The article presents a sociological analysis of the mortality rate of the population of post-Soviet Russia. The socio-demographic characteristics of respondents who passed away due to diseases of the circulatory system, respiratory system, cancer, suicide, homicide, external causes and due to the old age are analyzed. Statistical data of Rosstat (2018) and sociological databases of RLMS-HSE (2001–2017) are used as an empirical basis, the results of the analysis are consistent. In the conditions of Russia's depopulation, not only the health of an individual is important, but also the health of the nation as a whole. As the analysis reveals, only 5% of the population of modern Russia die of old age, while 95% of deaths are a consequence of social factors. In the opinion of the author the reasons for this situation are the optimization of the Russian health care system, the "modernization" of the economy, a dangerous ecological situation, degradation of the institutions of family and marriage, of the educational institutions, unemployment, low incomes of the population, etc. The article analyses the social factors affecting the causes of death of men and women, socio-demographic portraits of the deceased are given. The author demonstrates the unfolding demographic catastrophe of modern Russia - a decrease in the birth rate and an increase in mortality, that will inevitably be followed by an economic collapse and social chaos. The analyzed problem is not unique and characteristic only for Russia. This catastrophe looms over the countries of Eastern Europe and the Baltic states.
The data from the Russia Longitudinal Monitoring Survey by the National Research University Higher School of Economics and Demoscope LLC with the participation of the Population Center of the University of North Carolina (Chapel Hill) and the Institute of Sociology of the Federal Research Sociological Center of the Russian Academy of Sciences (Moscow) was used as an empirical sociological basis for the study.
This paper provides to the explanation of Russian peculiarities of excess mortality among young adults. Social and economic crises of the 1990s exerted a negative effect on the demographic processes in Russia of that time. The greatest increase in mortality was observed among young adults. The long-term effects of economic and social changes have also influenced the dynamics of mortality in post-crisis generations. The fact has conditioned the Russian peculiarities of mortality among young adults. The paper presents the results of the study which explains the phenomenon of excess mortality in young adults and shows the Russian peculiarities of mortality. The author identifies the changes in age limits of excess mortality among young adults, connected with the negative dynamics of mortality within the generation of the 1970s compared with neighbouring generations. To confirm the hypothesis that the dynamics of mortality in the generation of the 1970s will be a long-term one the author resorts to APC (Age, Period, Cohort) analysis. The study confirms the high importance of the cohort effect on mortality among the generation of the 1970s. This generation is most vulnerable to mortality and shows lower rates of decline in mortality in recent years as compared to other generations. If the trend of mortality continues, the aging generation of the 1970s will increase the negative impact on further rise in anticipated life expectancy.
Цель. Целью статьи является изучение младенческой смертности на Южном Урале в 1930-е годы и установление её причин. Актуальность исследуемой проблемы связана с трудностями решения демографических проблем в стране и с политикой государства по защите материнства и детства.Метод и методология проведения работы. Основу исследования составляют историко-сравнительный и историко-системный методы, а также критический анализ.Результаты. Младенческая смертность является самым существенным признаком демографического спада среди населения. Рассматриваемый период стал одним из самых сложных в истории нашего народа, так как сопровождался крупными экономическими, социальными и политическими процессами, повлиявшими на снижение естественного прироста среди населения. На основе анализа материалов центральных и местных архивов, статистических сведений смертности 1930-х гг. в регионе авторы составили погодную динамику численности смертей среди детей до 1 года и пришли к выводу, что причинами высокой смертности в начале 1930-х гг. стали не только политические процессы в обществе, а в первую очередь голод 1932–1933 гг. Авторами доказано, что для населения Южного Урала в 1930-е гг. уровень смертности детей до 1 года был высоким не только по отношению к численности населения региона, но и для страны в целом. По количеству младенческих смертей в рассматриваемом регионе лидировала Челябинская область, особенно среди городского населения. Это было связано с высоким уровнем численности населения, преобладанием городского населения над сельским, потребностями индустриализации, т.е. тяжелым физическим трудом женщин, недостаточно развитой системой здравоохранения.