The impact of macroeconomic crises on the investments made by parents in the human capital of their children is a question of considerable policy importance. Analysis of the effects of the profound 1988-92 macroeconomic crisis in Peru on the schooling and employment decisions of school-age children in urban areas finds no effect on attendance rates but a significant decline in the fraction of children who are both employed and attend school. It also finds significantly higher mean educational attainment for children exposed to the crisis than for those who were not. These findings may be related: children who are not employed have more time available and may therefore put more effort into school.

The Malawi Urbanization Review aims to provide fresh perspectives on urbanization in Malawi, by analyzing the current and potential contribution of urbanization to long-term national development and the current institutional and financial capacity of local governments to manage the process. Analyses presented in this report are particularly timely as Malawi is planning for the coming half decade through the Malawi Growth and Development Strategy (MGDS) III (2016-2020). Malawi is urbanizing at a moderate rate and has a good chance of proactively managing the urbanization process. Opportunities may arise from a positive structural change that Malawi's economy is undergoing, whereby the driver of growth and job creation moves from agriculture to non-agricultural sectors. Faster urbanization, with strong linkages with rural areas, can contribute further to deepening such structural change. To unlock the potential of urbanization as a catalyst for long-term economic development, it is necessary to strengthen the capacity of urban local governments to generate revenues and meet the key infrastructure and service needs in urban areas, which remain challenging even at the current rate of urbanization.

This background paper was prepared for the East Asia Pacific aging report. The East Asia and Pacific region grew at an unparalleled rate in the past 50 years. This economic boom is partly attributable to unprecedented demographic changes in East Asia during this period. But demographics are only part of the story. The size of the economic bonus or burden which results from population aging depends on how policy influences labor force participation, savings, human capital accumulation and total factor productivity.

The nutrition cluster is a very important entity to coordinate actions during emergencies. It is important that the nutrition cluster know in advance which institutions work where and what kind of inputs are pre-positioned. Risk management plans need to be ready at the local level in advance of emergencies. Although emergencies occur every year in Guatemala, the emergency response often fails to incorporate the management of malnutrition among its priority actions. Community programs, like AIN-C (a community- based child care program - atencion integral a la ninez comunitaria), can be an efficient mechanism to protect young children's nutrition and provide an important channel for the government or other assistance agencies to funnel support to communities in need during a period of crisis or emergency. This is because community workers know the families and those who are most vulnerable; they are willing to be called upon to help their community; and they can provide educational support to families to ensure rapid recovery among young children. Community-based growth promotion programs such as AIN-C can be strengthened and scaled up, and they are a good investment, in the aftermath of an emergency or during a time of economic crisis, to swiftly deliver services to affected families. Community agents can carry important information to the community and can distribute food, nutrition and health supplements such as micronutrient powders and oral rehydration salts for young children, as well as hygiene and water purification products. Community kitchens are an efficient approach to reduce hunger among the poor during times of high economic stress. They provide a social safety net and can have a nutrition effect when carefully planned. Community kitchens adjust to the labor market and general economic conditions; hence targeting of the poor and most in need through community kitchens is self-selective. The kitchens can expand and shrink as participants continually assess the trade-off between unpaid work in exchange for free or inexpensive meals and the pursuit of opportunities in the labor market.

Social funds represent a diverse universe of World Bank projects. Social funds are defined as agencies or programs that channel grants to communities for small scale development projects. Social funds typically finance some mixture of socio economic infrastructure (e.g. building or rehabilitating schools, health centers, water supply systems,), productive investments (e.g. micro?finance and income generating projects), social services (e.g. supporting nutrition campaigns, literacy programs, youth training, support to the elderly and disabled), and capacity building programs (e.g., training for community based organizations, nongovernmental organizations, and local governments). Social Fund programs are demand driven and aim to involve the active participation of several local actors, often using a community driven development approach. The main goal is usually to address the needs of poor and vulnerable communities while building social capital and empowerment at the local level. Social funds have several features that place them in the social protection (SP) realm. They typically target poor communities and/or vulnerable households. They finance social risk management interventions like temporary employment generation and expanded access to basic services by the poorest. Social funds are often employed to address immediate post?conflict needs and responses to natural disasters.

This monograph probes the role of information in service delivery by focusing on key sectors in Kenya and Ethiopia. Findings from comparative studies done in 2005-06 in the health, education, and water and sanitation sectors plus public and private partnerships (PPPs) illustrate the significance of information access to delivery of quality services. Linking information access to service delivery is instructive to deepen institutional reforms around transparency and governance. This volume contends that three constraints-governance, trust, and technology deficits-impede information flows for service delivery in Ethiopia and Kenya. Although these constraints are formidable, the cases reveal that reforms in the governance arena have began to make a difference in the domain of service delivery. Using local researchers for this study has contributed to the World Bank's vision and mission of strengthening knowledge production by local institutions, particularly in Africa, where the low rate of the input of indigenous voices in the development debates is a growing concern. Augmenting the corpus of knowledge about African issues by Africans also fits into the objective of capacity building: providing local experts the opportunities to research and highlight experiences on the ground. As more countries embark on public sector reforms that deepen transparent information mechanisms and better service delivery, the need for more of these studies will increase. What the authors of this study have articulated is a rich research agenda that ties information access and service provision-research that should help advance policy dialogue with actors involved in public sector and governance reforms.

Mención Internacional en el título de doctor ; Smoothing techniques have become one of the most popular modelling approaches in the unidimensional and multidimensional setting. However, out-of-sample prediction in the context of smoothing models is still an open problem that can significantly widen the use of these models in many areas of knowledge. The objective of this thesis is to propose a general framework for prediction in penalized regression, particularly in the P-splines context. To that end, Chapter 1 includes a review of the different proposals available in the literature, and results useful and necessary along the document, the formulation of a P-spline model and its reparameterization as a mixed model. In Chapter 2, we generalize the approach given by Currie et al. (2004) to predict with any regression basis and quadratic penalty. For the particular case of penalties based on differences between adjacent coefficients, we reparameterize the extended P-spline model as a mixed model and we prove that the fit remains the same as the result we obtain only fitting the data and show the crucial role of the penalty order, since it determines the shape of the prediction. Moreover, we adapt available methods in contexts such as mixed models (Gilmour et al. 2004) or global optimization (Sacks et al. 1989) to predict in the context of penalized regression and prove their equivalence for the particular case of Psplines. An extensive section of examples illustrates the application of the methodology. We use three real datasets with particular characteristics: one of them on aboveground biomass allow us to show that prediction can also be performed to the left of the data; other of them, on monthly sulphur dioxide levels, illustrates how prediction can take into account the temporal trends and seasonal effects by using the smooth modulation model based on P-splines suggested by Eilers et al. (2008); and other, on yearly sea level, shows that prediction can also be carried out in the case of correlated errors. We also introduce the concept of "memory of a P-spline" as a tool to know how much of the known information we use to predict new values. In the third chapter, we propose a general framework for prediction in multidimensional smoothing, we extend the proposal of Currie et al. (2004) to predict when more than one covariate is extended. The extension of the prediction method to the multidimensional case is not straightforward in the sense that, in this context, the fit changes when the fit and the prediction are carried out simultaneously. To overcome this problem we propose an easy but elegant solution, based on Lagrange multipliers. The first part of the chapter is dedicated to show how out-of-sample predictions can be carried out in the context of multidimensional P-splines and the properties satisfied, under certain conditions, by the coefficients that determine the prediction. We also propose the use of restrictions to maintain the fit, and in general, to incorporate