Background: Recent policy is marked by a shift towards enabling people with dementia to remain at home and in their neighbourhoods, yet little is known about the wider perspective of neighbourhood as an everyday place of connection, practice and meaning in the lives of people with experience of dementia. Aims: The aim of this thesis is twofold. The first aim is to explore the neighbourhood as an everyday place for people with experiences of dementia. The second aim is to explore neighbourhood as a place for practice. Methods and Designs: Five studies are included in the thesis with both quantitative and qualitative designs. Study I had a cross-sectional exploratory and descriptive design. A total cohort of 17, 405 people with a dementia diagnosis were identified and matched with data about home care services and housing, and were then associated with socio-demographic factors in three county councils: Östergötland, Stockholm and Västerbotten. Study II had a phenomenological design; 14 community-dwelling people diagnosed with dementia in the County of Östergötland participated using walking interviews. Study III had an inductive and exploratory qualitative design including 14 community-dwelling people living alone with dementia in England, Scotland and Sweden involving multiple data collection methods. Study IV had an inductive and explorative qualitative design that included 22 people with the lived, personal and professional experiences of dementia and used semi-structured individual and group interviews. Study V had an inductive and explorative qualitative design where the perspectives of 18 participants (registered and specialist nurses) were included using shadowing as the main method for data collection. Findings: In study I, 72% of the cohort was living in ordinary housing and 28% in special housing of the total of 17, 405 people with a dementia diagnosis. Overall, 52% of 17, 405 people with dementia in three county councils (Östergötland, Stockholm and Västerbotten) were living alone. Study II revealed that walking in the neighbourhood was an integral part of their day-to-day activities that helped them to manage life with dementia. Connection to nature by being outdoors was a restorative practice for people living with dementia. Neighbourhood was often described as a social context, although some participants living alone revealed that their social contacts were mainly staff working in municipal home care. In study III, participants across all three field sites channelled their efforts to stay connected to the neighbourhood into creating new ways of maintaining social networks and relationships. By participating in several activities (provided in the United Kingdom by the third-sector and charitable groups, and in Sweden, by the municipalities), bonds of friendship were created. However, the impact of stigma surrounding dementia was highlighted by the participants, which caused experiences of involuntary solitude or loneliness. Despite the impact of stigma, participants took control over their lives by searching for new daily social connections in the neighbourhood and were by no means passive in the face of the challenges in everyday life. In study IV, the participants discussed how dementia was stigmatized in the community. People living with dementia were often not being respected as active citizens with their own resources in the community. Being socially active in a group or in public spaces were strategies to maintain a social role in the community. Participants with different experiences of dementia wanted the day care centres and teams to be more centrally involved in person-centred care and health-promoting improvements. Finally, in study V, participants struggled with the commonly held view of their role and their workplace within the health care system, interpreting it as being invisible, as if placed in a black box. The tasks and responsibilities of the participants were shifting to assistant nurses, neighbours and family members according to the socio-economic level of the municipality. Nonetheless, the participants were clearly part of the neighbourhood. The findings of this thesis have been integrated into a combined thematic analysis based on the five studies to reach an overall representation of people's experiences of neighbourhood as an everyday place and a place for practice in the context of dementia. Five main themes (and three sub-themes) emerged from the analysis: (1) walkable attachment to the lived neighbourhood; (2) daily activities promote health and well-being; (3) opportunities for social connections; (4) just treat us as active citizens; (5) neighbourhood: a place for practice. The analysis suggested the neighbourhood was not only described as a walkable, social and citizenship arena in the context of dementia; it was also a place where practice was ongoing around the clock (studies II, III, IV and V) because most of the people with dementia are living in ordinary housing (study I). Conclusion: The thesis presents a new foundation and knowledge to understand neighbourhood as a place for everyday life and practice by applying a new lens for understanding. The neighbourhood can be understood as a place linked by connections that people actively searched out, and where the meaning of place emerges via movement of the body through the world. It is also a site where practices support everyday life for people with dementia, especially for those living alone with dementia. This points to the need to re-think nursing practice, where "neighbourhood nursing" as a formal model with a lifeworld perspective has to be established in dialogue with citizens. ; Bakgrund: Den senaste samhällsutvecklingen och de politiska ambitionerna har utgått alltmer från att göra det möjligt för personer med demenssjukdom att bo kvar allt längre i sina hem och grannskap. Trots denna utveckling finns det lite kunskap utifrån det bredare perspektivet om grannskapet som en vardaglig plats för relationer, omvårdnad samt vilken mening det har för personer som har erfarenhet av demens. Syfte: Syftet med avhandlingen var för det första att undersöka grannskapet som en plats i vardagen för personer som har erfarenheter av demens, och för det andra, att undersöka grannskapet som en plats för omvårdnad. Design och metod: Avhandlingen inkluderar fem delstudier med både kvalitativ och kvantitativ design. Studie I hade en explorativ och beskrivande tvärsnittsdesign. Totalt ingick det 17 405 personer med en demensdiagnos som identifierades och samkördes med information av hemtjänstinsatser och boende. Denna informationen förenades sedan med de socio-demografiska faktorerna i de tre regionerna Östergötland, Stockholm och Västerbotten. Studie II hade en femenologisk design som inkluderade 14 personer som bodde i ett eget boende med en demensdiagnos i Östergötlands län och som deltog i ´promenad intervjuer´. Studie III hade en induktiv och beskrivande kvalitativ design vilket inkluderade 14 personer som bodde ensamma i ett eget boende med en demensdiagnos i England, Skottland och Sverige. De 14 personer som bodde i ett eget boende med en demensdiagnos i Studie III deltog i flera datainsamlingsmetoder. Studie IV hade en induktiv och beskrivande kvalitativ design som inkluderade 22 personer med levda, personliga och professionella erfarenheter av demens som deltog i semistrukturerade individuella och gruppintervjuer. Slutligen, Studie V hade också en induktiv och beskrivande kvalitativ design där 18 deltagare (legitimerade sjuksköterskor och specialistsjuksköterskor) inkluderades skuggning som är den huvudsakliga metoden för datainsamlingen. Resultat: I Studie I fann vi att 72 % av den totala befolkningen av 17 405 personer med en demensdiagnos bodde i ordinärt boende och 28 % i särskilt boende. Sammantaget bodde 52 % av de 17 405 personerna med demens i singelhushåll i de tre regionerna Östergötland, Stockholm och Västerbotten. I studie III beskrev personer med demens i England, Skottland och Sverige hur de ansträngde sig för att hålla kvar kontakten med grannskapet men också hur de skapade nya sätt att upprätthålla och hitta nya sociala nätverk och kontakter. Dessa vänskapsband