L'elaborato affronta il tema del diritto alla salute del detenuto, sia dal punto di vista della normativa internazionale sia dal punto di vista della normativa nazionale; con un focus in particolare sul sovraffollamento carcerario e sul periodo emergenziale dovuto al Covid- 19. Analizzati gli interventi legislativi in merito al diritto alla salute del detenuto, si sono esaminate le sentenze più importanti della Corte EDU sul fenomeno del sovraffollamento carcerario ed infine, si è svolta un'analisi sull'impatto che ha avuto il Covid-19 sulla popolazione detenuta in merito al sovraffollamento e al diritto alla salute. The elaboration examins the right to the health of the prisoners, both from the point of view of intrenational law and from the point of view of national legislation; with a particular focus on prison overcrowding and on the emergency period due to Covid-19.The text analysed the legislative measures on the right to health of the prisoners,the most important judgments of the ECHR on the phenomenon of prison overcrowding and finally, an analysis has been carried out on the impact of Covid-19 on the detained population about overcrowding and the right to health.
The effects of the spread of Covid-19 have had important consequences on everybody's life but also had a dramatic impact for the population inside correctional centers in Italy; these issuses have finally received great attention over the last few months. This paper is primarily focused on the analysis of the critical aspects of overcrowding in Italian correctional centers, a cyclical issue which has become even more critical during the current pandemic emergency. Our research aims at describing the development of the number of prisoners before and after government legislative actions taken in early 2020 to reduce overcrowding of prisoners; furthermore we study these effects in combination with the main features of some Italian correctional centers. There is some evidence that the number of prisoners first decreased but then started increasing again, as it had always happened after similar governative decrees adopted in other cases in the past; moreover, it is also clear that the available capacity of jails is not homogeneous in Italy and that the situation is different between the different regions. The main difficulty encountered in preparing the research analysis for this contribution has been the access to data related to the dissemination of the pandemic inside correctional centers; in fact, in some frameworks such as prisons, this kind of information is not often available or completely updated. ; Gli effetti della diffusione del Covid-19 hanno avuto importanti conseguenze sulla vita di tutti noi e in particolare un impatto drammatico per le persone detenute nelle case circondariali in Italia; questi temi hanno ricevuto grande attenzione negli ultimi mesi. Il presente lavoro è principalmente rivolto all'analisi degli aspetti di rischio del sovraffollamento carcerario italiano, un problema che si ripresenta periodica-mente e che è diventato ancor di maggior criticità nell'attuale situazione di pandemia. Il contributo intende analizza la situazione carceraria prima e dopo le misure legislative adottate all'inizio del 2020 per attenuare il problema del sovraffollamento; inoltre vengono studiati gli effetti di tali provvedimenti alla luce di alcuni dati strutturali degli istituti penitenziari in Italia. Dai risultati emersi vi è evidenza di una diminuzione del numero di detenuti a cui tuttavia sta seguendo una fase di tendenziale risalita, in maniera analoga a quanto si è verificato a seguito di alcuni provvedimenti adottati in passato per fronteggiare il sovraffollamento carcerario. Emerge inoltre una situazione carceraria non omogenea con differenze anche notevoli nei diversi provveditorati. La principale difficoltà riscontrata durante la stesura di questo contributo è relativa all'accesso ai dati sulla diffusione della pandemia che, in determinati contesti (come ad esempio nelle carceri), risulta notevolmente complesso e le informazioni sono spesso non aggiornate.
Questo contributo si pone come obiettivo l'analisi dell'istituto della sospensione del procedimento con messa alla prova come metodo di deflazione penale e strumento per favorire l'estinzione del reato prima ancora che venga comminata una condanna; l'istituto è quindi in grado di incidere anche sul sovraffollamento carcerario. L'attenzione viene focalizzata sul carattere volontario e sui contenuti della messa alla prova che aprono la strada alla giustizia riparativa: la mediazione con la persona offesa, le condotte riparatorie, la restituzione ed il risarcimento del danno richiedono un comportamento attivo dell'imputato frutto di una sua libera scelta, dettata dall'aver compreso l'antigiuridicità della propria condotta. Sono questi gli aspetti che conferiscono legittimità costituzionale all'istituto e sui quali il legislatore deve intervenire per valorizzare al massimo la portata innovativa della messa alla prova. This contribution wants to analyze institute of probation as a method of criminal deflation and instrument to favor the extinction of the crime even before a conviction is communicated; the institute is therefore able to also affect prison overcrowding. Attention is focused on the voluntary nature and on the contents of the proof that open the way to restorative justice: mediation with the offended person, restorative conduct, restitution and compensation for damage require action of the accused, result of his free choice, dictated from having understood the anti-juridical nature of his conduct. These are the aspects that confer constitutional legitimacy on the institute and on which the legislator must intervene to maximize the innovative scope of probation.
