How Palliative and Hospice Care are Organized in Ukraine (Review

The article considers and covers the approaches of the development of palliative and hospice care in Ukraine, namely the departments of palliative and hospice medical institutions have extremely limited resources and are not able to provide patients with even the most necessary . An important problem is the creation of a system of personnel training in Ukraine that is involved in providing the service of palliative and hospice care. The duty of the state to ensure access to palliative care for all patients, regardless of age, the nosological category of the disease, social status, nationality, religious and political beliefs, place of residence, etc. ; У статті розглянуто та висвітлено питання, повязані з розвитком паліативної та хоспісної допомоги в Україні, а саме, відділення паліативного та хоспісних лікувально-профілактичних закладів є вкрай обмежені ресурси та не мають можливості забезпечити хворих навіть найнеобхіднішим. Важливою проблемою є створення в Україні системи підготовки кадрів, які беруть участь у наданні послуг паліативної та хоспісної допомоги. Обов'язок держави забезпечити доступ до паліативної допомоги всім пацієнтам, незалежно від віку, нозологічної категорії захворювання, соціального статусу, національності, релігійних та політичних переконань, місця перебування тощо. Якщо суспільство вважає себе цивілізованим, то неодмінно зобов'язане подбати про те, щоб невиліковно хворі отримали якісний і належний догляд. Байдуже суспільство немає майбутнього!

Stimulated by Downs(2003) article on Failure in Palliative Care the author reflects on the workings of a day hospice focusing on the difficult and challenging issues for patients and how the hospice setting can enable these issues to be raised and addressed. The issues are explored and illustrated using patient comments and experiences. Acknowledging a dearth of hospice day care research and literature the author seeks to motivate others and encourage a wider appreciation of day hospice and thereby influence future practice.

Dealing with existential questions is a constitutive part of palliative care. Interestingly, if we admit that border situations at the end of life give rise to clearly 'philosophical' questions, in the contexts of practice, these questions are perceived (nearly) without any reference to explicit philosophical traditions or to 'philosophical practice'. Existential questions in palliative care are usually approached from other perspectives and within other organisational forms of reflection and dialogue (psychotherapy, supervision, spiritual care, moral case deliberation). This piece considers how philosophical dialogue and reflection could contribute to palliative care and hospice work.

Purpose: medical-social analysis and determination of organizational aspects of the current state and priority directions of development of palliative and hospice care (PHC) system in Ternopil region in the conditions of reforming the health care system of Ukraine. Materials and Methods. In order to achieve this goal, common scientific methods of analysis, synthesis and generalization were used. Results. The unfavorable medical and demographic situation in Ukraine requires the justification, creation and development of an affordable, high-quality and effective PHC system, which requires the involvement of medical workers of health care facilities (HCF) of primary health care specialized HCF in coordinating the work of institutions, subordinating to different ministries and government departments, different forms of ownership. Today in the Ternopil region medical sector reforms are being continued in order to bring accessibility and improve the quality of service to the population, expand the range of medical services to patients in need of PHC by involving general practitioners-family doctors in outpatient palliative care at home as well. Conclusions. The medical-social analysis of demographics, morbidity and mortality indicates a rapid decline in population and aging, a high mortality rate from malignant neoplasms and severe complications of chronic noncommunicable diseases that determine the high need for PHC. Based on the assessment of the development of palliative and hospice care and its compliance with world standards, we understand that the system needs further development. ; Мета: провести медико-соціальний аналіз і визначити організаційні аспекти сучасного стану та пріоритетні напрямки розвитку системи паліативної та хоспісної допомоги (ПХД) на Тернопільщині за умов реформування системи охорони здоров'я України. Матеріали і методи. Для досягнення поставленої мети у роботі використано загальнонаукові методи аналізу, синтезу та узагальнення. Результати. Несприятлива медико-демографічна ситуація в Україні вимагає обґрунтування, створення і розвитку доступної, якісної та ефективної системи ПХД, що потребує залучення до надання паліативної та хоспісної допомоги медичних працівників закладів охорони здоров'я (ЗОЗ) первинної медичної допомоги і спеціалізованих ЗОЗ, координації роботи закладів та установ, підпорядкованих різним міністерствам та відомствам, різних форм власності. На сьогодні в Тернопільській області продовжуються реформи медичної галузі з метою наближення доступності та покращення якості обслуговування населення, розширення спектра надання медичних послуг пацієнтам, які потребують паліативної та хоспісної допомоги шляхом залучення лікарів загальної практики – сімейної медицини до надання амбулаторної паліативної допомоги пацієнтам, у тому числі вдома. Висновки. Медико-соціальний аналіз демографічних показників, захворюваності і смертності свідчить про швидке скорочення чисельності та постаріння населення, високий рівень смертності від злоякісних новоутворень і тяжких ускладнень хронічних неінфекційних захворювань, що визначають високу потребу населення у ПХД. На основі оцінки розвитку паліативної та хоспісної допомоги та відповідності її світовим стандартам розуміємо, що система потребує подальшого розвитку.

На основі оцінки розвитку паліативної та хоспісної допомоги та відповідності її світовим стандартам розуміємо, що система потребує подальшого розвитку.