skapades genom att delta i olika aktiviteter (som tillhandahålls i England av tredje sektorn och välgörenhetsgrupper, och i Sverige, av kommunerna). Även effekterna av stigmatiseringen kring att leva med demens betonades av deltagarna, de beskrev detta som en orsak till ofrivillig ensamhet. Trots påverkan av stigmatiseringen tog personerna med demens, kontroll över sina liv, genom att finna nya dagliga sociala kontakter i grannskapet. Personer med demens är intressearde av att anta nya utmaningarna i vardagen. I studie IV diskuterade personerna med olika erfarenheter av demens hur demenssjukdomen som ett stigmatiserat tillstånd i samhället påverkade dem. De ansåg att deltagarna som lever med en demensdiagnos ofta inte respekterades som aktiva medborgare med sina egna resurser i samhället. Personerna med olika erfarenhet av demens framhöll också att vara socialt aktiv i en grupp eller i offentliga rum var strategier för att upprätthålla en social roll i samhället. Förutom betydelsen av det sociala livet påpekade personerna med olika erfarenheter av demens att de skulle önska att dagverksamheterna och demensteamen skulle utgå mer utifrån person-centrerad vård och hälsofrämjande förhållningssätt. Slutligen, i studie V observerades det hur legitimerade sjuksköterskor och specialistsjuksköterskor kämpade med den vanligt förekommande synen på deras profession och arbetsplatsen inom hälso-och sjukvården som gav sig uttryck som osynlig. Deras uppgifter och ansvar hade även förändrats och allt mer hade överlåtits till undersköterskor, grannar och familjemedlemmar och kommunernas socioekonomiska status hade betydelse. Legitimerade sjuksköterskor och specialistsjuksköterskorna var en självklar del av grannskapet. Resultaten av denna avhandling har integrerats i en sammansatt tematisk analys baserad på de fem studierna för att nå en övergripande representation av människors upplevelser av grannskapet som en plats för det dagliga livet och omvårdnad med utgångspunkt utifrån erfarenheter av demens. Analysen resulterade i fem huvudteman (och tre underteman): (1) anknytning till det existerade grannskapet via promenader (2) dagliga aktiviteter främjar hälsa och välbefinnande; (3) möjligheter för sociala kontakter; (4) behandla oss som aktiva medborgare; (5) grannskapet som en plats för omvårdnad. Grannskapet beskrivs inte bara som en framkomlig, social medborgarskapsarena i relation till demens, utan var även en plats där sjuksköterskepraxis pågick dygnet runt (studierna II, III, IV och V) eftersom de flesta personer som lever med demens bor i ordinärt boende (studie I). Konklusion: Avhandlingen presenterar ett nytt underlag och ny kunskap för att förstå grannskapet som en plats för dagligt liv och omvårdnad genom att använda ett nytt perspektiv för att få förståelse. Grannskapet kan förstås som en plats förenad genom förbindelser som människor aktivt söker efter och där betydelsen av grannskapet inträder genom kroppens rörelse ut mot världen. Det är också en plats där omvårdnad sker som i sin tur stödjer vardagslivet för personer med demens, särskilt för dem som lever ensamma med demens. Detta visar på behovet av att tänka om i praktiken vad det gäller omvårdnaden, där "omvårdnad i grannskapet" som en modell med ett livsvärldsperspektiv behövs i dialog med medborgarna. ; Pozadina: Nedavni trend demencije ogleda se u ovome da osobe koje žive sa demencijom ostanu u kući ili u susjedstvu, ali još uvijek nije poznato mnogo u široj perspektivi koju nude ta susjedstva kao svakodnevno mjesto povezivanja, medicinske njege, te samog značaja u životu onih koji žive sa demencijom. Ciljevi: Cilj je istražiti susjedstvo kao svakodnevno mjesto boravka za ljude koji žive od demencije, te kao drugo istražiti susjedstva kao mjesta gdje bi se pružala medicinska njega. Metode i dizajni: Ukupno pet studija uključeno je u rad sa kvantitativnim i kvalitativnim dizajnom. Studija I imala je istraživački i opisni dizajn. Populacija od 17. 405 ljudi sa dijagnozom demencije bila je identifikovana, te usklađena sa podacima o kućnoj njezi i mjestu prebivališta, a zatim povezana sa tri općine: Östergötland, Štokholm i Vasterboten. Studija II imala je fenomenološki dizajn gdje je 14 osoba koje žive sa demencijom u porodičnoj kuci učestovalo u Östergötlandu u intervju pri šetnji. Studija III imala je induktivni i istraživački kvalitativni dizajn koji je uključivao 14 ljudi iz porodičnog domaćinstva koji žive sami sa demencijom u Engleskoj, Škotskoj i Švedskoj, uključujući više metoda prikupljanja podataka. Može se reći da je boravak na otvorenom, te povezivanje sa prirodom je vrlo praktično za ljude koji žive sa demencijom. Susjedstvo je često opisano kao socijalni kontekst, iako su neki učesnici u istraživanju koji žive sami otkrili kako je njihov jedini društveni kontakt bio sa uposlenicima kućne njege. U studiji III učesnici ispitivanja u Engleskoj, Škotskoj i Švedskoj su sve svoje napore da ostanu povezani sa susjedstvom kako bi stvorili nove načine koji bi pomogli stvaranje novih veza i odnosa. Učestvovanjem u nekoliko aktivnosti (koje su u Velikoj Britaniji omogućile dobrotvorne i slobodne organizacije, a u Švedskoj općine) stvorene su veze i prijateljstva. Uprkos utjecaju predrasuda, učesnici su preuzeli kontrolunad svojim životima, te su tražili nove svakodnevne društvene veze u susjedstvu, ne pokazujući tako pasivnost prema svakodnevnim izazovima s kojima se susreću. U studiji IV učesnici su razgovarali o predrasudama o demenciji u zajednici. Ljudi koji žive sa demencijom često nisu uvaženi kao aktivni članovi koji mogu doprinijeti zajednici. Kako bi održali svoje uloge u zajednici vrlo je bitno da ostanu društveno aktivni. Učesnici sa različitim iskustvom demencije izrazili su želju da se centri za svakodnevnu njegu i timovi više baziraju kao unapređenju njege i zdravlja, kao i da se akcenat stavi na osobu za demencijom. Na kraju, u studiji V medicinske sestre su se borile sa općeprihvaćenim stavom o njima i njihovoj ulozi, te njihovom radnom mjestu u sistemu zdravstvene zaštite, opisujući to kao nevidljivo. Zadaci i odgovornosti medicinskih sestara prebacivali su se na pomoćne sestre, komšije i članove porodica prema društveno-ekonomskom nivou opštine. Unatoč tome, medicinske sestre su očito bile dio susjedstva. Iskustva, odnosno pronalasci u ovoj tezi integrisani su u kombinovanu analizu prema pet tematski obrađenih studija, kako bi se dostigao sveobuhvatan prikaz iskustava u susjedstvu kao svakodnevnom mjestu, te mjestu zdravstvene podrške u kontekstu demencije. Iz ove analize pojavilo se pet glavnih tema (kao i tri podteme): (1) povezanost sa susjedstvom; (2) svakodnevne aktivnosti promovišu zdravlje i dobrobit; (3) mogućnosti za socijalne veze; (4) tretiranje kao aktivne građane; (5) susjedstvo je mjesto za medicinske prakse neprekidno traju. Analize pokazuju kako se susjedstvo ne opisuje samo kao prohodno, socijalno i građansko polje u kontekstu demencije, već kao i mjesto gdje medicinske prakse neprekidno traju (studije II, III, IV i V), jer većina ljudi s demencijom žive u običnom domaćinstvu (studija I). Zaključak: Teza predstavlja nove temelje i znanja, kako bi se lakše razumio pojam susjedstva kao svakodnevnog mjesta za život, kao i njege kroz nove objektive razumijevanja. Susjedstvo bi se moglo shvatiti kao mjesto spojeno vezama koje ljudi aktivno potražuju gdje se značenje mjesta spaja sa kretanjem tijela kroz svijet. To je također mjesto gdje je svakodnevno obezbjeđena njega za ljude koji žive sa demencijom, posebno za one koji žive sami. This dissertation holds the objective to contribute to an assessment of the climate-migration-conflict nexus as applied to the Syrian conflict. It does so by investigating how displaced Syrians perceive and react to the alleged climate-conflict links in the Syria-climate conflict thesis, and exploring their perceptions and experiences of the unrest that in Syria from 2011 and onwards. Overall, the dissertation contributes to discussion over what constitutes 'knowledge' in a time when 'facts' and 'truth' are publicly contested, and debate over how human life will be affected by the very real anthropogenic climate change problem we know is both accelerating and