The survey approachs the issue of health and the problem of its effective protection in a context of deprivation of liberty and coercion, which is the prison. The theoretical reflection born from the reform of the Legislative Decree 230/99 which marked the transition from an employee by the Prison Health within prison a fully integrated in the National Health Service. The comparison between an institution of health promotion and institution of punishment which may operate on the same subject held produces multiple attrits, making their relationship problematic. The work shows the daily difficulties in the management of prison health within the institution, physician-patient between different health care roles, and between the latter and prison workers. The coexistence, in fact, is not always harmonious though quite often it is common sense and the willingness of operators to reduce barriers: overcrowding, limited resources and insufficient staff make the application of the rule and therefore the right to goal a difficult to be pursued. It is designed for a scheme of semi-structured interview essay is divided into 3 sections covering: "staff and its functions", "health reform" and "health of the prisoner"; questions were directed to doctors, nurses and psychologists engaged inside the prison of Rimini with the specific aim of examining the ambivalent relationship between the demand for health care in prisons and the need for security and a clear - albeit partial - point of view. We tried to reconstruct the situation of prison health care through the perception of prison operators, capturing the problematic issues that deal on both issues is instrumental to the experience of persons detained by analyzing, in terms of operators , what happens inside of a prison institution in everyday health care.
The war with Venice from 1404-1405 represented a crucial moment in the history of Padua. In fact, it marked the definite end of the autonomy of the people of Padua and the passage of the city and its territory under the control of Venice. The final siege, narrated in the Chronicles of Bartolomeo Gatari, lasted almost five months and subjected Padua to a very severe test: overcrowding of man and animals within the city walls, lack of provisions, precarious health-hygiene conditions and epidemics of the plague. In this situation the relationship between the last nobleman of the city, Francesco Novello da Carrara, and the civies of Padua continued progressively to deteriorate: the emergency, in the end, induces a divaricator in the political views of one and the others, even if, at the time of surrender, the general public futilely attempted to plead in favour of the defeated nobles. Padua came out of the catastrophe with a new awareness of itself: while its civil traditions and the social regulations appeared to be well instilled, its capacity of political and military expression, of which the noblemen of Carrara were made interpreters for centuries, revealed insurmountable limits before all of the resources put on the battlefield from Venice. ; La guerra con Venezia del 1404-1405 rappresentò un momento cruciale nella storia di Padova. Essa segnò infatti la fine definitiva dell'autonomia patavina e il passaggio della città e del suo territorio sotto il diretto controllo di Venezia. L'assedio finale, narratoci nella Cronaca di Bartolomeo Gatari, durò quasi cinque mesi e sottopose Padova ad una prova molto severa: sovraffollamento di uomini e animali entro le muta urbane, carenza di viveri, precarie condizioni igienico-sanitarie, epidemia di peste. In questa situazione il rapporto tra l'ultimo signore della città, Francesco Novello da Carrara, e i cives padovani andò progressivamente logorandosi: l'emergenza indusse infine una divaricazione nella visuale politica dell'uno e degli altri, anche se, al momento della resa, la collettività cittadina tentò, inutilmente, di intercedere a favore dei signori sconfitti. Dalla catastrofe Padova uscì con una nuova consapevolezza di sé: mentre le sue tradizioni civiche e l'ordinamento sociale apparivano ben sedimentati, la sua capacità di espressione politica e militare, di cui si erano fatti interpreti per quasi un secolo i signori da Carrara, aveva rivelato limiti insuperabili di fronte alle risorse messe in campo da Venezia.
