллергический ринит, у 28 детей — бронхиальная астма, а у 24 — бронхиальная астма и аллергический ринит. Группу контроля составили 20 клинически здоровых детей соответствующего возраста и пола. Кроме общепринятых клинических, методы исследования включали анализ паттернов респираторной заболеваемости, определение уровней антимикробных пептидов LL-37, HbD-2 в секрете верхних дыхательных путей методами иммуноферментного и статистического анализа. Результаты. Установлено, что дети с аллергическими заболеваниями респираторного тракта характеризуются большей частотой ОРВИ (на 30 %) с более частым вовлечением в воспалительный процесс нижних дыхательных путей (на 72 %), что обусловливает более длительную продолжительность заболеваний (на 95 %) по сравнению со здоровыми детьми. У детей с аллергическим ринитом и бронхиальной астмой обнаружено существенное снижение уровней антимикробных пептидов в секрете верхних дыхательных путей по сравнению с группой контроля. На уровни антимикробных пептидов в секрете верхних дыхательных путей в наибольшей степени негативно влияет степень активности аллергического воспаления, сопровождающегося отсутствием контроля симптомов, а положительно — прием препаратов базисной терапии и отсутствие симптомов. Вместе с тем даже при наличии полного контроля симптомов аллергических заболеваний, то есть при минимальном уровне аллергического воспаления в дыхательных путях, уровни антимикробных пептидов были достоверно ниже, чем у детей контрольной группы. Установлено, что существует обратная связь между уровнями антимикробных пептидов и частотой и продолжительностью ОРВИ у детей с аллергическими заболеваниями, которая в наибольшей степени выражена при бронхиальной астме. Выводы. Прием пробиотического препарата, содержащего штамм Lactobacillus rhamnosus GG, в течение 1 месяца дополнительно к базисной терапии аллергического ринита и бронхиальной астмы способствовал достоверному повышению уровней HbD-2 (в среднем на 27 %) и LL-37 (в среднем на 60 %) в секрете верхних дыхательных путей, что сопровождалось снижением частоты ОРВИ в среднем на 26 % (в том числе и эпизодов с привлечением нижних дыхательных путей), уменьшением средней продолжительности ОРВИ на 22 % в течение следующего года и снижением вирус-индуцированных обострений на 25,7 %. ; Актуальність. На сьогодні алергічні захворювання респіраторного тракту, зокрема бронхіальна астма й алергічний риніт, у дітей є актуальною медико-соціальною проблемою, асоційованою з високим ризиком інвалідизації, суттєвим зниженням якості життя пацієнтів та їх сімей, а також значним фінансовим навантаженням на пацієнтів і державу. Бронхіальна астма й алергічний риніт часто співіснують, обтяжуючи перебіг одне одного. Діти, хворі на респіраторні алергічні захворювання, особливо вразливі до розвитку гострої респіраторної вірусної інфекції (ГРВІ), а респіраторні віруси, в свою чергу, відіграють важливу роль у патогенезі бронхіальної астми й алергічного риніту, сприяючи розвитку захворювань і виступаючи тригерами їх загострень. Однією з можливих причин схильності до частих ГРВІ у дітей з алергічною патологією респіраторної системи може бути недостатність неспецифічних факторів мукозального імунітету респіраторного тракту — ендогенних катіоноактивних амфіфільних антимікробних пептидів, зокрема кателіцидину (LL-37) та людських дефензинів. За даними низки досліджень, дані антимікробні пептиди проявляють противірусну, антимікробну та імуномодулюючу дію. При цьому даних щодо стану продукції антимікробних пептидів та їх впливу на респіраторну захворюваність у дітей з алергічними захворюваннями в сучасній літературі недостатньо. В той же час в низці літературних джерел описано позитивний вплив окремих пробіотичних штамів, зокрема Lactobacillus rhamnosus GG, на загальну резистентність до респіраторної вірусної інфекції та профілактику атопічних захворювань. Метою нашої роботи стало вивчення рівнів антимікробних пептидів (людського β-дефензину 2 (HbD-2) та LL-37) у секреті слизових оболонок верхніх дихальних шляхів у дітей з бронхіальною астмою й алергічним ринітом, з'ясування їх ролі у захисті від респіраторної вірусної інфекції у даного контингенту хворих та оцінка ефективності застосування пробіотичного