В результате исследования установлены и раскрыты основные причины высокой младенческой смертности в 1930-е гг. на Южном Урале. Несмотря на меры, предпринятые государством по поддержке материнства и детства, младенческая смертность на Южном Урале продолжает оставаться на высоком уровне, растет количество нелегальных абортов, инфекционных заболеваний в летние месяцы, не хватает медицинских специалистов.Область применения результатов. Результаты исследования могут быть применены при решении современных проблем младенческой смертности, органами учета и статистики. ; Purpose. The purpose of this paper is to study infant mortality in the southern Urals in the 1930 years and the establishment of its causes. The relevance of the researched problem is connected with difficulties of solving demographic problems in the country and the policy of the state to protect motherhood and childhood.Methodology. Basis of research is historical and comparative and historical and systematic methods and critical analysis.Results. Infant mortality is the most significant symptom of demographic decline among the population. This period has been one of the most difficult in the history of our nation, as it was accompanied by major economic, social and political processes that influenced the decline of natural increase among the population. Based on the analysis of Central and local archives, statistical information of mortality of the 1930 years in the region, the authors made the weather dynamics of the number of deaths among children under 1 year and came to the conclusion that the causes of high mortality in the early 1930 years were not only the political processes in society, and especially the famine of 1932–1933 years. The authors have proved that the population of the southern Urals in the 1930 years the mortality rate of children under 1 year was high, not only in relation to the population of the region but for the country as a whole. The number of child deaths in the region was the leader Chelyabinsk region, especially among the urban population. This was due to the high level of population, the predominance of urban population over the rural, the needs of industrialization, that is heavy physical labour of women, weak health care system. By the middle of the period under review, despite the measures taken by the state for the support of motherhood and childhood, the infant mortality rate in the southern Urals remains at a high level, increasing the number of illegal abortions, infectious diseases in the summer months, not enough health care professionals.The study established and revealed the main reasons for high infant mortality in the 1930 years in the southern Urals. This article is devoted to a detailed analysis of death causes in Mexico during the Revolution and Civil War period. Our study is based on the death registration records in six government-controlled municipalities in Veracruz State which promoted registration accuracy. A total of 2 876 records from 1918 were analyzed, most of which contained information about the causes of death. The main objectives of the study were to determine the main causes of death, to identify territorial, age and gender differences, to establish the time and extent of the spread of the Spanish flu epidemic and the overall mortality rate in 1918. The 10th revision of the International Classification of Diseases (ICD-10), developed by the World Health Organization, was used as a basis for coding the causes of death. Thus, the causes of diseases that led to the death of the inhabitants of Veracruz were divided into eight main classes: infectious; respiratory; related to pregnancy or childbirth; those caused by difficulties in the perinatal period of fetal development; external violent reasons; unclassifiable, those associated with infancy and childhood; the unclassifiable; and finally, the missing. The analysis revealed registration problems, especially in the agrarian regions, caused by the poor development of medical infrastructure and aggravated by wartime conditions. Based on the analysis of individual level data on the causes of death, the authors identified the 161 diagnostic options found in the sources, identified the main ten death causes for each of the six cities of Veracruz, determined the level and accuracy of registration and the age characteristics of the spread of certain classes of fatal diseases. The most common cause of death was infectious diseases, particularly children from one to five years old suffered from these. The rate of respiratory diseases was also high, the proportion of deaths from these rose sharply in the last quarter of 1918, which was caused by the spread of the Spanish flu. The analysis of the nominative data made it possible to refute the official statement of the Mexican government about the cessation of influenza at the end of 1918. The high mortality rate from various respiratory diseases in December 1918 among young men suggests that the flu pandemic was hidden behind various other concepts. ; Исследование посвящено анализу причин смертности в Мексике в ходе гражданской войны за независимость. В качестве источника использованы записи о регистрации смертей в шести муниципалитетах штата Веракрус, находившихся под контролем правительства, что обеспечило аккуратность ведения регистрации. Всего было проанализировано 2 876 записей, датированных 1918 г., большая часть которых содержала информацию о причинах смерти. Основными