Background: It is well-established that what is good for the heart is good for the brain. Vascular factors such as hypertension, diabetes, and high cholesterol, and genetic factors such as the apolipoprotein E4 allele increase the risk of developing both cardiovascular disease and dementia. However, the mechanisms underlying the heart–brain association remain unclear. Recent evidence suggests that impairments in vascular phenotypes and cerebrovascular reactivity (CVR) may play an important role in cognitive decline. The Heart and Brain Study combines state-of-the-art vascular ultrasound, cerebrovascular magnetic resonance imaging (MRI) and cognitive testing in participants of the long-running Whitehall II Imaging cohort to examine these processes together. This paper describes the study protocol, data pre-processing and overarching objectives. Methods and Design: The 775 participants of the Whitehall II Imaging cohort, aged 65 years or older in 2019, have received clinical and vascular risk assessments at 5-year-intervals since 1985, as well as a 3T brain MRI scan and neuropsychological tests between 2012 and 2016 (Whitehall II Wave MRI-1). Approximately 25% of this cohort are selected for the Heart and Brain Study, which involves a single testing session at the University of Oxford (Wave MRI-2). Between 2019 and 2023, participants will undergo ultrasound scans of the ascending aorta and common carotid arteries, measures of central and peripheral blood pressure, and 3T MRI scans to measure CVR in response to 5% carbon dioxide in air, vessel-selective cerebral blood flow (CBF), and cerebrovascular lesions. The structural and diffusion MRI scans and neuropsychological battery conducted at Wave MRI-1 will also be repeated. Using this extensive life-course data, the Heart and Brain Study will examine how 30-year trajectories of vascular risk throughout midlife (40–70 years) affect vascular phenotypes, cerebrovascular health, longitudinal brain atrophy and cognitive decline at older ages. Discussion: The study will generate one of the most comprehensive datasets to examine the longitudinal determinants of the heart–brain association. It will evaluate novel physiological processes in order to describe the optimal window for managing vascular risk in order to delay cognitive decline. Ultimately, the Heart and Brain Study will inform strategies to identify at-risk individuals for targeted interventions to prevent or delay dementia. ; The Heart and Brain Study is funded by a UK Alzheimer's Society Research Fellowship to SS (Grant Reference: 441). The article analyzes the political and legal aspects of the functioning of the Institute of Ukrainian presidency in the system of the higher bodies of state authority. It is determined that the quality of the head of state depends on the constitutionally established powers, normative vested rights and responsibilities as well as the individuality of a person occupying the president's position. The methodological basis of the study was the use of the historical method which allowed to identify the factors that influenced on the changes of Ukrainian polity in the period of independence, comparative method to determine the level of confidence changes of the presidents of Ukraine at the beginning of their cadence, a year later and in the year of re-election, and the system method helped determine the prospects for the development of the Institute of Presidency in Ukraine, taking into account the chosen model of the parliamentary republic.It is proved that the socio-political perception of the Ukrainians, excessive individualization and paternalism in relation to the position of the President of Ukraine directly affect the distribution of authority in the country, concentrating it in the hands of a single person. As a result of the analysis, it was possible to determine the mechanisms that can help Ukraine to avoid the processes related to the processes associated with usurpation of power, accumulation of powers in one decision-making centre at the present stage of democratization: a definitive constitutional distribution of powers, reducing the political dependence of the law enforcement system from the commander-in-chief, eliminating "immunity to corruptions" of the Ukrainians, preservation the Independence of the media and creation a nationwide state development strategy.The future of the Institute of Presidency in Ukraine for the Parliamentary Republic will be a symbolic one, limited powers and low political interest of citizens, which will positively affect the ability of the Ukrainians to a conscious and rational choices of representatives, the development of ideological multiparty system, the formation of a stable and consistent collegial cabinet of ministers and the responsible prime minister who will implement the parliamentary program and will bear responsibility in front of it. ; В статье анализируются политико-правовые аспекты функционирования института президентства Украины в системе высших органов государственной власти. Определено, что качество деятельности Главы государства зависит от конституционно установленных полномочий, нормативно закрепленных прав и обязанностей, а также особенности лица, занимающего должность Президента. Методологической основой исследования стало использование исторического метода, что позволило выявить факторы, которые повлияли на изменения формы правления Украины в период независимости, сравнительного метода для определения изменения уровня доверия к Президентам Украины в начале их каденции, через год и в год переизбрания, а системный метод помог определить перспективы развития института президентства в Украине, учитывая выбранную модель парламентской республики.Доказано, что социально-политическое восприятие украинцев, чрезмерная индивидуализация и патернализм по отношению к должности Президента Украины напрямую влияют на распределение власти в стране, концентрируя ее в одних руках. В результате анализа удалось определить механизмы, которые могут помочь Украине избежать процессов, связанных с узурпацией власти, аккумулированием полномочий в одном центре принятия решений на современном этапе демократизации: четкое конституционное распределение полномочий, снижение политической зависимости правоохранительной системы от Главнокомандующего, искоренение «иммунитета к коррупции»украинцев, сохранение независимости СМИ и выработка общенациональной стратегии развития государства.Будущее института президентства в Украине при парламентарной республике будет иметь символический характер, ограниченные полномочия и низкий политический интерес граждан, положительно повлияет на способность украинцев к сознательному и рациональному выбору своих представителей, развитию идеологической многопартийности, формированию стабильного и последовательного коллегиального Кабинета Министров и ответственного Премьер-министра, реализующего программу парламента, будучи ответственным перед ним. ; У статті аналізуються політико-правові аспекти функціонування інституту президенства України в системі вищих органів державної влади. Визначено, що якість діяльності Глави держави залежить від конституційно встановлених повноважень, нормативно закріплених прав та обов'язків, а також індивідуальності особи, що займає посаду Президента. Методологічною основою дослідження стало використання історичного методу, що дозволило виявити чинники, які вплинули на зміни форми правління України в період незалежності, порівняльного методу для визначення зміни рівня довіри до Президентів України на початку їхньої каденції, через рік та в рік переобрання, а системний метод допоміг визначити перспективи розвитку інституту президентства в Україні, враховуючи обрану модель парламентарної республіки.Доведено, що соціально-політичне сприйняття українців, надмірна індивідуалізація та патерналізм по відношенню до посади Президента України прямо впливають на розподіл влади в країні, концентруючи її в одних руках. У результаті аналізу вдалося визначити механізми, які можуть Україні допомогти уникнути процесів, пов'язаних з узурпацією влади, акумулюванням повноважень в одному центрі прийняття рішень на сучасному етапі демократизації: чіткий конституційний розподіл повноважень, зниження політичної залежності правоохоронної системи від Головнокомандувача, викорінення «імунітету до корупції» українців, збереження незалежності ЗМІ й вироблення загальнонаціональної стратегії розвитку держави.Майбутнє Інституту президенства в Україні за парламентарної республіки буде мати символічний характер, обмежені повноваження та низьку політичну цікавість громадян, що позитивно вплине на здатність українців до свідомого й раціонального вибору представників, розвитку ідеологічної багатопартійності, формуванню стабільного та послідовного колегіального Кабінету Міністрів та відповідального Прем'єр-міністра, що реалізовуватиме програму Парламенту та нестиме відповідальність перед ним.