any known information about the prediction. The second part of the chapter is dedicated to extend the methodology to the smooth mixed model framework. It is known that when a P-spline model is reparameterized as a mixed model, the structure of the coefficients is lost, that is, they are not ordered according to the position of the knots. This fact is not relevant when we fit the data, but if we predict and impose restrictions over the coefficients, we need to differentiate between the coefficients that determine the fit and the coefficients that determine the prediction. In order to do that, we define a particular transformation matrix that preserves the original model matrices. The prediction method and the use of restrictions is illustrated with one real data example on log mortality rates of US male population. We show how to solve the crossover problem of adjacent ages when mortality tables are forecasted and compare the results with the well-known method developed in Delwarde et al. (2007). The research in Chapter 4, is motivated by the need to extend the prediction methodology in the multidimensional case to more flexible models, the so-called Smooth-ANOVA models, which allow us to include interaction terms that can be decomposed as a sum of several smooth functions. The construction of these models through B-splines basis suffer from identifiability problems. There are several alternatives to solve this problem, here we follow Lee and Durbán (2011) and reparameterize them as mixed models. The first two sections of the chapter are dedicated to introduce the Smooth-ANOVA models and to show how out-of-sample prediction can be carried out in these models. We illustrate the prediction with Smooth-ANOVA models reanalyzing the dataset on aboveground biomass. Now, the Smooth-ANOVA model allows us to represent the smooth function as the sum of a smooth function for the height, a smooth function for the diameter of a tree, and a smooth term for the height-diameter interaction. At the end of this chapter, we provide a simulation study in order to evaluate the accuracy of the 2D interaction P-spline models and Smooth-ANOVA models, with and without imposing invariance of the fit. From the results of the simulation study, we conclude that in most situations the constrained S-ANOVA model behaves better in the fit and out-of-sample predictions, however, results depend on the simulation scenario and on the number of dimensions in which the prediction is carried out (one or both dimensions). In the fifth chapter we generalize the developed methodology for generalized linear models (GLMs) in the context of P-splines (P-GLMs) and mixed models (P-GLMMs). In both frameworks, the coefficients and parameters estimation procedures involve nonlinear equations. To solve them iterative algorithms based on the Newton-Raphson methods are used, regardless of the estimation criterion used (for instance, in the GLMMs context we can maximize the residual maximum likelihood (REML) or an approximate REML (based on Laplace approximation)). These iterative algorithms are based on a working normal theory model or a set of pseudodata and weights. Based on this idea, we extend the Penalized Quasilikelihood method (PQL) to fit and predict simultaneously in the context of GLMM. We highlight that, in the context of mixed models (even in the univariate case), to maintain the fit a transformation that preserves the original model matrices has to be used, since different transformations deal with different working vectors and therefore with different solutions. We also show how restrictions can be imposed in P-GLMs and P-GLMMs models. To illustrate the procedures we use a real dataset to predict deaths due to respiratory disease through 2D interaction P-splines and S-ANOVA models (both with and without the restriction the fit has to be maintained). Finally, Chapter 6 is devoted to summarize the main conclusions and pose a list of future lines of work. ; Las técnicas de suavizado se han convertido en uno de los enfoques de modelado más populares en el entorno unidimensional y multidimensional. Sin embargo, la predicción fuera del rango de valores conocidos en el contexto de los modelos de suavizado sigue siendo un problema abierto que puede ampliar significativamente el uso de estos modelos en muchas áreas de conocimiento. El objetivo de este documento es proponer un marco general para la predicción en regresión penalizada, particularmente en el contexto de P-splines. Con ese fin, el Capítulo 1 incluye una revisión de las diferentes propuestas disponibles en la literatura y los resultados útiles y necesarios a lo largo del documento, la formulación de un modelo P-spline y su reparametrización como modelo mixto. En el Capítulo 2, generalizamos el enfoque dado por Currie et al. (2004) para predecir con cualquier base de regresión y penalización cuadrática. Para el caso particular de penalizaciones basadas en diferencias entre coeficientes adyacentes, reparametrizamos el modelo P-spline extendido como un modelo mixto y demostramos que el ajuste sigue siendo el mismo que el resultado que obtenemos solo ajustando los datos, también mostramos el papel crucial del orden de penalización, ya que determina la forma de la predicción. Además, adaptamos los métodos disponibles en contextos como modelos mixtos (Gilmour et al. 2004) u optimización global (Sacks et al. 1989) predecir en el contexto de la regresión penalizada y demostramos su equivalencia para el caso particular de P-splines. Una extensa sección de ejemplos ilustra la aplicación de la metodología. Utilizamos tres conjuntos de datos reales con características particulares: uno de ellos, sobre biomasa, nos permite mostrar que la predicción también se puede realizar a la izquierda de los datos; otro de ellos, sobre los niveles mensuales de dióxido de azufre, ilustra como la predicción puede tener en cuenta las tendencias temporales y los efectos estacionales utilizando el modelo de modulación suave basado en P-splines sugerido por Eilers et al. (2008); y otro, sobre el nivel anual del mar, muestra que la predicción también se puede realizar en el caso de errores correlacionados. También presentamos el concepto de "memoria de un P-spline" como una herramienta para saber cuánta información conocida usamos para predecir nuevos valores. En el tercer capítulo, proponemos un marco general para la predicción en el suavizado multidimensional, ampliamos la propuesta de Currie et al. (2004) para predecir cuando se extiende más de una covariable. La extensión del método de predicción al caso multidimensional no es directa en el sentido de que, en este contexto, el ajuste cambia cuando el ajuste y la predicción se llevan a cabo simultáneamente. Para resolver este problema, proponemos una solución fácil, basada en multiplicadores de Lagrange. La primera parte del capítulo está dedicada a mostrar cómo se pueden realizar predicciones fuera de la muestra en el contexto de P-splines multidimensionales y las propiedades que satisfacen, bajo ciertas condiciones, los coeficientes que determinan la predicción. También proponemos el uso de restricciones para mantener el ajuste y, en general, para incorporar cualquier información conocida sobre la predicción. La segunda parte del capítulo está dedicada a extender la metodología al marco de modelos mixtos suaves. Se sabe que cuando un modelo de P-spline se reparametriza como un modelo mixto, la estructura de los coeficientes se pierde, es decir, no se ordenan de acuerdo con la posición de los nodos. Este hecho no es relevante cuando ajustamos los datos, pero si predecimos e imponemos restricciones sobre los coeficientes, necesitamos diferenciar entre los coeficientes que determinan el ajuste y los coeficientes que determinan la predicción. Para hacer eso, definimos una matriz de transformación particular que conserva las matrices del modelo original. El método de predicción y el uso de restricciones se ilustran con un ejemplo de datos reales sobre el logaritmo de las tasas de mortalidad de la población masculina estadounidense. Mostramos como resolver el problema de cruce de proyecciones edades adyacentes cuando se predicen tablas de mortalidad y comparamos los resultados con el método desarrollado en Delwarde et al. (2007). La investigación en el Capítulo 4 está motivada por la necesidad de extender la metodología de predicción en el caso multidimensional a modelos más flexibles, los modelos Smooth-ANOVA, que nos permiten incluir términos de interacción que pueden descomponerse como una suma de varias funciones suaves. La construcción de estos modelos a través de B-splines tiene problemas de identificabilidad. Hay varias alternativas para resolver este problema, nosotros seguimos Lee and Durbán (2011) y lo reparametrizamos como modelos mixtos. Las primeras dos secciones del capítulo están dedicadas a presentar los modelos Smooth-ANOVA y mostrar cómo se puede llevar a cabo la predicción fuera del rango de valores observados en estos modelos. Ilustramos la predicción con modelos Smooth-ANOVA reanalizando el conjunto de datos sobre biomasa. Ahora, el modelo Smooth-ANOVA nos permite representar la función suave como la suma de una función suave para la altura, un término suave para el diámetro y una función suave para la interacción altura-diámetro. Al final de este capítulo, proporcionamos un estudio de simulación para evaluar la precisión de los modelos de interacción 2D P-spline y los modelos Smooth-ANOVA, con y sin imponer la invariancia del ajuste. A partir de los resultados del estudio de simulación, concluimos que en la mayoría de las situaciones el modelo S-ANOVA restringido se comporta mejor tanto en el ajuste como en la predicción, sin embargo, los resultados dependen del escenario de simulación y del número de dimensiones en las que se realiza la predicción (una o ambas dimensiones). En el quinto capítulo generalizamos la metodología desarrollada para modelos lineales generalizados (GLM) en el contexto de P-splines (P-GLM) y modelos mixtos (P-GLMM). En ambos marcos, los procedimientos de estimación de coeficientes y parámetros involucran ecuaciones no lineales. Para resolverlos, se utilizan algoritmos iterativos basados en los métodos de Newton-Raphson, independientemente del criterio de estimación utilizado (por ejemplo, en el contexto de GLMMs podemos maximizar la máxima verosimilitud residual (REML) o un REML aproximado (basado en la aproximación de Laplace)). Estos algoritmos iterativos se basan en un modelo teórico normal o en un conjunto de pseudodatos y pesos. Basándonos en esta idea, ampliamos el método Penalized Quasilikelihood (PQL) para ajustar y predecir