Aims: To commemorate the centennial of the geriatrics, the authors present a review of the population aging, the elderly individual, the characteristics of the geriatric medicine, and its history. Source of data: A throughout review of the literature found on MedLine and LILACS, as well as textbooks, dissertations, thesis, and government directives was performed. Summary of the findings: Geriatrics is the area of medicine that cares for the health and treatment of diseases in old age, which works with physical, mental, functional and social aspects in the acute, chronic, rehabilitative, preventive and palliative care of the elderly. Geriatrics goes beyond "medicine centered on organs and systems", offering holistic treatment by interdisciplinary teams with the principal objective of optimizing functional capacity and improvement of quality of life and autonomy of the elderly, which make up the core of this medical specialty. Geriatrics, in daily assistance practice, works with particular questions such as: the heterogeneity of patients and of scenarios of care; concomitance of multiple diseases/disorders (plurimorbidity), including geriatric syndromes ("the giants of geriatrics"); difficulty of identifying all the problems (iceberg phenomenon); polypharmacy; fragility; vulnerability; the various losses that the elderly show; and terminality. Conclusions: Geriatrics refers to the medical specialty responsible for the clinical aspects of aging and for the various forms of health care necessary for the elderly. Because of the complexity allied to the aging process, geriatrics has become an intriguing, challenging and contemporaneous specialty. ; Objetivos: para comemorar o centenário da Geriatria, os autores apresentam uma revisão sobre o fenômeno do envelhecimento, o indivíduo idoso, as características da medicina geriátrica e um resumo da sua história. Objective: to examine the life trajectory of women with mastectomy members of a self-help group. Method: descriptive and exploratory study, with a qualitative approach, using the approach of the oral history of life as a methodological benchmark, meeting the operational phases. The participants were 20 women with mastectomy, whose narratives, considered discursive manifestations were submitted to the analysis of the collective subject discourse. Results: three thematic axes were elaborated, supported by their respective core ideas, each one was referent to before, during and after mastectomy: "to face life bravely" - "normal life", "separation", "change", "work" and "children"; "I have talked to God" - "prevention/care procedures", "diagnosis/treatment", "feelings/emotions" and "changes"; "reorganization with more objectivity and experience" - "creed", "change" and "feelings/emotions". Conclusion: it is recognized the need for studies into the mastectomy women to support public policies, or simply to enable health professionals to a humanized care.
Objective: To present the changes occurred in the daily life of men with cancer. Method:It is a qualitative research, accomplished from March 2006 to December 2007. The semi-structured interview was used as instrument and the data were organized starting from the thematic analysis. Results:It has verified that several modifications in the daily life of the men have been modified, especially, in aspects related to sexuality, corporal image,and social life, financial and labor aspects, in which they had to introduce strategies to face them. Conclusion: The study made possible to understand how the daily life of the individual with cancer has suffered modifications and it provided a greater sensibility about the health topic of men.
RESUMO Objetivo: refletir sobre a relação da Residência Multiprofissional em Saúde no processo saúde-doença e na qualidade de vida do profissional de saúde residente. Método: trata-se de um estudo qualitativo, do tipo reflexivo. Refere-se a coleta de dados a uma pesquisa documental e à revisão de literatura com busca nas bases de dados PUBMED/MEDLINE, LILACS, e biblioteca virtual SCIELO com 16 artigos submetidos à análise. Resultados: destacam-se as categorias "Condicionantes legislativos da residência e a interface na saúde do trabalhador" e "Residência versus qualidade de vida". Conclusão: percebem-se condicionantes negativos sobre a residência que podem influenciar o adoecimento do residente. Espera-se, porém, que esta pesquisa contribua para gerar subsídios tanto de políticas públicas sobre a residência, como de material para aperfeiçoar o regulamento interno dos cursos, políticas essas que ofereçam diretrizes que favoreçam o desenvolvimento de boas práticas em formação e saúde. Se objetivó discutir formas de organización del trabajo de enfermería en atención Prehospitalaria, considerando la caracterización de los eventos atendidos por una Unidad de Soporte Básico, de acuerdo a legislación que reglamenta y disciplina el ejercicio profesional de la categoría en Brasil. Estudio descriptivo, retrospectivo, realizado con el Servicio de Atención Móvil de Urgencias en Cuiabá-MT. Datos recolectados de fichas de atenciones realizadas en el primer trimestre de 2013. Fueron analizados 926 registros, constatándose que tanto los eventos como las acciones de enfermería configuran mayoritariamente acciones complejas, demandando competencias técnicas y legales para toma de decisiones, lo cual remite a la necesidad de presencia del enfermero. Se consideró así que la organización de la atención Prehospitalaria, normada por el Ministerio de Salud, se contrapone con la legislación de enfermería brasileña, y que los profesionales técnicamente responsables por el trabajo o actuantes en enfermería, infringen postulados ético-legales de la profesión. ; Objetivou discutir a forma de organização do trabalho de enfermagem na assistência pré-hospitalar, considerando a caracterização das ocorrências atendidas por uma Unidade de Suporte Básico, à luz da legislação que regulamenta e disciplina o exercício profissional da categoria no Brasil. Considerou-se assim que a forma de organização da