Objectives: The purpose of this study is to figure out the current state of patient safety research in Korea and to evaluate whether the research fund support project for patient safety research is proceeding in priority order. Methods: Through the literature search, a list of research projects in the field of patient safety in Korea were collected as of January 6, 2017. Four researchers independently evaluated the subject using the priority list of patient safety research derived from the previous study. Results: As of January 2017, a total of 21 research projects were conducted in the Korea Health Industry Development Institute and 42 research studies were conducted in the National Science & Technology Information. The results of the priority evaluation showed that there were no researches that ranked first and second priority. Many of the research topics were �쁀dverse drug events/drug errors,�� �쁇ealth information technology/information systems,�� and �쁋ess relevance.�� Conclusion: Patient safety research projects are not only diverse but also quantitative in Korea. Support for research topics that need to be prioritized is needed, such as developing safety indicators and measuring patient safelty levels, improving communication, and improving patient safety incident reporting systems. ; open

Purpose: The purpose of this study was to define and clarify the concept of political competence for nurses. Methods: A hybrid model method was used to investigate the dimensions, attributes, and definitions of the concept. In the theoretical stage of the study, literature on nursing, politics, and other discipline were reviewed. In the fieldwork stage, individual in-depth interviews and focus groups interviews were conducted with politically seasoned experts or activists who had an understanding of the concept of political competence for extensive descriptions in nursing and field of health care. Results: The concept of political competence was represented in four dimensions as political knowledge, political efficacy, political interaction, and political activity. In the political knowledge dimension, there were three attributes, namely, political knowledge, political information and systematic analysis ability. The political efficacy dimension had three attributes of internal political efficacy, external political efficacy, and self-pride of nursing profession. The political interaction dimension had three attributes of organizations and community service, networking, and persuasive power. The political activity dimension had six attributes of political leadership, political expression, assertive behavior, political advocacy, political participation, and policy intervention. Conclusion: This concept development might provide a basic understanding of developing a measurement tool and for constructing a theory promoting nurses�� political competence. ; open

Anesthesia facilitates surgery in millions of young children every year. Structural brain abnormalities and functional impairment observed in animals have created substantial concerns among clinicians, parents, and government regulators. Clinical studies seemed ambivalent; it remains unclear whether differential species effects exist towards anesthetic exposure. The current literature search and analysis attempts to unify the available clinical and animal studies, which currently comprise of > 530 in vivo animal studies and > 30 clinical studies. The prevalence of abnormalities was lowest for exposures < 1 hour, in both animals and humans, while studies with injurious findings increased in frequency with exposure time. Importantly, no exposure time, anesthetic technique, or age during exposure was clearly identifiable to be entirely devoid of any adverse outcomes. Moreover, the age dependence of maximum injury clearly identified in animal studies, combined with the heterogeneity in age in most human studies, may impede the discovery of a specific human neurological