simultáneamente en el contexto de GLMMs. Destacamos que, en el contexto de modelos mixtos (incluso en el caso univariante), para mantener el ajuste, se debe utilizar una transformación que conserve las matrices del modelo original, ya que las diferentes transformaciones tratan con diferentes vectores de trabajo y, por lo tanto, con diferentes soluciones. También mostramos como se pueden imponer restricciones en los modelos P-GLM y P-GLMM. Para ilustrar los procedimientos, utilizamos un conjunto de datos real para predecir las muertes por enfermedad respiratoria a través de modelos 2D P-splines y modelos S-ANOVA (con y sin la restricción el ajuste debe mantenerse). The article analyzes a scientific literature, international and national normative and legal documents, medical and statistical data that determine the topicality of the problem, and medical-social, bioethical, legal aspects of euthanasia and palliative and hospice care (PHC) as an alternative to euthanasia. Objective: To present an analysis of the situation in Ukraine and worldwide on regulatory assessment and public attitudes to euthanasia and its alternatives – PHC, strategies and approaches to the PHC service development within to reform the of the Public Health System in Ukraine. Materials research. The material for the study presented in the article are WHO documents and the international organizations, legislation of Ukraine, the domestic and foreign scientific literature, international and domestic legal documents relating euthanasia and PHC, medical statistics and reports of the State Statistics Committee and Ministry of Health of Ukraine. Research methods. The authors used the following methods: information-analytical, bibliosemantyc, systematic approach, structural and logical analysis and comparative content analysis. Results and discussion. Effective and accessible the PHC system using a multidisciplinary and intersectoral approach can adequately respond and meet the needs of palliative patients and their families, which helps to ensure proper quality of life and reduces the desire to palliative patients on the application of euthanasia. It defines the great medical and social, economic and humanitarian values of PHC – this innovative Health Care and Social Welfare approach, which is an important component of Public Health System reform in Ukraine. The most important are international regulations that say inalienable human rights and which, in terms of most lawyers, prohibiting euthanasia, although euthanasia is in legalization and using in some countries. The law clearly prohibits euthanasia in Ukraine. Conclusions. 1. The ensuring the maximum attainable quality of life level for palliative patients with limited life expectancy forecast in the last decade is a major humanitarian problem of modern society, due to the adverse medical and demographic processes, including the populations aging and mortality from chronic non-communicable diseases increased. 2. The above situation requires the optimal organization and development of modern efficient and accessible of PHC System, which is an innovative medical and social multidisciplinary approach that most adequately meets the requirements and adequate quality of life of palliative patients and their families, contributes to the preservation of human dignity at the end of life. 3. In accordance with modern international concepts and standards, PHC is required to provide four components: medical, psychological, social and spiritual. 4. Today, the most important are international regulations that say inalienable human rights and which, in terms of most lawyers, prohibiting euthanasia, although in some countries there is a tendency to euthanasia legalize and use. 5. Existing law clearly prohibits an euthanasia in Ukraine. 6. The above situation requires the creation and development efficient and effective PHC System as an euthanasia alternative in Ukraine. ; В статье анализируются научная литература, международные и отечественные нормативно-правовые документы, медико-статистические данные, определяющие актуальность проблемы, медико-социальные, биоэтические, правовые аспекты эутаназии и паллиативной и хосписной помощи (ПХП) как альтернативы эутаназии. Цель работы: представить анализ ситуации в Украине и в мире касательно нормативно-правовой оценки и отношения общества к эутаназии и ее альтернативы - ПХП, стратегий и подходов к развитию службы ПХП в рамках реформирования системы общественного здравоохранения в Украине. Материалы исследования. Материалами для исследования при выполнении представленной работы были документы ВОЗ и международных организаций, законодательная база Украины, отечественная и зарубежная научная литература, международные и отечественные нормативно-правовые документы, касающиеся эутаназии и ПХП, медико-статистические данные Госкомитета статистики и отчеты МЗ Украины. Методы исследования. Авторами были использованы следующие методы: информационно-аналитический, библиосемантический, системного подхода, структурно-логического анализа и сравнительного контент-анализа. Результаты исследования и обсуждение. Эффективная и доступная система ПХП, применяя мультидисциплинарный и межсекторальный подход может адекватно реагировать и удовлетворять потребности паллиативных пациентов и их родных, что способствует обеспечению надлежащего качества жизни и уменьшает желание у инкурабельных больных к применению эутаназии. Это определяет большое медико-социальное, экономическое и гуманитарное значение ПХП – этого инновационного направления здравоохранения и социальной защиты населения, является важной составляющей реформирования системы здравоохранения в Украине. Наиболее важными являются международные нормативно-правовые акты, утверждающие неотъемлемые права человека, и которые, с точки зрения большинства юристов, запрещают эутаназию, хотя в некоторых странах имеет место легализация и применение эутаназии. Законодательство Украины однозначно запрещает эутаназию. Выводы. 1. Обеспечение максимально достижимого уровня качества жизни инкурабельных больных с ограниченным прогнозом продолжительности жизни стало в последние десятилетия одной из важнейших гуманитарных проблем современного общества, что обусловлено неблагоприятными медико-демографическими процессами, в частности старением населения и увеличением смертности от хронических неинфекционных заболеваний. 2. Указанное требует хорошей организации и развития современной эффективной и доступной системы ПХП, которая является инновационным медико-социальным мультидисциплинарным подходом, который наиболее адекватно обеспечивает потребности и надлежащее качество жизни паллиативных пациентов и их родных, способствует сохранению человеческого достоинства в конце биологической жизни. 3. В соответствии с современными международными концепциями и стандартами, ПХП должна обеспечить 4 обязательных составляющих: медицинскую, психологическую, социальную и духовную. 4. Сегодня наиболее важными являются международные нормативно-правовые акты, утверждающие неотъемлемые права человека, и которые, с точки зрения большинства юристов, запрещают эутаназию, хотя в некоторых странах наблюдается тенденция к легализации и применению эутаназии. 5. Действующее законодательство Украины однозначно запрещает эутаназию. 6. Указанное требует создания и развития в Украине доступной и эффективной системы оказания ПХП населению как альтернативы эутаназии. ; У статті аналізуються наукова література, міжнародні та вітчизняні нормативно-правові документи, медико-статистичні дані, що визначають актуальність проблеми, медико-соціальні, біоетичні, правові аспекти еутаназії та паліативної і хоспісної допомоги (ПХД) як альтернативи еутаназії. Мета роботи: представити аналіз ситуації в Україні і у світі щодо нормативно-правової оцінки і ставлення суспільства до еутаназії та її альтернативи – ПХД, стратегій та підходів до розвитку служби ПХД в рамках реформування системи охорони громадського здоров'я в Україні. Матеріали дослідження. Матеріалами для дослідження при виконанні представленої роботи були документи ВООЗ та міжнародних організацій, законодавча база України, вітчизняна та закордонна наукова література, міжнародні та вітчизняні нормативно-правові документи, що стосуються еутаназії та ПХД, медико-статистичні дані Держкомітету статистики та звіти МОЗ України. Методи дослідження. Авторами були використані наступні методи: інформаційно-аналітичний, бібліосемантичний, системного підходу, структурно-логічного аналізу та порівняльного контент-аналізу. Результати дослідження та обговорення. Ефективна і доступна система ПХД, застосовуючи мультидисциплінарний та міжсекторальний підхід, може адекватно реагувати та задовольняти потреби паліативних пацієнтів та їхніх рідних, що сприяє забезпеченню належної якості життя і зменшує бажання в інкурабельних хворих щодо застосування еутаназії. Це визначає велике медико-соціальне, економічне та гуманітарне значення ПХД – цього інноваційного напрямку охорони здоров'я і соціального захисту населення, яка є важливою складовою реформування системи охорони здоров'я в Україні. Найбільш важливими є міжнародні нормативно-правові акти, що стверджують невід'ємні права людини, і які, з точки зору більшості юристів, забороняють еутаназію, хоча в деяких країнах має місце легалізація та застосування еутаназії. Законодавство України однозначно забороняє еутаназію. Висновки. 1. Забезпечення максимально досяжного рівня якості життя інкурабельних хворих з обмеженим прогнозом тривалості життя стало в останні десятиліття однією з найважливіших гуманітарних проблем сучасного суспільства, що зумовлено несприятливими медико-демографічними процесами, зокрема постарінням населення і збільшенням смертності від хронічних неінфекційних захворювань. 2. Зазначене потребує оптимальної організації та розвитку сучасної ефективної та доступної системи ПХД, яка є інноваційним медико-соціальним мультидисциплінарним підходом, що найбільш адекватно забезпечує потреби та належну якість життя паліативних пацієнтів та їхніх рідних, сприяє збереженню людської гідності наприкінці біологічного життя. 3. Відповідно до сучасних міжнародних концепцій та стандартів, ПХД має забезпечити 4 обов'язкових складових: медичну, психологічну, соціальну і духовну. 4. Сьогодні найбільш важливими є міжнародні нормативно-правові акти, що стверджують невід'ємні права людини, і які, з точки зору більшості юристів, забороняють еутаназію, хоча в деяких країнах спостерігається тенденція до легалізації та застосування еутаназії. 5. Чинне законодавство України однозначно забороняє еутаназію. 6. Зазначене потребує створення та розвитку в Україні доступної та ефективної системи надання ПХД населенню як альтернативи еутаназії.