atenção pré-hospitalar, normatizada pelo Ministério da Saúde, afronta a legislação da enfermagem brasileira e que os profissionais que respondem tecnicamente pelo trabalho de enfermagem ou nela atuam, infringem postulados ético-legais da profissão. ; The aim of this study was to discuss how nursing work is organized in pre-hospital care, considering the characterization of incidents assisted by a Basic Support Unit, observing the legislation that regulates this category's professional practice in Brazil. A descriptive, retrospective study conducted with the Emergency Mobile Care Service in Cuiabá, state of Mato Grosso. Data were gathered from the records of incidents from the first quarter of 2013. A total of 926 incidents were analyzed, and showed that injuries and nursing care are mostly of a complex nature, demanding technical and legal competencies for decision making, which takes us back to the necessary presence of a nurse. Thus, the model of organization of pre-hospital care, standardized by the Ministry of Health, is against the Brazilian nursing's legislation and workers who are technically responsible for nursing work or whose practice in nursing infringe upon the ethical and legal postulates of the profession.
Objectives: To analyze the perception of HIV positive women regarding the decision to become pregnant; investigate women's knowledge about HIV positive health care in the prevention of vertical transmission. Method: This is a descriptive and exploratory qualitative research. We used a script of questions and the interviews were taped and later transcribed. Results: The study revealed that positive serology for HIV is not a limiting factor for the decision to become pregnant. Although the fear of transmitting the virus is present, it is overcomed by the desire to be a mother and the expectation of having a healthy child. Conclusion: Reproductive decisions of HIV positive women should be valued during the health care assistance so they can make consciously, democratic and emancipatory life decisions.
Life and death in the daily: reflections with the professional of health was written with the purpose of contribute with the project of the State Politics of Humanization of the assistance and of the Health Management of the State of the Ceará, that defines in your guidelines " to provide the appropriate preparation to the experience of losses, of suffering, of dying and of the death ". The work is organized in thematic about of the senses of the death and of dying in the occident. It presents four chapters that talk about the life and the death in a valuable contribution about the understanding of the death as indispensable part of the life and your inclusion in the cares offered by the professionals of the health. The authors provoke in a competent and sensitive way the rethinking about the subjects of the death and of the dying, starting from the reality lived by the professionals. They put in evidence an indispensable theme for taking care humanized in the practices of health and not give the reference to the patients and your relatives, which in some way share with the professionals of the process of taking care. The first chapter: the death and the dying in the life and your senses, contains teachings retired of the poets that through your music and poems portray the death in the most different ways, but recognizes it as part of the life in your fullness. They present an abbreviation revision about the death´s history, the transformations happened along of the time and your several meanings. Concluding the chapter the authors reminds the fact that the man is the only animal that knows that will die and this conscience provokes fear and solitude feelings. They also portray importants subjects as the aging, the losses and your implications, that are going happening along the life, and last they talk about the science´s power, that tries to postpone the death considering that in this moment the biggest part of die´s process is the professional's of the health responsibility.The second chapter: the professional of the health before the death, treats of the professional's unpreparedness, it presents some basic concepts as euthanasia, medical futility, to discuss to subjects of the emotion, of sense of the life and of the death for each person, of the need of moments for that the professionals can to reflect on the own death. To think in the own death means to reflect about the life as not as a group of vital signs that have to be monitored. The reading invites us to consider the death as not as enemy, but as a complement of the life, in that life and death form the totality of being. The third chapter: the appeal to the re-humanization of the death. The authors reflect about the technological progress of the Medicine that at every cost tries the prolongation of the life and that not always those actions are accompanied of help for a good death. They point out, in this context, the solitude, the impersonal and the inhumanization of the Being that is dying. In this same chapter is discussed still the relevance of the palliative cares and the twelve beginnings for a good death published in England.