An issue, regarding famine (Holodomor) – genocide organization that was performed by the Soviet system on Kuban where the Ukrainians constituted majority, is considered in the article. The particular schemes of the deliberate policy oriented on physical annihilation of the Ukrainian population on Kuban, destruction of the national, ethnical and group (Cossacks') identity, cessation of national-cultural development, forced russification, all abovementioned are the characteristics of genocide, the main features of which were determined by a particular UN resolution in 1948. The authors provide the particular data that reveal a deliberate policy of the USSR aimed at annihilation of the Ukrainians on the Northern Caucasus (Kuban). Provided factual data is based on the wide range of archival sources, testimonies of witnesses, publications of the scientific studies. The authors' analysis permits to conclude that a deliberate policy was provided regarding the Ukrainians on Kuban that were an inherent part of the general Ukrainian national space during 1932-1933-s, namely the policy included mass physical annihilation via artificial famine, repressions, deportations as well as via different means of the Ukrainians' denationalization. The processes were on the territory of the USRR as well as in the places of high concentration of the Ukrainians beyond its borders, particularly on the Northern Caucasus. The specific implications of the work are to highlight the methods that the Soviet government used against the Ukrainians on Kuban at the abovementioned period, the authors consider the measures as genocide based on the UN resolution of 1948, moreover, the analysis of the statistical data is provided to prove the scale of physical loss of the Ukrainians on Kuban. Considering that the scientific issue of the his-tory of the Ukrainians on the Northern Caucasus (Kuban) at the period of famine (Holodomor) du¬ring 1932-1933-s only begins to be reflected, the results of the article can be a base for the further studies and more deep consideration of one of the greatest disasters in the 20th century – Famine (Holodomor)-genocide of the Ukrainian nation. ; В статье рассматривается вопрос о практике осуществления Голодомора - геноцида, которое организовала и осуществила советская система на Кубани, большинство населения которой составляли украинцы. Конкретные схемы проведения сознательной политики физического уничтожения украинского населения Кубани, ликвидации его национального сознания, этнической и сословной (казацкой) идентичности, прекращения национально-культурного развития, насильственной русификации, в полной мере укладываются в практику геноцида, основные признаки которого были определены в 1948 году отдельной резолюцией ООН. Авторы представляют конкретные данные, которые свидетельствуют о целенаправленной политике СССР, направленной на уничтожение украинцев на Северном Кавказе (Кубани). Приведенные в статье фактологические данные опираются на широкий круг архивных источников, свидетельства очевидцев и публикации в научных исследованиях. Авторский анализ позволяет сделать вывод, что в 1932-33 годах проводилась целенаправленная политика в отношении украинский Кубани, как составной части всеукраинского национального пространства, сочетающей как массовое физическое уничтожение путем искусственно организованного голода, репрессий, депортаций, так и различные мероприятия по денационализации украинского населения. И эти процессы происходили как на территории УССР, так и в местах компактного проживания украинцев за ее пределами, в частности на Северном Кавказе. Конкретными последствиями этой работы является освещение мероприятий, применявшихся советской властью по отношению к украинской Кубани в вышеупомянутый период, которые авторы расценивают как геноцид, опираясь на резолюцию ООН 1948 года, анализ статистических данных, доказывающих масштаб физических потерь украинский Кубани. Считая, что научная проблематика истории Украинского Северного Кавказа (Кубани) в период Голодомора 1932-33 годов только начинает осмысливаться, результаты этой статьи могут стать основой для дальнейших исследований и значительного более глубокого понимания одной из крупнейших катастроф ХХ века - Голодомора-геноцида украинской нации. ; У статті розглядається питання про практику здійснення Голодомору – геноциду, який організував та здійснив комуністичний тоталітарний режим на Кубані, більшість населення якої складали українці. Конкретні схеми проведення свідомої політики по фізичному знищенню українського населення Кубані, ліквідацію його національної свідомості, етнічної та станової (козацької) ідентичності, припинення національно-культурного розвитку, насильницька русифікація уповні вкладаються у практику геноциду, основні ознаки якого були визначені в 1948 році окремою резолюцією ООН. Автори подають конкретні дані, які свідчать про цілеспрямовану політику очільників комуністичного тоталітарного режиму, спрямовану на знищення українців на Північному Кавказі (Кубані). Наведені у статті фактологічні дані спираються на широке коло архівних джерел, свідчення очевидців та публікації у наукових дослідженнях. Авторський аналіз дозволяє зробити висновок, що в 1932-33 роках проводилась цілеспрямована політика по відношенню до українців Кубані, як складової частини загальноукраїнського національного простору, яка поєднувала як масове фізичне знищення, шляхом штучно організованого Голодомору, репресій, депортацій , так і різноманітні заходи з денаціоналізації українців. І ці процеси відбувалися як на території УСРР, так і в місцях компактного проживання українців за її межами, зокрема на Північному Кавказі. Конкретними наслідками цієї роботи є висвітлення заходів, які застосовував комуністичний тоталітарний режим по відношенню до українців Кубані у вищезгаданий період, які автори розцінюють як геноцид, спираючись на резолюцію ООН 1948 року, аналіз статистичних даних, які доводять масштаб фізичних втрат українців Кубані. Вважаючи, що наукова проблематика історії українців Північного Кавказу (Кубані) в період Голодомору 1932-33 років тільки починає осмислюватись, результати цієї статті вони порпонують як основу для подальших досліджень та значного більш глибокого розуміння однієї з найбільших катастроф ХХ ст. – Голодомору-геноциду української нації.

Background The UK Government's implementation in 2008 of the Improving Access to Psychological Therapies (IAPT) initiative in England has hugely increased the availability of cognitive behavioural therapy (CBT) for the treatment of depression and anxiety in primary care. Counselling for depression—a form of person-centred experiential therapy (PCET)—has since been included as an IAPT-approved therapy, but there is no evidence of its efficacy from randomised controlled trials (RCTs), as required for recommendations by the National Institute for Health and Care Excellence. Therefore, we aimed to examine whether PCET is cost effective and non-inferior to CBT in the treatment of moderate and severe depression within the IAPT service. Methods This pragmatic, randomised, non-inferiority trial was done in the Sheffield IAPT service in England and recruited participants aged 18 years or older with moderate or severe depression on the Clinical Interview Schedule-Revised. We excluded participants presenting with an organic condition, a previous diagnosis of personality disorder, bipolar disorder, or schizophrenia, drug or alcohol dependency, an elevated clinical risk of suicide, or a long-term physical condition. Eligible participants were randomly assigned (1:1), independently of the research team, and stratified by site with permuted block sizes of two, four, or six, to receive either PCET or CBT by use of a remote, web-based system that revealed therapy after patient details were entered. Those assessing outcomes were masked to treatment allocation. Participants were seen by appropriately trained PCET counsellors and CBT therapists in accordance with the IAPT service delivery model. Depression severity and symptomatology measured by the Patient Health Questionnaire-9 (PHQ-9) at 6 months post-randomisation was the primary outcome, with the PHQ-9 score at 12 months post-randomisation being a key secondary outcome. These outcomes were analysed in the modified intention-to-treat population, which comprised all randomly assigned patients with complete data, and the per-protocol population, which comprised all participants who did not switch from their randomised treatment and received between four and 20 sessions. Safety was analysed in all randomly assigned patients. The non-inferiority margin was set a priori at 2 PHQ-9 points. Patient safety was monitored throughout the course of therapy, adhering to service risk procedures for monitoring serious adverse events. This trial is registered at the ISRCTN Registry, ISRCTN06461651, and is complete. Findings From Nov 11, 2014, to Aug 3, 2018, 9898 patients were referred to step three treatments in the Sheffield IAPT service for common mental health problems, of whom 761 (7·7%) were referred to the trial. Of these, we recruited and randomly assigned 510 participants to receive either PCET (n=254) or CBT (n=256). In the PCET group, 138 (54%) participants were female and 116 (46%) were male, and 225 (89%) were White, 16 (6%) were non-White, and 13 (5%) had missing ethnicity data. In the CBT group, 155 (61%) participants were female and 101 (39%) were male, and 226 (88%) were White, 17 (7%) were non-White, and 13 (5%) had missing ethnicity data. The 6-month modified intention-to-treat analysis comprised 401 (79%) of the enrolled participants (201 in the PCET group; 200 in the CBT group) and the 12-month modified intention-to-treat analysis comprised 319 participants (167 in the PCET group; 152 in the CBT group). The 6-month per-protocol analysis comprised 298 participants (154 in the PCET group; 144 in the CBT group). At 6 months post-randomisation, PCET was non-inferior to CBT in the intention-to-treat population (mean PHQ-9 score 12·74 [SD 6·54] in the PCET group and 13·25 [6·35] in the CBT group; adjusted mean difference −0·35 [95% CI −1·53 to 0·84]) and in the per-protocol population (12·73 [SD 6·57] in the PCET group and 12·71 [6·33] in the CBT group; 0·27 [95% CI −1·08 to 1·62]). At 12 months post-randomisation, there was a significant adjusted between-group difference in mean PHQ-9 score in favour of CBT (1·73 [95% CI 0·26–3·19]), with a 95% CI exceeding the 2-point non-inferiority margin. There were two deaths, one death by suicide in the PCET group and one due to chronic obstructive pulmonary disease in the CBT group. Both were assessed by the responsible clinician to be unrelated to the trial. In terms of using emergency departments for depression-related events, four people (three in the PCET group; one in the CBT group) made more than a single use and six people (three in the PCET group; three in the CBT group) made a single use. One patient in the PCET group had inpatient treatment for a depression-related event. Interpretation This trial is the first to examine the two most frequently administered psychological therapies in the IAPT service. The finding of non-inferiority of PCET to CBT at 6 months supports the results from large, routine, non-randomised datasets from the IAPT programme. Given the high demand for psychological therapies and the need for patient choice, our findings suggest the need for continued investment in the training and delivery of PCET for improving short-term outcomes, but suggest that PCET might be inferior to CBT at 12 months.

