The total public funding allocated in Japan for public works each year is enormous when compared to what other industrialized countries generally invest in the improvement of their public infrastructure. Vast amounts of public funds are used to build roads, dams, community centers and other public facilities, taking into account not only the regular budgetary expenditure, but also the large economic stimulus packages the Japanese government has put in place on a nearly annual basis throughout the 1990s. Considering the widely varying impacts of infrastructure investment, not all of them positive with regard to demand, efficiency and environmental concerns, a fundamental question arises as to who decides which projects this money is spent on. To answer this question, the book provides an analysis of the formal decision-making process and the informal influence structures characteristic of political decision making in the field of public works in Japan. All levels of government, i.e., the central, prefectural and local governments, are covered by the study, which employs a network approach in order to take account of all relevant actors. The analysis focuses on the annual budgetary process, which is the arena in which the use of public funds for particular infrastructure programs and projects is actually authorized, and on the medium to long-term planning instruments for various types of public works. The results are analyzed from three perspectives: the respective roles of bureaucrats and politicians in policymaking, the degree of centralization of the political decision-making process, and the effects of specific political institutions on the mechanisms of political decision making.
How to assess Disaster Responds on Flood Events during the 20th Century – a conceptional framework and its application in Vietnam and Germany A comparison of disaster responds on flood events during the late 20th century in Vietnam and Germany promises to provide an insight on disaster responds from a normative perspective. The fear of natural disasters leads to a comprehensive program of sciences and politics to apply security technologies on a global scale. Historical Environmental Science traces the question whether societies learn from natural disasters. I aim to contribute to the question: Was the handling of flood events rational? The normative framework I base my research on to assess the physical and psychological costs reflects a multidimensional structure of welfare economics as well as the application of the ethnical theory of social subjectivism. The basic premise of the ethnical theory is marked by the transformable character of all evaluative statements into states about all individual preferences. As a baseline the ethnical theory of social subjectivism functions to focus on all preferences by natural persons. Herein, institutions are assumed outcomes of preferences aiming to strategically solve specific challenges. Human beings are always oriented to solve problems. While reducing this economical cost is imperative, the question remains whether this cost should be reduced. However, limited resources force decision makers to assess which problems may be solved and which not. In this context it may be of benefit and the most rational to not realize prevention measurements, as the linked opportunity costs end up being too high. In order to evaluate a response in this normative framework one has to compare the social benefits of this response with the linked social cost. However, due to missing historical data on social benefits and cost, such cost-benefit analysis cannot be performed. Therefore I developed a concept following Schnädelbach's theory of interpersonal communication as a basis of rationality which solves these uncertainties and allows a normative assessment of disaster responds on flood events. The concept uses an indirect approach and is based on accessible information.