phenotype. In summary, animal and human research studies identify a growing prevalence of injurious findings with increasing exposure times. However, the existing lack of definitive data regarding safe exposure durations, unaffected ages, and non-injurious anesthetic techniques precludes any evidence-based recommendations for drastically changing current clinical anesthesia management. Animal studies focusing on brain maturational states more applicable to clinical practice, as well as clinical studies focusing on prolonged exposures during distinct developmental windows of vulnerability, are urgently needed to improve the safety of perioperative care for thousands of young children requiring life-saving and quality of life-improving procedures daily. ; open

학위논문 (석사)-- 서울대학교 대학원 : 행정학과, 2012. 2. 김상헌. ; 중앙정부는 지방정부의 부족한 재원을 지방교부세, 국고보조금과 같은 지방재정조정제도를 통해 보전하고 있다. 우리나라의 경우 지방 재정조정제도의 하나인 지방교부세에 보통교부세와 특별교부세가 있 다. 특별교부세는 보통교부세의 경직적 산정에 따른 배분의 한계를 보완하여 각 지방재정력의 격차를 고려하여 보전한다는 방침으로 행 정안전부에서 운영하고 있다. 그러나 특별교부세는 배분의 기준과 결 정이 모호하고 불투명하여 그 결정이 정치적으로 이루어진다는 문제 점을 지적하는 연구가 있다. 선행연구로 이루어진 특별교부세 배분에 관한 실증분석에서는 배분의 결정에 영향을 미친다고 상정한 정치적 행위자를 국회 행정안전위원회 소속, 지역구 국회의원 수 등 의회의 영역으로만 한정하였다. 본 논문은 정치적 요인에 의회의 행위자뿐만 아니라 정부의 행위자 를 포함시켜 분석하였다. 본 논문은 전국 234개 시‧군‧구의 2003~2006년도 4개년도의 특별교부세 자료를 이용하여 특별교부세 배분의 결정에 영향을 미치는 요인들에 대해 정부 정무직 공직자를 포함하여 실증분석을 시도하였다. 그 결과 정부 정무직 공직자로 상 정한 변수가 배분의 결정에 영향을 미친다고 파악되었고, 정부 정무 직 공직자 중에서도 국무총리와 국무조정실‧차장이 다른 정무직 공직 자에 비해 더 큰 영향을 미친다고 파악되었다. 특별교부세 배분에 정무직 공직자가 자기 출생지에 더 많이 배분하 려 결정 과정에 영향을 미치는 것뿐만 아니라 정무직 공직자가 임기 후 선출직에 출마할 경우 특별교부세 배분의 결정에 더 큰 영향을 미 친다는 것을 보았다. 이로써 특별교부세 배분의 결정에 정부의 정무 직 공직자가 영향을 미친다는 것을 확인할 수 있었다. ; The national government makes up for any shortfall in finance of the local governments by the local finance coordination system, which includes local revenue sharings and national subsidies. The local revenue sharing, as a kind of the local finance coordination system, consists of an ordinary revenue sharing and a special revenue sharing. The latter is introduced to remedy the shortcomings of the former's allocation system caused by the rigidity of its calculation, and is currently operated by the Ministry of Public Administration and Security. There are some literature criticizing the problem that the SRS allocation is decided politically due to the fact that the principles for its allocation are equivocal and the decisions are arbitrary and not transparent. In previous studies analyzing the SRS allocation empirically, the scope of the political actors who are assumed to exert influences on the allocation decision is limited to the legislative area, such as the members of the National Assembly Public Administration and Security Committee and the number of lawmakers in each electoral district. This study presents a further analysis with a new political factor, the government actors, in addition to the existing legislative actors. Specifically, using the SRS data for the period of 2003 ~ 2006 for the entire 234 cities- counties-districts in the country, the present study attempts to analyze the factors, which affect SRS allocation decisions, including the new variable, the government's political masters. The results demonstrate that the new variable indeed affects the allocation decisions and that, among the political masters, prime ministers and directors generals and deputy directors in the Prime Ministers Office are shown to exert greater influences than others. Moreover, it is also shown that they exercise their power in the SRS allocation decision making process to provide more sharings to their places of birth, and in particular, their intervention is even greater when they decide to run for an elective office from their places of birth after the termination of their government offices. As such, this study verifies that the government's political masters exert influences on the SRS allocation decision. ; Master