L'oggetto di ricerca del presente elaborato è la condizione abitativa della popolazione straniera a Bologna. Attraverso una metodologia mixed method, abbiamo cercato di indagare il rapporto esistente tra integrazione dei migranti e questione abitativa, focalizzandoci sul peso giocato da eventuali forme di disuguaglianza e discriminazione. L'obiettivo della ricerca è duplice. Da un lato, ricostruendo il panorama delle politiche pubbliche adottate nell'ultimo decennio abbiamo voluto comprendere se e come si definiscano situazioni di dise-guaglianza per i nuclei familiari i cui bisogni non possono essere soddisfatti alle condizioni di mercato. Detto altrimenti, abbiamo voluto verificare se e come le housing policies adottate definiscano una di-seguaglianza strutturale di accesso alla casa, intercettando solo alcuni destinatari ed escludendone altri. In secondo luogo, abbiamo voluto indagare più nel dettaglio quanto i requisiti di accesso previsti dalle singole misure di intervento abbiano implicazioni sull'acceso al bene casa, in particolare per la popola-zione straniera. A partire dal quadro teorico degli housing studies e da una ricognizione della storia delle politiche abitative in Italia, l'analisi empirica si fonda da un lato sulla comparazione degli ultimi dati censuari di-sponibili (2001-2011) per le popolazioni autoctone e per gli stranieri, e dall'altro sui dati qualitativi, ossia interviste semi strutturate a migranti e agenti immobiliari, e note prodotte durante la fase di os-servazione diretta presso la principale associazione per il diritto alla casa: il Sunia. Alla luce della ricerca empirica condotta nella città di Bologna, la popolazione migrante, come di-mostra l'analisi quanti-qualitativa condotta, è quella che maggiormente risente di una condizione abita-tiva polarizzata considerando tutti i maggiori indicatori utilizzati (titolo di godimento, affordability, presenza di servizi, sovraffollamento), e che rispetto alla popolazione autoctona è maggiormente colpi-ta dalla povertà abitativa. ; The topic of this research is the housing condition of migrant in Bologna. Using a mixed method methodology, we have tried to investigate the relationship between migrants' integration and housing issues, focusing on the role played by possible forms of inequality and discrimination. The research objective is twofold. On the one hand, by reconstructing the panorama of public policies adopted in the last decade, we aimed at understanding if and how they entail situations of inequality for citizens and households whose needs cannot be satisfied under market conditions. In this regard, we aimed to verify if and how policies define structural inequalities in housing access, intercepting only some beneficiaries and excluding others. On the other hand, we aimed at investigating in detail to what extent the requirements provided by these intervention measures have implications for access housing, particularly for the non-national population. Starting from the theoretical framework of housing studies and from a reconstruction of the history of housing policies in Italy, the empirical analysis is based, on the one hand, on the comparison of the last available census data (2001-2011) for the native and for immigrants, and, on the other hand, on the qualitative data, i.e. semi-structured interviews with migrants and real estate agents, and fieldnotes from direct observation in the principal association for housing rights in Bologna: Sunia. Considering the empirical research conducted in Bologna, the migrant is the most affected by a polarized housing condition considering the most important indicators (such as tenure status, affordability, services availability, overcrowding), and compared to the native is more affected by housing poverty. The results, therefore, suggest that the relationship between inequality and the housing should be interpreted confirming that it is not the result of subjective choices and individual characteristics, but considering that these choices are socially structured.