exacerbating. The Syrian War has according to the Syrian Network for Human Rights claimed the lives of 227,413 civilians, including 29,457 children. Over 5.6 million people have fled Syria, and 6.6 million are internally displaced, accounting for the world's largest forcibly displaced population (UNHCR 2020). The magnitude of devastation left behind by a revolution gone bad, and the conflict's ongoing protraction is undeniable. Given this backdrop, it is little wonder that the war has been subject to proliferating analyses, with debates over its triggers coming under increasing scholarly scrutiny. Among the explanations of the cause of the Syrian war is the Syria-climate conflict thesis, which views climate change as a threat multiplier. The thesis argues an alleged link between antecedent anthropogenic drought in northeastern Syria, mass internal migration, which eventually fuelled Syria's descent into civil war. Such narratives supposedly depict what conflictual circumstances await humanity under warmer conditions in the human-induced 'Anthropocene'. This study directs a critical eye toward the contents and methods of the Syria-climate conflict thesis by means of qualitative and political ecology-aligned analysis. Little testimony from Syrians themselves is currently part of the thesis' foundation - a gap this dissertation aims to fill by negotiating 'insecurity' definitions with 'locals' themselves. Based on four focus group discussions and fifteen semistructured interviews with a total of 79 Syrian participants, I argue that the Syrian conflict was neither scarcity nor climate-driven, but rather a result of coarse living conditions that generations of Syrians had endured under Baʿathist rule, that culminated in a widespread uprising after key events in early 2011, such as the Arab Spring. My analysis uncovers colonial, Malthusian, and environment-centric undercurrents in the Syria-climate conflict thesis as well as in broader securitised notions of climate change found in the nexus literature. It is also critical of the widespread endorsement climate-conflict narratives enjoy in the media and 'grey literature' without, from my perspective, possessing the empirical backing to do so. Using the analytical tools of narrative analysis and political ecology, I call for a decolonisation of the Syria-climate conflict thesis with recognition and senses of justice as key components. Humanity does have an environmental crisis on its hands, but as with the Syrian War, its genesis appears to have had a more human creator, rather than 'natural' one. ; Denne oppgaven har målet å bidra til en evaluering av klima-migrasjon-konflikt forbindelsen slik den anvendes den syriske konflikten. Oppgaven gjør dette ved å utforske hvordan syrere oppfatter og reagerer på de påståtte sammenhengene mellom klimaendringer og konflikt funnet i den såkalte 'Syria-klimakonflikt avhandlingen' og ved å se på deres persepsjoner og erfaringer med uroen som preget Syria fra 2011 og utover. Alt i alt, bidrar denne oppgaven til diskusjonen om hva som utgjør 'kunnskap' i en tid der 'fakta' og 'sannhet' er offentlig bestridt, og debatten om hvordan samfunnet vil påvirkes av de veldig ekte og reelle problemene menneskeskapte klimaendringer bærer med seg. Ved å bruke analytiske verktøy som narrativ analyse og politisk økologi, oppfordrer jeg til en 'avkolonisering' av Syria-klimakonflikt teorien med 'anerkjennelse' og 'rettferdighetsforestillinger' som nøkkelkomponenter. Menneskeheten har en miljøkrise i hendene, men som med den syriske krigen, har dens opphav en tydelig menneskelig skaper, mer enn en 'naturlig' en. ; ملخص الأطروحة تھدف ھذه الأطروحة إلى المساھمة في تقییم العلاقة بین المناخ والھجرة والصراع بالطریقة التي یتم تطبیقھا على الصراع السوري. تقوم ھذه الأطروحة بمثل ھذا التقییم من خلال استكشاف الطریقة التي یدرك بھا السوریون ویتفاعلون مع الروابط المزعومة بین تغیر المناخ والصراع الموجود فیما یسمى (أطروحة الصراع السوري وتغییر المناخ) وأیضا ومن خلال النظر إلى تصوراتھم وتجاربھم بشأن الاضطرابات التي تمیزت بھا سوریا منذ عام 2011 فصاعدًا. بشكل عام، تساھم ھذه الأطروحة في مناقشة ما یشكل "المعرفة" في الوقت الذي تكون فیھ "الحقائق" محل نزاع علني، والنقاش حول كیفیة تأثر المجتمع بالمشكلات الحقیقیة المتعلقة بالتغیرات المناخیة الناتجة عن الانسان وما تنتج عنھا من تحدیات بحیث ان التغییرات التي نعرفھا تخضع للتسارع والتدھور المستمر. خلال فترة الحرب السوریة فقد 227،413 مدنیاً حیاتھم من بینھم 29،457 طفلاً، بحسب الشبكة السوریة لحقوق الإنسان. كما أجبر أكثر من 5.6 ملیون شخص على الفرار خارج سوریا و 6.6 ملیون شخص تم تھجیرھم داخلیًا، وھذا یشكل أكبر عدد من النازحین في العالم (حسب المفوضیة السامیة للأمم المتحدة لشؤون اللاجئین 2020 ). حیث لا یمكن إنكار حجم الدمار الذي خلفتھ الثورة التي لم تنجح ببلوغ اھدافھا واستمرار الصراع داخل الأراضي السوریة. لذلك لیس من المستغرب أن تكون الحرب السوریة في بؤرة التركیز في العدید من التحلیلات والدراسات، وأن الأسباب التي أدت إلى اندلاع الحرب قد خلقت جدلاً محتدمًا. من بین تفسیرات ونظریات اندلاع الحرب ھي نظریة الصراع المناخي في سوریا والتي تعرّف عامل تغیر المناخ على أنھ (عامل مضاعف للتھدید). النظریة تنص على أن موجات الجفاف والتصحر السابقة والتي كانت من صنع الإنسان في شمال شرق سوریا تسببت في ھجرة جماعیة داخلیة في سوریا، ثم ساھمت بعد ذلك الى إشعال فتیل الحرب الأھلیة. تحاول مثل ھذه الروایات وصف نوع النتائج الملیئة بالصراعات التي تنتظر البشریة في ظروف أكثر دفئًا في الأنثروبوسین من صنع الإنسان. تلقي ھذه الرسالة نظرة نقدیة على محتوى وطرق أطروحة الصراع المناخي السوري باستخدام الأسالیب النوعیة والتحلیل البیئي السیاسي. ھناك عدد قلیل جدا من الأصوات السوریة المدرجة في قاعدة الأدلة الخاصة بالأطروحة وھو أمر تحاول ھذه الأطروحة أن تفعل شیئًا حیالھ من خلال التفاوض على تعریفات مثل "انعدام الأمن" مع "السكان المحلیین" أنفسھم. استنادًا إلى تصریحات أربع مجموعات تركیز قابلتھم وخمسة عشر مقابلة شبھ منظمة مع ما مجموعھ 79 مشاركًا سوریًا، زعمت لھم أن الصراع السوري لم یكن نادرًا ولا مدفوعًا بالعوامل المناخیة، بل نتیجة لسوء الأحوال المعیشیة لعدة أجیال في ظل حكم البعث، والتي بلغت ذروتھا في انتفاضة واسعة في أوائل عام 2011 بعد أحداث رئیسیة مثل الربیع العربي. دراستي ھذه تزیل الغطاء عن تیارات استعماریة ومالثوسیة متمحورة حول البیئة في نظریة الصراع المناخي في سوریا وكذلك الأدبیات التي تستخدم الفھم الأمني لتغیر المناخ. من المستھجن أیضًا أن روایات الصراع المناخي تحظى بھذا الدعم الواسع في وسائل الإعلام و "الأدب الرمادي" دون وجود اي دلیل ملموس یبرره، حسب رأیي الشخصي. باستخدام أدوات تحلیلیة مھمة مثل التحلیل السردي والبیئة السیاسیة، أدعو إلى "إنھاء استعمار" نظریة الصراع المناخي السوري مع اضافة "الاعتراف" و "مفاھیم العدالة" كمكونات رئیسیة. أخیرا تواجھ البشریة أزمة بیئیة حقیقیة، ولكن كما ھو الحال مع الحرب السوریة، فإن أصول ھذه الازمة البیئیة یقف خلفھا مسبب بشري واضح، أكثر من ما ھي مسببات طبیعیة. ; M-IES

Childhood and adolescence are important stages of life with long-lasting implications for both the health and well-being of individuals and society as a whole. Accordingly, health-promoting efforts aimed at supporting the everyday lives of children and adolescents represent a key priority for public health. In Norway, there has been increased attention paid to neighborhoods and local communities as crucial settings for such efforts, and health-promoting changes to the built environment are deemed a promising strategy. This doctoral thesis has examined neighborhood and local community built environment determinants and their potential to support participation in activities and strengthening the well-being of children and adolescents. Such knowledge can contribute to provide inputs for policy making, development and planning to achieve good health and well-being in the younger population. This thesis has brought together data from different sources using a pragmatic multi-methodology research strategy based on quantitatively driven approaches and geographic information systems (GIS) technology. Through a step-based research process, review designs were utilized together with a cross-sectional epidemiological design. The first review focused on methodological issues and involved identifying, systematizing and evaluating previously applied GIS-derived measures and operational definitions of the built environment characteristics and the spatial units of analysis. The second systematic review addressed the health-promoting potential of the built environment by