Background. The COVID-19 pandemic, caused by the SARS-CoV-2 coronavirus, was announced by the World Health Organization on March 11, 2020 and identified as having a significant negative impact on the functioning of health systems and the economic development of countries. To date, we know the impact of the pandemic on the decrease in the availability of medical care, difficulties with the annual monitoring and screening of socially significant diseases. But we will be able to fully assess the medical, clinical and psychosocial consequences of the pandemic, which are just beginning to manifest themselves, only in the coming years. The purpose was to study and analyze the features of clinical manifestations, diagnosis, treatment, immunity and social significance of the COVID-19 pandemic in Ukraine. Materials methods. The available current normative and literary sources were investigated. Methods: bibliosemantic - for studying the lokal and world science and information space on the problem under study; a systematic approach and analysis - for a comprehensive study of an object, a subject in a systemic relationship, for analyzing problematic issues, ways to solve them. Results. The medical feature of COVID-19 in Ukraine is the two-stage course of the disease 1-10 days - active replication of the virus; 8-14 days or longer - immune dysfunction (eg, respiratory problems, other serious conditions). Often recorded injury of the lungs in the form of pneumonitis, although it is mainly the disease of mild or moderate severity. Diagnostic is based on the detection of the RNA of the virus using PCR or immunoglobulins by ELISA. Treatment is mostly symptomatic. The social significance of the COVID-19 pandemic was reflected in the increase in patients with mental disorders, victims of domestic violence, deterioration in the health of the population due to the limited availability of many types of medical services, increased workload and housework, decreased income and a deterioration in the quality of life. Conclusions. The COVID-19 pandemic over the year of its existence has caused changes in many spheres of life of the world's population, and Ukraine is not an exception. Disease COVID-19 has nonspecific symptoms, can proceed under the mask of many pathological conditions. especially in the presence of comorbid diseases. The pathogenetic features of the course of COVID-19 are poorly understood, which limits the possibilities of effective etiopathogenetic therapy. The virus constantly mutates and leaves an unstable and short-lived immunity, which explains its diversity of the clinical symproms in different populations. In addition to a direct increase in morbidity and mortality due to COVID-19, the virus can affect the psychoemotional state of people, reduce income and deterioration in nutrition, especially among the most vulnerable segments of the population, which requires taking these aspects into account when making decisions at the legislative level with an emphasis on addressing gender and social inequality. ; Актуальность. Пандемия COVID-19, причиной которого является коронавирус SARS-CoV-2, объявлена Всемирной организацией здравоохранения 11 марта 2020 и определена как такая, что нанесла значительное негативное влияние на функционирование систем здравоохранения и экономическое развитие стран. На сегодняшний день известно влияние пандемии на снижение доступности медицинской помощи, трудности с проведением ежегодного мониторинга и скрининга социально значимых болезней. Но в полной мере оценить медицинские, клинические и психосоциальные последствия пандемии, которые только начинают проявляться, мы сможем лишь в последующие годы. Цель: исследовать и проанализировать особенности клинических проявлений, диагностики, лечения, иммунитета и социального значения пандемии COVID-19 в Украине. Материалы и методы. Исследованы имеющиеся текущие нормативные и литературные источники. Методы: библиосемантичний - для изучения отечественного и мирового науковоинформацийного пространства по исследуемой проблеме; системного подхода и анализа - для комплексного исследования объекта, предмета в системной взаимосвязи, для анализа проблемных вопросов, путей их решения. Результаты. Медицинские особенности COVID-19 в Украине заключаются в двохстадийности течения заболевания 1-10 день - активная репликация вируса; 8-14 день или дольше - иммунная дисфункция (например, респираторные нарушения, другие тяжелые состояния). Часто фиксируется поражения легких в виде пневмонита, хотя преимущественно заболевание имеет легкий или среднетяжелое течение. Диагностика основывается на выявлении РНК вируса с помощью ПЦР или имунонглобулинив методом ИФА. Лечение преимущественно носит симптоматический характер. Социальное значение пандемии COVID-19 отразилось на увеличении пациентов с нарушением психического состояния, жертв насилия в семье, ухудшении состояния здоровья населения в связи с ограничением доступности многих видов медицинской помощи, увеличении нагрузки и работы по дому, уменьшении доходов и ухудшении качества жизни. Выводы. Пандемия COVID-19 за год своего существования вызвала изменения во многих сферах жизни населения земного шара, и Украина не есть исключением. Заболевания COVID-19 имеет неспецифические симптомы, может протекать под маской многих патологических состояний. особенно при наличии коморбидных заболеваний. Патогенетические особенности течения COVID-19 недостаточно изучены, что ограничивает возможности эффективной этиопатогенетической терапии. Вирус постоянно мутирует и оставляет неустойчивый и непродолжительный иммунитет, что объясняет его разнообразие клинической картины в разных популяциях. Кроме непосредственного увеличения заболеваемости и смертности за счет COVID-19, вирус может влиять на психоэмоциональное состояние людей, уменьшение внимания по борьбе с социально значимыми неинфекционными заболеваниями (сердечно-сосудистые и онкологические), вызвать значительные социально-экономические последствия - увеличение безработицы, насилия, уменьшение доходов и ухудшение питания, особенно среди наиболее уязвимых слоев населения, что требует учета этих аспектов при принятии решений на законодательном уровне с акцентом на решение проблем гендерного и социального неравенства. ; Актуальність. Пандемія COVID-19, причиною якого є коронавірус SARS-CoV-2, оголошена Всесвітньою організацією охорони здоров'я 11 березня 2020 року і визначена як така, що нанесла значний негативний вплив на функціонування системам охорони здоров'я та економічний розвиток країн. Насьогодні відомий вплив пандемії на зниження доступності медичної допомоги, труднощі з проведенням щорічного моніторингу та скринінгу соціально значущих хвороб. Але в повній мірі оцінити медичні, клінічні й психосоціальні наслідки пандемії, які лише починають проявлятися, ми зможемо в наступні роки. Мета: дослідити та проаналізувати особливості клінічних проявів, діагностики, лікування, імунітету та соціального значення пандемії COVID-19 в Україні. Матеріали та методи. Досліджені наявні поточні нормативні та літературні джерела. Методи: бібліосемантичний – для вивчення вітчизняного і світового науковоінформаційного простору з досліджуваної проблеми; системного підходу та аналізу – для комплексного дослідження об'єкта, предмета у системному взаємозв'язку, для аналізу проблемних питань, шляхів їх вирішення. Результати. Медичні особливості COVID-19 в Україні полягають у двохстадійності перебігу захворювання: 1–10 день - активна реплікація вірусу; 8–14 день або довше - імунна дисфункція (наприклад, респіраторні порушення, інші важкі стани). Часто фіксується ураження легень у вигляді пневмоніту, хоча переважно захворювання має легкий чи середньоважкий перебіг. Діагностика засновується на виявленні РНК вірусу за допомогою ПЦР або імунонглобулінів методом ІФА. Лікування переважно носить симптоматичний характер. Соціальне значення пандемії COVID-19 відобразилось у збільшенні пацієнтів з порушенням психічного стану, жертв насильства у сімї, погіршення стану здоров'я населення у зв'язку з обмеженням доступності багатьох видів медичної допомоги, збільшення навантаження та роботи по дому, зменшення доходів та погіршення якості життя. Висновки. Отже, пандемія COVID-19 за рік свого існування спричинила зміни в багатьох сферах життя населення земної кулі, і Україна не є виключенням. Захворювання COVID-19 має неспецифічні симптоми, може перебігати під маскою багатьох патологічних станів. особливо за наявності коморбідних захворювань. Патогенетичні особливості перебігу COVID-19 недостатньо вивчені, що обмежує можливості ефективної етіопатогенетичної терапії. Вірус постійно мутує і залишає нестійкий та нетривалий імунітет, що пояснює його різноманітність клінічної картини в різних популяціях. Окрім безпосереднього збільшення захворюваності і смертності за рахунок COVID-19, вірус має значний вплив на психоемоційний стан людей, зменшення уваги щодо боротьби з соціально значимими неінфекційними захворюваннями (серцево-судинні та онкологічні), спричинив значні соціально-економічні наслідки - збільшення безробіття, насильства, зменшення доходів і погіршення харчування, особливо серед найбільш вразливих верств населення, що вимагає врахування цих аспектів при прийнятті рішень на законодавчому рівні з акцентом на вирішення проблем гендерної та соціальної нерівності.