Background: Under Ontario's Public Hospitals Act, the scope of professional practice of hospital pharmacists is approved by each hospital's medical advisory committee. Some Ontario hospitals have adopted policies or medical directives related to prescription modification, allowing pharmacists to broadly adapt, discontinue, hold, or renew prescriptions as part of their clinical scope of practice. Objectives: The primary objective of this study was to describe Ontario hospital pharmacists' perception of their readiness to independently modify prescriptions. The secondary objectives of this study were to gather opinions on the perceived benefits, drawbacks, facilitators, and barriers to prescription modification by pharmacists and to determine how various factors affect perceived readiness. Methods: A confidential web-based survey with Likert-type quantitative questions and qualitative open-ended questions was distributed to 936 hospital pharmacists in Ontario between May and July 2019. Mean scores were calculated for the following constructs affecting prescription modification: self-efficacy, support from the practice environment, and support from interprofessional relationships. Independent t tests were conducted to compare responses between subgroups of interest. The answers to open-ended questions were analyzed thematically. Results: The survey had a 29% response rate (n = 271). The mean self-efficacy score was 5.2 out of 7 (standard deviation [SD] 1.0, Cronbach α = 0.88), equivalent to "quite sure". The mean score for support from the practice environment was 3.3 out of 5 (SD 0.4, Cronbach α = 0.75), equivalent to "not a factor". The mean score for support from interprofessional relationships was 4.2 out of 5 (SD 0.1, Cronbach α = 0.80), equivalent to "weak support". Improved efficiency of care, timelier interventions to improve medication safety and efficacy, and improved interprofessional collaboration were cited as benefits of prescription modification by pharmacists. Potential for inappropriate decision-making and miscommunication were cited as concerns. Respondents in hospitals who were already performing prescription modification reported higher self-efficacy to modify prescriptions in clinical areas of both familiarity and unfamiliarity and greater support from prescribers. Conclusions: A large proportion of respondents to a survey of Ontario hospital pharmacists expressed an encouraging level of readiness to independently modify prescriptions. Responses to open-ended questions in this study provided valuable insights to inform widespread adoption of this practice change. RÉSUMÉ Contexte : En vertu de la Loi sur les hôpitaux publics de l'Ontario, le comité consultatif de chaque hôpital approuve l'élargissement de la pratique professionnelle des pharmaciens d'hôpitaux. Certains hôpitaux de l'Ontario ont adopté des politiques ou des directives médicales concernant la modification de la prescription. Celles-ci autorisent les pharmaciens à adapter, cesser, suspendre ou renouveler largement les prescriptions dans le cadre de leur champ de pratique. Objectifs : L'objectif principal de cette étude visait à décrire la perception des pharmaciens d'hôpitaux de l'Ontario de leur degré de préparation à modifier des prescriptions de manière indépendante. Les objectifs secondaires consistaient à recueillir les opinions sur les avantages, les inconvénients, les éléments de facilitation et les obstacles perçus par les pharmaciens au sujet de la modification de la prescription et de définir comment divers facteurs influençaient la perception de leur degré de préparation. Méthodes : Entre mai et juillet 2019, 936 pharmaciens d'hôpitaux en Ontario ont reçu une enquête confidentielle menée sur Internet comportant des questions quantitatives de type Likert et des questions ouvertes qualitatives. Les scores médians ont été calculés pour les concepts suivants liés à la modification de la prescription : l'autoefficacité, le soutien de l'environnement de pratique et le soutien des relations interprofessionnelles. Des tests t indépendants ont été menés pour comparer les réponses entre les sous-groupes sous-groupes qui intéressaient les auteurs. Les réponses aux questions ouvertes ont été analysées par thème. Résultats : Le taux de réponses à l'enquête se montait à 29 % (n = 271). Le score moyen pour le thème « Autoefficacité » était de 5,2 sur 7 (écart type [ET] 1, Cronbach α = 0,88), ce qui équivaut à la réponse « Assez certain ». Le score moyen pour le thème « Soutien de l'environnement de pratique » était de 3,3 sur 5 (ET 0,4, Cronbach α = 0,75), ce qui équivaut à la réponse « N'est pas un facteur ». Le score moyen pour le thème « Relations interprofessionnelles » était de 4,2 sur 5 (ET 0,1, Cronbach α = 0,80), ce qui équivaut à la réponse « Soutien faible ». Les pharmaciens ont cité l'amélioration de l'efficacité des soins, les interventions en temps opportun visant à améliorer l'innocuité et l'efficacité des médicaments ainsi que l'amélioration de la collaboration interprofessionnelle comme étant des avantages de la modification indépendante des prescriptions. Ils ont aussi indiqué que le risque de prise de décision inappropriée ainsi que la mauvaise communication constituaient pour eux un sujet de préoccupation. Les répondants qui pratiquaient déjà la modification de la prescription en milieu hospitalier ont indiqué un gain d'autoefficacité de la modification des prescriptions dans des domaines cliniques qui leur sont familiers ou non, ainsi qu'un plus grand soutien de la part des prescripteurs. Conclusions : Une grande partie des répondants à une enquête menée auprès de pharmaciens d'hôpitaux de l'Ontario ont jugé que leur degré de préparation à la modification indépendante des ordonnances était prometteur. Les réponses aux questions ouvertes de cette étude fournissent des éclaircissements précieux sur l'adoption généralisée de ce changement de pratique.

Background. The purpose of the research: to determine approaches to ensuring financial accessibility of the population of Ukraine to specialized medical care in the context of administrative reform and health care reform (using the example of dental care). Perposes of the study were to analyze the legislative regulation of the financial availability of dental care in Ukraine; to propose approaches to improving the financial provision of dental care in the context of administrative reform. Materials and methods. We analyzed legislative and regulatory legal acts (14 units), scientific sources on the topic (13 units), domestic and international analytical documents (5 units). We used system approach, system analysis, generalization, graphic. Results. The program of medical guarantees within the framework of reforming secondary medical care includes a limited list of dental services for the population: emergency dental care for adults, emergency and planned dental care for children. This violates the constitutional rights of citizens to free medical care in public and municipal health care institutions with a high level of population needs for dental care, an increase in the proportion of the poor to 24.1 % in 2018, the unemployment rate - by more than 35.0 % from 2006 to 2016, limiting benefits for certain contingents of the population for dental prosthetics. It is shown that the receipt by the governing bodies of the united territorial communities of the authority to take care of providing the population of the community with social services and the ability to receive funds to local budgets from additional sources allows the distribution of funds taking into account the priority needs of the population. The most rational investment of funds should be considered financing targeted health programs, in particular, the prevention and treatment of dental diseases. Conclusions. The reserves for increasing the financial accessibility of the population to dental care are laid down in the legislative regulation of the activities of the united territorial communities within the framework of the reform of local self-government. ; Цель исследования: определить подходы к обеспечению финансовой доступности населения Украины к специализированной медицинской помощи в условиях административной реформы и реформы здравоохранения (на примере стоматологической помощи). Задачи исследования: провести анализ законодательного регулирования финансовой доступности стоматологической помощи в Украине; предложить подходы к совершенствованию финансового обеспечения стоматологической помощи в условиях административной реформы. Материалы и методы. Использовались законодательные и нормативно-правовые акты (14 ед.), научные источники по теме (13 ед.), отечественные и международные аналитические документы (5 ед.). Применялись методы системного подхода, системного анализа, обобщения, графический. Результаты и обсуждение. В программу медицинских гарантий в рамках реформирования вторичной медицинской помощи включен ограниченный перечень стоматологических услуг населению: ургентная стоматологическая помощь взрослым, ургентная и плановая стоматологическая помощь детям. Это нарушает конституционные права граждан на бесплатную медицинскую помощь в учреждениях здравоохранения государственной и коммунальной формы собственности при высоком уровне потребностей населения в стоматологической помощи, увеличении доли бедного населения до 24,1 % в 2018 году, уровня безработицы – более чем на 35,0 % с 2006 по 2016 годы, ограничении льгот для отдельных контингентов населения на зубопротезирование. Показано, что получение органами управления объединенных территориальных общин полномочий заботиться об обеспечении населения общины социальными услугами и возможностей получать средства в местные бюджеты из дополнительных источников позволяет осуществлять распределение средств с учетом приоритетных потребностей населения. Наиболее рациональным вложением средств следует считать финансирование целевых программ по здравоохранению, в частности, профилактики и лечения стоматологических заболеваний. Выводы. Резервы повышения финансовой доступности населения к стоматологической помощи заложены в законодательном регулировании деятельности объединенных территориальных общин в пределах реформы местного самоуправления. ; Мета дослідження: визначити підходи до забезпечення фінансової доступності населення України до спеціалізованої медичної допомоги в умовах адміністративної реформи та реформи охорони здоров'я (на прикладі стоматологічної допомоги). Це порушує конституційні права громадян на безоплатну медичну допомогу в закладах охорони здоров'я державної та комунальної форми власності при високому рівні потреб населення в стоматологічній допомозі, зростанні частки бідного населення до 24,1 % у 2018 році, рівня безробіття – більше ніж на 35,0 % з 2006 по 2016 роки, обмеженні пільг для окремих контингентів населення на зубопротезування. Показано, що наділення органів управління об'єднаних територіальних громад повноваженнями піклуватися про забезпечення населення громади соціальними послугами та можливостями отримувати кошти до місцевих бюджетів з додаткових джерел дозволяє здійснювати розподіл коштів з урахуванням пріоритетних потреб населення. Найбільш раціональним вкладенням коштів слід вважати фінансування цільових програм з охорони здоров'я, зокрема, з профілактики і лікування стоматологічних захворювань. Висновки. Резерви підвищення фінансової доступності населення до стоматологічної допомоги закладені у законодавчому регулюванні діяльності об'єднаних територіальних громад в межах реформи місцевого самоврядування.

