The aim is to analyze the educational policy of the state in the national districts of Katerinoslav province, later - on the territory of Dnipropetrovsk region with the biggest concentration of the national formations in the republic. Research methods: historical-genetic, historical-comparative and historical-systemic. The main results. It was detected that after the ending of the civil war, representatives of national minorities, perceived the ideology Bolshevik government without enthusiasm. To strengthen their influence in this environment, was taken the course to the creation of the human resources that were loyal to the party from local population. In the complex of measures that were directed to the decision of this problem, the most important place was taken away to the educational policy, on the base on the influence of consciousness of the society and in the first place to its growing generation. It was shown it was started to do by the way of creation the chain of the national general and secondary special educational institutions. It was opened the specific formation of this policy in the region. Quantitative indicators testified that on the territory of Katerinoslav (Dnipropetrovs'k) till the end of the 20th, in the sphere of creation the educational education, noticeable positive results were taken place. It testified that its leadership of different levels was followed in the way of soviet party- state leadership. But, despite the proclamation of priority in the development of national education, the state allocated not much money for this sphere. There were serious problems with the providing of educational literature on the mother tongue. Circulations and nomenclature of planned educational manuals didn't meet the needs. That's why teachers of schools and teachers of secondary special educational institutions needed to conduct the educational process due to pre-revolionary or Russian-speaking textbooks, or even due to fiction literature. There was a shortage of school facilities. However, it's noted that to the end of 20-s among national minorities, the percentage of primary education coverage ranged from 80 to almost 100 percent. It's emphasized that a network of various and secondary special educational institutions has been created. The training down specialists was under a significant ideological influence. It's noticed that since the early 30's began to curtail the policy of indigenization, liquidation of educational institutions and repressions against their pedagogical staff under the slogans of accusations of espionage and counter-revolutionary activities. In the middle of 1938 educational institutions of all national minorities ceased to exist. Concise conclusions Educational policy among national minorities in the Ekaterinoslav region was conducted in accordance with the proclamation, which consisted in the introduction of party ideology through the formation in the system of institutions of national education of personnel trained in the spirit of Bolshevik ideology. In the 1920s it was properly implemented. From the beginning of 1930s education begins to change and teaching in national languages begins to unfold accompanying repression against educators. The process ended in the middle of 1938 year by the liquidation of educational institutions of all types and translation into Russian of the Ukrainian language of insruction. In this way, the authorities initiated the process of assimilation and Russification of national minorities in the region. Originality: based on research materials and collections of archival documents. Scientific novelty: for the first time the educational policy of the Soviet power on the territory of Dnipropetrovs'k region in the ethno-national dimension is presented, through the prism of its realization in various national formations of the region. Sickle cell disease (SCD) is a common blood disease caused by a single nucleotide substitution (c.20T>A, p.Glu6Val) in the beta globin gene on chromosome 11. The prevalence of the disease is high throughout large areas in sub-Saharan Africa, the Mediterranean basin, the Middle East, and India due to the level of protection that the sickle cell trait, provides against severe malaria. Approximately 300,000 infants are born per year with sickle cell anemia, which is defined as homozygosity for the sickle hemoglobin (HbS). The majority (nearly 75%) of these births occur in sub-Saharan Africa, particularly in two countries: Nigeria, and the Democratic Republic of the Congo where there are poorly resourced healthcare systems. Early diagnosis, penicillin prophylaxis, blood transfusions, hydroxyurea, and hematopoietic stem-cell transplantation can dramatically improve survival and quality of life for patients with SCD. However, our understanding of the role of genetic and clinical factors in explaining the complex phenotypic diversity of this disease is still limited. Early prediction of the severity, and patients' responses to specific therapeutics of SCD could lead to more precise treatment and management. Beyond well-known modifiers of disease severity, such as fetal hemoglobin (HbF) levels and αthalassemia, other genetic variants might influence specific sub-phenotypes. New treatments and management strategies accounting for these genetic and nongenetic factors could substantially and rapidly improve the quality of life and reduce health care costs for patients with SCD. Patients with SCD are subjected to long term administration of drugs and there is a limited data on pharmacogenomics of SCD therapeutics. Vaso-occlusive crisis (VOC) are