Eye health and vision have widespread and profound implications for many aspects of life, health, sustainable development, and the economy. Yet nowadays, many people, families, and populations continue to suffer the consequences of poor access to high-quality, affordable eye care, leading to vision impairment and blindness. In 2020, an estimated 596 million people had distance vision impairment worldwide, of whom 43 million were blind. Another 510 million people had uncorrected near vision impairment, simply because of not having reading spectacles. A large proportion of those affected (90%), live in low-income and middle-income countries (LMICs). However, encouragingly, more than 90% of people with vision impairment have a preventable or treatable cause with existing highly cost-effective interventions. Eye conditions affect all stages of life, with young children and older people being particularly affected. Crucially, women, rural populations, and ethnic minority groups are more likely to have vision impairment, and this pervasive inequality needs to be addressed. By 2050, population ageing, growth, and urbanisation might lead to an estimated 895 million people with distance vision impairment, of whom 61 million will be blind. Action to prioritise eye health is needed now. This Commission defines eye health as maximised vision, ocular health, and functional ability, thereby contributing to overall health and wellbeing, social inclusion, and quality of life. Eye health is essential to achieve many of the Sustainable Development Goals (SDGs). Poor eye health and impaired vision have a negative effect on quality of life and restrict equitable access to and achievement in education and the workplace. Vision loss has substantial financial implications for affected individuals, families, and communities. Although high-quality data for global economic estimates are scarce, particularly for LMICs, conservative assessments based on the latest prevalence figures for 2020 suggest that annual global productivity loss from vision impairment is approximately US$410·7 billion purchasing power parity. Vision impairment reduces mobility, affects mental wellbeing, exacerbates risk of dementia, increases likelihood of falls and road traffic crashes, increases the need for social care, and ultimately leads to higher mortality rates. By contrast, vision facilitates many daily life activities, enables better educational outcomes, and increases work productivity, reducing inequality. An increasing amount of evidence shows the potential for vision to advance the SDGs, by contributing towards poverty reduction, zero hunger, good health and wellbeing, quality education, gender equality, and decent work. Eye health is a global public priority, transforming lives in both poor and wealthy communities. Therefore, eye health needs to be reframed as a development as well as a health issue and given greater prominence within the global development and health agendas. Vision loss has many causes that require promotional, preventive, treatment, and rehabilitative interventions. Cataract, uncorrected refractive error, glaucoma, age-related macular degeneration, and diabetic retinopathy are responsible for most global vision impairment. Research has identified treatments to reduce or eliminate blindness from all these conditions; the priority is to deliver treatments where they are most needed. Proven eye care interventions, such as cataract surgery and spectacle provision, are among the most cost-effective in all of health care. Greater financial investment is needed so that millions of people living with unnecessary vision impairment and blindness can benefit from these interventions. Lessons from the past three decades give hope that this challenge can be met. Between 1990 and 2020, the age-standardised global prevalence of blindness fell by 28·5%. Since the 1990s, prevalence of major infectious causes of blindness—onchocerciasis and trachoma—have declined substantially. Hope remains that by 2030, the transmission of onchocerciasis will be interrupted, and trachoma will be eliminated as a public health problem in every country worldwide. However, the ageing population has led to a higher crude prevalence of age-related causes of blindness, and thus an increased total number of people with blindness in some regions. Despite this progress, business as usual will not keep pace with the demographic trends of an ageing global population or address the inequities that persist in each country. New threats to eye health are emerging, including the worldwide increase in diabetic retinopathy, high myopia, retinopathy of prematurity, and chronic eye diseases of ageing such as glaucoma and age-related macular degeneration. With the projected increase in such conditions and their associated vision loss over the coming decades, urgent action is needed to develop innovative treatments and deliver services at a greater scale than previously achieved. Good eye health at the community and national level has been marginalised as a luxury available to only wealthy or urban areas. Eye health needs to be urgently brought into the mainstream of national health and development policy, planning, financing, and action. The challenge is to develop and deliver comprehensive eye health services (promotion, prevention, treatment, rehabilitation) that address the full range of eye conditions within the context of universal health coverage. Accessing services should not bring the risk of falling into poverty and services should be of high quality, as envisaged by the WHO framework for health-care quality: effective, safe, people-centred, timely, equitable, integrated, and efficient. To this framework we add the need for services to be environmentally sustainable. Universal health coverage is not universal without eye care. Multiple obstacles need to be overcome to achieve universal coverage for eye health. Important issues include complex barriers to availability and access to quality services, cost, major shortages and maldistribution of well-trained personnel, and lack of suitable, well maintained equipment and consumables. These issues are particularly widespread in LMICs, but also occur in underserved communities in high-income countries. Strong partnerships need to be formed with natural allies working in areas affected by eye health, such as non-communicable diseases, neglected tropical diseases, healthy ageing, children's services, education, disability, and rehabilitation. The eye health sector has traditionally focused on treatment and rehabilitation, and underused health promotion and prevention strategies to lessen the impact of eye disease and reduce inequality. Solving these problems will depend on solutions established from high quality evidence that can guide more effective implementation at scale. Evidence-based approaches will need to address existing deficiencies in the supply and demand. Strategic investments in discovery research, harnessing new findings from diverse fields, and implementation research to guide effective scale up are needed globally. Encouragingly, developments in telemedicine, mobile health, artificial intelligence, and distance learning could potentially enable eye care professionals to deliver higher quality care that is more plentiful, equitable, and cost-effective. This Commission did a Grand Challenges in Global Eye Health prioritisation exercise to highlight key areas for concerted research and action. This exercise has identified a broad set of challenges spanning the fields of epidemiology, health systems, diagnostics, therapeutics, and implementation. The most compelling of these issues, picked from among 3400 suggestions proposed by 336 people from 118 countries, can help to frame the future research agenda for global eye health. In this Commission, we harness lessons learned from over two decades, present the growing evidence for the life-transforming impact of eye care, and provide a thorough understanding of rapid developments in the field. This report was created through a broad consultation involving experts within and outside the eye care sector to help inform governments and other stakeholders about the path forward for eye health beyond 2020, to further the SDGs (including universal health coverage), and work towards a world without avoidable vision loss. The next few years are a crucial time for the global eye health community and its partners in health care, government, and other sectors to consider the successes and challenges encountered in the past two decades, and at the same time to chart a way forward for the upcoming decades. Moving forward requires building on the strong foundation laid by WHO and partners in VISION 2020 with renewed impetus to ultimately deliver high quality universal eye health care for all.