задачами исследования было определение основных причин смерти, выявление территориальных и половозрастных различий, установление времени и степени распространения эпидемии «испанского гриппа» и общего уровня смертности в 1918 г. Для кодирования причин смерти в качестве основы была использована международная классификация, разработанная Всемирной организацией здравоохранения, — IDC-10. В соответствии с поставленными задачами все варианты записей, в которых были указаны или не указаны причины заболеваний, приведших к смерти, для удобства анализа были распределены по восьми основным классам: инфекционные; респираторные; связанные с беременностью или родами; вызванные осложнениями в перинатальный период развития плода; внешние насильственные причины; не поддающиеся классификации, связанные с младенческим и детским возрастом; не поддающиеся классификации; без указания причин. Проведенный анализ выявил проблемы регистрации, особенно в аграрных областях, вызванные слабым развитием медицинской инфраструктуры, усугубленные условиями гражданской войны. На основе анализа индивидуальных данных о причинах смерти, авторы выявили 161 вариант диагнозов, встречающихся в источниках; определили основные десять причин по каждому из шести городов Веракруса, выявив территориальные особенности; определили уровень и аккуратность регистрации, а также возрастные особенности распространения отдельных классов смертельных заболеваний. Наиболее частой причиной смерти жителей Веракруса являлись инфекционные заболевания, в особенности от них страдали дети в возрасте от года до пяти лет. Высок был также уровень респираторных заболеваний, доля смертей от которых резко возросла в последней четверти 1918 г., что было вызвано распространением испанского гриппа. Проведенный анализ номинативных данных позволил опровергнуть официальное утверждение правительства Мексики об остановке гриппа в конце 1918 г. Высокий уровень смертности от различных респираторных заболеваний в декабре 1918 г. именно в среде молодых мужчин позволяет предположить, что за различными наименованиями скрывался именно грипп.
The reasons and dynamics of child mortality in Russian Federation during the XXI century are presented. The data of official statistics from Rosstat, World Bank and World Data Atlas have been analyzed. The following medical statistical regularities have been revealed: reduction of child mortality rate in the structure of total number of deaths in Russian population and infant mortality rate in the structure of child mortality; reduction of regional varieties of infant mortality rates; continued transfer of live births to stillborn births in some regions; external factors are also very significant not only in adolescents but also in infants. Priority areas for further reduction of child mortality in Russian Federation are provided: regional approach in the implementation of measures for reduction of child mortality; preventive measures aimed on reducing of genetic burden in population; full supply on the management of children with rare diseases by the government; establishment of the system for medical and social rehabilitation of children; implementation of the system of effective medical and social care for children with high social risk. ; Представлены причины и динамика смертности детского населения в Российской Федерации в XXI столетии. Проанализированы данные официальной статистики Росстата, Всемирного банка, Мирового атласа данных. Выявлены следующие медико-статистические закономерности: снижение доли умерших детей в общем числе смертей населения России и доли младенческой смертности в смертности детского населения; уменьшение региональных различий между показателями младенческой смертности; продолжение переброса родившихся живыми в мертворожденные в отдельных регионах; внешние причины по своему уровню весьма значимы не только в подростковом, но и младенческом возрасте. Обоснованы приоритетные направления по дальнейшему снижению смертности детского населения Российской Федерации: региональный подход в реализации мер по снижению смертности детского населения; проведение профилактических мероприятий, направленных на снижение генетического груза популяции; полное обеспечение государством лечения детей с редкими заболеваниями; создание системы медико-социальной реабилитации детей; внедрение системы эффективного медико-социального патронажа контингента детей высокого социального риска.
The article outlines the need for the allocation of deaths due to road traffic accidents (RTA) as an independent object of study. Disclosure is necessary to estimate the mortality due to accidents and based global problem under consideration. An assessment of current trends in mortality in Russia because of the accident. Conclusions about the effectiveness of government policy in the field of road safety are formulated. ; В статье охарактеризована необходимость выделение смертности из-за дорожно-транспортных происшествий (ДТП) в качестве самостоятельного объекта исследования. Раскрыта необходимость оценки смертности из-за ДТП и обоснована глобальность рассматриваемой проблемы. Проведена оценка существующих тенденций в смертности населения России из-за ДТП. Сформулированы выводы об эффективности проводимой государственной политики в области безопасности дорожного движения.