학위논문 (석사)-- 서울대학교 대학원 : 정치학과, 2012. 2. 권형기. ; 본 연구는 발전국가 이후 한국 자본주의 제도 변화 과정에서 나타난 국가 성격 전환에 관한 연구이다. 구체적인 사례로 하도급 제도를 대상으로 세계화 과정에서 국가 성격의 변화를 경험적으로 밝히고 이의 이론적 성격을 규명하고자 한다. 발전국가의 변천에 관한 기존의 연구는 주로 시장국가 혹은 최소주의적 규제국가라는 단일한 국가 모델로 수렴할 것인가라는 테제를 중심으로 접근해 왔다. 세계화론자들은 자본의 이동이 자유로워지고 자본을 유치하기 위한 경쟁이 치열해지면서 발전국가가 변화하는 세계경제에 부적합하기 때문에 단일한 규제국가 모델로 수렴할 것이라 예측한다. 반면, 신제도주의자들은 각 국가마다 착종된 제도와 문화가 다르기 때문에 "발전국가 이후" 규제국가로의 전환은 발전국가의 개입주의적 성격이 유지되는 경로의존적 제도 변화를 겪게 된다고 주장한다. 발전 경로가 다양할 수 있다는 점에서 제도주의의 입장에 동의하지만 한국의 국가성격 전환에 대한 기존 연구는 발전국가의 유산에만 천착하여 새롭게 재편되는 국가의 역할에 주목하지 못하고 있다. 본 연구에서는 개입주의의 유산을 통해 발전국가의 연속성을 찾는 기존 연구의 문제제기를 벗어나, 시장자유화라는 세계화의 일반적 추세에 따라 한국의 국가 성격 역시 규제국가로의 수렴 과정에 있다는 명제 하에서 한국의 규제국가가 갖는 다양성의 동학을 살피고자 했다. 한국의 발전 모델이 규제국가로 전환된 것은 사실이나 신자유주의적 국가가 상정하는 최소주의적 국가로 변화한 것은 아니다. 오히려 시장질서의 유지를 위한 협치적 개입주의라는 독특한 규제국가 모델을 형성하고 있다. 본 연구는 기존의 국가전환론이 지나치게 정태적인 분석 방법을 사용하여 비교균형론에 머무르는 한계를 지적하고 협치적 규제국가로의 전환을 이해하기 위해 행위자 중심의 이념, 사고, 담론의 정치적 과정을 분석하였다. 한국의 산업화 시기 형성된 발전연합은 공정거래법과 하도급법과 같은 경쟁법의 제도화를 억제했다. 80년대 신자유주의 관료의 등장과 정치엘리트의 정책 기조 전환이 맞물리면서 현재의 하도급 제도의 기원이 형성될 수 있었다. 이후 하도급법의 확대 및 강화를 통해 신자유주의적 규제국가가 아닌 질서 자유주의를 기반으로 하는 규제국가 모델로 재편되는 모습을 보여주었다. 이렇듯 한국의 규제국가가 '최소주의적'인 국가는 아니었지만 불충분한 제도화로 인해 자본의 양극화 문제를 충분히 해소하는 데에는 부족했다. 규제국가의 한계를 인식한 정부는 규제보다 사회적 규범을 통한 문제 해결을 선호했기 때문에 하도급법 강화에 저항적이었다. 이로 인해 중소기업의 불만을 조직하여 대변하는 정치세력의 연동제 도입 노력은 실패로 돌아가고 대신 점진적 개혁이 이뤄졌다. 자본의 패권에 대한 인식과 규제 국가의 능력에 대한 의심에도 불구하고 정부는 탈규제와 시장 방임으로 나아가지는 않았다. 오히려 사회적 규범을 형성하기 위한 사회적 조정을 시도하는 적극적 협치주의를 추구했다. 동반성장위원회는 공정거래위원회와 달리 사회적 합의 기구라는 제도적 조건과 적극적인 정치적 의지로 인해 이익공유제, 업종 적합성 등 중소기업의 불만을 반영한 다양한 대안들을 제시하여 대기업 측을 압박할 수 있었다. 전경련은 집단행동을 통해 이익공유제에 대한 항의를 전달했지만 법제화에 대한 두려움으로 동반위에서의 이탈보다는 강력하게 반대해 온 납품단가 연동제를 수용했다. 결론적으로 한국의 국가 성격은 발전주의 시기 이후 규제국가로의 수렴을 겪었다고 볼 수 있지만 이는 신자유주의 이념에 의한 것이 아니라 질서자유주의에 입각한 규제국가로의 전환이었다. 그러나 규제국가의 딜레마로 인해 강성규제국가보다는 협치적 규제국가라는 타협적 제도화가 이뤄지고 있다. 연성 규범 형성을 위한 협치적 개입주의는 사회적 조정을 통한 제도 변화의 급진성을 가져올 수 있는 새로운 가능성을 보여주지만 강제력의 부재라는 한계 역시 내포하고 있다. ; This study examines how the form of the Korean developmental state has changed in the process of economic liberalization. More specifically, the study analyzes the transformation of the Korean state empirically and theoretically investigating the changes in the subcontracting institutions under the globalization. The main question of the previous research on the tranformation of the developmental state is whether the state develops into one national model, 'minimalist state'. Capital mobility and the competition for capital make the interventionist states insufficiently competitive. Neoliberals predict the convergence of the national diversity under globalization. Institutionalists, on the other hand, insist that although the interventionist state has developed into a regulatory state, the direction of transformation is determined by path dependence. However, the literature on the transformation of the Korean state has paid little attention to the newly constructed role of the state. Unlike the previous literature, this research examines the dynamic process of the transformation of the Korean regulatory state. It is true that although the Korean state has changed from developmental model to regulatory state, it is distinct from minimalist model based on neo-liberal ideology. Rather, a new regulatory model has been formed. State intervention occurs not as a form of transformed developmental intervention, but as a form of governance. In order to understand this transformation, this study analyzes the political process based on the discursive institutionalism. The growth coalition blocked the introduction of the competition law in the era of industrialization. The newly empowered neoliberal bureaucrats and the turn of the policy stance under the new junta regime made it possible to enact the competition law. Since 1980s, the subcontracting law has been gradually reinforced. After enacting the law, the Korean state has developed into the regulatory state based on the ordo-liberalism, not on the neoliberalism. However, it is insufficient for the change to close the widening gap