Ein allgemeiner Trend im Gesundheitswesen unterstützt eine Atmosphäre von ver- stärkter Zusammenarbeit zwischen den Patienten und ihren jeweiligen Anbietern der Gesundheitsfürsorge. Die Forderung nach Einbeziehung und Berücksichtigung des Patienten im medizinischen Entscheidungsprozess wird von Politik und Patienten gleichermaßen unterstützt. Mit dem Ziel der aktiven Partizipation von Patienten bei Fragen der Gesundheitsversorgung hält eine neue Gewichtung von kommunikativen Fähigkeiten Einzug in die medizinische Ausbildung und die klinische Routine. Ärzte werden sensibilisiert für die Aufklärung des Patienten als eine weitere Facette ärztlich ethischer Verantwortung. Die partizipative Entscheidungsfindung (PEF) als Mittelstellung zwischen dem "paternalistischen Ansatz" und dem patientenzentrierten "Autonomiemodell" folgt einem klar strukturierten Aufbau im Arzt-Patient-Gespräch. Durch Umsetzung der PEF werden die Akzeptanz der Behandlung, die Zufriedenheit von Arzt und Patient, die Zuverlässigkeit bei der Behandlungsumsetzung und die klinischen Behandlungserfolge verbessert [96]. In der vorliegenden Arbeit wurde das Modell der PEF für zwei anästhesiologische Bereiche untersucht. Die Fragestellung der ersten Arbeitshypothese widmete sich dem Vergleich der zwei Patientengruppen (Rettungsstelle vs. Schmerzambulanz) im Hinblick auf ihre Präferenzen bezüglich der PEF und deren Einflussfaktoren. Die zweite Arbeitshypothese zielte darauf ab, die zwei Patientengruppen auf bisher unbeobachtete Heterogenität zwischen latenten Subpopulationen von Patienten zu untersuchen, die einen unterschiedlich starken Wunsch nach PEF aufwiesen. Als Maß der Patientenpräferenzen für Information und Beteiligung an der Entscheidungsfindung wurde der Autonomy Preference Index (API) neben einer Vielzahl weiterer soziodemographischer Fragen den Patienten beider Gruppen (n=569) zur Beantwortung vorgelegt. Können offene Gesellschaften, die über eine bestimmte Staatsideologie nicht verfügen, sondern unterschiedliche Interessen organisieren, Ziele für ein öffentliches Bildungssystem überhaupt begründen? Nachdem die fraglose Eingliederung des Bildungswesens in die Staatsverwaltung nicht mehr legitim erscheint und nachdem sich eine Organisation nach dem Demokratieprinzip als fragwürdig erwiesen hat, ist zunehmend von vertraglichen Regelungen auch im Bildungswesen die Rede, von Marktmechanismen oder korporativistischen Prinzipien. Privatisierung ist dafür das falsche Stichwort, es scheint eher um die vertragliche Organisation öffentlichen Handelns zu gehen. Die neuere bildungspolitische Entwicklung geht in diese Richtung: De-Institutionalisierung, Effizienzerwartung, Multikulturalität, Wahlfreiheit, Beteiligung. There are several reasons to assume that political decision making has to solve problems of considerably higher complexity than those to be dealt with in private business. At the same time the data base for describing the consequences of public decision making often is fairly weak. Cosequently, it often seems that public decision making lacks rationality. This in mind, is it worthwhile to apply model building techniques for public decision support ?For regional economic and port policy it is shown, that model building is able- to enhance transparency for a wide range of problems- to betterunderstand the questions to be dealt with- to clarify the consequences of particular courses of actionand thereby to increase efficiency of the resources publicly used.
In der hier vorliegenden Dissertation wurden anhand einer empirischen Fallstudie die Entscheidungs- und Aushandlungsprozesse zwischen Auftraggeber und Auftragnehmer in IT-Projekten analysiert und auf diesem Wege die Handlungszwänge des Projektmanagements beleuchtet. Damit sollte ein Beitrag zum näheren Verständnis der Eigendynamik von IT-Projekten und zur Wirkungsweise der gängigen Ansätze von Projektmanagement und QM geleistet werden. In dieser Arbeit wurden IT-Projekte und die Empfehlungen zum Projektmanagement aus politik- und organisationstheoretischer Perspektive beleuchtet, um Effekte begrenzter Rationalität, opportunistischen Verhalten und Inkrementalismus mit berücksichtigen zu können. Theoretische Ausgangspunkte für die Untersuchung sind auf der einen Seite die mikropolitischen Ansätze und auf der anderen Seite die Agenturtheorie. Gemeinsam ist diesen Ansätzen, die Auffassung, wonach das Handeln der Akteure in ergebnisoffenen, interdependenten Handlungskonstellationen stattfindet, in denen die Auswirkungen von Entscheidungen wenig bekannt sind und die benötigten Informationen den Akteuren in unterschiedlichem Maße zur Verfügung stehen. Bezüglich der Empfehlungen des Projektmanagements wurde auf Normen und Richtlinien sowie die Einführungs- und Praxisliteratur zum Projekt- und Qualitätsmanagement und zur Verwaltungs- und Organisationsberatung Bezug genommen. Die Fallstudie bezieht sich auf ein Realisierungsprojekt zur Einführung eines Personalmanagementsystems in einem Ministerium und seinen nachgeordneten Behörden. Das Projekt fand im Rahmen eines umfassenden Modernisierungsprogramms nach dem NSM statt und legte erstmals einen Schwerpunkt auf das Personalmanagement. Das Vorhaben war ursprünglich in drei Phasen von jeweils einem Jahr Laufzeit geplant. Mit der Realisierung wurde ein