Objective: Dynamics and causes of infant and child mortality in the Russian Federation in post-Soviet period (1990–2012) under conditions of demographic policy implementation and healthcare system reforming are presented. Methods: Federal State Statistics Service's official statistics data (1990–2012) had been analyzed. The data was compared with similar rates of European Union countries. Results: It's been revealed that infant mortality decreased (more than in 2 times) (from 17.4‰ to 8.6‰) through all its level indicators (early neonatal, neonatal, post-neonatal mortality) and due to almost all basic reasons (perinatal disorders, infectious diseases, respiratory diseases, congenital anomalies, accidents). Infant mortality rate in rural areas is higher (10.1‰ and 8.1‰), however, the gap between rates in rural regions and in cities is decreasing. In Russian Federation the infant mortality has regional distinctions, thus Siberian and Far Eastern regions are the least advantaged areas. Mortality of children under 5 y.o. is declining (from 21.3‰ to 10.6‰), but its rate is still higher than in European Union countries (3.5‰). External factors occupy the top position as a death cause of infants and toddlers; the number of clinical cases with a diagnosis of malignant neoplasm increases. Conclusion: Thus, improving of the system of motherhood and childhood protection in the Russian Federation has resulted in significant reduction of infant and child mortality rate. ; Цель исследования: оценить тенденции младенческой (МС) и детской смертности в Российской Федерации (РФ) в постсоветском периоде (1990–2012 гг.) в условиях проведения демографической политики и реформирования системы здравоохранения. Методы: выполняли медико-статистический анализ показателей МС и детской смертности по официальным данным Росстата за 1990–2012 гг. Результаты: установлено значительное (более чем в 2 раза) снижение МС (с 17,4 до 8,6%о) за счет всех ее составляющих (ранней неонатальной, неонатальной, постнеонатальной смертности) и почти от всех основных причин (состояния перинатального периода, инфекционных заболеваний, болезней органов дыхания, врожденных аномалий, несчастных случаев). Отмечается более высокий уровень МС в сельской местности (10,1 против 8,1%о), однако разрыв между городскими и сельскими показателями сокращается. В РФ МС имеет региональные особенности и взаимосвязь с демографическими показателями (общая смертность), социально-экономическими детерминантами (уровень прожиточного минимума, доля бедного населения и численность безработных) и технологическими показателями (уровень ВВП на душу населения). Сокращается смертность детей в возрасте до 5 лет (с 21,3 до 10,6%о), однако ее уровень остается выше, чем в странах Европейского союза (3,5%о). Среди причин гибели детей этой возрастной группы первые места начинают занимать внешние факторы, увеличивается значимость злокачественных новообразований. Заключение: совершенствование системы охраны материнства и детства в РФ позволило значительно снизить младенческую и детскую смертность.