препарату, що містить Lactobacillus rhamnosus GG, з метою профілактики ГРВІ та зниження частоти вірус-індукованих загострень алергічної патології. Матеріали та методи. Нами було проведено клініко-лабораторне обстеження 76 дітей віком від 7 до 17 років, з яких у 24 був діагностований алергічний риніт, у 28 дітей — бронхіальна астма, а у 24 — бронхіальна астма й алергічний риніт. Групу контролю становили 20 клінічно здорових дітей відповідного віку та статі. Окрім загальноклінічних, методи дослідження включали аналіз патернів респіраторної захворюваності, визначення рівнів антимікробних пептидів LL-37, HbD-2 у секреті верхніх дихальних шляхів методами імуноферментного та статистичного аналізу. Результати. Встановлено, що діти з алергічними захворюваннями респіраторного тракту характеризуються більшою частотою ГРВІ (на 30 %) з більш частим залученням у запальний процес нижніх дихальних шляхів (на 72 %), що обумовлює більшу тривалість захворювань (на 95 %) порівняно із здоровими дітьми. У дітей з алергічним ринітом і бронхіальною астмою виявлено суттєве зниження рівнів антимікробних пептидів у секреті верхніх дихальних шляхів порівняно з групою контролю. На рівні антимікробних пептидів у секреті верхніх дихальних шляхів найбільшою мірою негативно впливає ступінь активності алергічного запалення, що супроводжується відсутністю контролю симптомів, а позитивно — прийом препаратів базисної терапії та відсутність симптомів. Разом з тим навіть за наявності повного контролю симптомів алергічних захворювань, тобто при мінімальному рівні алергічного запалення в дихальних шляхах, рівні антимікробних пептидів були вірогідно нижчими, ніж у дітей контрольної групи. Встановлено, що існує зворотний зв'язок між рівнями антимікробних пептидів і частотою та тривалістю ГРВІ у дітей з алергічними захворюваннями, найбільш виражений при бронхіальній астмі. Висновки. Прийом пробіотичного препарату, що містить штам Lactobacillus rhamnosus GG, протягом 1 місяця додатково до базисної терапії алергічного риніту та бронхіальної астми сприяв вірогідному підвищенню рівнів HbD-2 (в середньому на 27 %) та LL-37 (в середньому на 60 %) у секреті верхніх дихальних шляхів, що супроводжувалось зниженням частоти ГРВІ в середньому на 26 % (в тому числі й епізодів із залученням нижніх дихальних шляхів), зменшенням середньої тривалості ГРВІ на 22 % протягом наступного року та зниженням вірус-індукованих загострень на 25,7 %.

Бронхиальная астма и аллергический ринит часто сосуществуют, обременяя течение друг друга. Дети, больные респираторными аллергическими заболеваниями, являются особенно уязвимыми к развитию острой респираторной вирусной инфекции (ОРВИ), а респираторные вирусы, в свою очередь, играют важную роль в патогенезе бронхиальной астмы и аллергического ринита, способствуя развитию заболеваний и выступая триггерами их обострений. Одной из возможных причин склонности к частым ОРВИ у детей с аллергической патологией дыхательной системы может быть недостаточность неспецифических факторов мукозального иммунитета респираторного тракта — эндогенных катионоактивных амфифильных антимикробных пептидов, в частности кателицидина (LL-37) и человеческих дефензинов. По данным ряда исследований, эти антимикробные пептиды проявляют противовирусное, антимикробное и иммуномодулирующее действие. При этом данных о состоянии продукции антимикробных пептидов и их влиянии на респираторную заболеваемость у детей с аллергическими заболеваниями в современной литературе недостаточно. В то же время в ряде литературных источников описано положительное влияние отдельных пробиотических штаммов, в частности Lactobacillus rhamnosus GG, на общую резистентность к респираторной вирусной инфекции и профилактику атопических заболеваний. Целью нашей работы стало изучение уровней антимикробных пептидов (человеческого β-дефензина 2 (HbD-2) и LL-37) в секрете слизистых оболочек верхних дыхательных путей у детей с бронхиальной астмой и аллергическим ринитом, выяснение их роли в защите от респираторной вирусной инфекции у данного контингента больных и оценка эффективности применения пробиотического препарата, содержащего Lactobacillus rhamnosus GG, с целью профилактики ОРВИ и снижения частоты вирус-индуцированных