>In the fourth chapter, practical application: like this in the life as in the death, the authors propose a series of practical activities for be developed in groups, with the objective of to sensitize the professionals to share, in wheels of chats in the services of health about the life and the death. Los autores provocan de una manera competente y sensible o repensar sobre las cuestiones de la muerte y de el morir, a partir de la realidad vivieron por los profesionales. Concluyendo el capítulo los autores recuerda el hecho que el hombre es que el único animal que sabe cuando se morirá y esa conciencia provoca sentimientos de miedo y soledad. Retratan también asuntos importantes como el envejecimiento, las pérdidas y sus implicaciones, que van pasando a lo largo de la vida, y último abordan el poder de la ciencia, que intenta posponer la muerte considerando que en este momento la mayoría del proceso del morir es responsabilidad el profesional de la de salud.El segundo capítulo: el profesional de la salud enfrente de la muerte trata de el desarreglo de los profesionales, presenta algunos conceptos básicos como la eutanasia, el distanasia, para discutir los asuntos de la emoción, del sentido de vida y muerte para cada persona, de la necesidad de momentos para que los profesionales puedan reflejar sobre la propia muerte. Pensar en la propia muerte significa reflejar sobre la vida no como un conjunto de señales vitales que se debe supervisar. Considerar la muerte no come al enemigo, pero como un complemento de la vida. La vida y la muerte forman la totalidad del ser.El tercer capítulo: la apelación para la re-humanización de la muerte. Zimbabwe is today affected by a grave & generalised crisis of unparalleled proportions. To add to the climate of violence & intimation that has characterised life in this Southern African country, a humanitarian emergency without precedents has unfolded with close to 4 million Zimbabweans requiring food aid & urgent medical assistance. The political dimensions of the crisis have dominated international policy towards Zimbabwe, in an effort to pressure Robert Mugabe's ZANU-PF regime to negotiate with the MDC, to respect human rights & fundamental liberties, to repeal the set of draconian laws which affect all areas of life in the country &, finally, to implement transparent & free & fair elections. This article provides a critical reflection on the multifaceted aspects of the crisis, with a view to providing the basis for a comprehensive discussion of possible multi-pronged strategies for a new strategic & operational level re-engagement with the country. Objective: Analyze how scientific studies describe the burnout syndrome in nurses. Methods: Integrative review of literature, in databases BDENF, LILACS, MEDLINE and Cochrane library, using descriptors of stress, burnout, hospital and nursing, in Portuguese and/ or its variations in English. Results: Only 14 articles entered in the selection because they fit the inclusion criteria and include answers to the guiding question (three articles spoke of leisure, hours of work and quality of life, five articles describing stress; seven articles detailing burnout, and only one article described the quality of life and stress simultaneously). Conclusion: The papers presented concern only in areas of the hospital environment, especially in closed areas, with lack of research outside of these.
This article considers, initially, the mobility of doctors throughout the world from a bibliographic database collection and identifies the lack of information regarding Brazilian doctors. The aim is to analyze aspects that determine the emigration of Brazilian doctors to the United States of America. The methodology is based on bibliographic research using the keywords brain drain, medical migration, physicians migration, data migration physicians; identification of articles related to the emigration of doctors throughout the world; elaboration and validation of the questionnaire "Emigration Motives"; identification of doctors that emigrated, using the "snowball" technique; sending of the questionnaire by e-mail to the doctors that emigrated to the USA; tabulation of the forwarded answers; Skype interviews aiming at the validation and illustration of the results obtained in the questionnaire. Initially, the doctors choose to emigrate for personal motives (family, professional opportunities, and, in general, absence of language barriers); when established in the USA, they experience a new way of life that makes them stay (better work condition, quality of life, family and general opportunities); external motives become the cause for staying in the USA (insecurity, professional, political and economic scenarios). The conclusion is that an emigrational process of Brazilian doctors to the USA exists and, at first, the reason to emigrate is not well defined; salary is not mentioned as a primary reason; the presence of the family eases the stay in the country; the proficiency in the English language is fundamental and necessary to restart the professional life as a recently graduated in medicine since there isn't an university degree or medical specialty degree validation. ; Este artigo aborda a mobilidade de médicos pelo mundo a partir de levantamento bibliográfico em base de dados e identifica a escassez de informações referentes aos médicos brasileiros. O objetivo é analisar aspectos que determinam a emigração de médicos brasileiros para os EUA. A metodologia baseia-se em pesquisa bibliográfica, utilizando as palavras-chave "brain drain", "medical migration", "physicians migration", "data migration physicians"; identificação de artigos relacionados à emigração de médicos pelo mundo; elaboração e validação do questionário "Motivos de Emigração"; identificação de médicos que emigraram utilizando a técnica "bola de neve" ; envio do questionário por e-mail aos médicos que emigraram para os EUA; tabulação das respostas encaminhadas; realização de entrevistas por Skype com a finalidade de corroborar e exemplificar os resultados obtidos nos questionários. Inicialmente, os médicos escolhem emigrar por motivos pessoais (família, oportunidades profissionais e, em geral, facilidade do idioma); ao se estabelecerem nos EUA vivenciam uma nova forma de vida, o que os faz permanecer (melhores condições de trabalho, qualidade de vida, família e oportunidades em geral); as causas do não retorno ao Brasil passam a ter motivos externos (insegurança, cenários profissional, político e econômico). Conclui-se que existe um processo emigratório de médicos brasileiros para os EUA e, a princípio, a motivação de emigrar não é bem determinada; o salário não é citado como questão primordial; a presença da família facilita a permanência no país; a fluência na língua inglesa é fundamental; e é necessário recomeçar a vida profissional como um recémformado em medicina, pois não existe processo de validação de diploma ou de especialidades.