synthesizing the existing empirical evidence of relations between the built environment and the participation in activities and well-being of children and adolescents. The cross-sectional studies were conducted within the Norwegian context. Data from 23 043 eight-year-olds in the Norwegian Mother and Child Cohort Study were linked to GIS-derived measures of population density, green spaces and facilities around the residential addresses of the study participants. Associations between these built characteristics and children's participation in leisure-time physical activity (PA), organized activities and social activity with friends were investigated. Further, mediation analysis techniques were applied to examine whether these built environment characteristics were related to children's subjective well-being and if participating in different leisure activities mediated any such associations. The methodological findings show numerous GIS-derived measures of diverse built environment characteristics for which consistency in operational definitions is very much needed. The empirical results suggested that the multifarious characteristics of built environments act as resources for participation in different activities, and could thereby contribute to strengthening well-being in childhood and adolescence. In particular, the synthesis of existing evidence showed that living in neighborhoods characterized by low traffic, proximate facilities, high walkability, more safety features and well-established infrastructure for walking and cycling likely promotes active travel and, to some extent, physical activity. Findings from the cross-sectional studies revealed that neighborhood green space was associated with more leisure-time PA among Norwegian 8-year-olds in both the summer and winter. The results also showed that more densely populated areas and more facilities, such as playgrounds/sports fields and schools, were associated with greater participation in organized and social activities. Further, positive indirect relations between the built environment characteristics and children's moods and feelings, through greater participation in leisure activities, counteracted some of the negative direct associations observed between children's emotional state and the determinants higher population density, access to a park, more playgrounds/sports fields. These findings from the Norwegian context underscore the role that the built environment may have in terms of enabling participation in a variety of leisure activities for children's subjective well-being. This thesis concludes that although many methodological issues and knowledge gaps remain, planning for public health cannot wait. The best available evidence at this very moment suggests that children and adolescents who live in neighborhoods with versatile built resources and activity venues likely engage more in leisure activities that in the long run might contribute to strengthening their health and well-being. Accordingly, holistic approaches to public health within these everyday settings are essential. Different stakeholders, including policy makers, public health professionals and planners should acknowledge the multifaceted nature of determinants and appreciate that a variety of resources for health and well-being can be found within built environments. ; Barndom og ungdomsårene er viktige stadier i livet som har langsiktige innvirkninger på både individets helse og livskvalitet og på samfunnet i sin helhet. Helsefremmende tiltak som tar sikte på å støtte barn og unge i deres hverdag er derfor en sentral prioritering innen folkehelsearbeidet. I Norge er det viet økt oppmerksomhet til nabolag og lokalsamfunn som avgjørende arenaer for slik innsats, og tilpasninger av våre fysiske omgivelser betraktes som en potensiell strategi. Det er derfor avgjørende å ha en god forståelse av hva som kjennetegner et helsefremmende nærmiljø. Denne doktorgradsavhandlingen har undersøkt de fysiske nærmiljøkvalitetene i nabolag og lokalsamfunn og deres potensiale for å fremme deltakelse i aktiviteter og styrke trivsel blant barn og unge. Slik kunnskap kan bidra med viktige innspill til politikkutforming samt samfunns- og arealplanlegging for å sikre god helse og trivsel blant de yngste i vår befolkning. Med utgangspunkt i en pragmatisk multimetodisk forskningsstrategi, ble data fra flere ulike kilder innhentet gjennom kvantitative tilnærminger og bruk av geografiske informasjonssystemer (GIS). Avhandlingen bygger på en trinnvis forskningsprosess der ble det benyttet ulike review design og et epidemiologisk tverrsnittdesign. Den første kunnskapsoversikten belyste metodologiske problemstillinger og involverte å identifisere, systematisere og vurdere eksisterende GIS-avledete mål og operasjonelle definisjoner av fysiske nærmiljøkvaliteter samt de romlige analyseenhetene. Den andre systematiske kunnskapsoppsummeringen fokuserte på nærmiljøets helsefremmende potensiale ved å sammenstille eksisterende forskning om sammenhenger mellom fysiske nærmiljøkvaliteter, deltakelse i aktiviteter og trivsel blant barn og ungdom. De epidemiologiske tverrsnittstudiene tar utgangspunkt i den norske konteksten. Data fra 23 043 åtte-åringer i Den Norske Mor-Barn Undersøkelsen ble koblet til GIS-avledede mål på befolkningstetthet, tilgang til grøntarealer og fasiliteter rundt barnas bostedsadresser. Sammenhenger mellom disse fysiske nærmiljøkvalitetene og barnas deltakelse i fysisk aktivitet, organiserte aktiviteter og sosial aktivitet med venner ble studert. Videre ble medieringsanalyser benyttet for å undersøke om de ulike nærmiljøkvalitetene var forbundet med barnas trivsel, samt hvorvidt deltakelse i fritidsaktiviteter fungerte som en potensiell mediator i slike eventuelle sammenhenger. Avhandlingens metodologiske funn avdekker at en rekke GIS mål og operasjonelle definisjoner av de fysiske nærmiljøkvalitetene finnes og benyttes i forskningen, og det er et stort behov for mer konsistent bruk av både mål og definisjoner. De empiriske resultatene tyder på at mangfoldige fysiske nærmiljøkvaliteter kan være viktige ressurser for aktivitetsdeltakelse og derigjennom bidra til å fremme trivsel blant barn og unge. Den systematiske kunnskapsoppsummeringen viste spesielt at nabolag kjennetegnet ved lite trafikkeksponering, flere sikkerhetselementer, fotgjengervennlige områder, nærhet til fasiliteter og veletablert infrastruktur for gående og syklister med stor sannsynlighet kan fremme aktiv transport, og til en viss grad, fysisk aktivitet. Resultater fra tverssnittstudiene viste at tilgang til grøntarealer var forbundet med mer fysisk aktivitet blant norske 8-åringer. Videre ble det funnet positive sammenhenger mellom høyere befolkningstetthet og tilgang til flere fasiliteter, slik som lekeplasser/aktivitetsanlegg og skoler, og økt deltakelse i organisert aktiviteter og sosial aktivitet med venner. Positive indirekte sammenhenger mellom fysiske nærmiljøkvaliteter og barnas humør og følelser, via økt deltakelse i fritidsaktiviteter, bidro til å motvirke noen av de negative sammenhenger observert mellom barnas emosjonelle tilstand og determinantene høyere befolkningstetthet, tilgang til park, flere lekeplasser/aktivitetsanlegg. Funnene fra den norske konteksten understreker hvilken rolle våre fysiske omgivelser kan ha med tanke på å fremme deltakelse i aktiviteter for barns trivsel. Avhandlingen konkluderer med at selv om mange metodologiske problemer og kunnskapshull fortsatt eksisterer så kan ikke planlegging for å sikre god helse og livskvalitet i befolkningen vente. Den beste tilgjengelige kunnskapen vi har på nåværende tidspunkt peker i retning av at barn og unge som bor i nabolag med ulike fysiske nærmiljøkvaliteter og arenaer for aktivitet trolig deltar mer i helsefremmende fritidsaktiviteter, noe som i det lange løp kan bidra til å styrke deres helse og trivsel. Derav er helhetlige tilnærminger til folkehelse på disse arenaene viktig. Både politikere, samfunns- og arealplanleggere bør anerkjenne de mangfoldige helsedeterminantene i nærmiljøet og sette pris på at en rekke ressurser for helse og livskvalitet finnes, og kan gjøres tilgjengelig, i våre fysiske omgivelser.