обострений аллергической патологии. Материалы и методы. Нами было проведено клинико-лабораторное обследование 76 детей в возрасте от 7 до 17 лет, из которых у 24 был диагностирован аллергический ринит, у 28 детей — бронхиальная астма, а у 24 — бронхиальная астма и аллергический ринит. Группу контроля составили 20 клинически здоровых детей соответствующего возраста и пола. Кроме общепринятых клинических, методы исследования включали анализ паттернов респираторной заболеваемости, определение уровней антимикробных пептидов LL-37, HbD-2 в секрете верхних дыхательных путей методами иммуноферментного и статистического анализа. Результаты. Установлено, что дети с аллергическими заболеваниями респираторного тракта характеризуются большей частотой ОРВИ (на 30 %) с более частым вовлечением в воспалительный процесс нижних дыхательных путей (на 72 %), что обусловливает более длительную продолжительность заболеваний (на 95 %) по сравнению со здоровыми детьми. У детей с аллергическим ринитом и бронхиальной астмой обнаружено существенное снижение уровней антимикробных пептидов в секрете верхних дыхательных путей по сравнению с группой контроля. На уровни антимикробных пептидов в секрете верхних дыхательных путей в наибольшей степени негативно влияет степень активности аллергического воспаления, сопровождающегося отсутствием контроля симптомов, а положительно — прием препаратов базисной терапии и отсутствие симптомов. Вместе с тем даже при наличии полного контроля симптомов аллергических заболеваний, то есть при минимальном уровне аллергического воспаления в дыхательных путях, уровни антимикробных пептидов были достоверно ниже, чем у детей контрольной группы. Установлено, что существует обратная связь между уровнями антимикробных пептидов и частотой и продолжительностью ОРВИ у детей с аллергическими заболеваниями, которая в наибольшей степени выражена при бронхиальной астме. Выводы. Прием пробиотического препарата, содержащего штамм Lactobacillus rhamnosus GG, в течение 1 месяца дополнительно к базисной терапии аллергического ринита и бронхиальной астмы способствовал достоверному повышению уровней HbD-2 (в среднем на 27 %) и LL-37 (в среднем на 60 %) в секрете верхних дыхательных путей, что сопровождалось снижением частоты ОРВИ в среднем на 26 % (в том числе и эпизодов с привлечением нижних дыхательных путей), уменьшением средней продолжительности ОРВИ на 22 % в течение следующего года и снижением вирус-индуцированных обострений на 25,7 %. ; Актуальність. На сьогодні алергічні захворювання респіраторного тракту, зокрема бронхіальна астма й алергічний риніт, у дітей є актуальною медико-соціальною проблемою, асоційованою з високим ризиком інвалідизації, суттєвим зниженням якості життя пацієнтів та їх сімей, а також значним фінансовим навантаженням на пацієнтів і державу. Бронхіальна астма й алергічний риніт часто співіснують, обтяжуючи перебіг одне одного. Діти, хворі на респіраторні алергічні захворювання, особливо вразливі до розвитку гострої респіраторної вірусної інфекції (ГРВІ), а респіраторні віруси, в свою чергу, відіграють важливу роль у патогенезі бронхіальної астми й алергічного риніту, сприяючи розвитку захворювань і виступаючи тригерами їх загострень. Однією з можливих причин схильності до частих ГРВІ у дітей з алергічною патологією респіраторної системи може бути недостатність неспецифічних факторів мукозального імунітету респіраторного тракту — ендогенних катіоноактивних амфіфільних антимікробних пептидів, зокрема кателіцидину (LL-37) та людських дефензинів. В той же час в низці літературних джерел описано позитивний вплив окремих пробіотичних штамів, зокрема Lactobacillus rhamnosus GG, на загальну резистентність до респіраторної вірусної інфекції та профілактику атопічних захворювань. Метою нашої роботи стало вивчення рівнів антимікробних пептидів (людського β-дефензину 2 (HbD-2) та LL-37) у секреті слизових оболонок верхніх дихальних шляхів у дітей з бронхіальною астмою й алергічним ринітом, з'ясування їх ролі у захисті від респіраторної вірусної інфекції у даного контингенту хворих та оцінка ефективності застосування пробіотичного препарату, що містить Lactobacillus rhamnosus GG, з метою профілактики ГРВІ та зниження частоти