Konsortium aus drei Firmen beauftragt. Die Studie bezieht sich auf die erste der drei Phasen der Systemeinführung, die Pilotierungsphase in drei Pilotbehörden. Diese Phase beanspruchte insgesamt zwei Jahre und acht Monate und hatte damit ein Jahr und acht Monate Verspätung vor allem wegen wechselnden Ziel- und Aufgabenstellungen durch das Ministerium und aufgrund interner Steuerungsdefizite und Entscheidungsblockaden auf beiden Seiten. Die Entwicklung einer Schnittstelle in die Personalabrechnung wurde erfolglos abgebrochen. Hauptergebnis der Arbeit ist, dass die Anwendung von Methoden und Instrumenten, die in den Projektmanagement-Leitfäden bereitgestellt werden, stets den jeweils individuellen Interessen der Akteure unterliegen und als Bausteine der akteursspezifischen Strategien fungieren. Dabei führen insbesondere Strategien der Unsicherheitsabsorption, die auf die gegenseitige Kontrolle von Auftraggeber und Auftragnehmer hinauslaufen, zu unproduktiven, sich selbst verstärkende Blockadezyklen ("Circuli vitiosi"), aus denen die Akteure nur durch Strategiewechsel ausbrechen können. Zwischen Auftraggeber und Auftragnehmer besteht eine strukturelle Informationsasymmetrie, weil der Auftragnehmer über die fachlichen-technischen Aspekte und über den aktuellen Projektstand besser informiert ist als der Auftraggeber. Gleichzeitig besteht jedoch auch eine entgegen gesetzte Entscheidungsasymmetrie, bei der der Auftraggeber formale Entscheidungsbefugnisse an sich ziehen kann und im Konfliktfall über das größere Drohpotenzial verfügt. Diese wird durch die Rechtsform des Werkvertrags verstärkt. Die Leitfäden des Projektmanagements werden hierbei zwar angewendet, doch die Anwendung beschränkt sich auf fachlich-operative Aspekte der Projektdurchführung, in denen die Empfehlungen einen hinreichenden Konkretisierungsgrad erreicht haben. Nur unzureichend umgesetzt werden jedoch die Empfehlungen zu politisch-strategischen Problemen, deren Anwendung entweder einen komplexen strategischen Entscheidungsprozess erfordert oder zwischen den Vertragsparteien strittig ist. Als äußerst kritisch lässt sich vor allem der Umgang mit dem Leistungsumfang im Projektverlauf bezeichnen, die von Seiten des Auftraggebers häufig im Projektverlauf verändert wird. Hier kollidiert das Gebot der Unabänderlichkeit der Leistungsbeschreibung mit den vitalen Interessen des Auftraggebers, und es zeigt sich anhand der Fallstudie, dass die vertrauensvolle Zusammenarbeit beider Vertragsparteien nötig ist, um zu eine befriedigenden Lösung zu gelangen. Gelingt dies nicht, so kann das Projekt nur mit Hilfe so genannter starker Ideologien wie dem NSM in einer Art Krisenmanagement fortgeführt werden, bedarf aber dann der strategischen Neuorientierung z.B. in Form von Personalwechsel oder Reorganisationen. Im Fallstudienprojekt hat sich herausgestellt, dass, eine Analyse der spezifischen Akteursstrategien (Policy-Analyse) den Akteuren helfen kann, die Ausgangskonstellationen besser zu verstehen und die eigene Strategie auf ihre Konsens- oder Durchsetzungsfähigkeit hin zu justieren. Hier halten die Leitfäden zum Projektmanagement jedoch keine befriedigenden Handlungsanweisungen bereit. ; In the here available thesis on the basis of an empirical case study the decision making processes between clients and contractor in IT-projects were analyzed and in this way the compulsions to act of the project management were lit up. Thus a contribution should be made to the closer understanding of the self-dynamics of IT-projects and to the impact of the usual beginnings of project management and quality management. In this work IT-projects and the recommendations regarding the project management were lit up out of politics-scientifically and organization-theoretical perspective, in order to be able to consider effects of limited rationality, opportunistic behaviour and incrementalism. Theoretical starting points for the investigation are on the one side the micro-political approaches and on the other side the agency theory. These approaches have the conception in common according to which acting of the participants takes place in result-open, interdependent action constellations, in which the effects of decisions are few known and the needed information is available to the participants in different degree. Concerning the recommendations of the project management on standards and guidelines as well as the introduction and practice literature to the project and quality management and for the administrative and system information purchase was taken. The case study refers to a realization project for the introduction of