This work is a review of the philosophical analisys surrounding the concept of "humanism" and what it means to be a human being, in relationship to daily life, education and medicine. The authors establish a direct relationship between humanism and bioethics as they relate to the new trends acquired through the development of institutional medicine and the increasing application of technological innovations in the health field. Both of these conditions tend to depersonalize the practice of medicine, and transform an ill person into a clinical file. Reflections are made about current topics, such as the knowledge and manipulation of human genoma, assisted reproduction, abortion, survival of premature infants, organ transplants, technological innovation, euthanasia and disthanasia. Concepts and ideas are reviewed in relation to medical institutions and the sick, the physician and the community, and the physician and the government. ; En este trabajo se revisan conceptos filosóficos acerca del humanismo y de las implicaciones del ser humano, de su vínculo con la vida cotidiana, con la educación y con la medicina. Se establece aquí una relación directa entre el humanismo y la bioética en lo que respecta a los nuevos aires que imponen el auge de la medicina institucional y la creciente aplicación de innovaciones tecnológicas en el campo de la salud, situaciones que tienden a despersonalizar la práctica médica y a convertir al paciente en un simple expediente clínico. Se hacen reflexiones sobre el conocimiento y la manipulación del genoma humano, la reproducción asistida, el aborto, la supervivencia de los niños prematuros, el trasplante de órganos, la innovación tecnológica, la eutanasia y la distanasia. Se examinan conceptos e ideas relativas a la relación entre las instituciones de atención médica y el enfermo; entre el médico y la colectividad; y entre el médico y el gobierno.

The position of physicians regarding induced abortion in Mexico deserves closer attention. The attitudes of physicians towards induced abortion have been a subject of study in different countries. It has been observed that such attitudes depend on ethical, religious, legal, political, or medical factors. In those countries where abortion is not penalized, physicians sustain different positions regarding this issue. The experiences of some countries indicate that the institutional provision of abortion services is a controversial and politically serious matter, and that offering or denying them generally depends to a great extent on the physician's discretion. In contexts like Mexico, where abortion is penalized, legal and other restrictions do not prevent many physicians from practicing it in a concealed way, generally for profit. On the other hand, even though abortion is recognized as an important social and public health problem, and laws regulate the conditions for its legal practice, the majority of medical professionals and health institutions maintain a conservative and reserved position on this matter. The professional training of physicians and the legal status of induced abortion as a criminal practice, are central elements in their attitudes towards this problem. ; La posición de los médicos frente al aborto inducido es un problema que merece mayor atención en México. Estudios realizados en algunos países muestran que las actitudes de los médicos frente al aborto están influidas por factores diversos (éticos, religiosos, jurídicos, políticos y médicos, entre otros), y que donde es legal coexisten profesionales con posiciones diversas. Recognizing the role of technology in the development of medicine and the impact of telecommunication advances, we reflect on the meaning and ethics of the use of Telemedicine, both in its general dimension for the use and distribution of knowledge, as well as in the delivery of health actions, scientific research, and data management. Teleconsultation is discussed in greater detail, analyzing its process and application, reviewing its possible advantages and disadvantages, from the point of view of providers and patients. We highlight the need to carry out an appropriate evaluation of each instance, from the point of view of both the patient and the professional who uses it. The importance of maintaining a doctor- patient relationship in agreement with the nature and practice of Medicine, respecting people's dignity, is emphasized. We mention the ethical conditions that must be bore in mind for the proper use of telemedicine. We discuss the eventual influence that this practice will have on the concept and practice of medical care, while suggesting the need to legislate on the matter. ; Versión publicada - versión final del editor

