Telemedicine has been practiced in France since the 1980s. In 2009, the HPST law proposes a legal definition: Telemedicine is "a form of medical practice remotely using ICT". From this date, the practice has been institutionalized, a national development strategy has been implemented and a sectoral public policy is applied. The objective is to remove regulatory and organizational barriers to the practice of telemedicine. Telemedicine is also becoming a tool for the renewal of public action. However, public policies are unsuccessful. Telemedicine is not developing as the public authorities have desired. The general hypothesis of our research is that instead of removing barriers to the development of telemedicine, public policies dedicated to its practice are actually impeding it. That is why we analyze power relations among stakeholders of regulation. To do this we adopt an institutionalist approach. We question the relevance of the governance model used for for deployment of telemedicine. We highlight that the solutions provided by the institutions in charge of the development of telemedicine can be improductive. ; La télémédecine existe et est pratiquée en France depuis les années 1980. Toutefois, malgré l'intérêt qui lui est porté, la pratique ne s'intensifie pas et les stratégies successives mises en œuvre par les institutions en charge de son déploiement peuvent être qualifiées d'échecs. La télémédecine ne se développe pas, ou plutôt, ne se développe pas comme le souhaiterait la puissance publique. L'hypothèse générale de nos recherches est qu'au lieu de lever les freins au développement de la pratique, la politique publique dédiée à la pratique est à l'origine même du blocage. À partir d'une démarche institutionnaliste nous interprétons les rapports de force entre les échelles de régulation de la télémédecine, ce qui nous amène à questionner la pertinence du modèle de gouvernance de son déploiement. Nous mettons alors en évidence que les solutions apportées par les institutions en charge du développement de la télémédecine peuvent être contre-productives.

À la mi-2006 on voit se dessiner en Espagne, sous l'impulsion du Gouvernement national, une politique industrielle de soutien aux groupements d'entreprises. Son objectif est de promouvoir une dynamique d'innovation à caractère territorial et sectoriel. Cet article présente une description et une analyse du programme visant à stimuler la création de ces groupements d'entreprises. The objective was to promote an innovative dynamics at the territorial and sectorial scales. On the basis of an analysis crossing different types of infrastructures (drinking water and sewerage networks, hydraulic structures, road structures (bridges, tunnels) and minor road network), we identify the factors explaining the observed organizational configurations and their evolution (public policies, specificities of territories and actors involved). Organizational efficiency is the result of a myriad of micro decisions taken as close as possible from the place and time at which contextual interference emerges. The Toyota Production System, on the management level, and the boom of the Apple Corporation, on the level of the intuitive tools for personal productivity, demonstrate this evolution where thought and action are no more segregated. Proof is the path taken by the main actors of the digital revolution : the founders of Linux, Yahoo!, Wikipedia, Amazon.com, Google or Facebook. It did not begin from a classroom theory, but by a trial and error approach that never disconnected the mind from the act.This virtuous pragmatism is also found in Bavaria, Flanders, Scotland, Euskadi, Catalonia, Lombardy, Veneto . Indeed, these regions have an autonomy - including and particularly at the cultural level - similar to that of the German Länder or the Swiss cantons. Coordination between stakeholders is at the heart of territory management. This is especially important in periurban territories, where balancing a sustainable agriculture and territorial development calls for the (re)definition of plural identity stakeholders coordination, i.e. the territorial governance, which is at the heart of political and scientific discussions. With an abductive approach, this thesis offers to explore how these stakeholders can learn to cooperate and coordinate themselves through the analysis of two case studies in France and Morocco. Using theories on organizational learning and instrumentation, I build a framework to identify governance learning in situations of collective action, but also in situations produced by the contextualisation of instruments for public action which the territory's stakeholders are confronted with. In particular, I identify learning allowing the strengthening of collective action (interest-building, teamworking), the negociation of common goals (instrument-seizing, first and second order learning), or the definition of a shared vision (sensemaking, sensegiving). Theseresults allow me to build a definition of governance learning and a conceptual framework to study it, in addition to new hypotheses on the links between learning and the contextualisation of instruments. A provisional theory is then created, connecting enrolment, "exploitation" learning and "exploration" learning, internal contextualisation and external contextualisation. ; La question de la coordination entre acteurs est au cœur de la gestion des territoires. J'identifie notamment des apprentissages permettant de renforcer l'action collective (intéressement, travailler ensemble), de négocier des objectifs communs (instrument-seizing, apprentissages de 1ère ou 2nde boucle) ou de produire une vision partagée (sensemaking, sensegiving). Les résultats obtenus me permettent de proposer une définition des apprentissages de gouvernance territoriale et un cadre conceptuel pour leur analyse, ainsi que des hypothèses sur les liens entre la contextualisation des instruments et les apprentissages. Une théorie intermédiaire est ainsi proposée, reliant l'enrôlement, les apprentissages de type « exploitation » et de type « exploration », le degré de contextualisation interne, et le degré de contextualisation externe reconnue.

In Tunisia the water resource is scarce. The study of the evolution of water management policies shows considerable institutional changes: from a central administration (offices) to associations of water users (GDA).All these institutional changes were not a request of the beneficiaries of irrigated activity. They are caused by a political system subject to the requirements of internal and external environment.Locally these institutional changes have important implications for the governance and performance of irrigation water management in irrigated areas.This thesis aims to understand the relationship between institutional transformation and local governance, on the one hand and to explain the relationship between governance and irrigation water performance, on the other hand,For methodology, it was conducted in three stages. In a first step, we use the model of David Easton to study the interactions between institutional changes and political system. In the second step, we adopt the Strategic Analyses approach of Michel Crozier (1997) to analyze the current governance of irrigation water. In the third step, we employ the game theory, specially prisoner and chicken dilemmas, to assess the effects of local governance on the farms performance. The irrigated areas in the delegation of Nadhour were chosen as investigation area. The results of this study have elucidated the research questions initially formulated. In fact, we confirmed that the difficult situation of the irrigated areas mainly due to problems in the governance of irrigation water. The institutional changes imposed by a political system lead to governance problems. This governance is the cause of the weak performances in the irrigated area.This study has concluded that an institution must be conceived as a social construct, which goes through a process of institutionalization allowing its internalization and acceptance by all local actors.Keywords: Coalition, conflict, cooperation, governance, institutions, games, power, performance, political ...