a personnel management system in a Ministry and its subordinate authorities. The project took place in the context of a comprehensive modernization program according to the New Public Management and put for the first time an emphasis on the personnel management. The project was originally planned by in three phases in each case one year running time. The realization a consortium of three companies was assigned. The study refers to first of the three phases of the system introduction, the pilot phase with three pilot authorities. This phase lasted altogether two years and eight months and had thereby one year and eight months delay particularly because of changing a setting of tasks and goal by the Ministry and due to internal control deficits and decision blockades on both sides. The development of an interface into the personnel account was unsuccessfully cancelled. Main result of the work is that the use of methods and instruments, which are made available in the project management manuals, are always subject to the individual interests of the participants and the participant-specific strategies. Thereby particular strategies of the uncertainty absorption, which come down to mutual control of client and contractor, lead to unproductive, intensifying blockade cycles ("Circuli vitiosi "), of which the participants can break out only by changes of strategy. Between clients and contractors a structural information asymmetry exists, because the contractor is better informed about the technical-technical aspects and about the current project conditions than the client. At the same time however also a contrarious decision asymmetry exists, with that the client can accroach formal powers of decision making and has the larger threat potential available in the conflict situation. This is strengthened by the legal form of the fixed price contract. The manuals of the project management are here used, but application is limited to technical-operational aspects of the implementation of the project, in which the recommendations achieved a sufficient concretisation degree. However the recommendations are only insufficiently applied regarding politico strategic problems, whose application either require a complex strategic decision-making process or is contentious between the Contracting Parties. Extremely critical is above all designated the handling of the scope definition within the project course, which is changed by the client frequently in the course of the project. Here the requirement of the unalterability of the performance specification collides with the vital interests of the client, and it shows up on the basis the case study that the trusting co-operation of both Contracting Parties is necessary, in order to arrive at a satisfying solution. If this does not succeed, then the project can be continued only with the help of strong ideologies such as the New Public Management in a kind of crisis management, it requires however then the strategic re-orientation e.g. in the form of personnel change or reorganizations. In the case study project it turned out that, an analysis of the specific participant strategies (Policy analysis) can help the participants to understand the output constellations better and to adjust the own strategy on its penetration ability or consent. Here the manuals hold no satisfying procedural instructions ready to the project management however.
This dissertation is guided by an overall interest in the manifold links between politics and EU economic policy in action. The EU is sometimes described as a 'policy-making state' or 'system' based on the insight that in terms of policy generation, the EU's capacities have come to resemble those of traditional states. Policy-making in the EU, as in other political systems, can be characterized as a multi-stage process, although the process is arguably more complex than in nation-states because it takes place in a more dynamic institutional setting and involves a variety of actors and multiple levels. The contributions assembled here move through the policy process and the various levels to provide a broad picture about the making, application and reception of EU economic policies. While each of the chapters engages with a different stage of this process, this is not based on the assumption that these stages are always clearly separated in political practice or that they follow a universal sequence. Rather, the underlying assumption is that each of these stages of the policy cycle follows a different logic and thus requires a different theoretical toolbox to be analyzed and understood.