вірус-індукованих загострень алергічної патології. Матеріали та методи. У дітей з алергічним ринітом і бронхіальною астмою виявлено суттєве зниження рівнів антимікробних пептидів у секреті верхніх дихальних шляхів порівняно з групою контролю. На рівні антимікробних пептидів у секреті верхніх дихальних шляхів найбільшою мірою негативно впливає ступінь активності алергічного запалення, що супроводжується відсутністю контролю симптомів, а позитивно — прийом препаратів базисної терапії та відсутність симптомів. Разом з тим навіть за наявності повного контролю симптомів алергічних захворювань, тобто при мінімальному рівні алергічного запалення в дихальних шляхах, рівні антимікробних пептидів були вірогідно нижчими, ніж у дітей контрольної групи. Встановлено, що існує зворотний зв'язок між рівнями антимікробних пептидів і частотою та тривалістю ГРВІ у дітей з алергічними захворюваннями, найбільш виражений при бронхіальній астмі. Висновки. Прийом пробіотичного препарату, що містить штам Lactobacillus rhamnosus GG, протягом 1 місяця додатково до базисної терапії алергічного риніту та бронхіальної астми сприяв вірогідному підвищенню рівнів HbD-2 (в середньому на 27 %) та LL-37 (в середньому на 60 %) у секреті верхніх дихальних шляхів, що супроводжувалось зниженням частоти ГРВІ в середньому на 26 % (в тому числі й епізодів із залученням нижніх дихальних шляхів), зменшенням середньої тривалості ГРВІ на 22 % протягом наступного року та зниженням вірус-індукованих загострень на 25,7 %.

Vitamin D može smanjiti rizik od virusnih infekcija respiratornog trakta te smanjiti težinu i dužinu trajanja bolesti. Sama činjenica da stanice respiratornog epitela, monociti/makrofazi, dendritične stanice, T i B limfociti posjeduju receptore za vitamin D i da mogu autonomno aktivirati vitamin D koji onda djeluje autokrino i parakrino, ukazuje na značajan imunomodulatorni učinak vitamina D u urođenome i stečenome imunom odgovoru. Mehanizmi obrane od respiratornih virusa, uključujući i virus SARS-CoV-2, a koji su pod utjecajem aktivnog oblika vitamina D uključuju: indukciju stvaranja katelicidina antivirusnog proteina respiratornog epitela koji može smanjiti opterećenje virusom, poticanje urođenog imunog odgovora na virusnu infekciju, čuvanje integriteta respiratornog epitela kroz učvršćivanje uskih spojeva između stanica te kroz smanjenje prekomjernog stvaranja proupalnih citokina i pojačanje stvaranja protuupalnih citokina omogućavajući uravnotežen imuni odgovor na infekciju, time smanjujući nepotrebno oštećenje tkiva i takozvanu citokinsku oluju i posljedičnu akutnu ozljedu pluća koja u najtežim slučajevima bolesti COVID-19 rezultira respiratornim distres sindrom. Nadoknada vitamina D je jednostavna, sigurna i jeftina te bi trebala biti neizostavni dio javnozdravstvene politike sprječavanja zaraznih bolesti uključujući i COVID-19, posebice stoga što su za vrijeme pandemije ljudi dodatno izloženi nedostatku vitamina D jer su prisiljeni boraviti u zatvorenom prostoru, čime je onemogućen glavni put sinteze vitamina D u koži djelovanjem UV zraka. Za vrijeme pandemije virusom SARS-CoV-2 racionalno je nadoknadu provesti višim dozama, do 10 000 IJ dnevno, kako bi se u kratkom vremenu postigle koncentracije u krvi potrebne za nekalcemijske učinke vitamina D. ; Vitamin D may reduce the risk of respiratory tract viral infection, and the severity and duration of the disease as well. Respiratory epithelial cells, monocytes/macrophages, dendritic cells, T and B lymphocytes express vitamin D receptors, and can autonomously activate vitamin D. This points to a significant immunomodulatory effect of vitamin D in the innate and acquired immune response to viral infection. Defense mechanisms against respiratory viruses, including SARS-CoV-2, that are under the influence of the active form of vitamin D are: enhanced production of cathelicidin - an antiviral protein of the respiratory epithelium, induction of innate immune response to viral infection, preservation of the integrity of the respiratory epithelium, suppression of pro-inflammatory cytokine production and enhancement of anti-inflammatory cytokine production. The immunomodulatory effect of vitamin D supports a balanced immune