Against the background of the events of the last decade in the security sphere, the scenario has already become clear, which envisages the return of NATO to a more serious approach to decisive steps in addressing issues of collective defense and security. The abandonment of such a prospect, given the already deployed units of the Allied forces in Eastern Europe, especially Poland, Romania and the Baltic States, will lead to the idea (if not certainty) of the inferiority of the measures taken, and a statement that NATO is not ready for global action. For Ukraine in its aspirations for membership in the Alliance, it is very important to predict with high probability the military-political development of NATO and, taking into account such forecasts, to formulate directions of its policy, including issues of military cooperation with the North Atlantic Treaty Organization. This article is a continuation of the discussion proposed by the authors of the article "The future of the North Atlantic Alliance in a context of uncertainty and complex geopolitical environment" which analyzes the long-term vision of the North Atlantic Treaty Organization. In fact, this article is an attempt to predict the directions of the Alliance's policy, the international security, and the military-political situation in general, in accordance with the proposed scenarios. The probability and, ultimately, the success of one of these scenarios depend on a number of strategic factors. In this article, the author tried to identify and analyze the most plausible scenarios for NATO's development, namely: "eurocentric NATO", "global NATO", "flexible NATO". Here each of the given scenarios is analyzed; all its positive and negative sides and consequences are defined. Also important is the significance of each of the scenarios for Ukrainian policy. The author singles out the "flexible NATO" scenario as the most favorable for more thorough cooperation and providing Ukraine with the necessary support. ; На фоне событий последнего десятилетия относительно обеспечения безопасности ситуации уже стал очевидным сценарий, который предусматривает поворот НАТО к более решительным шагам в решении вопросов коллективной обороны и безопасности. Отказ же от такой перспективы, с учетом уже развернутых подразделений вооруженных сил стран-членов на территории государств Восточной Европы, прежде всего, Польши, Румынии и странах Балтии, приведет к представлению (если не уверенности) в неполноценности принимаемых мер, и утверждение, что НАТО не готово к действиям в глобальном плане. Для України в її прагненні щодо набуття членства в Альянсі, дуже важливим є з високою долею вірогідності передбачити воєнно-політичний розвиток НАТО та, з урахуванням таких прогнозів, сформулювати напрями своєї політики, включаючи питання військової співпраці по відношенню до Організації Північноатлантичного Договору. Данная статья является продолжением дискуссии, предложенной авторами публикации "Будущее Североатлантического альянса в условиях неопределенности и сложного геополитического окружения" относительно будущего места НАТО в глобальной системе международных отношений, где проведен анализ видения развития Организации Североатлантического договора на долгосрочную перспективу. И, фактически является попыткой прогнозирования направления политики Альянса, международной ситуации и военно-политической обстановки в целом, в соответствии с предложенными сценариями. Достоверность и, в итоге, успешность реализации одного из указанных сценариев, зависит от ряда стратегических факторов. В статье проведен анализ наиболее вероятных сценариев развития НАТО, а именно: "евроцентричное НАТО", "глобальное НАТО", "гибкое НАТО". Проанализированы каждый из приведенных сценариев, определены все положительные и отрицательные его стороны и последствия. Также важно значение каждого из сценариев на политику Украины. Наиболее благоприятным для более основательного сотрудничества и предоставления Украине необходимой поддержки выделяется сценарий "гибкое НАТО". ; На тлі подій останнього десятиліття в царині безпекової ситуації вже став очевидним сценарій, який передбачає повернення НАТО до більш рішучих кроків у вирішенні питань колективної оборони та безпеки. Відмова ж від такої перспективи, з урахуванням вже розгорнутих підрозділів збройних сил країн-членів Альянсу на території країн Східної Європи, перш за все, Польщі, Румунії та країнах Балтії, призведе до уявлення (якщо не впевненості) в неповноцінності тих заходів, які вживаються, та ствердження, що НАТО не готове до дій в глобальному плані. Для України в її прагненні щодо набуття членства в Альянсі, дуже важливим є з високою долею вірогідності передбачити воєнно-політичний розвиток НАТО та, з урахуванням таких прогнозів, сформулювати напрями своєї політики, включаючи питання військової співпраці по відношенню до Організації Північноатлантичного Договору. Дана стаття є продовження дискусії, яка запропонована авторами публікації "Майбутнє Північноатлантичного Альянсу в умовах невизначеності та складного геополітичного оточення" стосовно майбутнього місця НАТО в глобальній системі міжнародних відносин, де проведений аналіз бачення розвитку Організації Північноатлантичного договору на довгострокову перспективу. І фактично є спробою спрогнозувати напрями політики Альянсу, міжнародну ситуацію, та воєнно-політичну обстановку в цілому, відповідно до запропонованих сценаріїв. Вірогідність та, у підсумку, успішність реалізації одного із зазначених сценаріїв, залежить від низки стратегічних факторів. В статті проведено аналіз найбільш вірогідних сценаріїв розвитку НАТО, а саме: "євроцентричне НАТО", "глобальне НАТО", "гнучке НАТО". Проаналізовано кожний з наведених сценаріїв, визначені всі позитивні і негативні його сторони та наслідки. Також важливим є значення кожного із сценаріїв на політику України. Найбільш сприятливим для більш ґрунтовного співробітництва та надання Україні необхідної підтримки виділяється сценарій "гнучке НАТО".

La gestion des ressources humaines est une discipline paradoxale. Alors qu'elle est considérée comme théoriquement pauvre—Watson (2007) rappelle qu'il n'y a pas à proprement parler de « théories de la GRH », mais bien que la GRH s'alimente de théories issues du management stratégique, de la psychologie, de la sociologie, de l'économie—elle occupe une place importante au sein d'autres disciplines telles que le management international, interculturel, stratégique ou le comportement organisationnel (Keenoy, 2009). De même, alors que la rhétorique propre à la GRH annonce une performance supérieure de la firme grâce à la mise en œuvre de certaines pratiques (rassemblées sous le label des HPWS-High Performance Work Systems) alignées avec la stratégie d'entreprise, ce lien entre une fonction RH stratégique source d'avantage concurrentiel et la performance de l'entreprise demeure empiriquement non-établi ou, du moins, incertain (Taskin et Pichault, 2012 ; Truss, 2001). C'est à partir de ces paradoxes—et d'un scepticisme à l'égard des modalités et desseins de la GRH—qu'une critique des discours, pratiques, modèles, finalités et fondements de la gestion des ressources humaines s'est développée et affirmée à partir des années 1990. Cette perspective critique fait partie intégrante du champ de la gestion des ressources humaines et alimente les recherches, les enseignements mais également la pratique dans le domaine. La critique porte souvent sur des pratiques particulières de GRH et d'organisation du travail, telles que la flexibilité (télétravail, nouvelles formes d'organisation du travail et de gouvernance, rémunération variable, etc.), la gestion des connaissances et des compétences, la sélection, l'appréciation ou la culture d'entreprise. Elle dénonce alors les effets de ces pratiques (souffrance, désengagement, individualisation, résistance…) en mobilisant des théories et des méthodologies diverses et variées et peut aussi questionner la rhétorique selon laquelle la GRH serait autant au service du bien-être des salariés que de l'intérêt des propriétaires de l'entreprise, par le déploiement de « bonnes pratiques » universelles. Un point commun rassemble toutefois ces différentes entreprises critiques : la volonté d'exprimer les rapports de pouvoir, de domination et d'émancipation, de rendre compte de la diversité d'intérêts des acteurs mais aussi de la nécessité de contextualiser l'étude de la GRH et de ses pratiques. Toutes les critiques ne prennent donc pas la même forme : certaines sont factuelles (on peut critiquer la méthodologie employée par Lawler et Boudreau pour évaluer le positionnement stratégique de la fonction RH), d'autres sont thématiques (on peut rapporter les effets observés de telle pratique de GRH sur différentes dimensions individuelles et organisationnelles), d'autres portent sur les fondements ou les finalités de la GRH et seraient davantage théoriques (on peut questionner la conception de l'Humain sous-jacente aux principaux modèles de GRH). Ces formes de critique sont poreuses entre elles (une critique thématique peut être factuelle mais aussi théorique) car elles sont liées à la nature de la problématisation, à l'épistémologie et aux méthodologies déployées, ainsi qu'aux référents théoriques mobilisés. C'est pourquoi, dans ce chapitre, nous avons reconstitué deux postures différentes de la critique en GRH qui se caractérisent par des objets, finalités, problématiques et modalités différents. La première posture « programmatique » dénonce le programme (capitaliste) que poursuivrait la GRH en l'incarnant, par le biais de pratiques, de discours ainsi que de modèles. Cette posture rassemble la majorité des travaux critiques en GRH qui, partant de l'étude de dispositifs spécifiques (la formation, le coaching, le travail par projet ou en équipe, p.ex.), dénoncent leur projet disciplinarisant ou les rapports de domination qui leur sont sous-jacents. Cette perspective est le fruit d'une critique politique du capitalisme et de ses institutions (t.q. l'entreprise mais aussi les écoles de commerce) et repose sur un appareil méthodologique de la critique en gestion qui s'articule autour des principes de dé-naturalisation, de réflexivité et de (re)considération des rapports de pouvoir. Cette perspective « programmatique » emprunte assez fidèlement les balises des perspectives critiques en management telles qu'institutionnalisées par les anglo-saxons depuis le début des années 1990 et dont l'École de Francfort et la Labour Process Theory en sont les inspirations originelles. La seconde posture « éthique-disciplinaire » se veut davantage performative et questionne la GRH en tant que discipline. Les travaux issus de cette posture proposent des cadres et modèles alternatifs à la GRH, le plus souvent émargeant d'une critique morale amenant à identifier des modes de gestion des personnes et du travail souhaitables, au-delà de la dénonciation. Cette seconde posture peut être riche d'une réflexion anthropologique à partir des sujets centraux de la GRH—l'Humain et le travail, qui demeurent étonnamment indéfinis dans la plupart des recherches et manuels de GRH. Cette posture éthique-disciplinaire présente des perspectives nouvelles et fécondes autorisant, par exemple, la compréhension des rapports de travail dans les registres du don, de la réciprocité et du « bien » (bien-être, bienveillance, bien commun…), au prix d'inspirations pluridisciplinaires. Ce chapitre présente les caractéristiques de ces postures critiques, les concepts et inspirations théoriques majeures qui les rassemblent ainsi qu'un certain nombre de travaux qui les illustrent. Au terme de cette présentation détaillée, les enjeux que ces approches critiques posent pour la recherche, l'enseignement et la pratique de la GRH sont abordés, offrant au lecteur quelques balises utiles s'il décidait de s'aventurer dans une critique de la GRH.