[spa] La relación médico paciente (RMP) es una relación interpersonal asimétrica que nace del encuentro entre médico y paciente, produce resultados y se desarrolla en tres dimensiones: individual, económica y social. El diagnóstico y el tratamiento son los resultados más considerados, pero no los únicos. Satisfacción, experiencia, adherencia y adhesión al tratamiento, continuidad, uso de los servicios de urgencia o acceso a los recursos sociales, son resultados que el paciente vive en términos de salud y de bienestar mientras que el financiador los traduce en términos de consumo de recursos. El financiador no considera estos productos de la RMP igual que aquellos, tiende a juzgarlos resultados tangenciales de la RMP y los desatiende por considerarlos indirectamente relacionados con ella, pese a que revelan la perspectiva del paciente. La complejidad de la atención contribuye a infravalorar dichos resultados al multiplicar los interlocutores del paciente y difuminar los resultados. Muchos promotores de la RMP piensan que potenciarla solo aporta beneficios marginales; aunque esa visión contradiga los hallazgos de los estudios sobre continuidad, coste de la atención a enfermedades crónicas o los obtenidos en términos de seguridad. Mediante su propia experiencia, el paciente protagoniza la RMP y la enriquece con valores cualitativos, como la necesidad de adecuarla a la situación propia, su voluntad y sus expectativas. La RMP se enmarca dentro de la actividad profesional del médico que se rige por obligaciones éticas, técnicas y por condicionantes económicos. La asimetría de poder en la RMP es debida al desequilibrio entre el estado de necesidad de la persona que busca ayuda sanitaria frente al profesional, poseedor de la autoridad y el poder conferidos por los conocimientos de la Medicina y el Estado. Para reducir esa asimetría se intenta contrapesar el poder del médico mediante disposiciones sociales, legales y personales. La confianza sustenta la RMP como manifiesta la custodia del cuerpo y de la información que surge por la intervención médica, sin embargo, aumenta la vulnerabilidad del paciente. Los conflictos de interés relacionados con los honorarios o con la promoción social o académica ponen en entredicho la confianza hacia el médico o hacia la profesión, al igual que el uso inadecuado de los conocimientos de la Medicina puede lesionar al paciente, al médico, a la profesión y a la sociedad. En el espacio de la RMP se toman decisiones que implican a la salud y a la vida del paciente, a la economía y la sociedad. Sobre los protagonistas se ejercen presiones, internas y externas, para que la RMP incorpore a terceros, con voz, voto y sin riesgos y puedan intervenir en la toma de decisiones, incluso en su propio beneficio en lugar del paciente. Estas presiones se ejercen directa e indirectamente mediante: los honorarios; la publicidad; las organizaciones gubernamentales; las leyes; los pacientes, sus asociaciones y las medidas que potencian su autonomía; la formación médica o los lobby. Todas ellas pueden lesionar al paciente, al médico, a la profesión, al conocimiento médico y a la sociedad, desembocando, además, en una práctica defensiva de la Medicina. ; [eng] The doctor-patient relationship (DPR) is an asymmetric interpersonal relationship that emerges when doctors and patients meet, produces outcomes and develops in three dimensions: individual, economic and social. Diagnosis and treatment are the outcomes most often considered but these are not the only ones. Satisfaction, experience, adherence and adhesion to treatment, continuity, the use of emergency services or access to social resources, are outcomes that patients live through in terms of health and well-being, while financial bodies analyze this factors in terms of resource consumption. The funder does not consider these second group of outcomes in the same way as diagnosis and treatment, tend to judge them as peripheral outcomes and neglect it because they are only considered indirectly related to DPR, and despite the fact that they reveal the patient's perspective. Health assistance complexity contributes to the undervaluing of these outcomes due to the proliferation of concerned partner and outcome blurring. Many promoters of DPR think that its enhancement brings only a minor benefits; although this point of view contradicts the findings of studies on continuity, cost of attention to chronic diseases and others studies related to safety. Through their own experience, the patient must own and lead the DPR and enrich it with qualitative values, such as the need to adapt to their own situation, wills and expectations. The DPR is framed within the professional activity of doctors that is ruled by ethical and technical obligations and by economic considerations. There is a power asymmetry due to imbalance caused by the needs of people looking for health care versus the professional, holder of the authority and power conferred by the knowledge of Medicine and the State. To reduce the asymmetry attempts are made to counterweigh the power of the doctor through social, legal and personal provisions. Trust underpins the DPR which is reflected in their responsibility for the care of the body and for the information that arises from medical intervention, nevertheless, the vulnerability of the patient increases. The conflicts of interest related to medical fees or to social or academic promotion raise doubts about the trust for the doctor or for the profession, as the improper use of medical knowledge can injure the patient, the doctor, the profession and the society. In the DPR's domain decisions are taken that involve the health and life of a patient, the economy and society. Pressures are exerted on the protagonists, both internal and external, so that DPR incorporates third-party voices, votes and without risks, can intervene in the decisions-making process, even to their own benefit in replacing the patient. These pressures are exerted directly and indirectly through: fees; advertising; governmental organizations; law; patients, their associations and measures to enhance patient autonomy; medical training or lobbies. All of them can injure the patient, the doctor, the profession, the medical knowledge attained and society, leading, additionally, to a defensive practice in Medicine. ; [cat] La relació metge-pacient (RMP) és una relació interpersonal asimètrica que neix de la trobada entre metge i pacient, produeix resultats i es desenvolupa en tres dimensions: individual, econòmica i social. El diagnòstic i tractament són els resultats més considerats, però no els únics. Satisfacció, experiència, adherència i adhesió al tractament, continuïtat, ús dels serveis d'urgència o accés als recursos socials, són resultats que el pacient viu en termes de salut i benestar mentre que el finançador els tradueix a termes de consum de recursos. El finançador no considera aquests productes de la RMP igual que aquells, tendeix a jutjar-los resultats tangencials de la RMP i els desatén perquè els considera només indirectament relacionats amb ella, tot i que revelen la perspectiva del pacient. La complexitat de l'atenció, que multiplica els interlocutors del pacient i difumina els resultats, contribueix a infravalorar aquests resultats. Dins l'espai de la RMP es prenen decisions que impliquen a la salut i a la vida del pacient, a l'economia i a la societat. Sobre els protagonistes s'exerceixen pressions, internes i externes, perquè la RMP incorpori terceres persones, amb veu, vot i sense riscos i puguin intervenir a la presa de decisions, inclús en el seu propi benefici substituint al del pacient. Aquestes pressions s'exerceixen directament i indirecta mitjançant: els honoraris; la publicitat; les organitzacions governamentals; las lleis; els pacients, les seves associacions y les mesures que potencien la seva autonomia; la formació o els lobby. Totes elles poden lesionar al pacient, al metge, a la professió al coneixement mèdic i a la societat, desembocant, a més, en una pràctica defensiva de la Medicina.