Currently (April 2009) the MCBR has registered over 26 000 births. The registry covers a geographical area known as the Kola Peninsula in Northwest Russia, which is almost entirely located above the Arctic Circle. Murmansk County is about half the size of Norway and had 857 000 inhabitants in 2008. All the 15 delivery departments in the county are involved and their locations stretch from Nikel in the Northwest to Kandalaksha in the south. The Registry Office is located in the city of Murmansk with a trusted staff of four. The MCBR annually registers over 99% of all deliveries in the region. Based on several measures such as quality control exercises and regional workshops, the registry seems to exhibit adequate validity. The registration of births in the MCBR is obligatory and embedded in regional legislation. It is a cooperative effort between the University of Tromsø and the Murmansk County Health Department. Together they have defined four major guidelines, or tasks for the registry: • Monitor the health condition of mothers and their newborn; • Monitor the availability of maternal and perinatal health care; • Develop standards and guidelines for maternal and perinatal health care; • Spawn new hypotheses and provide knowledge related to causal relationships for reproductive health risk factors. Comparisons of selected pregnancy outcomes from Murmansk County with the northern regions of other Nordic countries (Norway, Sweden and Finland) revealed several interesting differences. First of all, there was the divergence of the demographic composition of the respective delivering populations. The pregnant women were much younger in Murmansk County (about 3.5 years), and the percentage of teenage mothers was about twice that of Northern Norway and 5 times higher than in Northern Sweden. Further each woman tended to have fewer children in Murmansk County, the babies were lighter on average (about 200 g), and the proportion of children with a birth weight over 4500 g was 4.5 times higher in Northern Norway. A study comparing the birth weights, perinatal mortality and gestational ages between Northern Norway and Murmansk County disclosed valuable information. Based on WHO-guideline-calculations, the perinatal mortality among the women with a known gestational age was 11.0/1000 in Murmansk County (2006-2007) and 5.4/1000 in Northern Norway (2004-2006). The risk of perinatal mortality was higher at all gestational ages and at all birth weight increments in Murmansk County. There were large disparities between the two regions in the optimal perinatal-survival weights and the small-for-gestationalage 10 percent cut-off weight for term deliveries. Two further studies aimed to map out challenges related to the collection of human tissue samples in the Russian Arctic for the analyses of environmental contaminants. After all, a relevant and effective protocol is the core of any viable epidemiological study. It was concluded that relative to cord blood and breast milk, maternal plasma/blood is the most fundamental biomonitoring medium for organochlorines and toxic metals. Also, complicated statistical analyses will require a detection frequency of the individual contaminant levels in each sample to exceed 80%. And finally, the correlations between concentrations of different organochlorines in the body fluids (with a few exceptions) were sufficiently high so that measuring the levels of a few with high detection frequencies would give a suitable picture of the combined body burden of these contaminants. In conclusion, the MCBR constitutes an invaluable tool for reproductive health studies in the future such as the studies of adverse effects of environmental contaminants. ; Murmansk County Birth Registry (MCBR) ble offisielt startet 1. januar 2006. Frem til i dag (april 2009) har MCBR registrert over 26 000 fødsler. Registeret dekker et geografisk område som kalles Kola halvøya lokalisert i nordvest Russland. Nesten hele området ligger nord for polarsirkelen. Murmansk fylke (eller Murmansk regionen) er omtrent halvparten så stort som Norge og hadde 857 000 innbyggere i 2008. Det finnes 15 fødemottak i fylket som alle er involvert og leverer data til registeret. Fødemottakene strekker seg fra Nikel (ved norskegrensen) og ned til Kandalaksha, sør i fylket. Selve registerkontoret ligger i Murmansk by og har i dag fire ansatte. MCBR registrerer hvert år over 99% av alle fødsler i fylket og basert på resultater av flere kvalitetskontroller og plenumsmøter med alle involverte, ser registeret ut til å ha en validitet av tilfredsstillende omfang. Selve registreringen av fødsler er obligatorisk for alle kvinner og vedtatt gjennom regional lovgivning og er et samarbeidsprosjekt mellom Universitetet i Tromsø og helsedepartementet i Murmansk. Sammen har de definert flere retningslinjer og oppgaver som registeret skal oppfylle og utføre: • Overvåke mor og barns helse; • Overvåke tilgangen på helsetilbud; • Utvikle standarder og retningslinjer for mor/barn helse; • Generere nye hypoteser og frembringe kunnskap om kausale sammenhenger mellom risiko faktorer og perinatal helse. Sammenligninger av svangerskapsutfall fra Murmansk fylke med andre nordlige deler av de nordiske landene (Norge, Sverige og Finland) resulterte i mange interessante oppdagelser. For det første var den demografiske sammensetningen av de fødende kvinnene veldig forskjellig i disse ulike populasjonene. De gravide hadde en mye lavere gjennomsnittsalder in Murmansk fylke (omtrent 3.5 år), prosentandelen av tenåringsmødre var dobbel så høy som i Nord-Norge of fem ganger høyere enn i Nord-Sverige. Videre viste det seg at hver kvinne fikk færre barn gjennom livet i Murmansk fylke, de nyfødte hadde en lavere gjennomsnittlig fødselsvekt (omtrent 200 g) og andelen av barn med en fødselsvekt over 4500 g var fire og en halv gang høyere i Nord-Norge. Den ene studien som sammenlignet fødselsvekter, perinatal dødelighet og svangerskapslengder mellom Nord-Norge og Murmansk Fylke ga oss mer nyttig informasjon. Basert på WHO sine retningslinjer for utregninger av perinatal dødelighet bland kvinner med kjent svangerskapslengde ble det funnet at den perinatale dødeligheten var 11.0/1000 i Murmansk fylke (2006-2007) og 5.4/1000 in Nord-Norge (2004-2006). Risikoen for perinatal dødelighet var høyere ved alle svangerskapslengder og i alle fødselsvektkategorier i Murmansk fylke. Det var også store forskjeller i den optimale perinatale overlevelsesvekten og i det som kunne oppfattes som "liten for gestasjonsalder", spesielt for de som ble født på termin. To videre studier prøvde å finne løsninger på problemer relatert til innsamling av vevsprøver og miljøgifter i den arktiske delen av Russland. En skikkelig protokoll er tross alt hjørnesteinen i en hver ordentlig epidemiologisk studie. Det ble konkludert med at maternalt blod/plasma var det mest fundamentale bioovervåkningsmedium for organiske klorider og giftige metaller. Det viste seg også at avanserte statistiske utregninger krevde tilstedeværelse av målbare verdier av kontaminantene i over 80% av tilfellene. Til slutt ble det funnet at korrelasjonene mellom nivåene av de forskjellige organiske kloridene (med noen få unntak) var så høye at det å måle nivået av noen få av dem kunne gi et klart bilde av den kombinerte kroppsbelastningen av de respektive kontaminantene. Konklusjonen er at MCBR kan bli et viktig og uunnværlig instrument for perinatale helsestudier i fremtiden. ; Регистр родов Мурманской области (РРМО) был официально начат 1 января 2006 года. До настоящего времени (до апреля 2009 г.) в РРМО зарегистрировано свыше 26000 родов. Регистр охватывает географическое пространство, известное как Кольский полуостров, расположенный на Северо-западе России. Это пространство почти полностью находится за Полярным кругом. Мурманская область составляет почти половину территории Норвегии. В 2008 г. в ней проживало 857 000 жителей. В области насчитывается 15 родильных отделений. Все они поставляют данные для регистра. Родильные отделения расположены на территории, которая простирается от г. Никеля (находящегося у норвежской границы) на Северо-западе до г. Кандалакши на юге. Офис Регистра находится в г. Мурманске. Его персонал составляет 4 человека. Ежегодно в РРМО регистрируется свыше 99% всех родов области. Результаты различных измерений контроля качества, а также региональные семинары подтверждают надежность данных регистра. Регистрация родов в РРМО является обязательной, и это записано в региональном законодательстве. РРМО является плодом совместных усилий Университета Тромсё и Отдела Здравоохранения Мурманской области. Вместе они разработали четыре основные директивы, или задачи регистра: • контролировать состояние здоровья матерей и их новорожденных детей; • контролировать эффективность материнского и перинатального здравоохранения; • разработать стандарты и директивы для материнского и перинатального здравоохранения; • выдвинуть новые гипотезы и обеспечить знание о взаимосвязи между факторами риска и репродуктивным здоровьем. Сравнение некоторых исходов беременностей Мурманской области с данными северных регионов скандинавских стран (Норвегия, Швеция и Финляндия) показало некоторые интересные различия. Прежде всего, это расхождение в демографическом составе соответствующих групп рожающего населения. Беременные женщины в Мурманской области были гораздо моложе (разница около 3,5 лет). Процент матерей- подростков был почти в два раза выше, чем в Северной Норвегии и в 5 раз выше, чем в Северной Швеции. Каждая женщина Мурманской области склонна иметь меньше детей, младенцы в среднем легче на 200 грамм. Доля детей с весом при рождении свыше 4500 грамм в 4.5 раза выше в Северной Норвегии. Сравнение данных Северной Норвегии и Мурманской области по весу при родах, перинатальной смертельности и гестационному возрасту дало нам ценную информацию. На основе директивных расчетов ВОЗ перинатальная смертность среди женщин с известным гестационным возрастом в Мурманской области была 11.0/1000 (2006-2007 гг.) и 5.4/1000 в Северной Норвегии (2004-2006 гг.). Риск перинатальной смертности в Мурманской области был выше для всех гестационных возрастов и для любого веса при родах. Большое несоответствие в этих двух регионах было и по оптимальному перинатальному весу, при котором младенец выживал, и теми младенцами, которые были рождены в срок, но были рождены маленькими для своего гестационного возраста (10% ниже нормального веса). Целью двух других исследований было найти решение проблем, касающихся отбора проб тканей и загрязняющих веществ окружающей среды в Российской Арктике. Основой любого эпидемиологического изучения является эффективный протокол. Были сделаны выводы, что материнская плазма/кровь является самой основной средой для биомониторинга органохлоридов и токсичных металлов по сравнению с кровью из пуповины и грудным молоком. Также для сложного статистического анализа необходимо, чтобы частота обнаружения концентраций отдельного загрязняющего вещества в каждом образце превышала 80%. И в заключение, корреляции между концентрациями различных органохлоридов в биологических жидкостях (за некоторым исключением) были достаточно высокими. Т.о. определение концентрации только нескольких органохлоридов, тех, у которых высокая частота обнаружения, даст соответствующую картину комбинированной нагрузки на организм этих загрязняющих веществ. В заключение Будем надеяться, что РРМО даст бесценный инструмент для изучения в будущем репродуктивного здоровья, например инструмент для изучения неблагоприятного эффекта от веществ, загрязняющих окружающую среду.