The impediment of individuals affected by mental illness at risk of suicide is well justified. But the impediment to rational suicide is complicated by the conflict between defending life and defending autonomy, as illustrated by the various philosophical approaches to the ethical dimension of suicide. In this article we argue that, insofar as punctual in nature and limited in scope, paternalistic intervention of the autonomous individual is also justified. To do so, we will use the distinction between shallow and deep autonomy, as well as complicating factors from recent medical literature: the question of ambivalence and the cry for help model. We will also pay special attention to the ethical implications of new strategies for risk detection that use artificial intelligence applied to social network databases like Facebook. Although preliminary evidence suggests these are effective tools, their lack of transparency, regulation and consent puts civil liberties and rights at risk.
This book develops a legal history of colonial women as a methodological approach to studying the women of Paraíba, a captaincy on the northeast coast of Brazil, from the end of the Dutch occupation (1661) to Brazilian independence in 1822. It uses the concept of multiple normativities to analyse dozens of daily life cases from Portuguese and Brazilian archives. To study women's everyday normative contexts in a colonial space, the author analyses traditional Ius Commune and Portuguese legal sources from different jurisdictions, but also legal doctrines, medical treatises, moralist works and literature to enrich interpretations in women's history, gender studies, feminist legal theory and legal history. Furthermore, she examines the impact of these normative traditions in the colonial Captaincy of Paraíba and focuses on normativities of a more pragmatic character, analysing archival documents portraying women's daily life situations relating to both secular and religious jurisdictions. The analysis demonstrates that the law from the metropole neither offered pre-established solutions for women's daily lives, nor was it applied unchanged in the colony. On the ground, law was dynamic, and the interplay of multiple normativities provided different possibilities that depended on the intersection of women's condition and status, religion and sexual options, proving that sex and gender categories are not immutable, but, on the contrary, flexible according to the practices of law in colonial Paraíba.
Objective: Knowing the relationships of socialization between the community of Caxias do Sul city, and the residents of Therapeutic Residential Service (SRT). Method: This is an excerpt of the Research Project about Networks which Rehabilitates - evaluates innovative experiences of composing networks about psychosocial care (REDESUL), which used the field diaries of four researchers who performed 700 hours of participant observation on the daily lives of residents and their social network. Results: The results comprise two themes: SRT Strategies for Reintegrating residents in the community, understanding of community neighboring the SRT about psychosocial rehabilitation. Conclusion: The service demonstrates compromise in reintegration of people in psychological distress, and observed that the community has a good relationship with the locals, recognizing that the best way to treat is freedom.
Objective: To comprehend the knowledge and practices of caregivers of elderly people with Alzheimer disease (AD). Method: A descriptive and qualitative conducted with relatives and/or caregivers of AD patients enrolled in the Unimed of North of Minas Gerais. Data were obtained by interview and recorded. Results: The results showed the following categories: "Knowledge about Alzheimer disease", "Care provided to patients with the disease", "Feelings when caring for the elderly with Alzheimer Disease". It is observed that the knowledge of caregivers about AD is confused. Due to this daily care performed by these caregivers, many impacts can be seen in the lives of respondents, such as changes in daily life, financial difficulties and impacts on their health. Conclusion: The need of a multidisciplinary team of caregivers for guidance on issues related to the disease in order to they can learn to deal with the elderly suffering from Alzheimer disease.