BACKGROUND: The UK Government's implementation in 2008 of the Improving Access to Psychological Therapies (IAPT) initiative in England has hugely increased the availability of cognitive behavioural therapy (CBT) for the treatment of depression and anxiety in primary care. Counselling for depression-a form of person-centred experiential therapy (PCET)-has since been included as an IAPT-approved therapy, but there is no evidence of its efficacy from randomised controlled trials (RCTs), as required for recommendations by the National Institute for Health and Care Excellence. Therefore, we aimed to examine whether PCET is cost effective and non-inferior to CBT in the treatment of moderate and severe depression within the IAPT service. METHODS: This pragmatic, randomised, non-inferiority trial was done in the Sheffield IAPT service in England and recruited participants aged 18 years or older with moderate or severe depression on the Clinical Interview Schedule-Revised. We excluded participants presenting with an organic condition, a previous diagnosis of personality disorder, bipolar disorder, or schizophrenia, drug or alcohol dependency, an elevated clinical risk of suicide, or a long-term physical condition. Eligible participants were randomly assigned (1:1), independently of the research team, and stratified by site with permuted block sizes of two, four, or six, to receive either PCET or CBT by use of a remote, web-based system that revealed therapy after patient details were entered. Those assessing outcomes were masked to treatment allocation. Participants were seen by appropriately trained PCET counsellors and CBT therapists in accordance with the IAPT service delivery model. Depression severity and symptomatology measured by the Patient Health Questionnaire-9 (PHQ-9) at 6 months post-randomisation was the primary outcome, with the PHQ-9 score at 12 months post-randomisation being a key secondary outcome. These outcomes were analysed in the modified intention-to-treat population, which comprised all randomly assigned patients with complete data, and the per-protocol population, which comprised all participants who did not switch from their randomised treatment and received between four and 20 sessions. Safety was analysed in all randomly assigned patients. The non-inferiority margin was set a priori at 2 PHQ-9 points. Patient safety was monitored throughout the course of therapy, adhering to service risk procedures for monitoring serious adverse events. This trial is registered at the ISRCTN Registry, ISRCTN06461651, and is complete. FINDINGS: From Nov 11, 2014, to Aug 3, 2018, 9898 patients were referred to step three treatments in the Sheffield IAPT service for common mental health problems, of whom 761 (7·7%) were referred to the trial. Of these, we recruited and randomly assigned 510 participants to receive either PCET (n=254) or CBT (n=256). In the PCET group, 138 (54%) participants were female and 116 (46%) were male, and 225 (89%) were White, 16 (6%) were non-White, and 13 (5%) had missing ethnicity data. In the CBT group, 155 (61%) participants were female and 101 (39%) were male, and 226 (88%) were White, 17 (7%) were non-White, and 13 (5%) had missing ethnicity data. The 6-month modified intention-to-treat analysis comprised 401 (79%) of the enrolled participants (201 in the PCET group; 200 in the CBT group) and the 12-month modified intention-to-treat analysis comprised 319 participants (167 in the PCET group; 152 in the CBT group). The 6-month per-protocol analysis comprised 298 participants (154 in the PCET group; 144 in the CBT group). At 6 months post-randomisation, PCET was non-inferior to CBT in the intention-to-treat population (mean PHQ-9 score 12·74 [SD 6·54] in the PCET group and 13·25 [6·35] in the CBT group; adjusted mean difference -0·35 [95% CI -1·53 to 0·84]) and in the per-protocol population (12·73 [SD 6·57] in the PCET group and 12·71 [6·33] in the CBT group; 0·27 [95% CI -1·08 to 1·62]). At 12 months post-randomisation, there was a significant adjusted between-group difference in mean PHQ-9 score in favour of CBT (1·73 [95% CI 0·26-3·19]), with a 95% CI exceeding the 2-point non-inferiority margin. There were two deaths, one death by suicide in the PCET group and one due to chronic obstructive pulmonary disease in the CBT group. Both were assessed by the responsible clinician to be unrelated to the trial. In terms of using emergency departments for depression-related events, four people (three in the PCET group; one in the CBT group) made more than a single use and six people (three in the PCET group; three in the CBT group) made a single use. One patient in the PCET group had inpatient treatment for a depression-related event. INTERPRETATION: This trial is the first to examine the two most frequently administered psychological therapies in the IAPT service. The finding of non-inferiority of PCET to CBT at 6 months supports the results from large, routine, non-randomised datasets from the IAPT programme. Given the high demand for psychological therapies and the need for patient choice, our findings suggest the need for continued investment in the training and delivery of PCET for improving short-term outcomes, but suggest that PCET might be inferior to CBT at 12 months. In the article based on the results of comparative analysis of the dynamism of the securities market of Ukraine and its key component – the stock market, the trends are clarified and the directions of its further development are determined. The role of the stock market in the development of the country's economy from the standpoint of its impact on various areas of activity and functional purpose is emphasized. According to the results of critical analysis of scientific works of modern economists and financiers in the field of studying the problems of stock markets functioning, the range of issues that deserve priority attention from the standpoint of identifying the problematic nature of their development and intensification of exchange activities are specified. The reasons for the downward dynamics and stagnation of development of the securities market of Ukraine from the standpoint of the international online platform Trading Economics in the period 2010-2020 are revealed. The tendencies of change of volumes of exchange trade in the securities market of Ukraine in comparison with dynamics of GDP by calculation of a partial indicator of influence on its dynamics in the period of 2015-2020 are defined. The presented comparative data on the volume of exchange and over-the-counter trade in the securities market of Ukraine revealed a tendency to change the focus of trade from the over-the-counter market to organized securities markets. It is established that the modern stock market of securities of Ukraine tends to a polycentric model, which is based on FB "Perspective" and FB "PFTS", which in the study period accounted for 91.94-99.67% of exchange turnover, which indicates the processes of consolidation of securities trade in such a market. Analysis of the situation on the stock market of securities of Ukraine during the spread of COVID-19 (January-September 2020) revealed that in the study period compared to the same period in 2019, trading volumes increased by 3.14%, and this indicates a lack of pandemy impact on the dynamics of exchange trade in stock values. At the same time, the reason for such changes can be considered a significant previous decline in the domestic stock market and its gradual recovery, which would have had a faster growth rate, if not for the impact of the spread of COVID-19. It should also be noted the low activity of stockbrokers in the primary securities market, and relatively high in the secondary market, where Domestic government bonds were the most frequently traded, and trading was widespread in the spot market. Exit from the state of stagnation is possible if the National Commission on Securities and Stock Market adheres to the Strategy for the Development of the Financial Sector of Ukraine until 2025, the global financial market will be free of shocks similar to February-March 2020, there will be consolidated development of financial investment, organizational, legal and socially significant support of the country's exchange activities. ; Луцький національний технічний університет В статті на основі результатів порівняльного аналізу динамізму розвитку ринку цінних паперів України та його ключової складової – біржового ринку, уточнено тенденції та визначено напрями її подальшого розвитку. Підкреслено роль біржового ринку цінних паперів у розвитку економіки країни з позицій його впливу на різні сфери діяльності та функціонального призначення. За результатами критичного аналізу наукових праць економістів і фінансистів сучасності у сфері вивчення проблем функціонування фондових ринків країн уточнено коло питань, які заслуговують першочергової уваги з позицій виявлення проблемного характеру їх розвитку та активізації біржової діяльності. Розкрито причини низьхідної динаміки та стагнації розвитку ринку цінних паперів України з позиції міжнародної онлайн-платформи Trading Economics в період 2010-2020 рр. Визначено тенденції зміни обсягів біржової торгівлі на ринку цінних паперів України порівняно з динамікою ВВП шляхом розрахунку часткового показника впливовості на його динаміку в період 2015-2020 рр. Наведені порівняльні дані обсягів біржової та позабіржової торгівлі на ринку цінних паперів України виявили тенденцію зміни акцентів торгівлі з позабіржового ринку на організовані ринки цінних паперів. Встановлено, що сучасний біржовий ринок цінних паперів України тяжіє до поліцентричної моделі, основу якої становлять ФБ "Перспектива" та ФБ "ПФТС", на які у досліджуваному періоді припадає 91,94-99,67% біржового обороту, що вказує на процеси консолідації торгівлі цінними паперами на такому ринку. Аналіз ситуації на біржовому ринку цінних паперів України в період поширення COVID-19 (січень-вересень 2020 р.) виявив, що у досліджуваному періоді порівняно з аналогічним періодом 2019 р. обсяги торгівлі зросли на 3,14%, і це вказує на відсутність впливу пандемії на динаміку розгортання біржової торгівлі фондовими цінностями. Разом з тим, причиною таких змін можна вважати й значне попереднє падіння вітчизняного біржового ринку та його поступове відновлення, яке мало б швидші темпи зростання, якби не вплив поширення COVID-19. Також слід відзначити низьку активність біржовиків на первинному ринку цінних паперів, та відносно високу на вторинному ринку, де предметом торгів найчастіше виступали ОВДП, а торгівля була поширена на спотовому ринку. Вихід зі стану стагнації є можливим за умов, якщо Національна комісія з цінних паперів та фондового ринку буде дотримуватись Стратегії розвитку фінансового сектору України до 2025 року, світовий фінансовий ринок буде позбавлений потрясінь, аналогічних лютого-березня 2020 р., матиме місце консолідований розвиток фінансово-інвестиційного, організаційно-правового та соціально значимого забезпечення біржової діяльності країни.