between big businesses and small businesses. The government prefers constructing the governance to strengthening the law in order to resolve the problem. Facing the government's resist, political parties' efforts to rapidly change the law failed. Although the government concerned about the capital mobility, it did not seek de-regulation and laissez-faire policy. Rather, the government tried to form a social norm using the governance. Unlike Fair Trade Commission, the Commission on Shared Growth for largecorporations and Small and medium Enterprises exerted pressure on the big business by proposing alternative plans including profit sharing and lists for SMB business items. Although the Federation of Korean Industries took collective action against the profit sharing plan to adjust the cost of subcontracting to the changes in raw material prices, it had to accept the plan which the FKI had strongly opposed. In conclusion, the Korean state developed into the regulatory state after the "developmental state". However, it does not mean that the role of the state becomes minimal as the neo-liberalsts argue. Nor does it that the state create a new role dependent on the developmental legacy. Using a new-formed governance, the Korean state tried to change the subcontracting institutions. ; Master

학위논문 (석사)-- 서울대학교 대학원 : 외교학과, 2012. 2. 신범식. ; 본 논문의 핵심 질문은 색깔 혁명 발생으로 민주화에 성공한 그루지야와 우크라이나의 이후 민주주의 발전 양상은 왜 달랐는가이다. 색깔 혁명 이후, 우크라이나의 민주주의는 진전을 보였지만, 그루지야는 그렇지 못했다. 본 연구는 색깔혁명 이후 미국과 러시아의 갈등 구도와 양국의 대(對) 그루지야, 우크라이나 정책이 각국의 국내 엘리트-대중 관계에서 엘리트 자율성, 그리고 민주화에 미친 영향에 주목하였다. 9.11 테러 이후 미국과 러시아는 협력 체제에 있었지만, 색깔 혁명의 발생으로 인해 양국은 갈등 구도에 들어서게 된다. 그루지야와 우크라이나는 미․러 갈등 구도의 중심에 있었다. 활용가능성의 요인으로 인해, 그루지야는 우크라이나보다 전략적 가치가 더 큰 곳이었다. 미․러 갈등 구도 속에서 그루지야 집권 엘리트들은 높은 엘리트 자율성을 가질 수 있었다. 미국은 그루지야에게 큰 경제 원조, 정치적 지원을 하였다. 또한 그루지야-러시아 관계 악화와 2008년 전쟁 발발은 정권의 권위주의 정책을 정당화하고 국내를 결집시키는 기제로 작용하여 그루지야 집권 엘리트들이 높은 자율성을 갖게 해주었고, 이는 민주화 저해로 이어졌다. 우크라이나는 그루지야와 반대였다. 미․러 갈등 구도 속에서 우크라이나 엘리트들은 낮은 자율성을 가졌다. 우크라이나의 집권 엘리트들은 서방과 러시아 양측 모두로부터 확고한 정치적, 경제적 지원을 얻을 수 없었다. 오히려 러시아의 정책으로 인해 국내의 반 오렌지, 대항 엘리트가 공고해지면서, 집권 엘리트의 자율성은 낮아졌다. 엘리트들의 낮은 자율성과 대중에 대한 높은 의존성은 대중 영합주의 정책과 정국 불안의 결과를 낳았으나, 이는 오렌지 혁명 이후 우크라이나 민주주의의 발전을 제한적으로나마 가능하게 해주었다. 본 연구는 민주화 연구에서 그동안 비교적 주목을 덜 받아온 국제적 요인을 다루고, 외교 정책, 국내 엘리트-대중 관계, 민주화의 연계를 통합적으로 보여준다는 점에서 함의가 있다. 본고는 많은 기존 연구들이 상정해 온, 민주화에 대한 서구 민주화 증진 정책의 긍정적 영향과 권위주의 국가의 부정적 영향이라는 단순한 명제의 한계를 드러내주고 있다. 또한 민주주의 지원 정책에서 민주화 단계별로 정책을 달리할 필요성에 대해서 시사한다. ; What caused the difference in democratic trajectories between Georgia and Ukraine after the Color Revolutions? Since the occurrence of the Color Revolutions, Georgia's democracy has laid in tatters, whereas Ukraine has achieved a relatively successful democracy. Georgia and Ukraine share similar domestic determinants which have been discussed in previous democratization literature. This study, however, focuses on the international factor as an explanation for the different democratic fates of democracy between the two countries. Comparing both countries, this thesis uses a framework that integrates international factors, elite autonomy in domestic elite-mass relations, and democratization. The study argues that international factors, specifically the foreign policies of the US and Russia, provide an explanation for the difference in democratic outcomes for the two countries. Following the events of September 11, 2001, US-Russian relations improved. However, the occurrence of the Color Revolutions caused deterioration in the ties between the two countries, and both started to adopt different foreign policies towards the post-Soviet region. Caught in the middle of this conflict between the two great powers were Georgia and Ukraine. The unconditional US support for Georgia and Russia's assertive policy towards Georgia which culminated in the Russian-Georgian War in 2008 enabled the Georgian elites to attain high elite autonomy, thus resulting in democratic stagnation for Georgia. In contrast, Ukraine