This report gives a comprehensive and updated presentation of the impact socioeconomic resources have on the living conditions of Norwegian youth. It aims to describe a broad spectrum of social inequalities between Norwegian youth, based on new data from a national survey - Ungdata. The report covers many aspects of their lives; their relationships with parents and friends, how they spend their leisure time, their expectations and general well-being, as well as issues such as sexuality and sexual offences. Our results are consistent with other studies, which generally show that Norwegian youth are doing quite well, and lead active and meaningful lives. However, one major finding is that there are systematic social inequalities in living conditions and quality of life in almost all of the areas covered in this report. The report shows that adolescents with relatively few socioeconomic resources at home, in general have more complicated relationships with their parents and friends. In addition, these youths enjoy school less, have more health issues, are bullied more, are more pessimistic, they participate less in organized activities, and spend more of their spare time in front of screens than youth who grow up in higher socioeconomic groups. The report uses data from Ungdata – a survey conducted in 183 Norwegian municipalities in 2014–2015. The data stems from responses from 118,000 young people, aged 13-19 (from lower and upper secondary school). The response rate is high, ranging from 81 percent in the lower to 65 percent in upper secondary school. The study is representative for Norwegian youth attending school. In this report, we use a measure of the parents' socioeconomic status, as reported by the respondents. This measure captures the socioeconomic resources the teenagers have access to via their families. The measure was constructed using information about the parents' educational background, amount of books at home and access to various types of material goods and benefits. On this basis, we divided the respondents into five socioeconomic groups. In this report, youth from the different socioeconomic tiers are analyzed and compared. The report also illustrates gender inequalities and variations between youth in different stages of adolescence. Vulnerable families and different parenting styles The report shows that most teenagers are pleased with their parents and have a healthy, open and trusting relationship with them. The vast majority of parents know what their children do in their spare time. Not only do most parents know their children's friends, but they also know the friends' parents. Concurrently the analysis also demonstrates that there are clear socioeconomic differences in the relationships the young have with their parents. More frequently, youth from families in the lower social tiers have a more confrontational relationship with their parents and are more often dissatisfied with them. They argue more often with their parents and report that their parents know less about how they spend their spare time. They also report less emotional parental support. The socioeconomic inequalities are not dramatic, and regardless of their background, most young feel that their parents are positive, supporting and involved. Yet the results suggest that parents in the higher socioeconomic strata more often practice what research usually describes as "authoritative parenting". Authoritative parents are described as being sensitive to the needs of the young, open to input, but also as having firm boundaries, high demands and to communicate their expectations clearly. Research has shown that this type of parenting often is positively associated with the youngsters' social, emotional and cognitive development. The report also shows that youth from lower social tiers to a larger extent use grownups, other than their parents, when they need help with personal problems. Friendships, bullying and social background Peer relationships are a topic in various sections of the report. In general, most youth are satisfied with their friends. Still as much as ten percent reply that they are not sure if they have close friends they can trust. Youth from the lower social strata lack, more than others, confidential friends and they are not as pleased with their friends. It seems as if these teenagers have a weaker standing among their peers than youth in general. Regarding bullying, the report shows a higher prevalence of bullying, being either the bully or the victim, among youth in the lower social strata, regardless of whether the bullying is "face to face" or digital. Youth from lower social strata are both more often victims of bullying, and they participate more often actively in conflicts. Social differences related to school The report confirms findings from other studies, which show that most youth are comfortable at school: they feel that the teachers care and they are socially well adjusted. Although most are comfortable, a minority is unhappy and dread going to school. The percentage who do not feel well at school is approximately twice as large amongst young in the lower socioeconomic strata, compared with those in the highest strata. When teenagers from lower socioeconomic strata report that they are less comfortable at school, it might refer to both the social and educational environment. Youth from families in the lower social strata do less often feel that they fit in socially at school – among peers. However, they report to the same extent as youth from higher socioeconomic strata, that the teachers care about them. The report also shows quite clear patterns in parents' involvement with school, based on their socioeconomic background. Youth from families with higher socioeconomic status more often experience parental help with their schoolwork, and that their parents talk more about school, in addition to being more interested in and praising them for their schoolwork. Analyses of time spent doing homework during weekends indicate that youth from higher socioeconomic strata invest more time in obtaining good grades than youth from families with lower social-economic standing. Still, the differences in time spent on schoolwork outside school is less than one might expect from previous research, which indicates relatively large difference in grades between youth from different social backgrounds. The report shows that already in their teens, youth with different socioeconomic backgrounds, differ in their expectations about what they can achieve in life. Most Norwegian teenagers are optimistic about their future. The higher the family's socioeconomic strata the more optimistic the young are in their perception of their opportunities in the future, be it getting a higher education, happiness, owning a home or employment. Leisure - different arenas different experiences The report illustrates some clear, but opposing social patterns in the young's leisure time habits. Youth from higher social strata more often than others participate in organized activities in their spare time, while youth from lower socioeconomic strata have the most screen time. When young people from different social strata participate in different types of leisure activities, it also implies that they learn different things and acquire different experiences. Organized leisure activities offer meeting places for youth and give them opportunity to develop skills. Often grownups are present to set the terms for the activities, and additionally they contribute help and support. Organizations can also be an arena where young people can develop their identity and gain an understanding of democracy. When youth from lower socioeconomic strata participate less in such activities, they have less access to this type of learning and socializing activities. While youth from families with high socioeconomic status spend more time on organized activities away from home, the opposite is the case with screen activities. The socioeconomic differences are largest between the heavy users – those who spend more than four hours a day in front of a screen – and the social gradient is largest among the girls. The largest difference is found in the girls' gaming activities (computer games and smaller games on cellphones / tablets), watching TV, movies or series, in additions to using social media. Girls from homes with lower socioeconomic status use these more extensively. The survey shows that most parents, regardless socioeconomic status, are relatively open-minded and have a liberal attitude towards their youngsters' online activities. Only a minority reported having rules for usage and few say they hide their online activities from their parents. Youth in families with lower socioeconomic status experience less frequently that their parents know what they are up to online, and they more rarely experience their parents as good role models for use of social media. Social differences in health and wellbeing Regarding health, the report shows that social background matters. Compared with young from the higher social tiers, youth from lower strata in general are less pleased with themselves and their looks, and fewer report that they are satisfied with their own health. Youth from lower social strata more frequently experience physical ailments in their daily life and use painkillers more extensively. There are also social differences in mental health; especially girls from the lower strata more often suffer from anxiety and depressive symptoms. As in most other areas we have studied, there are gradients in the social health differences. The extent of the difficulties decrease gradually as the socioeconomic resources in the family increase. A consistent finding is that social inequalities in health are more evident among girls than among boys; the girls stand out as having most ailments. An example is that headaches and other physical pains bother significantly more girls than boys. The extent of mental health problems is two to three times as high amongst girls as boys. Girls are also far less satisfied with their appearance and self-image than boys are. Regarding the use of health services, the report generally shows rather small socioeconomic differences. Meanwhile, some differences go in slightly different directions for boys and girls. Visits to youth health, school health and psychologist / psychiatrist are, however, more common among youth in families with low socioeconomic status, particularly for girls. Both boys and girls state that physical health issues are the most common reason to seek school health services. Many also seek out school health services to get help or answers to questions about sexual health, mental health, well-being and friendship, and family issues. Overall, we do not find systematic patterns in the reasons youth with different socioeconomic backgrounds report. The exception is that young people from families with low socioeconomic status more frequently than others turn to school health services because of family problems. Few systematic differences in sexual experiences Adolescence is a time in life when many people have their first sexual experiences. The proportion who have ever had sex with anyone increases tenfold from the beginning of middle school to the end of high school, from seven to 73 percent. The increase is largest from mid-teens upward. The report concludes that there are no socioeconomic differences in terms of how many boys have made their sexual debut. However, we see a tendency that relatively more girls from families with lower socioeconomic status have made their sexual debut, compared to girls from the higher strata. The analyses also show that few of the sexually active young have experienced involuntary pregnancy. Simultaneously about 40 percent of the boys and one in four girls state that they did not use contraception the last time they had sex with someone. The analyses of social inequalities does not provide grounds to conclude that youth from lower social strata have unprotected sex more often than others. However, the analyses of sexual experiences are based on a smaller sample than other topics in this report. Therefore, we cannot be quite as sure, as elsewhere in the report, that these data are representative of most youth in Norway. Quite a few youngsters report exposure to various types of sexual offenses, this applies more to girls than to boys. Exchanging sex for material goods however is not prevalent. The boys report such actions somewhat more frequently, but the analyses only show minor differences in the adolescents' socioeconomic background. Nor, when it comes to sexual harassment, is there a clear pattern to suggest that young people from lower social strata are more susceptible. In some areas, these data nevertheless suggest that there are social patterns according to families' socioeconomic status. For example, we see a tendency that some types of sexual offenses – such as being exposed to rumors of a sexual nature and physical harassment - are more prevalent among girls from families with lower socioeconomic status. ; Har ungdommenes sosioøkonomiske bakgrunn betydning for det livet de lever til daglig? Basert på nye tall fra Ungdata gir rapporten en bred beskrivelse av sosiale forskjeller blant gutter og jenter i Norge. Rapporten tar for seg ungdoms forhold til foreldre og venner, trivsel og innsats i skolen, framtidsplaner, deltakelse i organiserte fritidsaktiviteter, digital fritid, helse og trivsel, samt seksualitet og seksuelle krenkelser. Hovedfunnet er at selv om norske ungdommer har det bra på mange av disse områdene, er det systematiske mønstre i hvem som ikke har det så bra: desto flere sosioøkonomiske ressurser ungdommene har hjemme, desto høyere er ungdommenes livskvalitet.