The Philippines is one of the most rapidly urbanizing countries in Asia. This trend reflects the effects of years of high rates of natural population growth and consistent rural-to-urban migration. Urbanization in the Philippines has been led by Metro Manila, also known as the National Capital Region (NCR), since the 1950s. Metro Manila and the other five metropolitan areas of the country produce 80% of the Philippines' Gross Domestic Product (GDP), and no development strategies for the rural areas are to be implemented in the foreseeable future. The spread of informal settlements in the Philippines has become a phenomenon associated with big cities and unplanned expanding urban centres. From the early 1970s to more recent years, estimates of the number of informal settlers in the country have varied, ranging from 470,000 to 2.5 million families. Current estimates2, place the number of informal settlement families (ISFs) at about 1.5 million, 15% of the Philippines' total urban population. Many ISFs live in chronic urban poverty and are confronted by physical, economic, social, legal and environmental risks on a day-to-day basis. They have limited or no access to security of tenure, capital, social networks, environmental safety and legal security. Clearly, as in other developing countries, the pervasiveness of informal settlements in the Philippines can be traced back to low income, inadequate urban planning, lack of serviced land, lack of affordable social housing, and many other factors.3 In Metro Manila and other growing urban centres, informal settlers live in sprawling slums that do not meet the most basic hygienic needs (or, worse, are used as dumping grounds for hazardous wastes) where they are constantly exposed to serious health risks. This issue has been too often addressed in ineffective ways: demolitions, relocations to extra urban areas, overcrowded tenement buildings. The aim of this work is to demonstrate that the informality that shapes this settlements and the social housing provided by the government so far can't be repressed, and should be incorporated in what is commonly considered "proper" architecture. These two aspects can successfully work together to generate an environment that responds to the needs of the people. Formality can lay the basis for informality to happen and evolve in a solid, safe and hygienic environment. This is the principle that defines the whole strategy here proposed for the chosen case study, the Smokey Mountain and Paradise Heights area in Tondo, Manila. The project is focused on the creation of public spaces at the street level using abandoned areas and existent and new buildings, in order to continuously connect parts of the city that share the same margins, but are not linked to one another. The proposal presents a series of covered public spaces realized using parts of some existing buildings and the alleviation of the overcrowding issue that affects this neighbourhood; a new and incremental social hosing building prototype, that would provide families highly flexible spaces; a diffused system of public toilets and wash-houses in the slum area, to provide ISFs with the sanitary and hygienic facilities they don't have access to; a family planning centre, with day-clinics for medical visits and classrooms for educational meetings and initiatives; and a junk shop, that could be the first step for a cooperative recycling business in the area. Another aspect that is pointed out is that in contexts characterized by scarcity of resources in terms of funds and materials, vernacular traditional buildings are the greatest resources for architects and engineers, as people managed to protect themselves from the weather elements with simple and effective precautions and no need of high-tech solutions. Of course, these precautions and architectural forms are to be actualized and adapted to the needs of a family living in a city of the 21st century, and this is were architects, engineers and planners should contribute with their expertise. Numerous NGOs have been and are working in the Smokey Mountain area, mainly carrying on important and indispensable feeding and schooling programs; I hope that this project, although probably utopian, will be able to raise awareness towards the living conditions of so many people also from an urban and architectural point of view.