did not garner the support of either the US or Russia; rather, it was pressured by Russia. The anti-Orange elites in Ukraine were effectively supported by Russia, and, thus, were able to function as a forceful political opposition to the incumbent elites. Therefore, the Ukrainian elites only garnered low autonomy, which resulted in democratic progress in Ukraine despite domestic political chaos. This study contributes to further understanding of the impact of international factors upon democratization. In addition, this thesis creates implications for democracy promotion policy. The conclusion of the thesis challenges the predominant assumption that Western democracy promotion is conducive to actually democratization, and that authoritarian countries' authoritarian promotion policy is harmful to democratization. In the process of democracy promotion policy, policymakers and implementers need both careful monitoring of the process of democratic assistance and coordination with other countries. ; Master

학위논문 (석사)-- 서울대학교 대학원 : 정치학과, 2012. 2. 박찬욱, 강원택. ; 본 연구는 제18대 국회 전반기 2년간 본회의 표결 기록을 분석하여 개인 의원의 당론이탈투표 행위를 결정하는 유의미한 변수를 찾아내는데 그 목적이 있다. 구체적으로 본 논문은 강한 정당기율의 전통에도 불구하고 국회의원들이 표결 현장에서 당론을 이탈해 독립적으로 행동하는 현상을 목격하고, 이를 결정하는 요인을 분석하고자 한다. 이러한 목적에서 18대 국회 전반기동안 의원직을 유지한 한나라당과 민주당 의원 241명의 본회의 전자표결 기록을 분석하여 이들이 당론과 다른 표결을 하도록 결정하는 요인을 찾는다. 한국의 국회 내 표결행태에 관한 다양한 연구는 의원 행위를 결정하는 다양한 요인 중 정당의 제도적 영향력을 크게 평가한다. 이에 따르면 한국의 정당은 형성기부터 권위주의 시기를 거치면서 그 기율을 강화하였고, 의원들의 행위를 제한하고 통제하는 역할을 수행해왔다. 그러나 국회의원이 단순히 정당기율만을 따라 수동적으로 행동하는 존재가 아니라는 증거 또한 여럿 찾아볼 수 있다. 구체적으로 표결의 영역에서 의원들은 당론에 배치되는 두 가지 선택을 할 수 있다. 즉, 의원은 적극적으로 본회의에서 당론에 일치하지 않는 방향으로 투표하거나 소극적으로 기권이나 표결불참을 택할 수 있다. 비록 많은 비중을 차지하지 않더라도 국회의원들이 당론으로부터 이탈하여 독립적으로 표결하는 경우가 발견된다는 사실은 강한 정당기율만을 강조해왔던 기존의 연구 전통에서 벗어나 새로운 관점에서 의원 표결을 바라볼 필요성을 제기한다. 이러한 관점에서 본 연구는 의원을 전략적 행위자로 가정하고, 18대 국회 전반기에 의원들이 당론을 이탈하여 표결하는 데 영향을 미친 유의미한 결정요인을 찾는데 주목하였다. 이를 위하여 특정 의원의 소속 정당 다수가 투표한 선택지를 당론으로 정의하고, 개인 의원이 모든 표결에 대하여 당론을 이탈하여 투표한 비율을 종속변수로 설정하였다. 연구는 표결불참을 분석대상에 포함한 포괄적 당론이탈투표 모형과 표결불참을 제외한 단순 당론이탈투표 모형의 두 차원으로 구성된다. 의원의 당론이탈투표를 결정하는 요인을 찾기 위하여 제도적, 지역구적, 의원 개인적 요인의 세 가지로 분류된 독립변수를 설정하였고, 가중최소제곱 그룹 로지스틱 회귀분석을 통한 계량 분석을 시행하였다. 연구 결과는 다음과 같이 요약될 수 있다. 첫째, 포괄적 당론이탈투표 모형에서 국회의원들이 당론을 이탈하여 투표하는 데 영향을 미친 유의미한 결정요인은 제도적 변수로 밝혀졌다. 먼저, 국회의원들은 그들이 정당으로부터 자율성을 획득할 때 정당 다수로부터 이탈하여 표결할 가능성이 높다. 구체적으로 당내에서 경험과 연륜을 통하여 비공식적인 리더십을 획득하고 있는 선수가 높은 의원들이 상대적으로 더 자율적이므로 당론을 이탈할 가능성이 높다. 반면, 정당 지도부에 위치한 의원들은 정당 전체를 책임져야 하는 입장에 있으므로 정당으로부터 자유로운 선택을 할 수 없으며 따라서 당론을 고수할 가능성이 높다. 둘째, 단순 당론이탈투표 모형에서 그 동안 크게 주목받지 못하였던 변수 중 하나인 이념이 유의미한 변수로 확인되었다. 분석에 따르면, 진보적인 의원일수록 당론으로부터 이탈하여 표결할 가능성이 높다. 특히, 표결불참을 분석에서 제외했을 때 이념이 중요한 결정요인으로 대두되었다는 사실이 시사하는 바는 크다. 의원들이 표결을 통해 전면적으로 당론에 배치되는 입장을 표명할 수 있는 것은 정당이 줄 수 있는 제도적 인센티브를 뛰어넘는 개인의 강한 이념적 속성 때문이라는 것이다. 그러나 본 연구에서 이념 변수는 정책과 이념과의 연계성 측면에서 해석된 것이 아니라 의원이 당론을 따를 것인가 이탈할 것인가를 결정하는 일반적인 이념성향으로, 이념이 정책과의 연계 속에서 당론이탈투표에 직접적으로 미치는 영향력을 측정하지는 못하였다는 한계를 갖는다. 본 연구는 건강한 대의 민주제도를 수립하는 데 필요한 요인들에 대한 함의를 제공하였다는 점에서 의의를 갖는다. 당론이탈투표의 결정요인을 연구한 결과 당론이탈투표가 의원들의 제도적 자율성에서 비롯된 것임을 알 수 있었다. 이는 그동안 많은 학자들이 한국 국회의 갈등과 비효율성을 고질적인 문제로 지적하면서 그 원인으로 강한 정당기율과 당내 비민주성을 꼽아온 것과도 일맥상통하는 결과이다. 나아가, 이념이 당론으로부터의 이탈을 촉진하는 요소 중 하나임이 밝혀졌다는 점에서 의원들이 보다 자율성을 가질 수 있는 민주적인 원내외 정당구조가 확립된다면 의원들이 보다 이념 등에 기반하여 표결할 수 있는 정치적 환경이 확립될 것이라 예상할 수 있다. ; The purpose of this study is to identify meaningful variables effecting individual national assembly member's party defection votes by analyzing floor voting records for the first two years of the 18th National Assembly of the Republic of Korea. Specifically, the study will analyze the factors that result in party defection voting behavior by assembly members, a practice that betrays the tradition of strong party discipline. To this end, the study analyzes the roll-call voting record of 241 Grand National Party and Democratic Party assembly members having held office during the first half of the 18th National Assembly to analyze the factors that cause party defection votes. Various studies