Цель анализ международных методических материалов, нормативно-правовых документов, регламентирующих механизмы планирования объемов и оплаты стационарной медицинской помощи. Материалы и методы. Проведен литературный обзор организации систем здравоохранения, планирования объемов и финансовых затрат на оказание медицинской помощи в условиях стационара девяти зарубежных стран (Германия, Франция, Дания, Норвегия, Австрия, Великобритания, Канада, Республика Беларусь и Казахстан). Результаты. Во всех рассмотренных странах наблюдается рост расходов на здравоохранение как в денежном выражении, так и в процентном отношении к ВВП. Основными направлениями для снижения расходов на здравоохранение являются: регулирование фармацевтического рынка, оценка эффективности медицинских технологий и снижение объемов оказываемой стационарной помощи путем оказания помощи в дневных стационарах, амбулаторно или на дому выездными бригадами. В большинстве рассмотренных стран ответственность за организацию и планирование стационарной помощи лежит на региональных властях. Во всех рассмотренных странах постоянно проводятся мероприятия по снижению количества коек в учреждениях, оптимизации их загрузки, уменьшению времени пребывания в стационаре и увеличению оборота койки с целью снизить операционные издержки и расходы на содержание инфраструктуры. Преобладающим методом оплаты стационарной помощи является оплата за законченный случай лечения, включенного в соответствующую группу заболеваний (клинико-статистическую группу (КСГ) или diagnosis related group (DRG)). Многие из рассмотренных стран при планировании медицинской помощи исходят из финансовых возможностей, основываясь на показателях предыдущих лет. Методики планирования находятся в плоскости политических решений, часто разрабатываются на региональном или муниципальном уровнях, вследствие чего субъекты федерации и регионы могут по-разному подходить к вопросу планирования и финансирования стационарной медицинской помощи. ; Objective. To examine approaches to organization and planning as well as expenditures of inpatient care in countries outside of Russia. Methods. A narrative review of published articles and legal documents was conducted. The review focused on health systems organization, planning volumes and costs associated with inpatient care in nine foreign countries (Germany, France, Denmark, Norway, Austria, Great Britain, Canada, Belarus and Kazakhstan) was conducted. Results. In all countries, there is a growth in health care costs both in monetary terms and as a percentage of GDP. The primary policy responses to expenditure growth have focused on outpatient pharmaceuticals, evaluation of medical technologies and reducing volumes of provided hospital care by providing assistance in day hospitals, outpatient or with home-visiting teams. In most countries, responsibility for the organization and planning of inpatient care rests with regional authorities. In all of countries, quality measures are regularly taken as a means to optimize or reduce numbers of beds, reduce hospital length of stay or increase turnover as a means to reduce transaction costs and costs of infrastructure maintenance. The main method of payment for acute care services is based on per case of treatment for a relevant group of diseases (diagnosis related group (DRG)). Planning of medical care in many of these countries is based on financial capacity and performance of previous years. Approaches to planning are typically part of political decision-making, developed at regional or municipal level, leading to variable approaches in health systems characterized by a federation of health regions.
The paper considers the question how the Ost-Akademie in Lüneburg, Federal Republic of Germany, tackled the problems of West-East-relations in Germany and in Europe between 1952 and 2004. As an institute for political and cultural education on basis of the carried out research, the Academy worked with adult and young participants. It dealt with problems deriving from the special position of Germany after the defeat of National Socialist Germany and from German and European partition. In the initial years it took up the challenges connected with the expulsion of millions of Germans from the Eastern Prussian provinces. Afterwards it picked up the splitting between the Western and the Eastern zone of Germany, between FRG and GDR, and investigated the socialist development of the GDR. In the last years of the German partition it succeeded in establishing immediate encounter with everyday life in the GDR. After the end of the German partition and the end of the Cold War it engaged in bringing together unified Germany and its Eastern neighbours, Poland, Russia, the Baltic states, and the new emerging republics of the CIS. In this way the Academy could make a contribution to the development of understanding between citizens in Germany and in Central and Eastern Europe. Exactly that was the intention of the victors over National Socialism and of the new founded Federal Republic of Germany. ; Статья посвящена вопросу деятельности Академии «Восток» в Люнебурге (Западная Германия), которая в период с 1952 по 2004 гг. занималась актуальными проблемами отношений между Западом и Востоком Германии и Европы. Как институт политического и культурного образования и на основе собственных исследований Академия занималась с взрослыми и молодыми участниками проблемами, которые вытекали из особого положения Германии в Европе после крушения национал-социализма, раскола Германии и Европы. В первые годы своей работы она изучала проблемы, связанные с изгнанием миллионов немцев из провинций Восточной Пруссии, а затем специализацией Академии становится изучение ГДР и восточных соседей Германии в период европейской интеграции. После окончания холодной войны и воссоединения Германии Академия налаживала и развивала новые связи между Западом и Востоком: с Польшей, странами Балтии, Россией и другими странами СНГ. Таким образом, Академия внесла свой вклад во взаимопонимание между гражданами Германии и стран Центральной и Восточной Европы, что отвечало намерением победителей германского нацизма и лежало в основании новой Федеративной республики Германии.
PUBLISHED ; Using the case of Northern Ireland, this article examines how post-conflict transition impacts sports organisations which have traditionally underpinned societal division and what factors facilitate or restrain such organisations from fulfilling peacebuilding functions. The article identifies three peacebuilding functions of sport: in-group socialisation, promoting social cohesion and symbolically representing new identities. Then, after outlining the sporting and political context in Northern Ireland, the article explores changes within the sporting environment stemming from the peace process as well as persistent socio-political barriers to sport?s capacity to encourage reconciliation and integration, especially symbolic contestation and territorial segregation. The analysis draws on a public attitudes survey (n?=?1210) conducted by the authors as part of a major government-funded study. The findings demonstrate the potential of the three peacebuilding functions of sport, yet also, crucially, the constraints imposed by the distinct and divided political context. Avenues for further and comparative research are identified.

Deep Water Web is a poetic meditation around contemporary and historical geopolitical contexts, underscored by London and Sydney's situation around large bodies of tidal water in the forms of the River Thames and Sydney Harbour. These bodies of water bear material evidence of the local impact of global warming, such as rising tide levels caused by melting ice caps, leading to flooding, and increasingly extreme climate fluctuation. Both cities are also centres of neoliberal capitalism, inscribing the effects of privatisation, fiscal austerity and deregulation of markets across the planet. Deep Water Web weaves rhetorical explication of postcolonial relationship, elaborating the precarious material forms of climate change, and post-labour late capitalist neoliberal urban developments of waterfronts of former Docklands, considered within the geological and rhetorical ecology of the Anthropocene.