Introduction. Chronic kidney disease (CKD) is commonly diagnosed in patients with cardiovascular diseases (CVDs) and also manifests itself in most patients with urolithiasis. Numerous studies have shown that renal dysfunction is not only directly related to the high risk of developing various CVDs and chronic heart failure (CHF) as one of the most common complications but also the mortality rate in comorbid patients. CKD and CHF have similar pathogenetic mechanisms and common target organs; co-existing, both pathological conditions accelerate the progression of major diseases and significantly aggravate their course. In patients with recurrent nephrolithiasis combined with CVDs, all the causes leading to the formation of CKD (recurrent obstructive pyelonephritis, nephroangiosclerosis, etc.) are present to some extent.Purpose of the study. To evaluate the incidence and characteristics of CKD in patients suffering from recurrent urolithiasis associated with CVDs.Materials and methods. The prospective study included 406 patients who were treated for recurrent nephrolithiasis and concomitant CVDs from 2007 to 2020 (Urology Division, Burdenko Principal Military Clinical Hospital). From long-term follow-up respondents who lived at least 10 years after inclusion in the study (n = 52), three groups were formed: group I (n = 18) included patients with a combination of essential hypertension (EH) and ischemic heart disease (IHD), complicated by CHF; group II (n = 15) consisted of patients with uncomplicated CVDs (EH – 7 patients, IHD – 8 patients). The control group III (n = 19) included respondents suffering from nephrolithiasis without CVDs. The glomerular filtration rate (GFR) was determined by the CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) following the Russian National Guidelines for «Chronic Kidney Disease». The analysis of the obtained data was carried out using Statistica 8.0; the Fisher and Wilcoxon criteria were calculated; the differences were considered significant at p < 0.05.Results. All patients included in the study were repeatedly hospitalized urgently and as planned and underwent at least one non-invasive manipulation or surgery. The average age of the patients was 58.9 ± 2.95 years; men predominated (~ 75 – 78%). A GFR decrease was recorded in 41.1% of patients included in the study, in 40.5% of patients with a combination of nephrolithiasis and uncomplicated CVDs, Also, its decrease was found in 60 (58.8%) of patients with chronic heart failure (CHF) in 41.1% of cases from the general sample and 40.5% of patients without CHF. CKD stage II occurred in 44 (43.1%) cases of CHF; CKD stages III Ca and Cb were detected in 10 (9.8%) and 4 (1%) cases, respectively; CKD stage IV developed in 1 (0.25%) patient with one of the re-hospitalizations. Of the 52 patients included in the second study part, the ratio of men and women was 41/11 (78.8 and 21.2%, respectively). All three groups were also dominated by men. The initial values of GFR in group I patients significantly differed from those in the control group; in group II, statistically significant differences appeared 4 years after the s the study initiation, and in group I – after 2 years. A sharp (1.5-fold) significant decrease in renal filtration function was registered in group I by the 6th research year, in group II (1.3-fold) – by the 8th research year, and in group III (1.28-fold) – only by the 10th research year. The GFR level in group I and group II decreased during the 1st follow-up year by 2.36 and 1.65 times, respectively.Conclusion. CKD in patients suffering from recurrent nephrolithiasis in combination with IHD and EH is generally benign. The progression rate of filtration deficiency is relatively low and is (at least in the early stages) about 4.5 ml/min per year. The addition of CHF increases the rate of decline in renal filtration function by up to 25% (from 4 ml/min per year to 5 ml/min per year). The main negative effect of concomitant CVDs (especially complicated CHF) is not an ultrahigh decrease in GFR but a reduction in kidney functioning stable period up to complete cessation. ; Введение. Хроническая болезнь почек (ХБП), часто диагностируемая у пациентов с сердечно-сосудистыми заболеваниями (ССЗ), развивается и у большинства пациентов с мочекаменной болезнью. Многочисленные исследования показывают, что нарушение почечной функции не только напрямую связано с высоким риском развития различных ССЗ и с одним из наиболее часто встречающимся их осложнением (хронической сердечной недостаточностью (ХСН)), но и уровнем смертности у коморбидных пациентов. ХБП и ХСН имеют схожие патогенетические механизмы и общие органы-«мишени»; сосуществуя, оба патологических состояния ускоряют прогрессирование основных заболеваний и значительно отягощают их течение. У пациентов с рецидивирующим нефролитиазом, сочетающимся с ССЗ, присутствуют в той или иной степени все причины, приводящие к формированию ХБП (рецидивирующий обструктивный пиелонефрит; нефроангиосклероз и др.).Цель исследования. Оценить частоту и характеристики ХБП у пациентов с рецидивирующей мочекаменной болезнью, сочетающейся с ССЗ.Материалы и методы. В проспективное исследование было включено 406 больных, находившихся на лечении в урологическом отделении филиала № 1 ФГБУ «ГВКГ им. Н.Н. Бурденко» Минобороны России, по поводу рецидивирующего нефролитиаза и сопутствующими ССЗ в период с 2007 по 2020 год. Из длительно наблюдавшихся респондентов, проживших не менее 10 лет с момента включения в исследование (n = 52), были сформированы три группы: I (n = 18), в которую были включены пациенты с сочетанием гипертонической болезни (ГБ) и ишемической болезни сердца (ИБС), осложненных ХСН; II группа (n = 15) состояла из больных с неосложненными ССЗ (ГБ –7 человек, ИБС – 8). В III (контрольную) группу (n = 19) были включены респонденты, страдающие нефролитиазом без ССЗ. Скорость клубочковой фильтрации (СКФ) в соответствии с Национальными рекомендациями «Хроническая болезнь почек» определяли по формуле CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration). Анализ полученных данных проводили с помощью программы Statistica 8.0; вычисляли критерии Фишера и Уилкоксона; различия считали достоверными при p < 0,05.Результаты. Все пациенты, включённые в исследование, были неоднократно госпитализированы по экстренным и плановым показаниям и перенесли как минимум одну неинвазивную манипуляцию или оперативное вмешательство. Средний возраст пациентов был равен 58,9 ± 2,95 лет; преобладали мужчины (~ 75–78%). Снижение СКФ зафиксировано у 41,1% пациентов, включённых в исследование, у 40,5% – с сочетанием МКБ и неосложнённых ССЗ, а также у 60 (58,8%) больных с ХСН в 41,1% случаев из общей выборки, у 40,5% пациентов без ХСН. В 44 (43,1%) случаях при ХСН имела место II стадия ХБП; III стадия, категория Са – в 10 (9,8%), категория Сб – в 4 (1%); IV стадия при одной из повторных госпитализаций развилась у 1 (0,25%) больного. Из 52-х пациентов, включённых во вторую часть исследования, соотношение мужчин и женщин составило 41 / 11 (78,8 и 21,2% соответственно). Во всех трёх группах также преобладали мужчины. Исходные значения СКФ у пациентов I группы достоверно отличались от таковых группы контроля; во II группе статистически значимые различия появились через четыре года от начала исследования, а в I – уже через два года. Резкое (в 1,5 раза) достоверное снижение фильтрационной функции почек зарегистрировано в I группе к 6-му году исследования, во II (в 1,3 раза) – к 8-му, а в III (в 1,28 раза) – только к 10-му. В I группе к 10-му году наблюдения уровень СКФ снизился в 2,36 раза, во II группе– в 1,65 раз.Выводы. ХБП у пациентов, страдающих рецидивирующей МКБ в сочетании с ИБС и ГБ, протекает в целом доброкачественно: скорость прогрессирования дефицита фильтрации сравнительно невелика и составляет (по крайней мере, на ранних стадиях) около 4,5 мл/мин в год. Присоединение ХСН увеличивает скорость падения фильтрационной функции почек до 25% (с 4 мл/мин в год до 5 мл/мин в год). Основное негативное влияние сопутствующих ССЗ (особенно осложненных ХСН) заключается не в сверхвысоком снижении СКФ, а в сокращении, вплоть до полного отсутствия, стабильного периода функционирования почки.