on voting behavior within the National Assembly assess that party is the major influence on assembly member's voting behavior. The logic is that Korean political parties, from their inception, and, experiencing phases of authoritarian government, strengthened party discipline in kind, giving birth to a tradition of party exercising significant control over assemblymen's behavior. However, plenty of evidence indicating that assembly members are more than just passive beings submissive to party discipline is to be found. In the act of voting, two choices in disagreement with the party line are available. The first is active party defection voting, wherein a vote is cast in a direction out of line with one's party. Second is passive defection behavior, wherein one chooses abstention or absence to avoid casting a party-line vote. Although not common, the fact that assembly members on occasion display independent voting behavior, defecting from party majority, raises the need for a new explanation other than strong party discipline, which has been identified in the existing literature as the significant determinant of voting behavior. The study assumes that assemblymen are strategic actors seeking utility through floor voting, and focuses on identifying significant determinants of assembly members' party defection votes in the first half of the 18th National Assembly. For this purpose, the party-line was identified as the voting choice of the majority of assembly members in a party, and party defection voting behavior by individual assembly member was set as the dependent variable. The study is divided into two models: In the comprehensive defection model, absences from floor voti are included in the analysis, where as in the simple defection model, they are not. To identify deciding factors of assembly members party defection votes, independent variables were categorized as institutional, representational, and individual variables. Weighted least squares group logistic regression analysis was performed. The results of the study can be summarized in three points. First, institutional variables were identified as having a significant effect on assembly members' party defection voting. Assembly members are more likely to defect from the party-line in voting when they have autonomy from their party. This is because assembly members having served multiple terms, who are not part of the official party leadership, but who, with relatively more experience, in effect are part of party leadership, can afford to be more autonomous. On the other hand, assembly members who are officially in party leadership are accountable for the entire party, and are relatively less able to vote in a direction not in agreement with the party line. They can therefore be expected to vote in line with the party. Second, in the simple defection model, ideology, a variable that had largely been dismissed in the past was confirmed as having a statistically significant influence. Analysis showed that the more progressive an assembly membern, the higher the likelihood of party defection voting behavior. The fact that the influence of ideology was only visible when absences were not included in analysis has wide implications. This indicates that assembly members' making known their personal standing on issues through floor voting, directly opposing the party-line, is only possible because the strength of individual ideology is such that it outweighs the institutional incentives offered by party. However, in the study, the ideology variable was not interpreted in connection with policy; rather, ideology was defined as a general tendency to defect or vote in line with party majority. Therefore, the direct influence of ideology on party defection in a policy context could not be measured. The study's significance lies in unearthing new implications regarding the building blocks of a healthy representative democracy. The results of the study, which analyzed