The increase in the volume of passenger transportation in megalopolises and large urban agglomerations is efficiently provided by the integration of urban public transit systems and city railways. Traffic management under those conditions requires creating intelligent centralised multi-level traffic control systems that implement the required indicators of quality, comfort, and traffic safety regarding passenger transportation. Besides, modern control systems contribute to traction power saving, are foundation and integral part of the systems of digitalisation of urban transit and the cities. Building systems solving the traffic planning and control tasks is implemented using algorithms based on the methods of artificial intelligence, principles of hierarchically structured centralised systems, opportunities provided by Big Data technology. Under those conditions it is necessary to consider growing requirements towards software as well as theoretical design and practical implementation of network organisation.This article discusses designing architecture and shaping requirements for developed applications and their integration with databases to create a centralised intelligent control system for the urban rail transit system (CICS URTS). The article proposes the original architecture of the network, routing of information flows and software of CICS URTS. The routing design is based on a fully connected network. This allows to significantly increase the network bandwidth and meet the requirements regarding information protection, since information flows are formed based on the same type of protocols, which prevents emergence of covert transmission channels. The implementation of the core using full connectivity allows, according to the tags of information flows, to pre-form the routes for exchange of information between servers and applications deployed in CICS URTS. The use of encrypted tags of information flows makes it much more difficult to carry out attacks and organise collection of information about the network structure.Platforms for development of intelligent control systems (ICS), which include CICS URTS, high computing power, data storage capacity and new frameworks are becoming more available for researchers and developers and allow rapid development of ICS. The article discusses the issues of interaction of applications with databases through a combination of several approaches used in the field of Big Data, substantiates combination of Internet of Things (IoT) methodology and microservice architecture. This combination will make it possible to single out business processes in the system and form streaming data processing requiring operational analysis by a human, which is shown by relevant examples.Thus, the objective of the article is to formalise the principles of organising data exchange between CICS URTS and automated control systems (ACS) of railway companies (in our case, using the example of JSC Russian Railways), URTS services providers, and city government bodies, implement the developed approaches into the architecture of CICS URTS and formalise principles of organisation of microservice architecture of CICS URTS software. The main research methods are graph theory, Big Data and IoT methods. ; Рост объёма пассажирских перевозок в условиях крупных городских агломераций и мегаполисов эффективно обеспечивается объединением общественного транспорта и городских линий железных дорог. Управление движением в этих условиях требует создания интеллектуальных централизованных многоуровневых систем управления, реализующих заданные показатели качества, комфорта и безопасности перевозок пассажиров. Современные системы управления дополнительно решают задачи экономии энергии на тягу подвижного состава, являются фундаментом и составной частью систем цифровизации городского транспорта и города в целом. Построение систем, решающих задачи планирования и управления движением, реализуется с применением алгоритмов, использующих методы искусственного интеллекта, принципы иерархического построения централизованных систем, возможности технологии Big Data. В этих условиях необходимо учитывать возросшие требования не только к программному обеспечению, но и к теоретическим и практическим решениям организации сети.В данной статье рассматриваются вопросы формирования архитектуры и требований к разрабатываемым приложениям и их интеграции с базами данных для создания централизованной интеллектуальной системы управления городской рельсовой транспортной системой (ЦИСУ ГРТС). В статье предлагаются оригинальные подходы к архитектуре сети, маршрутизации информационных потоков и программному обеспечению ЦИСУ ГРТС. В основе построения маршрутизации лежит использование полносвязной сети. Это позволяет значительно увеличить пропускную способность сети и выполнить требования по защите информации, так как информационные потоки формируются на базе однотипных протоколов, что препятствует образованию скрытых каналов передачи. Реализация ядра с использованием полносвязности позволяет по меткам информационных потоков заранее сформировать маршруты обмена информации между серверами и приложениями, развёрнутыми в ЦИСУ ГРТС. Использование шифрованных меток информационных потоков значительно усложняет проведение атак и организацию сбора информации о структуре сети.Платформы для разработки интеллектуальных систем управления (ИСУ), к которым относится ЦИСУ ГРТС, огромные вычислительные мощности, хранилища данных и новые фреймворки становятся всё более доступными для учёных и разработчиков и позволяют быстро развиваться ИСУ. В статье рассматриваются вопросы взаимодействия приложений с базами данных на основе комбинации нескольких подходов, используемых в области Big Data, обосновывается сочетание методологии интернета вещей (IoT) и микросервисной архитектуры. Данная комбинация позволит выделить в системе бизнес-процессы и сформировать потоковую обработку данных, требующих оперативного анализа человеком, что доказывается соответствующими примерами.Таким образом, целью статьи является формализация принципов организации информационного обмена ЦИСУ ГРТС и автоматизированных систем управления (АСУ) железнодорожных компаний (в нашем случае – на примере ОАО «РЖД»), организаций, предоставляющих услуги ГРТС, и городских органов управления, реализация сформулированных принципов в архитектуре ЦИСУ ГРТС и формализация принципов организации микросервисной архитектуры программного обеспечения ЦИСУ ГРТС. Основными методами исследования являются теория графов, методы Big Data, IoT.

Die vorliegende Studie zeigt, dass Daten in der Krise eine herausragende Bedeutung für die wissenschaftliche Politikberatung, administrative Entscheidungsvorbereitung und politische Entscheidungsfindung haben. In der Krise gab es jedoch gravierende Kommunikationsprobleme und Unsicherheiten in der wechselseitigen Erwartungshaltung von wissenschaftlichen Datengebern und politisch-administrativen Datennutzern. Die Wissensakkumulation und Entscheidungsabwägung wurde außerdem durch eine unsichere und volatile Datenlage zum Pandemiegeschehen, verbunden mit einer dynamischen Lageentwicklung, erschwert. Nach wie vor sind das Bewusstsein und wechselseitige Verständnis für die spezifischen Rollenprofile der am wissenschaftlichen Politikberatungsprozess beteiligten Akteure sowie insbesondere deren Abgrenzung als unzureichend einzuschätzen. Die Studie hat darüber hinaus vielfältige Defizite hinsichtlich der Verfügbarkeit, Qualität, Zugänglichkeit, Teilbarkeit und Nutzbarkeit von Daten identifiziert, die Datenproduzenten und -verwender vor erhebliche Herausforderungen stellen und einen umfangreichen Reformbedarf aufzeigen, da zum einen wichtige Datenbestände für eine krisenbezogene Politikberatung fehlen. Zum anderen sind die Tiefenschärfe und Differenziertheit des verfügbaren Datenbestandes teilweise unzureichend. Dies gilt z.B. für sozialstrukturelle Daten zur Schwere der Pandemiebetroffenheit verschiedener Gruppen oder für kleinräumige Daten über Belastungs- und Kapazitätsparameter, etwa zur Personalabdeckung auf Intensivstationen, in Gesundheitsämtern und Pflegeeinrichtungen. Datendefizite sind ferner im Hinblick auf eine ganzheitliche Pandemiebeurteilung festzustellen, zum Beispiel bezüglich der Gesundheitseffekte im weiteren Sinne, die aufgrund der ergriffenen Maßnahmen entstanden sind (Verschiebung oder Wegfall von Operationen, Behandlungen und Prävention, aber auch häusliche Gewalt und psychische Belastungen). Mangels systematischer Begleitstudien und evaluativer Untersuchungen, u.a. auch zu lokalen Pilotprojekten und Experimenten, bestehen außerdem Datendefizite im Hinblick auf die Wirkungen von Eindämmungsmaßnahmen oder deren Aufhebung auf der gebietskörperschaftlichen Ebene. Insgesamt belegt die Studie, dass es zur Optimierung der datenbasierten Politikberatung und politischen Entscheidungsfindung in und außerhalb von Krisen nicht nur darum gehen kann, ein "Mehr" an Daten zu produzieren sowie deren Qualität, Verknüpfung und Teilung zu verbessern. Vielmehr müssen auch die Anreizstrukturen und Interessenlagen in Politik, Verwaltung und Wissenschaft sowie die Kompetenzen, Handlungsorientierungen und kognitiv-kulturellen Prägungen der verschiedenen Akteure in den Blick genommen werden. Es müssten also Anreize gesetzt und Strukturen geschaffen werden, um das Interesse, den Willen und das Können (will and skill) zur Datennutzung auf Seiten politisch-administrativer Entscheider und zur Dateneinspeisung auf Seiten von Wissenschaftlern zu stärken. Neben adressatengerechter Informationsaufbereitung geht es dabei auch um die Gestaltung eines normativen und institutionellen Rahmens, innerhalb dessen die Nutzung von Daten für Entscheidungen effektiver, qualifizierter, aber auch transparenter, nachvollziehbarer und damit demokratisch legitimer erfolgen kann. Vor dem Hintergrund dieser empirischen Befunde werden acht Cluster von Optimierungsmaßnahmen vorgeschlagen: (1) Etablierung von Datenstrecken und Datenteams, (2) Schaffung regionaler Datenkompetenzzentren, (3) Stärkung von Data Literacy und Beschleunigung des Kulturwandels in der öffentlichen Verwaltung, (4) Datenstandardisierung, Interoperabilität und Registermodernisierung, (5) Ausbau von Public Data Pools und