The broader aim of this thesis is to contribute to the understanding of the production and reproduction of spatial inequalities following from the restructuring of the public healthcare system. More specifically, by analyzing the contention around healthcare restructuring related to two cases spanning a longer period in northern Sweden, I aim to investigate the changing conditions for healthcare provision in rural and sparsely populated areas, and I explore the forms of collective action that local people engage in to sustain the access to healthcare, as well as how state authorities' attitudes towards such collective action have shifted. In the context of larger public healthcare restructuring in contemporary Sweden, where the marketization and privatization of healthcare since the 1990s have impacted the provision of healthcare across the country, rural areas are experiencing deteriorating accessibility to both primary healthcare as well as emergency healthcare. This development is increasingly contentious, and is frequently met with resistance from rural populations as well as various strategies to rework these uneven conditions. The first case concerns the preceding protests as well as the occupation and opening of a citizen cooperative primary care center in Sollefteå, Västernorrland, in response to cutbacks at the local hospital. The second case follows the worker-cum-citizen cooperative primary and occupational healthcare centers in Offerdal, Jämtland. Through these two cases I explore people's experiences of public healthcare restructuring, their motivations for engaging in contention around it, their experiences of self-organizing cooperative healthcare, as well as their visions and desires for a future healthcare. As shown throughout this thesis, healthcare restructuring is highly contentious and comes in many forms, ranging from protests, demonstrations, and occupations of healthcare facilities to the self-organization of healthcare services through worker and citizen cooperatives. Healthcare restructuring marked by spatial concentration and withdrawal has thus given rise to a number of drawn-out and spectacular collective actions in contemporary Sweden, but responses can also take the form of low-key efforts to maintain healthcare provision. The healthcare authorities' attitude towards such low-key efforts by not-for-profit healthcare providers has shifted from a favorable approach in the 1990s to emphasizing their role in safeguarding fair market conditions in the healthcare market. This shift has created a more hostile welfare state landscape for not-for-profit healthcare providers in rural areas, which exacerbates the already unfavorable conditions they operate under. Rural populations' efforts to remedy the withdrawal of public healthcare are thus highly precarious. While reworking uneven healthcare provision, they operate in this increasingly hostile welfare state landscape, which is not adapted to either rural areas or not-for-profit healthcare. In practice, public healthcare restructuring and withdrawal amount to a cutback in healthcare provision for rural populations. This transfers the work of sustaining social reproduction to the private sphere, in this case not-for-profits healthcare providers. The public healthcare restructuring and withdrawal outlined in this thesis thus present an example of a form of 'rural neoliberalism', whereby rural populations are dispossessed of welfare services that instead accumulate in urban areas, which both increases and is connected to larger questions around spatial (in)equalities and the restructuring of the public sector in contemporary Sweden. Nevertheless, those engaged in contention around and the self-organization of healthcare nurture visions and desires for a future healthcare system that would take a holistic approach to the patient and make possible a more equitable access to healthcare. ; Offerdal och Sollefteå – lokalt engagemang och motstånd mot sjukvårdens omstrukturering Denna studie tar sin utgångspunkt i två fall av konflikter runt sjukvårdens omvandling på olika platser i norra Sverige för att studera de erfarenheter som finns av denna omvandling och dess effekter. Det första fallet är protesterna kring nedskärningarna på Sollefteå sjukhus som började 2015. Dessa protester har utvecklat sig från demonstrationer under 2015-2016, till en ockupation av sjukhusentrén i januari 2017, till att öppna en medborgarägd kooperativ vårdcentral i december 2017. Medan ockupationen fortfarande pågår när detta skrivs, så gick den medborgarkooperativa vårdcentralen som ägdes av 200 medlemmar i konkurs inom ett år, i november 2018. Det andra fallet är den kooperativa vårdcentralen Hälsorum Offerdal i Änge, Jämtland som har existerat sedan 1992 då arbetarna tog över vårdcentralen efter att regionen beslutat att stänga vårdcentralen. Under 2010-2011 omvandlades vårdcentralen från att vara ett arbetarkooperativ till att bli ett medborgarkooperativ ägt av 600 medlemmar i bygden och 2012 öppnade kooperativet också en företagshälsovård. I dessa två fall finns erfarenheter från nedskärningar och fråndragande av sjukvård i lands- och glesbygd. Dessa erfarenheter skiljer sig dock åt. Medan det i Sollefteås fall handlade om akutsjukvården och BB som lades ner, så var det i Offerdals fall primärvården som skulle stängas. Hur medborgarna har agerat skiljer sig också åt. I Offerdal bestämde sig arbetarna på den landstingsdrivna vårdcentralen att ta över driften när landstinget ville lägga ner den i början av 1990-talet. Både landstinget och kommunen var väldigt intresserade av att utforska möjligheterna med en annan driftsform och att de tog över vårdcentralen som just ett arbetarkooperativ var en av förutsättningarna för att landstinget skulle gå med på omvandlingen. De drev vårdcentralen tillsammans under många år där de förnyade olika aspekter av arbetet, bl.a. genom att tidigt digitalisera journalerna. Under årens lopp har de vid ett flertal tillfällen varit nedläggningshotade, men har varje gång lyckats avvärja hoten genom att visa på vilka bra resultat de gjort i jämförelser mellan landstingets egna vårdcentraler och kooperativet. Under 2000-talet började det dock bli svårare att driva vårdcentralen, dels eftersom flera medlemmar och personal började gå i pension, dels för att dessa var svåra att ersätta, speciellt de pensionerade läkarna. Att rekrytera läkare utanför städerna började bli svårare och det var under den här tiden som stafettläkarna som fenomen började växa. När den nya Lagen om vårdvalssystem (LOV) kom runt 2010 behövde kooperativet tänka om eftersom det skulle innebära mer ansträngande ekonomiska villkor för dem. De kontaktade Offerdals befolkning för att se om de var intresserade av att engagera sig i vårdcentralens överlevnad. På ett stormöte i Kaxås bestämdes det att kooperativet skulle omvandlas till ett medborgarkooperativ och när den omvandlingen var klar några månader senare hade kooperativet gått från att ha tre kvarvarande medlemmar till att ha mellan 600-650 medlemmar, nästan en i varje hushåll i Offerdalsbygden. Trots detta var bekymren inte över, utan de behövde på något sätt hitta en stabilare bas för att klara av de nya ekonomiska villkoren. Samtidigt planerade Krokoms kommun för att förnya sin företagshälsovård och upphandla den genom en kvalitetsupphandling som fokuserade på att förbättra de anställdas hälsa. Där föddes idén att starta en företagshälsovård som skulle kunna fungera som ett andra ben att stå på för kooperativet. Hälsorum