the deciding factors of party defection votes, showed that party defection was the result of assembly members' institutional autonomy. Such results are in line with existing literature, which points to strong party discipline and undemocratic intra-party structure as the key causes of the Korean National Assembly's chronic inefficiency and confrontations. The study identified ideology as a factor that increases the likelihood of party defection; it can therefore be forecast that once a more democratic party system is established both on and off the floor, assemblymen will be able to exercise greater autonomy, leading to a political environment in which factors such as ideology may have a more pronounced influence on voting behavior. ; Master

Background Low-quality services of emergency medical institutions, unclear function of institution classification, and overcrowding of emergency departments (ED) of regional emergency medical centers (regional centers) have been major problems in the Korean emergency medical services system (EMSS). In 2015, the Middle East Respiratory Syndrome (MERS) outbreak spread from patients waiting for hospitalization in the overcrowded ED. The MERS outbreak was an event that revealed the vulnerability of EMSS and opened the policy window to solve the problem of EMSS in Korea. Several policies for EMSS improvement were implemented after the 2015 MERS outbreak focusing the improvement of emergency medical services (EMS) delivery system and ED infection control. This study investigates the policy processes for improving emergency EMSS and infection control program after the MERS outbreak. The purpose of this study was to examine the policy processes regarding EMSS from the view of policy actors and draw lessons for improving the EMSS in Korea. 2. Methods A mixed method was utilized to identify and explain the complex subject of the policy processes of Korean EMSS during and after the MERS outbreak in 2015. As a quantitative study, changes in EMSS after MERS were investigated according to the WHO's model of a national health system infrastructure. Literature review and data analysis of Korean Health Insurance Service (NHIS) database were conducted. In Korea, the NHIS covers all citizens. A quantitative analysis was performed on 16,965,755 cases using ED during 2014-2016. In-depth interviews were conducted on 30 experts, who were involved in the emergency medical policy process, including clinicians, policy experts, and hospital management experts. The total interview time was 55 hours and 4 minutes. Changes in EMSS and the policy process after the MERS outbreak were evaluated from the view of policy actors. In this study, quantitative data was interpreted qualitatively, and independent qualitative evaluation was conducted, as well. Therefore, this research is a mixed-methods study focusing on qualitative research. SAS version 9.4 was used for the quantitative analysis and NVivo 12.0 was used for the qualitative analysis. 3. Results The quantitative analysis results showed that in the area of EMS delivery system, the human resource of ED and number of regional centers have been increased and facility, equipment, and manpower standards have been revised in order to expand emergency medical resources. As a result, the number of medical personnel and emergency intensive care units in regional centers increased and profitability of ED improved as well. The number of monthly ED visits temporarily decreased during the MERS outbreak but then recovered afterward. As ED overcrowding indicators were emphasized in the evaluation of EMS institutions, monthly mean ED length of stay of regional centers was decreased. However, despite the emphasis on providing critical care, transfer rate of severe emergency patients was not decreased. In the policy processes of EMS, experts evaluated that EMS delivery system was set up as a pseudo-agenda and exploration and comparison of policy alternatives were not