Open Data Nutzung, (6) Effektivere Verbindung von Datenschutz und Datennutzung, (7) Entwicklung eines hochfrequenten, repräsentativen Datensatzes, (8) Förderung der europäischen Daten-Zusammenarbeit. ; This study shows that data is of outstanding importance for scientific policy advice, administrative decision preparation and political decision-making in the crisis. During the crisis, however, there were serious communication problems and uncertainties in the mutual expectations of scientific data providers and political-administrative data users. Knowledge accumulation and decision-making were also hampered by uncertain and volatile data on the pandemic, combined with a dynamic development of the situation. Awareness and mutual understanding of the specific role profiles of the actors involved in the scientific policy advisory process, as well as their demarcation in particular, are still to be assessed as insufficient. The study has also identified a variety of deficits with regard to the availability, quality, accessibility, shareability and usability of data, which represent considerable challenges to data producers and users and reveal a need for extensive reform, since, on the one hand, important data sets for crisis-related policy advice are lacking. On the other hand, the depth of focus and differentiation of the available data stocks are partly insufficient. This applies, for example, to socio-structural data on the severity of the pandemic impact of different groups or to small-scale data on burden and capacity parameters, such as staffing levels in intensive care units, health offices and care facilities. There are also data deficits with regard to a holistic pandemic assessment, for example with regard to the health effects in a broader sense that have arisen as a result of the measures taken (postponement or discontinuation of operations, treatments and prevention, but also domestic violence and psychological stress). In the absence of systematic accompanying studies and evaluative research, there are also data deficits with regard to the effects of containment measures or their removal at the territorial level. Overall, the study shows that optimising data-based policy advice and political decision-making in times of crisis as well as between crises is not just a matter of producing "more" data and improving its quality, linkage and sharing. Rather, the incentive structures and interests in politics, administration and science as well as the competences, action orientations and cognitive-cultural imprints of the various actors must also be taken into account. Incentives must be set and structures have to be created in order to strengthen the interest, the will and the skill to use data. This does not only apply for political and administrative decision-makers, but also for academics who are needed to feed data into the system. In addition to preparing information in a way that is appropriate for the target group, this also involves designing a normative and institutional framework within which the use of data for decision-making can be more effective, more qualified, but also more transparent, more comprehensible and thus more democratically legitimate. Against the background of these empirical findings, eight clusters of optimisation measures are proposed: (1) Establishment of data routes and data teams, (2) Creation of regional data competence centres, (3) Strengthening data literacy and accelerating cultural transformation in public administration, (4) Data standardisation, interoperability and register modernisation, (5) Expanding public data pools and open data use, (6) Linking data protection and data use more effectively, (7) Development of a high-frequency, representative data set, (8) Promoting European data cooperation.

This article examines the role of public diplomacy in the Ukraine-Belarus relations. Effective interaction between state bodies and local self-government bodies, public organizations and associations of the three countries can be considered the result of the activity of public diplomacy - a dialogue that exists and develops outside official diplomacy and protocol. The existence of public diplomacy outside the social framework, outside a particular society of the nationstate is impossible. The diplomatic model of Belarus underlines the vitality and dynamism of its foreign policy and diplomacy. Strengthening the strategic partnership in the CIS space is a priority area of the policy of Belarus. The fact of friendly and benevolent bilateral foreign policy and diplomatic relations between Belarus and Ukraine is also important. High-level meetings and negotiations traditionally play an important role in strengtheningand developing partnership between the Republic of Belarus and Ukraine, which contribute to strengthening political, economic, cultural and humanitarian cooperation between the two states. Ukraine and the Republic of Belarus have all the necessary conditions for the transition to a qualitatively new stage of interstate cooperation - partnership, with the prospect of transition into a strategic partnership. A striking example of public diplomacy in action is the implementation of joint projects of international technical assistance of the European Union within the framework of the Cross-Border Cooperation Programme Poland-Belarus-Ukraine. Thus, public diplomacy is an integral auxiliary tool for the implementation of the foreign policy of both Ukraine and Belarus. The development of good-neighbourly relations by deepening cultural ties at the level of civil society certainly needs to be paid more attention, taking into account the existing security challenges in the region. In the near future, public diplomacy, as a tool, should become a locomotive in solving problematic aspects of relations between the two states, and the improvement of its mechanisms on the example of Ukraine-Belarus relations should become the subject of study of political science. ; В этой статье рассматриваются вопросы роли публичной дипломатии в украинско-белорусских отношениях. Эффективное взаимодействие между государственными органами и органами местного самоуправления, общественными организациями и объединениями трех стран можно считать результатом действия публичной дипломатии - диалога, существующего и развивающегося за рамками официальной дипломатии и протокола. Существование публичной дипломатии вне социальных рамок, вне конкретного общества национального государства является невозможным. В дипломатической модели Беларуси подчеркнуты жизненность и динамичность ее внешней политики и дипломатии. Укрепление стратегического партнерства на пространстве СНГ являетсяприоритетным направлением политики Беларуси. Немаловажным является факт дружеских и доброжелательных двухсторонних внешнеполитических и дипломатических отношений между Беларусью и Украиной. Важную роль в укреплении и развитии партнерства Республики Беларусь и Украины традиционно играют встречи на высоком уровне и переговоры, которые способствуют укреплению политического, экономического и культурно-гуманитарного сотрудничества двух государств. Украина и Республика Беларусь имеют все необходимые условия для перехода на качественно новый этап межгосударственного сотрудничества - партнерства с перспективой на переход в стратегическое партнерство. Ярким примером публичной дипломатии в действии служит реализация совместных проектов международной технической помощи Европейского Союза в рамках «Программы трансграничного сотрудничества Польша-Беларусь-Украина». Таким образом, публичная дипломатия является неотъемлемым вспомогательным инструментом реализации внешней политики как Украины, так и Беларуси. Вопросу развития добрососедских отношений путем углубления культурных связей на уровне гражданского общества безусловно необходимо уделять больше внимания с учетом существующих вызовов безопасности в регионе. Публичная дипломатия как инструмент в ближайшей перспективе должна стать локомотивом в решении проблемных аспектов взаимоотношений двух государств, а усовершенствование ее механизмов на примере украинско-белорусских отношений должно стать предметом изучения политологии. ; У статті розглядаються питання ролі публічної дипломатії в українсько-білоруських відносинах. Організацію ефективної взаємодії між державними органами і органами місцевого самоврядування, громадськими організаціями та об'єднаннями трьох країн можна вважати результатом дії публічної дипломатії - діалогу, який існує і розвивається за рамками офіційної дипломатії і протоколу. Існування публічної дипломатії поза соціальними рамками, поза конкретним суспільством національної держави є неможливим. У дипломатичній моделі Білорусі підкреслені життєвість і динамічність її зовнішньої політики і дипломатії. Зміцнення стратегічного партнерства на просторі СНД є пріоритетним напрямом політики Білорусі. Важливим є факт дружніх і доброзичливих двосторонніх зовнішньополітичних і дипломатичних відносин між Україною та Республікою Білорусь. Важливу роль у зміцненні та розвитку партнерства Республіки Білорусь і України традиційно відіграють зустрічі на високому рівні і переговори, які сприяють зміцненню політичного, економічного і культурно-гуманітарного співробітництва двох держав. Україна і Республіка Білорусь мають усі необхідні умови для переходу на якісно новий етап міждержавного співробітництва - партнерства, з перспективою на перехід у стратегічне партнерство. Яскравим прикладом публічної дипломатії в дії є реалізація спільних проектів міжнародної технічної допомоги Європейського Союзу в рамках «Програми транскордонного співробітництва Польща-Білорусь-Украї-на». Таким чином, публічна дипломатія є невід'ємним допоміжним інструментом реалізації зовнішньої політики як України, так і Білорусі. Питанню розвитку добросусідських відносин шляхом поглиблення культурних зв'язків на рівні громадянського суспільства безумовно необхідно приділяти більше уваги з урахуванням існуючих викликів безпеки в регіоні. Публічна дипломатія як інструмент у найближчій перспективі повинна стати локомотивом у вирішенні проблемних аспектів взаємовідносин двох держав, а вдосконалення її механізмів на прикладі українсько-білоруських відносин має стати предметом вивчення політології.

