Téhéran s'est imposé depuis 1979 comme un acteur régional majeur s'appuyant sur l'islamisme et son nationalisme perse. Son anti-occidentalisme brutal et revendiqué entre pourtant en contradiction avec les aspirations sociétales d'une société civile plus ouverte au monde, obligeant les Ayatollahs à un certain pragmatisme.
L'Iran est un pays paradoxal où le régime islamique despotique en place depuis 1979 n'a pas effacé les idéaux de liberté et de république qui, associés à l'islam populaire, avaient fait tomber la monarchie. Il ne s'agit pas d'une opposition simpliste entre un peuple qui serait « démocrate » et des élites despotiques, mais de dynamiques contradictoires. L'expérience souvent dramatique de plusieurs décennies d'islam politique fait de l'Iran un laboratoire politique et social qui mérite l'attention, dans un monde musulman où la démocratie cherche sa voie .
L'opposition géographique entre le centre du plateau iranien peuplé de persans chiites et les périphéries peuplées de minorités ethniques souvent sunnites a longtemps formé, avec justesse, le modèle de base de la géographie politique de l'Iran. Depuis un demi-siècle, la forte urbanisation et la généralisation de l'alphabétisation ont transformé le pays de façon aussi profonde et durable que la chute de la monarchie en 1979. Les élections ont montré qu'il n'y avait plus de corrélation simple entre les tendances politiques et l'ethnicité. L'opposition ville-campagne qui constituait un autre paradigme a perdu de sa pertinence puisque la population rurale est désormais très minoritaire et intégrée à la culture urbaine par l'éducation. La géographie politique actuelle de l'Iran doit donc être centrée sur l'analyse des rapports entre divers types de villes. Téhéran, ouvert à l'international, est un miroir déformant de l'Iran tandis que les grandes métropoles régionales manquent de pouvoir et que les villes moyennes et petites restent sous l'influence et le contrôle des forces traditionnelles malgré les ambitions d'ouverture économique et politique des habitants. L'ouverture des frontières offre de nouvelles perspectives de développement aux villes de la périphérie qui étaient jadis marginalisées. Cette évolution renforce le nationalisme iranien.
La rivalité entre Iraniens et Arabes, ou l'opposition sunnite/chiite, est souvent donnée comme un facteur explicatif majeur de la géopolitique du Moyen-Orient. La guerre Irak-Iran (1980-1988) fut la première confrontation directe entre l'Iran et ses voisins arabes depuis le vii e siècle. Les populations iranienne et arabe entretiennent des relations de bon voisinage dans le golfe Persique (Dubaï) ou en Irak (Najaf). Du Maghreb à Oman, les relations entre États sont diverses et fondées sur la politique. L'Iran, craignant d'être encerclé, a toujours voulu disposer d'une zone d'influence au-delà de ses frontières. Depuis la révolution de 1979 l'Iran a été marginalisé, mais les États de la péninsule Arabique se sont imposés comme les nouveaux acteurs dominants de la région, les nouveaux « gendarmes du Golfe ». La rivalité n'est pas ethno-religieuse entre l'Iran et le monde arabe, mais plus globale, entre deux puissances émergentes : la République iranienne et les monarchies pétrolières.

L'Iran souffre des sanctions occidentales mais la République islamique estime avoir gagné la bataille du nucléaire. Si l'économie iranienne est en crise, elle n'est pas au bord de l'effondrement. Les responsables iraniens pensent donc, avec un nouveau consensus national, pouvoir gagner du temps, sans grande concession aux alliés occidentaux. Ces derniers cherchent sans doute aussi à gagner du temps : les négociations ont donc peu de chances de progresser rapidement.
Though Iran is suffering due to Western sanctions, the Islamic Republic feels it has won the nuclear battle. If the Iranian economy is in fact in crisis, it is not on the verge of collapse. Iranian officials think, therefore, that with a new national consensus they could gain more time with little concession to Western allies. Si l'Iran considère le Proche-Orient comme une dimension très importante de sa politique extérieure, les contradictions au sein de la classe politique et de la société civile peuvent la relativiser au profit d'autres priorités liées à la sécurité du pays et plus largement à sa place dans le monde. En ce sens, le voyage au Liban de Mahmoud Ahmanidejad en octobre 2010 qui fut un incontestable succès populaire est peut-être un chant du cygne. En tout cas, l'issue des relations entre l'Iran et cette région se trouve dans la compétition désormais ouverte pour l'élection présidentielle iranienne de mai 2013.
