PrécisIn 1989 the Canadian Broadcasting Corporation aired a five-part radio series, "It's a Matter of Survival." Anita Gordon was the originator and executive producer of the series. David Suzuki is Professor of Zoology at the University of British Columbia and the host of "The Nature of Things," a science television program. This book grew out of the radio series and is described on the jacket cover by Edward O. Wilson as "the best piece of extended environmental journalism I've seen to date." Cited in the end notes are publications in the popular press (e.g., The New York Times) and CBC interviews with a range of environmental commentators such as Lester Brown and Paul Erlich. The book indicts the environmental irresponsibility of human beings as a species and is intended as a response to the radio listeners who wrote to the CBC following the 1989 broadcasts asking what they could do to forestall environmental catastrophe.Gordon and Suzuki begin with a hypothetical glimpse into the "nightmare world of 2040." Subsequent chapters question how we reached the stage of environmental crisis, explore myths that have blinded us to the crisis, predict future growth trends, describe the ethic of domination over nature, and review the devastation wrought by prevailing definitions of "progress." The authors end with an alternative (and positive) look at the year 2040 that can be possible if the resolutions they discuss are sought. They conclude that humans as a species have "lost the ability to hear the warning cries of nature" (p. 234), but they hold the hope that humans can emerge from the crisis with a "new collective image of ourselves as a species integrated into the natural world" (p. 238).

Research concerning sexual and gender minority (SGM) health has flourished in recent years in conjunction with a period of intense social, political, and legal discourse about SGM persons. While this attention has increased understanding and recognition of SGM experiences, recent advances have often been met with resistance and backlash rooted in social stigma and long histories of discrimination. This volume of Advances in Medical Sociology showcases rich theoretical and empirical contributions on SGM health and wellbeing. The chapters address a variety of topics, drawing from classic and contemporary sociological frameworks and constructs, and reflecting intersecting interdisciplinary approaches to SGM health. Research presented in this volume provides an in-depth focus on sexual and/or gender minority populations, as well as the diverse sub-populations within them; theoretical and empirical explanations for SGM health disparities and resilience; aging and life course perspectives on the health experiences of SGM persons; health in the context of critical relationships in the lives of SGM persons; and the experiences of seeking general and specialized health care among SGM. Background: In Colombia, recent legislation regarding end-of-life decisions includes palliative sedation, advance directives and euthanasia. We analysed which aspects influence health professionals´ decisions regarding end-of-life medical decisions and care for cancer patients. Methods: Qualitative descriptive–exploratory study based on phenomenology using semi-structured interviews. We interviewed 28 oncologists, palliative care specialists, general practitioners and nurses from three major Colombian institutions, all involved in end-of-life care of cancer patients: Hospital Universitario San Ignacio and Instituto Nacional de Cancerología in Bogotá and Hospital Universitario San José in Popayan. Results: When making decisions regarding end-of-life care, professionals consider: 1. Patient's clinical condition, cultural and social context, in particular treating indigenous patients requires special skills. 2. Professional skills and expertise: training in palliative care and experience in discussing end-of-life options and fear of legal consequences. Physicians indicate that many patients deny their imminent death which hampers shared decision-making and conversations. They mention frequent ambiguity regarding who initiates conversations regarding end-of-life decisions with patients and who finally takes decisions. Patients rarely initiate such conversations and the professionals normally do not ask patients directly for their preferences. Fear of confrontation with family members and lawsuits leads healthcare workers to carry out interventions such as initiating artificial feeding techniques and cardiopulmonary resuscitation, even in the absence of expected benefits. The opinions regarding the acceptability of palliative sedation, euthanasia and use of medications to accelerate death without the patients´ explicit request vary greatly. 3. This article discusses the right to assisted suicide, a right not derived from anything explicit or implied in any textual provision of the Constitution. It is a right derived from the two underlying political philosophies, which form the basis of the entire U.S. Constitutional enterprise: John Locke's Social Contract and Civic Republicanism. In Part I, this article discusses Glucksberg's fundamental rights analysis. So much has been written about this case that this article will limit comments to briefly adding thoughts as to why, given the combination of the Court's motivation, both institutional and pragmatic, in approaching this case, and its methodology for analyzing fundamental rights, a nine to zero decision was fairly predictable, even in this difficult, emotionally compelling case. Part II gives more consideration to the equal protection claim. Those entering the debate in Vacco v. Quill regarding whether there is an equivalence between terminating life support (which the law permits) and assisted suicide (which the law forbids), have done so in a rather conclusory fashion, whether contending for" or against" equivalence. Many entering the debate also have failed to explicitly question whether, even if what we condone in end-of-life care cannot be morally distinguished from assisted suicide, there may be meaningful distinctions in policy of which the law may properly take into account. A detailed equal protection analysis of legally accepted medical practices in end-of-life care-pulling the plug, refusing treatment, the principle of double effect, and terminal sedation is, therefore, appropriate. In Part III, this article discusses the Social Contract theory and Civil Republicanism. I explain why the combination of these political philosophies underlying the Constitution provide people like my father, both past the age of Social Security retirement and incapable of ever again participating in the political process, with a right to end their lives.