Background. The COVID-19 pandemic, caused by the SARS-CoV-2 coronavirus, was announced by the World Health Organization on March 11, 2020 and identified as having a significant negative impact on the functioning of health systems and the economic development of countries. To date, we know the impact of the pandemic on the decrease in the availability of medical care, difficulties with the annual monitoring and screening of socially significant diseases. But we will be able to fully assess the medical, clinical and psychosocial consequences of the pandemic, which are just beginning to manifest themselves, only in the coming years. The purpose was to study and analyze the features of clinical manifestations, diagnosis, treatment, immunity and social significance of the COVID-19 pandemic in Ukraine. Materials methods. The available current normative and literary sources were investigated. Methods: bibliosemantic - for studying the lokal and world science and information space on the problem under study; a systematic approach and analysis - for a comprehensive study of an object, a subject in a systemic relationship, for analyzing problematic issues, ways to solve them. Results. The medical feature of COVID-19 in Ukraine is the two-stage course of the disease 1-10 days - active replication of the virus; 8-14 days or longer - immune dysfunction (eg, respiratory problems, other serious conditions). Often recorded injury of the lungs in the form of pneumonitis, although it is mainly the disease of mild or moderate severity. Diagnostic is based on the detection of the RNA of the virus using PCR or immunoglobulins by ELISA. Treatment is mostly symptomatic. The social significance of the COVID-19 pandemic was reflected in the increase in patients with mental disorders, victims of domestic violence, deterioration in the health of the population due to the limited availability of many types of medical services, increased workload and housework, decreased income and a deterioration in the quality of life. Conclusions. The COVID-19 pandemic over the year of its existence has caused changes in many spheres of life of the world's population, and Ukraine is not an exception. Disease COVID-19 has nonspecific symptoms, can proceed under the mask of many pathological conditions. especially in the presence of comorbid diseases. The pathogenetic features of the course of COVID-19 are poorly understood, which limits the possibilities of effective etiopathogenetic therapy. The virus constantly mutates and leaves an unstable and short-lived immunity, which explains its diversity of the clinical symproms in different populations. In addition to a direct increase in morbidity and mortality due to COVID-19, the virus can affect the psychoemotional state of people, reduce income and deterioration in nutrition, especially among the most vulnerable segments of the population, which requires taking these aspects into account when making decisions at the legislative level with an emphasis on addressing gender and social inequality. ; Актуальность. Патогенетические особенности течения COVID-19 недостаточно изучены, что ограничивает возможности эффективной этиопатогенетической терапии. Кроме непосредственного увеличения заболеваемости и смертности за счет COVID-19, вирус может влиять на психоэмоциональное состояние людей, уменьшение внимания по борьбе с социально значимыми неинфекционными заболеваниями (сердечно-сосудистые и онкологические), вызвать значительные социально-экономические последствия - увеличение безработицы, насилия, уменьшение доходов и ухудшение питания, особенно среди наиболее уязвимых слоев населения, что требует учета этих аспектов при принятии решений на законодательном уровне с акцентом на решение проблем гендерного и социального неравенства. ; Актуальність. Але в повній мірі оцінити медичні, клінічні й психосоціальні наслідки пандемії, які лише починають проявлятися, ми зможемо в наступні роки. Мета: дослідити та проаналізувати особливості клінічних проявів, діагностики, лікування, імунітету та соціального значення пандемії COVID-19 в Україні. Матеріали та методи. Досліджені наявні поточні нормативні та літературні джерела. Методи: бібліосемантичний – для вивчення вітчизняного і світового науковоінформаційного простору з досліджуваної проблеми; системного підходу та аналізу – для комплексного дослідження об'єкта, предмета у системному взаємозв'язку, для аналізу проблемних питань, шляхів їх вирішення. Результати. Медичні особливості COVID-19 в Україні полягають у двохстадійності перебігу захворювання: 1–10 день - активна реплікація вірусу; 8–14 день або довше - імунна дисфункція (наприклад, респіраторні порушення, інші важкі стани). Часто фіксується ураження легень у вигляді пневмоніту, хоча переважно захворювання має легкий чи середньоважкий перебіг. Діагностика засновується на виявленні РНК вірусу за допомогою ПЦР або імунонглобулінів методом ІФА. Лікування переважно носить симптоматичний характер. Соціальне значення пандемії COVID-19 відобразилось у збільшенні пацієнтів з порушенням психічного стану, жертв насильства у сімї, погіршення стану здоров'я населення у зв'язку з обмеженням доступності багатьох видів медичної допомоги, збільшення навантаження та роботи по дому, зменшення доходів та погіршення якості життя. Висновки. Отже, пандемія COVID-19 за рік свого існування спричинила зміни в багатьох сферах життя населення земної кулі, і Україна не є виключенням. Захворювання COVID-19 має неспецифічні симптоми, може перебігати під маскою багатьох патологічних станів. особливо за наявності коморбідних захворювань. Патогенетичні особливості перебігу COVID-19 недостатньо вивчені, що обмежує можливості ефективної етіопатогенетичної терапії. Вірус постійно мутує і залишає нестійкий та нетривалий імунітет, що пояснює його різноманітність клінічної картини в різних популяціях. Окрім безпосереднього збільшення захворюваності і смертності за рахунок COVID-19, вірус має значний вплив на психоемоційний стан людей, зменшення уваги щодо боротьби з соціально значимими неінфекційними захворюваннями (серцево-судинні та онкологічні), спричинив значні соціально-економічні наслідки - збільшення безробіття, насильства, зменшення доходів і погіршення харчування, особливо серед найбільш вразливих верств населення, що вимагає врахування цих аспектів при прийнятті рішень на законодавчому рівні з акцентом на вирішення проблем гендерної та соціальної нерівності.

Кроме непосредственного увеличения заболеваемости и смертности за счет COVID-19, вирус может влиять на психоэмоциональное состояние людей, уменьшение внимания по борьбе с социально значимыми неинфекционными заболеваниями (сердечно-сосудистые и онкологические), вызвать значительные социально-экономические последствия - увеличение безработицы, насилия, уменьшение доходов и ухудшение питания, особенно среди наиболее уязвимых слоев населения, что требует учета этих аспектов при принятии решений на законодательном уровне с акцентом на решение проблем гендерного и социального неравенства. ; Актуальність. Пандемія COVID-19, причиною якого є коронавірус SARS-CoV-2, оголошена Всесвітньою організацією охорони здоров'я 11 березня 2020 року і визначена як така, що нанесла значний негативний вплив на функціонування системам охорони здоров'я та економічний розвиток країн. Насьогодні відомий вплив пандемії на зниження доступності медичної допомоги, труднощі з проведенням щорічного моніторингу та скринінгу соціально значущих хвороб. Але в повній мірі оцінити медичні, клінічні й психосоціальні наслідки пандемії, які лише починають проявлятися, ми зможемо в наступні роки. Мета: дослідити та проаналізувати особливості клінічних проявів, діагностики, лікування, імунітету та соціального значення пандемії COVID-19 в Україні. Матеріали та методи. Досліджені наявні поточні нормативні та літературні джерела. Методи: бібліосемантичний – для вивчення вітчизняного і світового науковоінформаційного простору з досліджуваної проблеми; системного підходу та аналізу – для комплексного дослідження об'єкта, предмета у системному взаємозв'язку, для аналізу проблемних питань, шляхів їх вирішення. Результати. Медичні особливості COVID-19 в Україні полягають у двохстадійності перебігу захворювання: 1–10 день - активна реплікація вірусу; 8–14 день або довше - імунна дисфункція (наприклад, респіраторні порушення, інші важкі стани). Часто фіксується ураження легень у вигляді пневмоніту, хоча переважно захворювання має легкий чи середньоважкий перебіг. Діагностика засновується на виявленні РНК вірусу за допомогою ПЦР або імунонглобулінів методом ІФА. Лікування переважно носить симптоматичний характер. Соціальне значення пандемії COVID-19 відобразилось у збільшенні пацієнтів з порушенням психічного стану, жертв насильства у сімї, погіршення стану здоров'я населення у зв'язку з обмеженням доступності багатьох видів медичної допомоги, збільшення навантаження та роботи по дому, зменшення доходів та погіршення якості життя. Висновки. Отже, пандемія COVID-19 за рік свого існування спричинила зміни в багатьох сферах життя населення земної кулі, і Україна не є виключенням. Захворювання COVID-19 має неспецифічні симптоми, може перебігати під маскою багатьох патологічних станів. особливо за наявності коморбідних захворювань. Патогенетичні особливості перебігу COVID-19 недостатньо вивчені, що обмежує можливості ефективної етіопатогенетичної терапії. Вірус постійно мутує і залишає нестійкий та нетривалий імунітет, що пояснює його різноманітність клінічної картини в різних популяціях. Окрім безпосереднього збільшення захворюваності і смертності за рахунок COVID-19, вірус має значний вплив на психоемоційний стан людей, зменшення уваги щодо боротьби з соціально значимими неінфекційними захворюваннями (серцево-судинні та онкологічні), спричинив значні соціально-економічні наслідки - збільшення безробіття, насильства, зменшення доходів і погіршення харчування, особливо серед найбільш вразливих верств населення, що вимагає врахування цих аспектів при прийнятті рішень на законодавчому рівні з акцентом на вирішення проблем гендерної та соціальної нерівності.

