Out-of-hospital cardiac arrest (OHCA) is a potential situation faced by all actors of the prehospital rescue chain. The prehospital emergency physician is the last link in this chain of survival and must decide to continue or stop initial resuscitation. The situation is further complex because the out-of-hospital environment does not provide physicians with the usual benchmarks of the in-hospital environment. Finally, the family or other bystanders are most often present and assist in the resuscitation attempts. OHCA is a pathology that requires invasive, sometimes aggressive medical gestures to restart the cardiac pump. The physician, first confronted with the choice of resuscitation, must then explain the situation to the family, and finally make the announcement of death to the relatives when it occurs. The objective of this study is to explore the physician's initial choice to act or not to act medically upon arrival at the bedside of a patient with OHCA. Qualitative research involving semi-structured interviews with prehospital physicians was conducted in February 2020. A total of 13 military and civilian physicians were interviewed. Nine interviews were analyzed due to the unprecedented COVID-19 health crisis. Our preliminary results suggest that physicians perform "technical" gestures while waiting for objective information in terms of prognosis, in a personal impulse of hope in life, in an approach of protection toward the family, against a possible "medico-legal risk," or for "oneself" in the face of the violence of a direct confrontation with death. The doctor legitimizes his or her actions for the principle of beneficence. To which need do they react and what are the consequences of their departure for Romanian society and health care system? The first part of the text is a synthesis of the causes of the immigration of Romanian physicians? Objective: identifying military personnels' criteria of preference for civilian or military physician as regular doctor and expectations toward the military physician. Method: prospective descriptive cross-sectional study conducted among 745 militaries with a self-administrated questionnaire. Results: if 69,9% of the sample appoint a civilian practitioner as their regular doctor, 65,6% think that military physicians should be able to carry out this mission. 80,8% think determining abilities and treatment are compatible. During the past year, 20,2% of the sample preferred consulting the military doctor, 23,1% used to consult him by opportunity but preferred a civilian physician. The rate of the sample preferring a military physician increases with rank, seniority and participation in external operations, particularly because of his knowledge of working conditions. Soldiers, corporals and recently enrolled militaries are suspicious of the military physician because they think that a conflict of interest exists between different missions: treatment, notifications about abilities and advisor to the command, thus inducing difficult care relationship. For patients, the military physician must be a general practitioner who knows emergency and military injuries specificities. He has to be humane and must have an officer's behaviour. Conclusion: 38,2% of the sample claim to consult military doctor for treatment. A suspicious population exists because of ignorance about the military physician's competence and the kind of information he transfers to the command. Depuis 40 ans en Occident, l'émergence d'une nouvelle génération d'individus, appelée 3e âge située à l'intervalle entre la vie professionnelle et le grand âge transforme la perception classique du vieillissement. Ces nouveaux vieux sont le reflet d'une vieillesse qui bouleverse les représentations usuelles de cette catégorie d'âge. Comment ces « jeunes vieux » perçoivent-ils leur vieillissement et appréhendent-ils les risques liés à celui-ci et comment les médecins généralistes accompagnent-ils les jeunes seniors face aux défis du vieillissement ? Il s'agit d'une enquête qualitative au travers d'entretiens semi-dirigés auprès de treize patients âgés de 65 à 75 ans à leurs domiciles et de leurs cinq médecins généralistes respectifs issus de cabinets de médecine générale en Alsace et en Lorraine. La perception du vieillissement chez les jeunes seniors s'insère dans une perspective globale de l'individu incluant l'expérience sensorielle, psychologique et socio-familiale de cette période qui implique des dynamiques complexes. La représentation du bien vieillir s'inscrit en contraste avec la figure du « vieux » avec l'exigence de « vieillir jeune ». Le médecin traitant est un partenaire important dans la promotion de la santé, la prévention des risques et l'accompagnement qu'il propose à son patient vieillissant au travers de la communication interactive sur les dépistages, les facteurs de risque et l'éducation à la santé. L'avènement de la médecine préventive et des politiques de lutte contre le vieillissement ouvre la voie à de nouvelles perspectives en matière de prévention, pour autant que le patient reste au centre de la prise en soin.

Hamas' leaders rarely speak publicly. So we particularly appreciated this interview w ith Mahmoud Zahar, who agreed to speak with Aude Marcovitch for Politique Internationale. He retraces the journey that led a young Palestinian doctor to an involvement in trade union activism and then to 1987, when he was instrumental in the formation of Hamas, a national resistance movement with religious underpinnings. Like Islamic Jihad, also based in Gaza, Hamas makes armed confrontation with Israel the foundation of his doctrine. I refuses to recognize the Jewish state and rejects the principle of a negotiated solution to the conflict. Having served briefly as Foreign Minister in the government led by Ismail Haniyeh in 2006 and 2007, Mahmoud Zahar is considered Hamas' number-two leader in Gaza and one of the Islamist party's more stubborn hardliners. Research activity funding in French hospitals is partly based on the number of publications, while taking also into account author position (first, second, third, second-to-last, last, investigator list, and Other) and journal category (A being the highest category followed by B, C, D, E, and NC). Over the 2004-2014 decade, the profile and the evolution of publications for the six existing types of hospital, i.e public teaching hospital, cancer centre, public non-teaching hospital, not-for-profit private hospital, military hospital, and for-profit private hospital were analysed. Among a total of 192 886 publications analysed, the most frequent category was B for cancer centres, whereas this was E in the other types. The first position was the most frequent for public teaching hospitals and the military hospital, whereas the Other position was the most frequent for the other types. The author position indicated that all types of hospital are involved in research projects. Public non-teaching hospitals, not-for-profit private hospitals, for-profit private hospitals, and cancer centres collaborated with other institutions; cancer centres were often distinguished by publishing in high-category journals. Over the same period, for the public teaching hospitals, means, Spearman correlation coefficients and Pearson regression lines were calculated between the following variables from 2004 to 2014 per full time equivalent (FTE) physicians: the number of publications, the number of hospital stays, the number of FTE university hospital practitioners, and finally the number of residents. The mean per FTE physicians was 0.73 publications, 235.8 hospital stays, and 0.63 residents. The correlations between these three variables two by two were weak. The correlations between the proportion of university hospital practitioners and number of publications per FTE physicians, number of residents per FTE physicians, AB categories publications per FTE physicians, or first and last authors' publications per FTE physicians was strong. ...