The French Military Health Service established an action plan to detect early post-traumatic stress disorder (PTSD) in the French Armed Forces. Our purpose is to set up a protocol to delegate medical procedure to nurses according to High Authority of Health criteria. This study was carried out during our internship in Bricy Airbase from November 2015 to October 2016. The nurses are first trained in clinical interview's specificities to detect PTSD, and besides, counselled by a mental health specialist. Each soldier must bring its reduced medical file back to the medical centre after the return from an overseas operation. Then, a mandatory meeting with a nurse is set three months later. The soldier is asked to fill the post-traumatic stress disorder checklist scale (PCLS). The nurse is receptive to each warning signs, each symptoms and suffering expressed by him or her. If a PTSD is detected, a specialized care is proposed in connection with the Medical Officer of the centre and specified in the soldier's medical file. Soldiers and nurses' satisfaction survey (n=159) shows the protocol relevance in terms of feasibility and global acceptability. Moreover, we noticed that the screening must be obligatory to be efficient. This cannot be carried out without the participation of the chain of command. The generalisation to the other military medical centres and its adaptation in the Public Health field should be proposed. ; Dans le cadre du plan d'action relatif au repérage précoce des troubles psychiques post-traumatiques (TPPT) dans les forces armées, nous avons proposé de formaliser un protocole de délégation d'actes à l'infirmier en référence aux critères de la Haute Autorité de santé. Une étude a été menée lors de notre stage chez le praticien à l'antenne médicale de la base aérienne d'Orléans-Bricy de novembre 2015 à octobre 2016. Au préalable, l'infirmier est formé aux spécificités de l'entretien clinique de dépistage des TPPT. Il bénéficie également d'une supervision technique avec un spécialiste de la santé mentale. Dès son retour d'opération extérieure, tout militaire doit rendre son dossier médical réduit à l'antenne médicale. Un rendez-vous avec l'infirmier est systématiquement fixé trois mois plus tard. Le militaire remplit tout d'abord l'auto-questionnaire PCLS (Post-traumatic Check List Scale). L'infirmier est ensuite attentif aux signes cliniques observables, aux symptômes décrits voire aux difficultés exprimées. Si des manifestations évocatrices d'un TPPT ou d'une autre forme de souffrance sont repérées, une prise en charge spécialisée est proposée. Cet acte infirmier de prévention est inscrit dans le dossier médical de l'intéressé. Les questionnaires de satisfaction du personnel reçu (n=159) et des infirmiers ont souligné la pertinence de ce protocole tant sur le plan de sa faisabilité que de son acceptabilité globale. Toutefois, le dépistage doit prendre un caractère obligatoire pour être effectif et ne peut être réalisé sans l'adhésion du commandement. Fifty years ago, health outcomes in the countries of Eastern Europe and Central Asia were not far behind those in Western Europe and well ahead of most other regions of the world. But progress since then has been slow. While life expectancy in the ECA region today is close to the global average, the gap with its western neighbors has doubled, and other middle-income regions have all surpassed ECA. Some countries in the region are doing better, but full convergence with the world's most advanced health systems is still a long way off. At the same time, survey evidence suggests that the health sector is the top priority for additional investment among populations across the region. The experience of high-income countries also suggests that popular demand for strong and accessible health systems will only grow over time. Yet these aspirations must be reconciled with current fiscal realities. In brief, health sector issues are a challenge here to stay for policy-makers across the ECA region. This report draws on new evidence to explore the development challenge facing health sectors in ECA, and highlights three key agendas to help policy-makers seeking to achieve more rapid convergence with the world's best performing health systems. The first is the health agenda, where the task is to strengthen public health and primary care interventions to help launch the "cardiovascular revolution" that has taken place in the West in recent decades. The second is the financing agenda, in which growing demand for medical care must be satisfied without imposing undue burden on households or government budgets. The third agenda relates to broader institutional arrangements. Here there are some key reform ingredients common to most advanced health systems that are still missing in many ECA countries. Das Projekt ´Lebensqualität und Wohlbefinden hochaltriger Menschen in NRW (NRW80+)´, das vom Ministerium für Innovation, Wissenschaft und Forschung Nordrhein-Westfalen gefördert und dem Forschungsverbund CERES an der Universität zu Köln durchgeführt wird, soll repräsentative Aussagen über die Lebensbedingungen hochaltriger Menschen in Nordrhein-Westfalen ermöglichen. Ziel ist es, umfassende Informationen darüber zu erhalten, in welchem Umfeld hochaltrige Personen leben bzw. leben möchten, was ihre gesellschaftliche Rolle ist und wie zufrieden sie mit ihrer Lebenssituation sind.

Im Rahmen des Projekts wurde in zwei Wellen repräsentativ erfasst, unter welchen Umständen hochaltrige Menschen leben. In der zweiten Welle wurden zum einen die zu einer Panelteilnahme bereiten Teilnehmer der ersten Welle erneut befragt. Zusätzlich wurde eine Aufstockungsstichprobe gezogen, um weiterhin repräsentative Aussagen zu ermöglichen.

Der hier vorliegende Datensatz enthält die Daten aller Befragten der Welle 2 (Panel und Aufstockung).