We discuss the current reality of peruvian physicians considering a number of variables, mainly the Reform in which our Health Sector is involved and the seculary trends in the training, qualifying and performance of physicians. This is a first step in order lo make an outline of the work that our physicians are performing. ; En este trabajo se pretende, a partir de fuentes secundarias, describir la configuración actual de la profesión médica teniendo en cuenta, en lo posible, un doble marco de referencia: la reforma sanitaria que se procesa en el país y las tendencias seculares en los campos de formación, capacitación y trabajo médico. El estudio es un primer esfuerzo de mirar el acontecer del trabajo médico desde aportes que viene realizando la teoría de los sistemas dinámicos no lineales.

Editorial del vol.18 n.3 (2015) de la publicació ; Continuing professional development, the processes involved in its accreditation and all the initiatives that the central and autonomic authorities, scienti c societies and medical associations have woven around this concept have played a vital role in creating a collective awareness of the fact that the recerti ca- tion of doctors is the most coherent way to guarantee the quality of the professional actions, as well as bringing us closer to what is already the norm inter- nationally. Moreover, the European Union has passed legislation in this sense. One of the initiatives that deserves special attention is Periodic Membership Validation, not because it is better or worse than any of the others, but because it is the rst that has man- aged to implement the process on a regular basis.

La adherencia a controles prenatales constituye una herramienta fundamental para disminuir la mortalidad materna. Usualmente se piensa la adherencia a partir de las características de las mujeres asistidas o de las particularidades del sistema de salud y sus agentes. Aquí se la aborda como emergente del modo en que se articulan las racionalidades y mundos de vida de profesionales y mujeres asistidas.Se realizó un estudio de casos múltiple buscando a comprender la adherencia a controles prenatales de mujeres rurales. Se realizaron 47 entrevistas en el norte argentino a mujeres embarazadas o que habían sido madres en los últimos años y a agentes del sistema de salud. El contenido de las entrevistas fue codificado con el apoyo del software Atlas Ti. El estudio permitió identificar dos tipos de interacción médico-paciente. El primero se caracteriza por una representación de las pacientes como ignorantes y desinteresadas, lo que lleva al agente a insistir con sus recomendaciones sin atender a las dificultades vividas por las mujeres. El segundo parte de concebir a las mujeres como cumplidoras y responsables, lo que permite un mayor diálogo, aún sin que se observe un interés claro por conocer el contexto de vida de las mujeres atendidas. ; Adherence to prenatal care is fundamental for diminishing maternal mortality. Usually, adherence is considered in terms of assisted women's characteristics or the specificities of the health systems and its professionals. Adherence is addressed here as the emergence of the articulation between the rationales and the life realities of physicians and pregnant women. Aiming to understand the adherence to prenatal care, a multiple case study was conducted in the North of Argentina. A total of 47 interviews of pregnant women or women that had been given birth in the last years and to agents of the health system were conducted. The content of the interviews was coded with the support of Atlas Ti software. The research identified two types or interaction between physicians and patients. The first one is based on physicians' representation of assisted women as being ignorant and disinterested, which leads professionals to insist on their recommendations without taking into account problems faced by women. The second part of conceptions is supported in the representation of women as obedient and responsible, which enables a better dialogue between physicians and patients, albeit without the knowledge of a clear interest of the life context of the women being known. ; Fil: Gonzalez Cowes, Valeria Laura. Since the end of the sixteenth century the Hanseatic city of Hamburg received a group of impostant Portuguese physicians that became members of its Iberian Jewish community. This article examines the intellectual contribution and the political implications of two of them, Rodrigo de Castro (1546-1627), who authored an important book on gynaecology, and Manuel Bocarro Rosales (ca. 1588-1662/8?), who wrote different astrological and astronomical works with a messianic-political content. ; Desde fines del siglo XVI Hamburgo recibió una constelación de distinguidos médicos portugueses que se integran dentro de la comunidad judía de origen ibérico. En este artículo se examina la contribución intelectual y las implicaciones políticas de las obras de dos de ellos, Rodrigo de Castro (1546-1627), autor de un importante tratado ginecológico, y Manuel Bocarro Rosales (ca. 1588-1662/8?), que escribe varias obras sobre astronomía y astrología de contenido político-mesiánico.

