In the context of the US health care reform debate, the declining status of primary care medicine is examined, asserting that primary care has failed its market test. Discussion begins with a look at the negative aspects of the contemporary primary physician's profession. Why there are not more multispeciality clinics, eg, the Mayo Clinic, or comprehensive care systems like Kaiser Permanente to support primary care is considered.