Klimaanpassungspolitik wird in Deutschland auf verschiedenen politischen Ebenen betrieben. Dieses Papier untersucht auf Grundlage der ökonomischen Theorie der Anpassungspolitik Defizite dieser Politikansätze. Diese liegen in erster Linie in den folgenden Bereichen: unklare Abgrenzung von staatlicher und privater Verantwortung, Nichtberücksichtigung von Verteilungsgerechtigkeit und Versorgungssicherheit, unklare institutionelle Zuständigkeiten und mangelnde Sicherstellung der Rationalität politischer Entscheidungen. Welche dieser Defizite mit Hilfe von Entscheidungsunterstützungssystemen beseitigt werden können, hängt von prozeduralen, ergebnisbezogenen und praktischen Anforderungen an Klimaanpassungsentscheidungen ab, die in diesem Papier ebenfalls formuliert werden. Die Vorteilhaftigkeit von Entscheidungsunterstützungssystemen für politische Entscheider wird anhand eines einfachen Modells aus der Neuen Politischen Ökonomie dargestellt. ; Climate change adaptation in Germany is performed on several policy levels. Based on the economic theory of adaptation policy, this paper analyses shortcomings of the German adaptation policy approach. These were found in the following areas: ambiguous distinction between justified government interventions and private autonomous adaptation responsibilities, non-consideration of both distributional fairness and security of supply, ambiguous institutional responsibilities and lacking assurance of the rationality of political decisions. Which of these shortcomings can be handled by the use of decision support systems depends on procedural, outcome-related and practical requirements. These adaptation decision requirements are also put forth in this paper. The advantageous use of decision methods for political decision makers is shown in a simple public choice model.
Patientenverfügungen sind ein wichtiges Instrument zur Umsetzung der Patientenautonomie am Lebensende. Nur etwa 10 Prozent der Bevölkerung in Deutschland haben bisher eine Patientenverfügung erstellt. Gründe hierfür scheinen eine allgemeine Unsicherheit betreffend Patientenverfügungen und die Schwierigkeit der zu treffenden Entscheidungen zu sein. Nach langer politischer Auseinandersetzung trat im September 2009 in Deutschland erstmals eine gesetzliche Regelung zur Patientenverfügung in Kraft. Jedoch besteht weiterhin ein hoher Aufklärungs- und Informationsbedarf zu Patien-tenverfügungen in der Bevölkerung. Möglicherweise stellt das Modell der Partizipativen Entscheidungsfindung eine geeignete Unterstützung bei der Beratung zu Erstellung einer Patientenverfügung dar. In der vorliegenden Arbeit sollte mittels eines systematischen Reviews untersucht werden, ob sich in der Literatur Hinweise dafür finden, dass die Partizipative Entscheidungs-findung (PEF) einen geeigneten Ansatz zur Unterstützung der Entschei-dungsfindung im Rahmen der Erstellung einer Patientenverfügung darstellen könnte. In der durchgeführten Literaturrecherche ließen sich viele Publikationen identifizieren, die sich mit Patientenbeteiligung und hierbei auch den ver-schiedenen Kernelementen der PEF bei der Erstellung von Patientenverfü-gungen beschäftigen, auch wenn der Begriff PEF nicht explizit verwendet wird. So wurden Maßnahmen durchgeführt, die den Aufbau einer partner-schaftlichen Beziehung zwischen Arzt und Patient, die Darstellung der verschiedenen Entscheidungsoptionen und die Exploration der Patientensicht fördern sollten. Die Studien zeigten, dass die reine Informationsvermittlung nicht genügt. Vielmehr scheint die Kombination von Information und Beratung durch qualifizierte Ansprechpartner erforderlich zu sein. Wiederholt wird in der Literatur die Beratung durch den Hausarzt bei der Erstellung der Patientenver-fügung empfohlen. Das Modell der PEF stellt einen erfolgversprechenden Ansatz zur Optimierung der Beratung zur Patientenverfügung dar. Dieser Ansatz sollte in weiteren Studien, insbesondere in der hausärztlichen Praxis, untersucht werden. ; Background: An Advance Directive (AD) is an instrument of patient-autonomy. Advanced planning of medical treatment in a situation of being incapable of deciding should be done in a participated decision making process, generally known as Shared Decision Making (SDM). This is to ensure the highest possible authenticity of ADs and compliance of patients in issuing an AD. In 1991, the PSDA was passed in the USA which led to numerous publications concerning AD and the effects of this new