SENTIERI Project (Mortality study of residents in Italian polluted sites) studies mortality of residents in the sites of national interest for environmental remediation (Italian polluted sites, IPS). IPSs are located in the vicinity of industrial areas, either active or dismissed, near incinerators or dumping sites of industrial or hazardous waste. SENTIERI includes 44 out of 57 sites comprised in the "National environmental remediation programme". For each IPS contamination data were collected, both from the national and local environmental remediation programmes. Contamination data are mainly for private industrial areas; municipal and/or green and agricultural areas were poorly studied, therefore it is difficult to assess the environmental exposure of populations living inside and/or near IPSs. Each one of 44 SENTIERI IPSs includes one or more municipalities. Mortality in the period 1995-2002 was studied for 63 single or grouped causes at municipality level computing: crude rate, standardized rate, standardized mortality ratios (SMR), and SMR adjusted for an ad hoc deprivation index. Regional populations were used as reference for SMR calculation. The deprivation index was constructed using 2001 national census variables on the following socioeconomic domains: education, unemployment, dwelling ownership and overcrowding. A characterizing element of SENTIERI Project is the a priori evaluation of the epidemiological evidence of the causal association between cause of death and exposure. Exposures for which epidemiological evidence was assessed are divided into IPSs environmental exposures and other exposures. The former are defined on the basis of the decrees defining sites' boundaries; they are coded as chemicals, petrochemicals and refineries, steel plants, power plants, mines and/or quarries, harbour areas, asbestos or other mineral fibres, landfills and incinerators. The other exposures, considered for their ascertained adverse health effects are: air pollution, active and passive smoking, alcohol intake, occupational exposure and socioeconomic status. The epidemiologists in SENTIERI Working Group (WG) developed a procedure to examine the epidemiological literature published from 1998 to 2009; the WG identified a hierarchy in the literature examined to classify each combination of cause of death and exposure in terms of strength of causal inference. The selected epidemiological information included primary sources (handbooks and Monographs and Reports of international and national scientific institutions), statistical re-analyses, literature reviews, multi-centric studies and single investigations. This hierarchy relies on the epidemiological community consensus, on assessments based on the application of standardized criteria, weighting the studies design and the occurrence of biased results. Therefore, to put forward the assessment, the criteria firstly favoured primary sources and quantitative meta-analyses and, secondly, consistency among sources. The epidemiological evidence of the causal association was classified into one of these three categories: Sufficient (S), Limited (L), and Inadequate (I). The procedures and results of the evidence evaluation have been presented in a 2010 Supplement of Epidemiologia & Prevenzione devoted to SENTIERI. SENTIERI studied IPS-specific mortality and the overall mortality profile in all the IPSs combined. Some IPS-specific results are noteworthy and are herementioned. The presence of asbestos (or asbestiform fibres in Biancavilla) was the motivation for including six IPSs (Balangero, Emarese, Casale Monferrato, Broni, Bari-Fibronit, Biancavilla) in the "National environmental remediation programme". In these sites (with the only exception of Emarese) increases in malignant pleural neoplasm mortality were observed, in four of them the excess was in both genders. In six other sites (Pitelli, Massa Carrara, Aree del litorale vesuviano, Tito, Area industriale della Val Basento, Priolo), in which additional sources of environmental pollution were reported, mortality from malignant pleural neoplasm was increased in both genders in Pitelli, Massa Carrara, Priolo and Litorale vesuviano. In the twelve sites where asbestos was mentioned in the decree, a total of 416 extra cases of malignant pleural neoplasms were computed. Asbestos and pleural neoplasm represent an unique case. Unlike mesothelioma, most causes of death analyzed in SENTIERI have multifactorial etiology, furthermore in most IPSs multiple sources of different pollutants are present, sometimes concurrently with air pollution from urban areas: in these cases, drawing conclusions on the association between environmental exposures and specific health outcomes might be complicated. Notwithstanding these difficulties, in a number of cases an etiological role could be attributed to some environmental exposures. The attribution could be possible on the basis of increases observed in both genders and in different age classes, and the exclusion of a major role of occupational exposures was thus allowed. For example, a role of emissions fromrefineries and petrochemical plants was hypothesized for the observed increases in mortality from lung cancer and respiratory diseases in Gela and Porto Torres; a role of emissions frommetal industries was suggested to explain increased mortality from respiratory