Se observa con creciente evidencia que una comunicación empática y próxima entre el médico y el paciente tiene un efecto positivo tanto en el diagnóstico como en el tratamiento y la recuperación de la persona enferma. Sin embargo, la existencia de ciertas barreras comunicacionales hoy en día impide que se optimice una relación eficiente y beneficiosa para ambos interlocutores. El presente trabajo tiene la finalidad de analizar distintos aspectos que influyen en la relación médico-paciente desde una perspectiva comunicativa y comparar las percepciones que ambos interlocutores tienen de su interacción en la consulta médica. Para ello, se encuestaron un total de 103 pacientes y los 24 médicos del Institut Clinic Cardiovascular del Hospital Clínic de Barcelona con preguntas relativas a la autonomía del paciente, la proximidad del vínculo asistencial, el uso de recursos empáticos o de cortesía y factores externos como el espacio o el tiempo de consulta. Así, se observó que los médicos consideran que su conducta hacia el paciente es empática y que su relación se basa en un modelo más democrático que paternalista. Por el contrario, los usuarios perciben menor autonomía y un vínculo más distante. Además, tanto un grupo como otro rehúsa cambios en la distribución del espacio de la consulta como medida de proximidad y, concretamente, los pacientes rechazan también alargar el tiempo de la consulta. Se concluye así, que las estrategias de mejora deben orientarse preferiblemente hacia la enseñanza y el desarrollo de habilidades comunicativas entre los profesionales sanitarios. Palabras clave: paciente, médico, relación, comunicación, empatía. ; Analysis of the doctor-patient communication in the Hospital Clínic: self-perceptual differences between doctors and patients. It is observed with increasing evidence that empathic and close communication between the doctor and the patient has a positive effect both in the diagnosis and in the treatment and recovery of the sick person. However, the existence of certain communication barriers today prevents an efficient and beneficial relationship for both partners from being optimized. The purpose of the present study is to analyze different aspects that currently influence the doctor-patient relationship from a communicative perspective and to contrast the perceptions that doctors and patients have of their interaction in the medical consultation. To this aim, 103 patients and 24 physicians who attended them at the Institut Clinic Cardiovascular of the Hospital Clínic de Barcelona were surveyed with questions regarding patient autonomy, the proximity of the care link, the use of empathic or courtesy resources and the presence of external factors such as the room or the time of consultation. We observed that doctors consider that their behavior towards the patient is empathetic and that their relationship is based on a more democratic than paternalistic model. On the contrary, patients perceive less autonomy and a more distant link. In addition, both groups refuse changes in the distribution of the consultation room as a measure of proximity and, specifically, patients also refuse to extend the time of the consultation to improve their relationship with the doctor. Learning and development of communication skills should be promoted among health professionals in order to optimize their beneficial potential.

The discussion of a bill that allows medically assisted death (MAD) in Chile, revived the debate about the ethics of this practice. The Department of Ethics of the Chilean Medical Association herein analyzes arguments in favor or against the participation of the medical profession in MAD. Among the main arguments against the participation of physicians in this practice are that MAD conflicts with the basic ethical principles of medical practice, that it is contrary to the purposes of medicine and that it could erode the patients' and society's confidence in physicians. The arguments in favor are related to physician's compassion and non-abandonment of patients during their illness, choosing palliative care and ushering them to the final instance. Additionally, there is social expectation that this practice will be carried out by trained physicians who can verify that the strict criteria established by the legislation are met, guarantee that it obeys to a repeated request of a fully capable patient, and who is able to deal with the complications of the procedure. In this document we aimed to represent the different perspectives about physicians' participation in MAD, offering arguments to colleagues and stimulating their participation in this important debate.