law. Since Germany is just about to implement such a law, it is time to summarise the results/findings of the past two decades. This systematic review tries to provide an overview of the state of current research on the implementation of SDM in the context of issuing ADs. Review questions: In this review we want to contribute to the ongoing debate on AD on promoting SDM in the process of issuing ADs and on the conclusions that can be drawn 17 years after the PSDA. Methods: On November17th 2008, we conducted a search of the electronic databases Medline and Embase to identify articles published up to that date which/that include the terms "shared decision making" and "advance directive". This search yielded 437 citations in Medline and 19 in Embase, amounting to a total of 456. Of these, 290 were rejected because they were neither published in English nor German (n=4) or they contained only the keywords, but not in the context of Advance Directives (n=286). The remaining (n=166) were purchased in full text and evaluated by two assessors in consent rating, and divided in/grouped into 6 categories (RCT, controlled trial, observation survey, qualitative survey, comment/editorial, review). In addition, we checked the references within these articles and included the 20 most often cited sources. Results: Within the selected literature/this corpus the following issues/topics were highlighted frequently: barriers and facilitators of ADs, preferences of patients, actual situation, specific ADs like psychiatric and research ADs. Few papers dealt specifically with shared decision making in the context of issuing ADs but some included essential components of SDM and physician-patient relationship in advance care planning. The (vast) majority of publications were published in the USA between 1991 and 1995 – after the PSDA – reaching a peak in 1992. Despite the temporarily high number of publications there are only few quantitative surveys. Conclusion: More research, particularly (in the form of) controlled trials, is necessary to ascertain the role of SDM in facilitating and composing ADs. The general statement of the assessed literature supports the assumption that SDM is crucial for increasing the number of ADs and the acceptance of ACP in the public. Furthermore, it is necessary to consider the varying cultural backgrounds of the patients (in order) to achieve a high degree of acceptance and authenticity of ADs.
In der Politik sind wie im Geschäftsleben oft riskante beziehungsweise gefährliche Entscheidungen zu treffen, bei denen die Ergebnisse nur schwer oder gar nicht prognostizierbar sind. Da Menschen in ihren Einstellungen gegenüber Risiken und Gefahren variieren, ist es vernünftig, dass in Lebensbereichen, die unterschiedlich riskante Entscheidungen erfordern, Menschen mit unterschiedlichen Risikoeinstellungen tätig sind. So wurde schon immer beobachtet, dass Unternehmer ein höheres Maß an Risikofreude zeigen als abhängig Beschäftigte. Für diesen Bericht wurde Ende des Jahres 2011 mit Hilfe einer statistischen Erhebung die Risikoeinstellung von Mitgliedern des Deutschen Bundestags (MdB) ermittelt. Demnach sind diese weit überdurchschnittlich risikofreudig; sie sind sogar deutlich risikofreudiger als Selbständige. Man kann dies kritisch sehen: Politiker sind bereit, höhere Risiken einzugehen, als es die Bevölkerung im Durchschnitt tun würde. Insofern sind Politiker keine repräsentativen Vertreter des Volkes. Wir interpretieren diesen Befund jedoch positiv, als eine gesellschaftlich sinnvolle Arbeitsteilung zwischen Bürgern, Wählern und Politikern im Rahmen einer repräsentativen Demokratie, die Risikofreude und Macht institutionell begrenzt. ; The article analyzes the question of whether career politicians differ systematically from the general population in terms of their attitudes toward risk. Das Papier geht der Frage nach, ob sich Berufspolitikerinnen und Berufspolitiker in ihren Risikoeinstellungen systematisch von der allgemeinen Bevölkerung unterscheiden. In einer schriftlichen Befragung von Mitgliedern des 17. Deutschen Bundestags wurde Ende 2011 die Risikoeinstellungen ermittelt und die erhobenen Daten in Relation zu Befragten des Sozio-oekonomischen Panel (SOEP) aus dem Erhebungsjahr 2009 gesetzt (2004 bis 2012). Abgeordnete des Deutschen Bundestags weisen für die allgemeine Risikobereitschaft im Vergleich mit den Ergebnissen des SOEP eine hochsignifikante und deutlich höhere Risikobereitschaft