Marine ecosystems are facing difficult times as climate change and heavy exploitation lead to biodiversity loss, the need of advanced ecosystem services, and deliberate adjustments especially in the food production industry. Cod (Gadus morhua L.), as one of the most important species for human consumption worldwide, is the focus of the presented study along with herring (Clupea harengus L.) and sprat (Sprattus sprattus L.) in the Baltic, as the species' recruitment success oscillate according to environmental changes. The present study consists of three chapters that deal with the question of environmental and intrinsic indicators underlying recruitment success in marine fish species. Apart from Eastern Baltic (EB) (Chapter II) and Atlantic cod (Chapter IV), Baltic herring (Clupea harengus L.) and sprat (Sprattus sprattus L.) (Chapter III) were species examined in this study. The overall idea of the presented approach is to find, besides fishing pressure, key indicators per species, area and period of factors, that fundamentally affect recruitment success. In Chapters II and III, environmental indicators connected with recruitment success were obtained through a simple linear regression approach that can be applied in an easy and timesaving manner in other contexts. From these indicators, individual threshold values are derived by means of a 5-fold crossvalidation approach, that define the barrier between a "good" and "bad" recruitment environment for the analyzed species. As the response variable to recruitment success, the recruitment residuals (RecRes) gained from the recruitment – spawning stock biomass (SSB) relationship were used. By subtracting the threshold value of an environmental indicator, the degree of how "good" and how "bad" recruitment environment was in certain years was assessed. The standard deviation of the crossvalidation method serves as a range of uncertainty, where "good" and "bad" becomes neutral and hence gives more space for natural variability regarding recruitment success. In Chapter II, the main indicators found for EB cod were the depth of the 11 psu isohaline as well as the reproductive volume (RV) (water body [km³], that measures more than 11 psu and oxygen content exceeds 2 ml / l and therefore provides optimal condition for cod egg development) in the central Baltic and in the Gotland Basin. Derived indicators and their correspondent thresholds for EB cod indicate, that abiotic factors seem to be the main drivers affecting Baltic cod recruitment success, as ideal conditions for successful egg development and survival are the most important mechanisms influencing year class strength. The depth of the isohaline was considered to be the most important indicator as it showed the strongest correlation with recruitment. Therefore, it was used for the evaluation of the RecRes as a good measure of response to recruitment and for possible forecast scenarios. Chapter III is an evaluation paper of the method applied in Chapter II. Here, the same approach was conducted within the same area (Subdivisions of Central Baltic) but for different species (sprat and herring). Key indicators for sprat were found to be the depth at 11 psu isohaline in the Bornholm and Gotland Basin, cod recruitment, as well as temperature in the reproductive volume of the Bornholm Basin and prey abundance (Bosmina spp.) in summer. For the evaluation of the RecRes, depth of the 11 psu isohaline (Bornholm Basin) was used. For herring, the main indicators affecting recruitment were found to be Acartia spp. abundance in summer, the depth of the 11 psu isohaline in the Bornholm Basin, as well as cod SSB. Prey abundance (Pseudocalanus spp.) as well as the depth of the 11 psu isohaline in the central Baltic also showed a correlation with herring recruitment but were not used for further analysis as the standard deviation was high. In contrast to Chapter II, biotic factors were also found to affect recruitment success of the clupeid stocks in the Baltic. The method used to derive a suitable set of environmental indicators and their corresponding thresholds per species in both chapters generates the opportunity to use indicators selected for predictive scenarios (i.e. future recruitment success) if underlying mechanisms are well understood and always with the understanding in mind, that systems are continuously changing. Also, revealed recruitment mechanisms could be falsified due to the lack of independence between SSB and recruitment data (i.e. autocorrelation) and therefore must be handled very carefully if implemented in a management context. In Chapter IV the focus was put on physiological, and therefore, intrinsic indicators responsible for recruitment success. Here, the parental contribution to the successful offspring was investigated in an extensive laboratory set up. Intrinsic indicators affecting Atlantic cod recruitment success were found to be egg size (amount of yolk) and fitness of mothers (Fulton's condition factor K) that contributed significantly to decreasing mortality rate in the early life stage of Atlantic cod. The results of Chapter IV indicate that there are more mechanisms driving recruitment success other than field observations and data gained from assessment models, as the indicators found are of intrinsic and genetic nature (i.e. yolk supply, body size, fitness). An obvious conclusion could be stated, that it may be better to estimate recruitment success using length-/weight-at age data of females as female size and fitness contribute significantly to recruitment success. Findings serve as an enhancement to the indicators found in the previous chapters, as well as a cautionary reminder to handle data holistically, if predictions derived from this are to be reliable. ; Marine Ökosysteme sind weltweit schweren Zeiten ausgesetzt, denn durch klimatische Veränderungen und übermäßige Ausbeute kommt es allerorts zur Verringerung der Artenvielfalt, und die Notwenigkeit für sorgfältig durchdachte Ökosystemleistungen und Nahrungsproduktionen wird dringender. Einer der global bedeutendsten Nahrungsfische stellt der Dorsch/Kabeljau (Gadus morhua L.) dar, welcher zusammen mit Hering (Clupea harengus L.) und Sprotte (Sprattus sprattus L.) im Fokus der vorliegenden Arbeit steht, da der Erfolg der Anzahl der Nachkommen als Antwort auf Umweltveränderungen verstanden werden kann. Die vorliegende Arbeit besteht aus drei Kapiteln, welche sich mit der Frage nach den inneren und äußeren Einflüssen beschäftigt, welche im Zusammenhang mit dem Rekrutierungserfolg mariner Fischpopulationen beschäftigt. Äußere Einflüsse sind durch die Umwelt bedingt und können biotischer (z.B. Nahrungsverfügbarkeit, Räuberaufkommen) oder abiotischer (z.B. Salzgehalt, Temperatur) Natur sein. Innere Faktoren beziehen sich auf die Gesundheit und die genetischen Voraussetzungen der Population oder, im kleineren Zusammenhang, der Elterntiere. In den Kapiteln II und III wurden umweltbedingte Indikatoren, welche in unmittelbaren Zusammenhang mit dem Rekrutierungserfolg stehen, anhand von einfachen linearen Modellen erster und zweiter Ordnung berechnet. Diese Methode kann sehr einfach und zeitsparend in anderen Zusammenhängen mit entsprechenden Daten angewandt werden. Als Resonanzgröße auf die Rekrutierung wurden die Residuen aus dem Verhältnis von Rekruten- und Laicherbiomasse genutzt und als RecRes bezeichnet. Durch die Subtraktion der Grenzwerte von den identifizierten Indikatoren, wurde der jährliche Wert der "guten" oder "schlechten" Rekrutierungsvoraussetzung ermittelt. Die Standardabweichung der Vergleichsprüfung definiert einen Raum, in welchem "gut" und "schlecht" neutralisiert werden und so Möglichkeit für natürliche Schwankungen im Rekrutierungserfolg bieten. Die Hauptindikatoren für den Dorsch in Kapitel II waren sowohl die Tiefe der 11 psu Isohaline, als auch die Größe des 'Reproduktiven Volumen' (Wasserkörper [km³], welcher eine höhere Salzkonzentration als 11 psu aufweist und eine Sauerstoffkonzentration von über 2 ml / l misst. Die gefundenen Indikatoren wurde auch für die Evaluation der RecRes genutzt, um zu prüfen, ob sich die RecRes als Resonanzgröße für mögliche Vorhersagen eignen. Die in diesem Kapitel abgeleiteten Indikatoren und ihre entsprechenden Schwellenwerte für EB Dorsch weisen darauf hin, dass abiotische Faktoren die Hauptfaktoren für den Rekrutierungserfolg von Dorsch im Baltikum zu sein scheinen, da ideale Bedingungen für eine erfolgreiche Entwicklung und das Überleben der Eier die wichtigsten Mechanismen sind, die die Stärke der Jahresklasse beeinflussen. Im Kapitel III wurden die gleichen Methoden angewendet, allerdings wurde nun der Rekrutierungserfolg von Hering und Sprotte untersucht. Die Hauptindikatoren für die Sprottenpopulation waren hier ebenfalls die Tiefe der 11 psu Isohalinen im Bornholm und im Gotland Becken, die Größe des Rekrutierungserfolges des Dorsches, die Temperatur im 'Reproductive Volume' und die Nahrungsabundanz von Bosmina sp. im Sommer. Für die Evaluierung der RecRes wurde der stärkste Indikator, die Tiefe der 11 psu Isohalinen (Bornholm Becken) genutzt. Die Hauptindikatoren die das Rekruitment der Heringspopulation in der Zentralen Ostsee beeinflussen waren die Nahrungsabundanz (Acartia sp.) im Sommer, die Tiefe der 11 psu Isohalinen (Bornholm Becken) sowie die die Größe der geschlechtsreifen Dorschpopulation. Im Gegensatz zu Kapitel II zeigten sich hier auch biotische Faktoren, die den Rekrutierungserfolg der Bestände der Clupeiden beeinflussten. Die angewendete Methode zur Identifikation geeigneter Umweltindikatoren und ihrer entsprechenden Grenzwerte pro Art in beiden Kapiteln bietet die Möglichkeit, Indikatoren zu verwenden, die für Vorhersageszenarien (z.B. künftige Rekrutierungserfolge) ausgewählt werden können, wenn zugrundeliegende Dynamiken eines Systems gut verstanden und stets im Auge behalten werden. Außerdem kann die hier verwendete Testvariabel (RecRes) aufgrund der mangelnden Unabhängigkeit zwischen Laicherbestand (SSB) und Rekrutierungsabschätzung durch z.B. Autokorrelation verfälscht sein und muss daher sehr sorgfältig gehandhabt werden, wenn sie in einem Managementkontext implementiert werden soll. In Kapitel IV wurde der Fokus auf die intrinsischen Indikatoren gelegt, die zu einem erfolgreichen Rekruitment führen. In diesem Kapitel wurde der elterliche Beitrag zu einer erfolgreichen Nachkommenschaft in einem umfangreichen laboratorischen Experiment erforscht. Die intrinsischen Indikatoren, die den Rekrutierungserfolg des atlantischen Kabeljaus am meisten beeinflussen waren in dieser Studie Eiergröße und somit der Gehalt an Dotter, sowie die Gesundheit der Mutter. Hier liegt der Rückschluss nahe, dass für die Abschätzung des Rekrutierungserfolges auch die Längen-Gewichtsbeziehungen der Weibchen in Betracht gezogen werden sollten, da diese Merkmale maßgeblich zu dem Rekrutierungserfolg beitragen. Die Ergebnisse dienen als Erweiterung der Indikatoren aus den vorherigen Kapiteln und sollen daran erinnern, dass Daten jeglicher Art mit Vorsicht bearbeitet werden sollen, um nicht voreilige Rückschlüssen (z.B. Vorhersagen) zu ziehen, da nicht jede Dynamik der natürlichen Umstände in jedem Datensatz wiedergegeben ist.