Создание баз индивидуальных данных позволило изучать смертность на уровне индивидов, отдельных семей и поколений. При этом роль ближайшего окружения, не состоявшего в кровном родстве, учитывалась редко. Между тем, стремительная урбанизация разрушила семейные связи и каналы трансляции традиционных моделей демографического поведения. Одновременно возрастала роль внесемейных факторов, главным из которых был церковный приход, с конца XIX в. постепенно трансформировавшийся в соседское сообщество — прообраз городского микрорайона. Предлагаемая работа нацелена на исследование смертности взрослого населения Екатеринбурга конца XIX — начала XX в. и ее дифференциации между приходами Русской православной церкви. Источниковую базу составили официальная статистика и база данных «Регистр населения Урала», созданная на основе метрических книг за 1880–1919 гг. пяти православных приходов Екатеринбурга. В ходе исследования была выполнена реконструкция динамики смертности по каждому приходу и городу в целом; восстановлена структура смертности; вычислен средний возраст смертности; проведен анализ причин, ее вызвавших; проанализирована сезонность. В итоге можно заключить, что каждый из пяти православных приходов имел определенную демографическую специфику. На смертность влиял экономический профиль района и направление городской застройки, большое значение имело расположение крупных социальных объектов на территории прихода. Проведенное исследование позволило выявить определенную стабильность и респектабельность Богоявленского и отчасти Вознесенского прихода; «мигрантскую» специфику Александро-Невского и Свято-Духовского; и некий средний, близкий общегородскому, характер района, соответствовавшего приходу Екатерининского собора. ; Traditionally, studies of historical mortality have focused on the national, regional, or local levels. Currently, the creation of individual level databases has made it possible to study mortality at the individual and family levels, also following people over generations. However, this research rarely considered non-family relations; at the same time, rapid urbanisation during the late nineteenth century severed many family ties and hindered the transmission of traditional models for demographic behaviour. Thus, the role of non-family factors increased, the main of which was the church parish, which since the end of the nineteenth century gradually transformed into a neighborhood community — the prototype of the urban microdistrict. This research aims to study the mortality of the adult population of Ekaterinburg during the decades around 1900, differentiating between the parishes of the Russian Orthodox Church. The sources consist of official statistics and the Ural Population Project database, which was created based on the metric (church) books. The authors reconstruct the full development of mortality for each parish and for Ekaterinburg as a whole; map the structure of mortality, calculate the average age at death, as well as analyse the causes of death and its seasonality. As a result, it may be concluded that each of the five Orthodox parishes indeed had a certain demographic specificity. The purpose of this study was to evaluate the mortality rate of St. Petersburg population from external causes in 2010–2015 in comparison with similar indicators for the Northwest Federal District and the Russian Federation. Materials and methods. The authors used the following data for analysis: annual long-term monitoring of St. Petersburg trauma services status conducted by coordination and methodological center of trauma and orthopedics; government reporting on mortality; data of Federal State Statistics Service (Rosstat); data of St.Petersburg Statistics Service (Petrostat); data of St. Petersburg Medical Information Analysis Center (MIAС); studies on mortality issues as well as methods of descriptive statistics. Results. From 2003 to 2014 St. Petersburg population mortality rate demonstrated a steady decline as well as generally in Russia. 2015 featured a slight increase in absolute number of deaths per year (+1.2%) in St.Petersburg, while the similar indicator across the Russian Federation was still declining (-5.9%). In 2015 mortality from external causes in St. Petersburg constituted 68 deaths per 100 thousand inhabitants, which is 1.6 times less than in the Northwest federal district (109.6 per 100 thousand inhabitants), and 1.7 times less than in the Russian Federation – 112 per 100 thousand population. In the overall mortality structure of St. Petersburg population the external causes have been consistently occupying third place within recent years. In 2015 the proportion of deaths from external causes in general mortality amounted to 5.7%. Conclusions. According to official statistics since 2000 in St. Petersburg the mortality rate from all the major external causes reduced gradually, as well as in the Northwest federal district and on the whole in Russia. Various national and regional programs that have been rolled out in Russia allowed to reduce by half the mortality of working age population from homicide and from suicide by almost 30% in men and 40% in women. The most reliable evaluation can be made by taking into account a consistent reporting data of overall traffic accidents mortality. In calculation of mortality rate from external causes there is a possibility to underreport injury and poisoning mortality rates described by the following ICD-10 codes: X40-X49, R95-R99, Z57, Z58, Z63, R54. ; Цель исследования: изучить показатели смертности жителей Санкт-Петербурга (СПб) от внешних причин за 2010–2015 г. в сравнении с аналогичными показателями по Северо-Западному федеральному округу (СЗФО) и Российской Федерации (РФ). Результаты. С 2003 по 2014 г. смертность населения в СПб, как и в целом в РФ, стабильно снижалась. В 2015 г. произошло незначительное увеличение абсолютного числа умерших за год на 1,2%, при этом аналогичный показатель по РФ по-прежнему снижался (5,9%). Показатель смертности населения от внешних причин в 2015 г. в СПб составил 68 умерших на 100 тыс. чел., что в 1,6 раза ниже показателя по СЗФО (109,6 на 100 тыс. чел.) и в 1,7 раза ниже показателя в целом по РФ – 112 на 100 тыс. чел. В общей структуре смертности населения СПб внешние причины в течение последних лет стабильно занимают третье место. Удельный вес смертей от внешних причин в общей смертности населения в 2015 г. составил 5,7%. Выводы. Начиная с 2000 г. показатель смертности от всех основных внешних причин устойчиво снижался как в СПб и СЗФО, так и в РФ в целом. Различные программы позволили снизить уровни смертности трудоспособного населения от убийств вдвое, от самоубийств – почти на 30% у мужчин и на 40% у женщин. Наиболее объективная оценка ситуации возможна при едином учете общего уровня смертности от всех транспортных происшествий. При расчете показателей смертности от внешних причин существует возможность недоучета смертности от травм и отравлений за счет таких блоков, как: Х40-Х49, R95-R99, Z57, Z58, Z63, R54.