diseases in Taranto and in Sulcis-Iglesiente-Guspinese. An etiological role of air pollution in the raise in congenital anomalies and perinatal disorders was suggested in Falconara Marittima, Massa-Carrara,Milazzo and Porto Torres. A causal role of heavy metals, PAH's and halogenated compounds was suspected for mortality from renal failure in Massa Carrara, Piombino, Orbetello, Basso Bacino del fiume Chienti and Sulcis-Iglesiente- Guspinese. In Trento-Nord, Grado and Marano, and Basso bacino del fiume Chienti increases in neurological diseases, for which an etiological role of lead, mercury and organohalogenated solvents is possible, were reported. The increase for non- Hodgkin lymphomas in Brescia was associated with the widespread PCB pollution. SENTIERI Project assessed also the overall mortality profile in all the IPSs combined. The mortality for causes of death with a priori Sufficient or Limited evidence of causal association with the environmental exposure showed 3 508 excess deaths for all causes, corresponding to 439 per year; the number of excess deaths was 1 321 for respiratory diseases, 898 for lung cancer and 588 for pleural neoplasms. When considering excess mortality with no restriction to causes of death with a priori Sufficient or Limited evidence of causal association with the environmental exposure, the number of excess deaths for all causes was 9 969 (SMR 102.5, about 1 200 excess deaths per year; the excess was 4 309 for all neoplasms (SMR 103.8, about 538 excess deaths per year), 1 887 for circulatory systemdiseases, and 600 for respiratory systemdiseases. Most of these excesses were observed in IPSs located in Southern and Central Italy. The distribution of the causes of deaths showed that the excesses are not evenly distributed: cancer mortality accounts for 30% of all deaths, but it is 43.2% of the excess deaths (4 309 cases out of 9 969). Conversely, the percentage of excesses in noncancer causes is 19%, while their share of total mortality is 42%. SENTIERI is affected by some limitations, such as the ecological study design and a time window of observation possibly inappropriate to account for induction-latency time; the analyzed outcome (mortality instead than incidence) might be unsuitable as well. Despite its limitations, SENTIERI documented increased mortality for single IPSs and an overall burden of disease in residents in Italian polluted sites. These excesses could be attributed to multiple risk factors, that include also the environmental exposures. The study results will be shared with the Ministries of Health and Environment, Regional governments, Regional environmental protection agencies, Local health authorities and municipalities. A collaborative approach between institutions in charge of environmental protection and health promotion will foster, among else, a scientifically sound and transparent communication process with concerned populations. ; Il Progetto SENTIERI (Studio Epidemiologico Nazionale dei Territori e degli Insediamenti Esposti a Rischio da Inquinamento) riguarda l'analisi della mortalit? delle popolazioni residenti in prossimit? di una serie di grandi centri industriali attivi o dismessi, o di aree oggetto di smaltimento di rifiuti industriali e/o pericolosi, che presentano un quadro di contaminazione ambientale e di rischio sanitario tale da avere determinato il riconoscimento di "siti di interesse nazionale per le bonifiche" (SIN). Lo studio ha preso in considerazione 44 dei 57 siti oggi compresi nel "Programma nazionale di bonifica", che coincidono con i maggiori agglomerati industriali nazionali; per ciascuno di essi si ? proceduto a una raccolta di dati di caratterizzazione, e successivamente a una loro sintesi. La maggior parte dei dati raccolti proviene dai progetti di bonifica ipotizzati per i diversi siti, da cui si evince che oggetto di caratterizzazione e di valutazione del rischio sono state prevalentemente le aree private industriali, quelle, cio?, ritenute causa delle diverse tipologie di inquinamento (definite in SENTIERI esposizioni ambientali). Le aree pubbliche cittadine e/o a verde pubblico e le aree agricole comprese all'interno dei SIN sono state poco investigate. I SIN studiati sono costituiti da uno o pi? Comuni. La mortalit? ? stata studiata per ogni sito, nel periodo 1995-2002, attraverso i seguenti indicatori: tasso grezzo, tasso standardizzato, rapporto standardizzato di mortalit? (SMR) e SMR corretto per un indice di deprivazione socioeconomica messo a punto ad hoc. Nella standardizzazione indiretta sono state utilizzate come riferimento le popolazioni regionali. L'indice di deprivazione ? stato calcolato sulla base di variabili censuarie appartenenti ai seguenti domini: istruzione, disoccupazione, propriet? dell'abitazione, densit? abitativa. Gli indicatori di mortalit? sono stati calcolati per 63 cause singole o gruppi