auf. Personalabteilungen gelten traditionell als wenig datenaffin, geraten aber immer stärker unter Druck, die Wirksamkeit betrieblicher Personalmanagementaktivitäten und Mitarbeiterinvestitionen auszuweisen. Vor diesem Hintergrund wächst das Interesse an "Workforce-" bzw. "People Analytics", die innerhalb der Managementliteratur sowie von Beratungs- und Softwareeunternehmen als revolutionäres Tool und Wegbereiter eines datengetriebenen und evidenzbasierten Managements propagiert werden. Über eine algorithmenbasierte Analyse der Zusammenhänge zwischen dem Arbeitsverhalten und den Kompetenzen von Mitarbeitenden, der Personalmanagement- und Führungspraxis sowie unternehmensbezogenen Performancegrössen sollen die Qualität der Managemententscheide und die Ausschöpfung von Mitarbeiterpotenzialen massgeblich erhöht werden. Unser Beitrag analysiert auf Basis von zwei explorativen Unternehmensfallstudien die organisationale Einbettung von People Analytics aus einer kritischen Perspektive. ZUSAMMENFASSUNG Hintergrund und Ziele: Medizinische Innovationen sind unerlässlich für die Erhaltung und Verbesserung des Gesundheitssystems und der Gesundheitsversorgung. Ein hohes medizinisches Versorgungsniveau verringert die Mortalität der Gesellschaft und verbessert die Lebensqualität von Patienten. Man kann davon ausgehen, dass dies zwangsläufig zu immer höheren Gesundheitskosten führt, während die Bereitstellung finanzieller und personeller Ressourcen für das Gesundheitswesen aufgrund von Sparzwängen immer stärker eingeschränkt wird. Weltweit werden bereits Maßnahmen entwickelt und angewendet, um mit einer wissenschaftlich belegten, wirtschaftlichen Verbesserung der Gesundheitsversorgung diesem Paradoxon entgegen zu wirken (vgl. [10], [147], [88]). Um Fehlinvestitionen im Gesundheitswesen zu vermeiden, sollte eine Bewertung medizinischer Innovationen in den frühen Phasen der ressourcenintensiven Entwicklung stattfinden. Dadurch können Entwicklungsprozesse noch vor großen zeitlichen, finanziellen und personellen Investitionen angepasst und optimiert werden. Mit Hilfe von Simulationsverfahren können solche Analysen frühzeitig durchgeführt werden. Dadurch lassen sich sowohl positive als auch negative Auswirkungen innovativer Ideen auf das Gesundheitssystem und die Gesundheitsversorgung darstellen. Simulationen benötigen neben quantifizierbaren und belegbaren Daten eine valide und zweckmäßige Informationsgrundlage, um zuverlässige und realistische Ergebnisse liefern zu können (vgl. [114], [143]). Das Gesundheitswesen kann aufgrund seiner vielen unterschiedlichen Beteiligten aus Politik, Wirtschaft und Gesellschaft als sehr komplex und dynamisch beschrieben werden. Dieses Papier untersucht Einflüsse von Deliberation auf Entscheidungen im Kontext repräsentativer Politik. Es testet Hypothesen der deliberativen Demokratietheorie, dass eine hohe Diskursqualität Konsense wahrscheinlicher macht (formale Dimension einer Entscheidung) und distributiv gerechtere Resultate fördert (substanzielle Dimension einer Entscheidung). Die mehrheitlich quantitativen Analysen stützen sich auf Wortprotokolle mehrerer Debatten des deutschen Vermittlungsausschusses. Dafür werden Indikatoren entwickelt, die verschiedene Dimensionen diskursiver Politik operationalisieren und zu einem Diskursqualitätsindex (DQI) kombiniert werden können. Die empirischen Resultate zeigen, dass die generelle Diskursqualität einen eigenständigen positiven Effekt auf die formale Dimension einer Entscheidung hat. Hingegen ist die Diskursqualität praktisch ohne Einfluss auf die substanzielle Dimension einer Entscheidung, die fast ausschließlich von den Mehrheitsverhältnissen abhängt. ; This paper explores effects of deliberation on decisions in a context of representative politics. It tests claims of deliberative democratic theory that a high quality of discourse leads to more consensual decisions (formal outputs) and to policies that approximate normative ideals of distributive justice (substantive outputs). The mostly quantitative empirical analyses are based on recorded debates of the German Conference Committee (Vermittlungsausschuss). To this end, indicators which operationalize diverse dimensions of discursive politics are developed and transformed into a discourse quality index (DQI). The results imply that discourse quality is a strong determinant of the formal outputs but is quite ineffective to counteract voting power in respect to the substantive outputs.