the main clinical events of SCD and are associated with recurrent and long-term use of antalgics/opioids and HU. This project aimed to investigate the clinical and genetic predictors of painful vaso-occlusive crisis (VOC) among SCD Cameroon patients by exploring pharmacokinetic determinants of treatment responses as well as post-transcriptional signatures triggered by hydroxyurea treatment, particularly, miRNA expression. SCD patients were recruited from Yaounde Central Hospital and Laquintinie Hospital in Douala (Wonkam et al., 2018, Mnika et al., 2019 (b)), and recent migrants SCD patients from the DRC, recruited at the Haematology Clinic, Groote Schuur Hospital in Cape Town, South Africa (Mnika et al., 2019 (a) and Mnika et al., 2019 (b)). Sociodemographic and clinical data were collected by means of a structured questionnaire. Patients' medical records were reviewed to extract their clinical features over the past 3 years. Specifically, the occurrences of VOC, hematological parameters, hospital outpatient visits, hospitalisation, overt strokes, blood transfusions, and administration of hydroxyurea were recorded. Height, weight, body mass index (BMI), systolic and diastolic blood pressures (SBP and DBP) were measured. Detailed descriptions of patients and sampling methods used in the Cameroonian patients have been reported previously (Wonkam et al., 2018 Mnika et al., 2019 (a) and Mnika et al., 2019 (b)). For the purpose of comparing frequencies of variants, ethnically matched Cameroonian controls were randomly recruited from apparently healthy blood donors in Yaounde for participation in the study. All blood samples were collected for genomic characterisation and analysis. DNA was extracted from peripheral blood, following instructions on the available commercial kit [QIAamp DNA Blood Maxi Kit ® (Qiagen, United States)]. Genotyping (TaqMan and MassArray) was performed for 40 variants in 17 pain-related genes, three fetal haemoglobin (HbF)-promoting loci, two kidney dysfunction-related genes, and HBA1/HBA2 genes for 436 patients. A subset of these samples was also genotyped to analyse 32 core and 267 extended pharmacogenes using commercially available PharmacoScan® platform for characterisation of pharmacokinetic determinant of response. We also compared the pharmacogenes variants from these African groups, to data extracted from the 1000 genomes Project. Moreover, association studies were carried out on pharmacogenes variants with SCD clinical variability. Additionally, protein-protein interaction (PPI) network and enriched biological processes and pathways were investigated. For association studies, statistical models using regression frameworks to analyse 40 variants were performed in R®. For miRNA expression, total RNA was isolated using the miRNeasy kit according to protocol of the Manufacturer (QIAGEN, Hilden, Germany); and sequenced by the Genomic and RNA Profiling Core at Baylor College of Medicine, United States, using the NanoString Platform (NanoString Technologies, Inc., Seattle, WA, United States), according to manufacturer's instructions. Genes with statistically significant changes in expression were analysed using the significance analyses of microarrays (SAM) tools. Female sex, body mass index, Hb/HbF, blood transfusions, leucocytosis and consultation or hospitalisation rates significantly correlated with VOC. Three painrelated gene variants correlated with VOC (CACNA2D3-rs6777055, P = 0·025; DRD2- rs4274224, P = 0·037; KCNS1-rs734784, P= 0·01). Five pain-related gene variants correlated with hospitalization/consultation rates (COMT-rs6269, P = 0·027; FAAHrs4141964, P = 0·003; OPRM1- rs1799971, P = 0·031; ADRB2-rs1042713; P < 0·001; UGT2B7-rs7438135, P = 0·037). The 3·7 kb HBA1/HBA2 deletion correlated with increased VOC (P = 0·002). HbF-promoting loci variants correlated with decreased hospitalisation (BCL11A-rs4671393, P = 0·026; HBS1L-MYB-rs28384513, P = 0·01). APOL1 G1/G2 correlated with increased hospitalisation (P = 0·048). A commercial genotyping array platform (PharmacoScan®) with 4627 markers located in 1191 genes was used to investigate 299 pharmacogenes (32 ADME core and 267 extended pharmacogenes). Based on the PharmacoScan analyses, no statistically significant differences in allele frequencies were detected between SCD cases and controls from Cameroon. A principal component analysis (PCA) revealed that Cameroonians' data clustered with other Africans, but this population is significantly distinct from American, European and Asian populations data. Variant allele frequencies in 21/32 core pharmacogenes were significantly different between the two SCD groups (Cameroon vs. Congo). No correlation between clinical variability and variants in the core genes was detected for both populations under study. An association study of the core and extended PharmacoScan variants to VOC identified statistically significant associations between two single nucleotide polymorphisms (SNPs) to VOC after correction of multiple testing. These two SNPs mapped to 50 genes, with two SNPs located in core pharmacogenes (SLCO4A1- rs118042746, p=1.21e-07; UGT1A10, UGT1A8- rs10176426, p=1.22e-07). Functional enrichment analyses revealed that these 50 genes are involved in three biological processes and four pathways relevant to SCD pathophysiology, including xenobiotic glucuronidation (GO:0052697, p = 2.3e-03), and drug metabolism - other enzymes (p = 2.1e-02). Further analyses of the 50 genes, identified key genes in human proteinprotein networks: NTSR1, LRMDA, SMAD SMAD4 and CDH2. These four genes also interacted with