Offerdal vann upphandlingen och började 2012 bygga upp en företagshälsovård. Genom denna unika kombination av kooperativ vårdcentral och företagshälsovård har de lyckats behålla primärvård i glesbygd under snart tre årtionden. Sollefteå's historia är en annan. När regionen under 2015 annonserar ett sparförslag som innebär att de ska skära ner på sjukvården och speciellt BB i Sollefteå och Örnsköldsvik, men också akutsjukvården i Sollefteå ska läggas ner börjar både befolkningen i stort men också arbetarna på Sollefteå sjukhus att mobilisera sig. Den första demonstrationen sker ganska spontant i Örnsköldsvik i september 2015 samtidigt som nedläggningen av BB där tas bort från sparförslaget. Några veckor senare följs demonstrationen upp av en andra demonstration i Kramfors som samlar uppåt 14000 deltagare. Efter denna massiva mobilisering ersätts regiondirektören och den styrande majoriteten lovar ett omtag; de ska skicka tillbaka sparförslaget för vidare utredning. När lite verkar hända med omtaget anordnas en tredje demonstration i Härnösand i april 2016 för att pressa politikerna att stoppa nedskärningarna. Trots dessa demonstrationer fortsatte nedskärningarna av de olika delarna att ske stegvis, först akutortopedi innan sommaren, sen BB som beslutades att stängas sista januari 2017, och sist akutkirurgi strax efteråt. Stängningen av BB var det som orsakade mest missnöje eftersom det symboliserade livet och möjligheterna att födas, leva och bo i glesbygd. Några veckor innan stängningen av BB organiserade ABF i Sollefteå en studiecirkel i bilbarnfödslar, hur man ska agera om man behöver föda på vägen till BB. Detta skapade stora rubriker och medias fokus hamnade på nedskärningarna på Sollefteå sjukhus. Dagen innan nedläggningen av BB gick en grupp medborgare in och ockuperade entrén till sjukhuset för att tvinga politikerna att lyssna på protesterna. Ockupanterna var ovana att ockupera och organiserade sig allt eftersom och byttes av i olika pass. Ännu i mars 2020 pågår ockupationen av Sollefteå sjukhus och är på väg att bli Sveriges längsta ockupation och genom åren har den fungerat som en samlingsplats för både politiker och medborgare att samlas och diskutera välfärdsfrågor. Ungefär samtidigt som ockupationen startade, började en annan grupp att planera för att starta en kooperativ vårdcentral. Först fanns planer på att ta över sjukhuset i kooperativ form men dessa omvandlades till att handla om primärvården. Efter mycket arbete öppnade en medborgarkooperativ vårdcentral i Sollefteå i december 2017. Denna vårdcentral fick tidigt många anställda läkare och annan personal och många patienter listade sig där. Trots detta fick vårdcentralen efter en tid ekonomiska bekymmer, då ersättningssystemet inte ersatte dem tillräckligt för de utgifter de hade för de många äldre och multisjuka patienterna. När detta blev känt under hösten 2018 började en insamling av pengar för att möjliggöra för kooperativet att dels kunna hantera en akut ekonomisk kris men också att kunna fortsätta driva vårdcentralen på en stabilare ekonomisk grund. Trots dessa insatser gick den medborgarkooperativa vårdcentralen i konkurs i november 2018, mindre än ett år efter att den öppnade. Vårdens och välfärdens förändrade villkor Sedan 1990-talet har den svenska välfärdsstaten genomgått en omvandling som har inneburit att relationen mellan stat, näringsliv och civilsamhället har förändrats. Detta har betytt att civilsamhällets organisationer har gått från att vara en röst åt folket, till att i allt större utsträckning producera välfärdstjänster själva. Denna förändring är del av en större omvandling som har lett till förändringar i hur och vem som producerar välfärdstjänster, från att dessa tidigare i hög utsträckning producerades inom det offentligas ramar, till att allt fler av dessa välfärdstjänster produceras av privata aktörer på en marknad, eller av det offentliga enligt marknadsliknande principer. Detta är del av en större trend mot marknadisering av välfärdstjänster i Europa, där också tankar om aktivt medborgarskap ingår. Dessa idéer går ut på att ett större ansvar för välfärden läggs på medborgarna som ett sätt att täcka upp för välfärdsstatens fråndragande när staten inte längre anser sig ha råd med välfärd på lika villkor som tidigare. Dessa förändringar har omvandlat också den offentliga sjukvården i Sverige. Medan den svenska sjukvården fortsatt är offentligt finansierad, så har sjukvården i stort genomgått en marknadisering. Samtidigt har främst primärvården genomgått en snabb privatisering, speciellt efter införandet av lagen om vårdvalssystem (LOV) 2010. Denna privatisering har lett till en stor andel vinstdrivande privata aktörer för individen att välja mellan på primärvårdsmarknaden. Samtidigt har resultaten av denna privatisering varit ökande geografiska skillnader mellan stad och landsbygd, men också mellan olika klasser i samma område. Det finns få icke-vinstdrivande företag inom den svenska sjukvården, där de två kooperativ inom primärvården som studeras här utgör hälften av den kooperativa primärvård som finns. Dessa två fall ger insyn i hur regionernas syn på icke-vinstdrivande vårdcentraler har förändrats över tid. Från en vilja att experimentera och stötta alternativa organisationer under början av 1990-talet, har den gått mot 2010-talets fokus på att skapa en sjukvårdsmarknad. Detta blir speciellt tydligt genom Offerdals erfarenheter av att gå in i det nya vårdvalssystemet 2010, då de inte längre kunde räkna med någon support från regionen för att kunna hantera detta, trots att de hade levererat primärvård i Offerdalsområdet i nästan två årtionden. När vi sen några år senare kommer fram till Sollefteå medborgarkooperativs grundande och senare ekonomiska problem så framstår denna attitydförändring ännu tydligare, det nya välfärdslandskapet var oerhört svårt att navigera som icke-vinstdrivande kooperativ vårdcentral och de ogynnsamma villkoren ledde till slut till medborgarkooperativets konkurs. Dessa ogynnsamma villkor för icke-vinstdrivande företag inom vårdsektorn blir extra prekära i situationer när den offentliga sjukvården drar sig tillbaka, vilket till stor del sker i svensk land- och glesbygd. När medborgarna då själva tar över produktionen av sin egen sjukvård faller en tung börda på kooperativa organisationer och dess medlemmar och anställda för att försöka hantera och navigera dessa villkor. Detta innebär i praktiken en privatisering av ansvaret för att bibehålla sjukvård på landsbygden, där staten frånsäger sig ansvaret och lämnar över det på individer, familjer och olika typer av icke-vinstdrivande organisationer. Denna privatisering är ett fråndragande av sjukvård från vissa områden av landet vilket möjliggör en koncentration av dessa sjukvårdsresurser i andra områden och en ackumulation av resurser i privata vinstdrivande företags händer. Framtidens sjukvård? Deltagarna i denna studie har många tankar och idéer om hur de skulle vilja att framtidens sjukvård såg ut. Dessa önskningar och idéer rör hur patienterna ses som en helhet, en hel människa som är mer än sin diagnos eller sjukdom, där olika aspekter av hälsa beaktas och patienter inte skickas mellan olika delar av sjukvården. Dessutom ser de också möjligheter för att organisera sjukvården på olika nivåer som skulle kunna möjliggöra en mer jämlik sjukvård. Dessa idéer och önskningar handlar om var ansvaret för olika typer av vård ska ligga; på statlig, regional eller kommunal nivå. Medan det finns olika idéer om lösningar är fokus på hur jämlikhet kan skapas.