response to viral infection and may reduce the risk of a cytokine storm and consequent acute lung injury, which in the most severe cases of COVID-19 disease results in respiratory distress syndrome. Vitamin D supplementation is simple, safe and inexpensive and should be an indispensable part of public health policy to reduce the risk of infectious diseases, including COVID-19. During a pandemic, the risk of vitamin D deficiency is increased because people are forced to stay indoors, thus disabling the main route of vitamin D synthesis in the skin under the effects of ultraviolet light. During the SARS-CoV-2 pandemic, Vitamin D supplementation should be prescribed with higher doses, up to 10,000 IU per day, to more rapidly achieve blood concentrations required for non-calcemic effects of vitamin D.

[Abstract] Mycobacteria of the Mycobacterium tuberculosis complex (MTBC) greatly impact human and animal health worldwide. The mycobacterial life cycle is complex, and the mechanisms resulting in pathogen infection and survival in host cells are not fully understood. Eurasian wild boar (Sus scrofa) are natural reservoir hosts for MTBC and a model for mycobacterial infection and tuberculosis (TB). In the wild boar TB model, mycobacterial infection affects the expression of innate and adaptive immune response genes in mandibular lymph nodes and oropharyngeal tonsils, and biomarkers have been proposed as correlates with resistance to natural infection. However, the mechanisms used by mycobacteria to manipulate host immune response are not fully characterized. Our hypothesis is that the immune system proteins under-represented in infected animals, when compared to uninfected controls, are used by mycobacteria to guarantee pathogen infection and transmission. To address this hypothesis, a comparative proteomics approach was used to compare host response between uninfected (TB-) and M. bovis-infected young (TB+) and adult animals with different infection status [TB lesions localized in the head (TB+) or affecting multiple organs (TB++)]. The results identified host immune system proteins that play an important role in host response to mycobacteria. Calcium binding protein A9, Heme peroxidase, Lactotransferrin, Cathelicidin and Peptidoglycan-recognition protein were under-represented in TB+ animals when compared to uninfected TB- controls, but protein levels were higher as infection progressed in TB++ animals when compared to TB- and/or TB+ adult wild boar. MHCI was the only protein over-represented in TB+ adult wild boar when compared to uninfected TB- controls. The results reported here suggest that M. bovis manipulates host immune response by reducing the production of immune system proteins. However, as infection progresses, wild boar immune response recovers to limit pathogen multiplication and promote survival, facilitating pathogen transmission. ; [Author Summary] Mycobacteria of the Mycobacterium tuberculosis complex (MTBC) are zoonotic pathogens representing a serious health problem for humans and animals worldwide. The life cycle of mycobacteria is complex, and the mechanisms resulting in pathogen infection and survival in host cells are not fully understood. Eurasian wild boar are natural reservoir hosts for MTBC and a model for mycobacterial infections and tuberculosis. The results of this study broaden our understanding of the molecular epidemiology of zoonotic tuberculosis and fill important gaps in knowledge of this topic. The results suggested that mycobacteria manipulate host immune response by reducing the production of immune system proteins. However, as infection progresses, wild boar immune response recovers to limit pathogen multiplication and promote survival, facilitating pathogen transmission. As previously reported in other obligate intracellular bacteria, host-mycobacteria interactions probably reflect a co-evolutionary process in which pathogens evolved mechanisms to subvert host response to establish infection, but hosts also evolved mechanisms to limit pathogen infection and promote survival. Subsequently, mycobacteria benefit from host survival by increasing the probability for transmission to continue their life cycle.