di cause. La presenza di amianto (o di fibre asbestiformi a Biancavilla) ? stata la motivazione esclusiva per il riconoscimento di sei SIN (Balangero, Emarese, Casale Monferrato, Broni, Bari-Fibronit e Biancavilla). In tutti i siti (con l'esclusione di Emarese) si sono osservati incrementi della mortalit? per tumore maligno della pleura e in quattro siti i dati sono coerenti in entrambi i generi. In sei siti con presenza di altre sorgenti di inquinamento oltre all'amianto, la mortalit? per tumore maligno della pleura ? in eccesso in entrambi i generi a Pitelli, Massa Carrara, Priolo e nell'Area del litorale vesuviano. Nel periodo 1995-2002 nell'insieme dei dodici siti contaminati da amianto sono stati osservati un totale di 416 casi di tumore maligno della pleura in eccesso rispetto alle attese. Quando gli incrementi di mortalit? riguardano patologie con eziologia multifattoriale, e si ? in presenza di siti industriali con molteplici ed eterogenee sorgenti emissive, talvolta anche adiacenti ad aree urbane a forte antropizzazione, rapportare il profilo di mortalit? a fattori di rischio ambientali pu? risultare complesso. Tuttavia, in alcuni casi ? stato possibile attribuire un ruolo eziologico all'esposizione ambientale associata alle emissioni di impianti specifici (raffinerie, poli petrolchimici e industrie metallurgiche). Tale attribuzione viene rafforzata dalla presenza di eccessi di rischio in entrambi i generi, e in diverse classi di et?, elementi che consentono di escludere ragionevolmente un ruolo prevalente delle esposizioni professionali. Per esempio, per gli incrementi di mortalit? per tumore polmonare e malattie respiratorie non tumorali, a Gela e Porto Torres ? stato suggerito un ruolo delle emissioni di raffinerie e poli petrolchimici, a Taranto e nel Sulcis-Iglesiente-Guspinese un ruolo delle emissioni degli stabilimenti metallurgici. Negli eccessi di mortalit? per malformazioni congenite e condizioni morbose perinatali ? stato valutato possibile un ruolo eziologico dell'inquinamento ambientale a Massa Carrara, Falconara, Milazzo e Porto Torres. Per le patologie del sistema urinario, in particolare per le insufficienze renali, un ruolo causale di metalli pesanti, IPA e composti alogenati ? stato ipotizzato a Massa Carrara, Piombino, Orbetello, nel Basso bacino del fiume Chienti e nel Sulcis-Iglesiente-Guspinese. Incrementi di malattie neurologiche per i quali ? stato sospettato un ruolo eziologico di piombo, mercurio e solventi organo alogenati sono stati osservati rispettivamente a Trento Nord, Grado e Marano e nel Basso bacino del fiume Chienti. L'incremento dei linfomi non-Hodgkin a Brescia ? stato messo in relazione con la contaminazione diffusa da PCB. Ulteriori elementi di interesse sono stati forniti dalle stime globali della mortalit? nell'insieme dei siti oggetto del Progetto SENTIERI. In particolare, ? emerso che la mortalit? in tutti i SIN, per le cause di morte con evidenza a priori Sufficiente o Limitata per le esposizioni ambientali presenti supera l'atteso, con un SMR di 115.8 per gli uomini (IC 904.4-117.2, 2 439 decessi in eccesso) e 114.4 per le donne (IC 902.4-116.5; 1 069 decessi in eccesso). Tale sovramortalit? si riscontra anche estendendo l'analisi a tutte le cause di morte, cio? non solo per quelle con evidenza a priori Sufficiente o Limitata: il totale dei decessi, per uomini e donne, ? di 403 692, in eccesso rispetto all'atteso di 9 969 casi (SMR 102.5%; IC 902.3-102.8), con una media di oltre 1 200 casi annui. Si ritiene opportuno ricordare che il Progetto SENTIERI, per obiettivi, disegno e metodi, rappresenta uno strumento descrittivo che verifica, in prima istanza, se e quanto il profilo di mortalit? delle popolazioni che vivono nei territori inclusi in aree di interesse nazionale per le bonifiche si discosti da quello cause delle popolazioni di riferimento. Ai fini dell'interpretazione dei risultati, si ricorda che la presenza di eccessi di mortalit? pu? indicare un ruolo di esposizioni ambientali con un grado di persuasivit? scientifica che dipende dai diversi specifici contesti; invece, un quadro di mortalit? che non si discosti da quello di riferimento potrebbe riflettere l'assenza di esposizioni rilevanti, ma anche l'inadeguatezza dell'indicatore sanitario utilizzato (mortalit? invece di incidenza) rispetto al tipo di esposizioni presenti, o della finestra temporale nella quale si analizza la mortalit? rispetto a quella rilevante da un punto di vista dell'esposizione. La condivisione dei risultati con i ministeri della salute e dell'ambiente, le Regioni, le ASL, le ARPA e i Comuni interessati consentir? l'attivazione di sinergie fra le strutture pubbliche con competenze in materia di protezione dell'ambiente e di tutela della salute, e su questa base l'avvio di un processo di comunicazione con la popolazione scientificamente fondato e trasparente.