sufficiently carried out. As a result, the physicians did not comply with the policy implementation and policy goals were not achieved. In the area of ED infection control program, the human resources for infection control and the number of isolation unit of ED were increased. Budgets for emerging infectious disease management were increased and profitability of infection control was also improved. The EMS Act was revised to respond to emerging infectious diseases and indicators related to infection control were strengthened in evaluation of EMS institutions. In the qualitative research, experts said that since the ED infection control program was such an intuitive and obvious problem, it was easy to become a policy-agenda. Despite the resistance of small hospitals, ED infection control program was implemented by powerful policy measures such as the revision of the Law. As a result, policy goals of expanding the infection control resources and improving the infection management environment were able to be achieved. From results of observing EMS policy process, the following characteristics of policy actors were found. First, there was difference of opinions within the physician subgroup as well as between government and medical professionals. ED staff, other department staff, and hospital executives, who have different views on policy alternatives could have all served as veto points in the policy process. Second, while the EMSS expert organization, which was an unofficial participant in the emergency medical policy process, could have played both roles of cause group and sectional group in the policy process, their sectional position has been emphasized, thereby weakening their insider status during policy process. Third, policy actors understood the publicness of EMS differently in terms of its ownership and function. The ED staff insisted that government support is needed because emergency medical care is based on public interest, however, some staffs were unwilling to provide less profitable cares, leading to conflict. Main characteristics of the emergency medical policy process that made it difficult to successfully implement are as follows: First, there was lack of consideration regarding variation of EMSS in Korea, such as distribution of medical resources among different regions, incentives to hospitals, and understanding of functional variations in ED. Second, the problem of the EMS delivery system was a problem of the low quality of emergency medical care. Third, the lack of professionalism in some physicians has lowered the capacity of emergency medical care, as well as trust between patient and doctor, therefore made the problem more difficult to solve. 4. Discussion Based on the analysis of the policy processes of the EMSS after the MERS outbreak in Korea, following lessons are suggested. First, setting the goals of EMS from the perspectives of citizen is warranted. The final performance goal should focus on improving health status of citizen and satisfaction with emergency medical care. Second, to close the gaps among policy actors in understanding policy content and policy process, actors should work together to explore the root causes of the problems and establish a policy agenda together. Third, a policy alternative should be causal in solving the problems in EMSS considering variation of EMS. Fourth, compliance with policies could be secured by internal persuasion among policy actors with proper incentives and reasonable punishment, especially EMSS expert organization. Fifth, policy outcomes should be evaluated based on both policy output and side effects together to identify policy impact. Sixth, policy governance regarding EMSS should be established and the roles of EMSS expert organization should be strengthened as the cause group rather than sectional group. This study examined the emergency medical policy processes from the view of policy actors through the 2015 MERS outbreak in Korea.