В 2015 г. произошло незначительное увеличение абсолютного числа умерших за год на 1,2%, при этом аналогичный показатель по РФ по-прежнему снижался (5,9%). Показатель смертности населения от внешних причин в 2015 г. в СПб составил 68 умерших на 100 тыс. чел., что в 1,6 раза ниже показателя по СЗФО (109,6 на 100 тыс. чел.) и в 1,7 раза ниже показателя в целом по РФ – 112 на 100 тыс. чел. В общей структуре смертности населения СПб внешние причины в течение последних лет стабильно занимают третье место. Удельный вес смертей от внешних причин в общей смертности населения в 2015 г. составил 5,7%. Выводы. Начиная с 2000 г. показатель смертности от всех основных внешних причин устойчиво снижался как в СПб и СЗФО, так и в РФ в целом. Различные программы позволили снизить уровни смертности трудоспособного населения от убийств вдвое, от самоубийств – почти на 30% у мужчин и на 40% у женщин. Наиболее объективная оценка ситуации возможна при едином учете общего уровня смертности от всех транспортных происшествий. При расчете показателей смертности от внешних причин существует возможность недоучета смертности от травм и отравлений за счет таких блоков, как: Х40-Х49, R95-R99, Z57, Z58, Z63, R54.
Основные предпосылки научной работы. При переходе от централизованного государственного планирования к рыночной экономике Россия испытала беспрецедентный скачок смертности, главным образом среди работоспособного мужского населения. Беларуси в этот период удалось избежать подъема уровня смертности. Что могло послужить причиной подобного различия? Имеющиеся научные работы указывают на связь между приватизационны-ми реформами ранних 1990-х гг. и смертностью, главным образом из-за безработицы. Однако имеющаяся научная литература основывается на традиционных данных по смертности из регистров народонаселения, что не позволяет делать выводы о влиянии приватизации на смертность изолированно от других факторов. Методы. Основой анализа выступает уникальная база данных, собранная научно-исследовательским проектом PrivMort. Она содержит информацию о населенных пунктах, предприятиях и индивидуальных респондентах, а также об их живых и умерших родственниках. Исследование основывается на непараметрическом методе отбора подобного по коэффициенту склонности вероятности для сравнения смертности и ее детерминант среди моно- и мультииндустриальных городов с разной скоростью и типом приватизации в России, где массовая приватизация была предпринята в начале 1990-х гг., и в Беларуси, где экономика была либерализована без массовой приватизации при сохранении сильного государственного контроля. Использована регрессия Пуассона с возрастной стандартизацией и смещением переменной (времени) по количеству лет, которые субъект прожил в 1992–1998 гг. Результаты. Показано, что скорость приватизации оказала наиболее сильный эффект на смертность среди мужчин в возрасте 20–65 лет в моноиндустриальных городах России, которые, в отличие от Беларуси, прошли массовую приватизацию. Выявлен сильный предохраняющий от смертности эффект проживания в Беларуси вблизи с польской границей, а также эффект наличия высшего образования. Кроме того, продемонстрировано, что безработица в 1990-е гг., а также отсутствие семьи повысили вероятность мужской смертности. Интерпретация результатов. Политика массовой приватизации имела крайне негативное влияние на мужскую смертность, особенно среди мужчин работоспособного возраста. Россия, которая применила политику массовой приватизации, включая быструю продажу государственных акций и быструю либерализацию цен, значительно хуже справилась с социально-экономическими последствиями транзиций, чем Беларусь, где применялась политика постепенной либерализации. Присутствие в экономике сильного государственного контроля может существенно амортизировать эффект тотальных экономических изменений на структурном уровне, так как оно значительно снижает уровень смертности за счет меньшего урезания социального бюджета, сокращения индустриального производства, более низкого уровня безработицы и бедности, экономического неравенства. = Background. While right after the onset of economic and political reforms transforming central planning to free market economy Russia experienced an unprecedented rise in