The field of translation is undergoing various profound changes. On the one hand it is being thoroughly reshaped by the advent and constant improvement of new technologies. On the other hand, new forms of translation are starting to see the light of day in the wake of social and legal developments that require that products and content that are created, are accessible for everybody. One of these new forms of translation, is audio description (AD), a service that is aimed at making audiovisual content accessible to people with sight loss. New legislation requires that this content is accessible by 2025, which constitutes a tremendous task given the limited number of people that are at present trained as audio describers. A possible solution would be to use machine translation to translate existing audio descriptions into different languages. Since AD is characterized by short sentences and simple, concrete language, it could be a good candidate for machine translation. In the present study, we want to test this hypothesis for the English-Dutch language pair. Three 30 minute AD excerpts of different Dutch movies that were originally audio described in English, were translated into Dutch using DeepL. The translations were analysed using the harmonized DQF-MQM error typology and taking into account the specific multimodal nature of the source text and the intersemiotic dimension of the original audio description process. The analysis showed that the MT output had a relatively high error rate, particularly in the categories of Accuracy - mistranslation and Fluency - grammar. This seems to indicate that extensive post-editing will be needed, before the text can be used in a professional context. ; El camp de la traducció està experimentant canvis profunds. D'una banda, està patint una autèntica transformació gràcies a l'arribada i la millora constant de les noves tecnologies. De l'altra, noves formes de traducció comencen a veure la llum arran de l'evolució social i legal que exigeix que els productes i continguts que es creen siguin accessibles per a tothom. Una d'aquestes noves formes de traducció és l'audiodescripció (AD), un servei que té com a objectiu fer accessibles els continguts audiovisuals a les persones amb pèrdua de visió. La nova legislació exigeix que aquests continguts siguin accessibles abans del 2025, la qual cosa constitueix una tasca immensa atès el nombre limitat de persones que actualment tenen formació com a audiodescriptors. Una possible solució seria utilitzar la traducció automàtica per traduir les audiodescripcions ja existents a diferents idiomes. L'AD, caracteritzada per frases curtes i un llenguatge senzill i concret, podria ser una bona candidata per a la traducció automàtica. En aquest estudi pretenem demostrar la hipòtesi per a la combinació lingüística anglès-holandès. Concretament, fragments de 30 minuts d'AD de tres pel·lícules holandeses que es van audiodescriure originalment en anglès, han estat traduïts a l'holandès per mitjà de l'eina DeepL. Les traduccions s'han analitzat utilitzant la tipologia d'error harmonitzada DQF-MQM i tenint en compte la naturalesa multimodal específica del text font i la dimensió intersemiòtica del procés d'audiodescripció original. L'anàlisi ha mostrat que la producció de TA té una taxa d'error relativament alta, especialment en les categories de precisió -errors de traducció- i fluïdesa gramatical. Això sembla indicar que caldrà una extensa postedició, abans que el text es pugui utilitzar en un context professional. ; El campo de la traducción está experimentando diversos cambios profundos. Por un lado, la llegada de nuevas tecnologías en constante desarrollo la está reconfigurando completamente. Por otro, están empezando a surgir nuevas formas de traducción como resultado de avances sociales y legales que establecen que los productos y contenidos que se crean han de ser accesibles para todo el mundo. Una de estas nuevas formas de traducción es la audiodescripción (AD), un servicio orientado a hacer accesible para las personas con pérdida de visión los contenidos audiovisuales. La nueva legislación establece que estos deberán ser accesibles para 2025, lo que representa una tarea desmesurada, dado el limitado número de personas actualmente cualificadas en AD. Una posible solución pasaría por utilizar la traducción automática para traducir las audiodescripciones existentes a diferentes idiomas. Teniendo en cuenta que la AD se caracteriza por el uso de frases cortas y un lenguaje sencillo y concreto, sería una buena candidata para la traducción automática. El presente estudio ha puesto a prueba esta hipótesis con la combinación lingüística inglés-holandés. Para ello, se utilizó DeepL para traducir al holandés tres fragmentos de 30 minutos de AD extraidos de varias películas holandesas que habían sido audiodescritas en inglés. Para analizar las traducciones se utilizó la tipología de error DQF-MQM armonizada y se tuvo en cuenta la naturaleza multimodal específica del texto de origen y la dimensión intersemiótica del proceso original de audiodescripción. El análisis mostró que el resultado de la TA presentaba una tasa de error relativamente elevada, especialmente en las categorías de Exactitud/Error de traducción y Fluidez/Gramática. Esto parece indicar que, antes de que el texto pueda ser utilizado en un contexto profesional, será necesario un proceso de posedición exhaustive.

The article analyzes the political and legal aspects of the functioning of the Institute of Ukrainian presidency in the system of the higher bodies of state authority. It is determined that the quality of the head of state depends on the constitutionally established powers, normative vested rights and responsibilities as well as the individuality of a person occupying the president's position. The methodological basis of the study was the use of the historical method which allowed to identify the factors that influenced on the changes of Ukrainian polity in the period of independence, comparative method to determine the level of confidence changes of the presidents of Ukraine at the beginning of their cadence, a year later and in the year of re-election, and the system method helped determine the prospects for the development of the Institute of Presidency in Ukraine, taking into account the chosen model of the parliamentary republic.It is proved that the socio-political perception of the Ukrainians, excessive individualization and paternalism in relation to the position of the President of Ukraine directly affect the distribution of authority in the country, concentrating it in the hands of a single person. As a result of the analysis, it was possible to determine the mechanisms that can help Ukraine to avoid the processes related to the processes associated with usurpation of power, accumulation of powers in one decision-making centre at the present stage of democratization: a definitive constitutional distribution of powers, reducing the political dependence of the law enforcement system from the commander-in-chief, eliminating "immunity to corruptions" of the Ukrainians, preservation the Independence of the media and creation a nationwide state development strategy.The future of the Institute of Presidency in Ukraine for the Parliamentary Republic will be a symbolic one, limited powers and low political interest of citizens, which will positively affect the ability of the Ukrainians to a conscious and rational choices of representatives, the development of ideological multiparty system, the formation of a stable and consistent collegial cabinet of ministers and the responsible prime minister who will implement the parliamentary program and will bear responsibility in front of it. ; В статье анализируются политико-правовые аспекты функционирования института президентства Украины в системе высших органов государственной власти. Определено, что качество деятельности Главы государства зависит от конституционно установленных полномочий, нормативно закрепленных прав и обязанностей, а также особенности лица, занимающего должность Президента. Методологической основой исследования стало использование исторического метода, что позволило выявить факторы, которые повлияли на изменения формы правления Украины в период независимости, сравнительного метода для определения изменения уровня доверия к Президентам Украины в начале их каденции, через год и в год переизбрания, а системный метод помог определить перспективы развития института президентства в Украине, учитывая выбранную модель парламентской республики.Доказано, что социально-политическое восприятие украинцев, чрезмерная индивидуализация и патернализм по отношению к должности Президента Украины напрямую влияют на распределение власти в стране, концентрируя ее в одних руках. В результате анализа удалось определить механизмы, которые могут помочь Украине избежать процессов, связанных с узурпацией власти, аккумулированием полномочий в одном центре принятия решений на современном этапе демократизации: четкое конституционное распределение полномочий, снижение политической зависимости правоохранительной системы от Главнокомандующего, искоренение «иммунитета к коррупции»украинцев, сохранение независимости СМИ и выработка общенациональной стратегии развития государства.Будущее института президентства в Украине при парламентарной республике будет иметь символический характер, ограниченные полномочия и низкий политический интерес граждан, положительно повлияет на способность украинцев к сознательному и рациональному выбору своих представителей, развитию идеологической многопартийности, формированию стабильного и последовательного коллегиального Кабинета Министров и ответственного Премьер-министра, реализующего программу парламента, будучи ответственным перед ним. ; У статті аналізуються політико-правові аспекти функціонування інституту президенства України в системі вищих органів державної влади. Визначено, що якість діяльності Глави держави залежить від конституційно встановлених повноважень, нормативно закріплених прав та обов'язків, а також індивідуальності особи, що займає посаду Президента. Методологічною основою дослідження стало використання історичного методу, що дозволило виявити чинники, які вплинули на зміни форми правління України в період незалежності, порівняльного методу для визначення зміни рівня довіри до Президентів України на початку їхньої каденції, через рік та в рік переобрання, а системний метод допоміг визначити перспективи розвитку інституту президентства в Україні, враховуючи обрану модель парламентарної республіки.Доведено, що соціально-політичне сприйняття українців, надмірна індивідуалізація та патерналізм по відношенню до посади Президента України прямо впливають на розподіл влади в країні, концентруючи її в одних руках. У результаті аналізу вдалося визначити механізми, які можуть Україні допомогти уникнути процесів, пов'язаних з узурпацією влади, акумулюванням повноважень в одному центрі прийняття рішень на сучасному етапі демократизації: чіткий конституційний розподіл повноважень, зниження політичної залежності правоохоронної системи від Головнокомандувача, викорінення «імунітету до корупції» українців, збереження незалежності ЗМІ й вироблення загальнонаціональної стратегії розвитку держави.Майбутнє Інституту президенства в Україні за парламентарної республіки буде мати символічний характер, обмежені повноваження та низьку політичну цікавість громадян, що позитивно вплине на здатність українців до свідомого й раціонального вибору представників, розвитку ідеологічної багатопартійності, формуванню стабільного та послідовного колегіального Кабінету Міністрів та відповідального Прем'єр-міністра, що реалізовуватиме програму Парламенту та нестиме відповідальність перед ним.