mortality primarily among the working-age male population, Belarus did not experience such a shock. What can the difference between the mortality rates in two post-Soviet Slavic states be attributed to? Previous studies have indicated a link between privatisation reforms and mortality mostly via unemployment. However, existing studies rely on conventional registry-based mortality data or cross-national data, which makes them unable to single out the effect of privatization on mortality. Methods. The study uses a novel dataset acquired by the PrivMort project, containing data on settlement- and enterprise levels as well as individual-level data on the respondents and their alive and dead relatives. The study using propensity score matching compares mortality and its determinants across mono- and multi-industrial settlements with different pace and style of privatization in Russia, where rapid mass-privatization policies were introduced in the early 1990s, and Belarus, which liberalised its economy maintaining a strong state. We used Poisson regressions with offset variable (time of exposure) as a number of years a person lived in the exposure period (1992–1998). Findings. Using an age-adjusted Poisson regression, we demonstrate that among men aged 20–65, the speed of privatization has the strongest effect on mortality in mono-industrial towns that have undergone rapid mass privatization. Since Belarus has not undergone mass privatisation, the effect is only observable in Russia. We also identify a strong protective effect of living close to the border with Poland among Belarusian men and a strong protective effect of academic higher education. Furthermore, we demonstrate that unemployment in the 1990s and marital status have a strong effect on mortality. Single men when compared to men in stable partnerships are significantly more likely to die. Interpretation. The policies of rapid mass privatization had a negative effect on mortality among working-age men. Russia, which implemented mass-privatisation policies including quick sale of state assets, rapid price liberalisation and stabilisation, fared significantly worse in terms of mortality levels than Belarus, which employed a slow liberalization strategy. The presence of strong state can significantly cushion the effect of large-scale economic crises as it a) directly affects mortality rates by less reduction in public spending; and b) indirectly affect mortality due to a smaller fall in output, smaller unemployment and poverty rates as well as by less income inequality.
The article is devoted to evaluation of situation with providing fist aid in the Russian Federation. It discusses the necessity to establish first aid system in the Russian Federation and formulates its principles. The need in establishing such system is caused by necessity to draw a wide range of persons, including those who are not medically educated, to provide first aid service to patients. Also substantiated the need of development and adoption of the legislation that adjusts different aspects of first aid, as well as alteration in a current legislation. Proved the necessity of establishment and functioning of the intersectional coordination council. Consideration is given to principles of functioning of the training system for first aid providers. Principles, which will help to provide them with first aid tools, are substantiated. ; Статья посвящена оценке существующей ситуации с оказанием первой помощи в Российской Федерации. Обоснована необходимость и сформулированы основные принципы создания в Российской Федерации системы первой помощи. Потребность в такой системе обусловлена необходимостью привлечения широкого круга лиц, в том числе не имеющих медицинского образования, для оказания первой помощи пострадавшим. Обоснована необходимость в разработке ряда нормативных актов по регулированию различных аспектов первой помощи, а также внесения изменений в действующие нормативные документы. Рассматриваются вопросы создания межведомственного координационного совета по оказанию первой помощи. Предложены принципы функционирования системы по подготовке участников, а также их оснащения необходимыми средствами для оказания первой помощи.