three core pharmacogenes associated with VOC: UGT1A8, UGT1A10 and SLCO4A1. We found 22/798 miRNAs to be differentially expressed under HU treatment, with the majority (13/22) being functionally associated with HbF-regulatory genes, including BCL11A (miR-148b-3p, miR-32-5p, miR-340-5p, miR-29c-3p), MYB (miR-105-5p), KLF-3 (miR-106b-5), and SP1 (miR-29b-3p, miR-625-5p, miR-324-5p, miR-125a-5p, miR-99b-5p, miR-374b-5p, miR-145-5p). The present thesis started by highlighting the scarcity of studies investigating variable responses to pain in SCD patients and then proceeded to addressing this research gap. To our knowledge this is the first body of from Africa to provide evidence supporting the possible development of a genetic risk model for pain in SCD. This is also the first body of work to report an association between these two SNPs and VOC in core and extended pharmacogenes. Our data reveals that the commercial pharmacogenes arrays investigated might need additional evidence for appropriateness among Africans. Therefore, it advocates the need to invest in research exploring population-specific arrays, drug design, targeting, and efficacy, for improved clinical management of patients of African descent. Previous studies have investigated various mechanisms to understand the genomic variations affecting responses to HU, but full understanding of the variable HU-mediated HbF production among individuals affected by SCD remains elusive. The present study showed that mechanisms of HbF production in response to HU, could particularly be mediated through miRNA regulation. The data reveals some alternative perspectives and routes towards identifying new therapeutic targets and approaches for SCD. However, this study needs to be replicated in larger samples in multiple African populations.

Global change is a major threat to local groundwater resources. Climate change and population growth are factors that directly or indirectly augment the increasing uptake of groundwater resources. To outbalance the pressure on aquifers, managed aquifer recharge (MAR) schemes are increasingly being implemented. They enable the subsurface storage of surplus water for times of high demand. The complexity of MAR schemes makes their planning and implementation multifaceted and requires a comprehensive assessment of the local hydrogeological and hydrogeochemical conditions. Despite the fact that MAR is a widely used technique, its implementation is not well regulated and comprehensive planning and design guidelines are rare. The use of supporting tools, such as numerical and physical models or geographic information systems (GIS), is rising for MAR planning but their scope and requirements for application are rarely reflected in the available MAR guidelines. To depict the application potential and the advantages and disadvantages of the tools for surface infiltration MAR planning, this thesis comprises reviews on the past use of the tools as well as suggestions to improve their applicability for MAR planning. GIS is not mentioned by most MAR guidelines as a planning tool even though it is increasingly being used for MAR mapping. Through a review of GIS-based MAR suitability studies, this thesis shows that the MAR mapping process could be standardized by using the often-applied approach of constraint mapping, suitability mapping by using pairwise comparison for weight assignment and weighted linear combination as a decision rule, and a subsequent sensitivity analysis. Standardizing the methodology would increase the reliability and comparability of MAR maps due to the common methodological approach. Thus, the proposed standard methodology was incorporated into a web GIS that simplifies MAR mapping through a pre-defined workflow. Numerical models are widely used for the assessment of MAR schemes and are included into some MAR planning guidelines. However, only a few studies were found that utilized vadose zone models for the planning and design of MAR schemes. In this thesis, a review and a subsequent case study highlight that numerical modelling has many assets, such as monitoring network design or infiltration scenario planning, that make its utilization during the MAR planning phase worthwhile. Consequently, this study advocates the use of vadose zone models for MAR planning by showing their potential areas of application as well as their uncertainties that need to be regarded carefully during modelling. Physical models used for MAR planning are typically field or pilot sites, as some MAR legislation requests pilot sites as part of the preliminary assessment. Laboratory experiments are used less often and are mostly restricted to the analysis of very specific issues, such as clogging. This thesis takes on the issue of scaling laboratory results to the field scale by comparing results from three physical models of different scales and dimensionality. The results indicate that preferential flow paths, air entrapment and boundary influence limit the quantitative validity of laboratory experiments. The use of 3D tanks instead of 1D soil columns and the application of statistical indicators are means to increase the representativeness of laboratory measurements. Nevertheless, physical models have the potential to improve MAR planning in terms of detailed process assessment, scenario and sensitivity analyses. All tools discussed in this thesis have their merits for MAR scheme planning and should be advocated better in MAR guidelines by depicting their application potential, advantages and disadvantages. Klimawandel und Bevölkerungswachstum sind Faktoren, die, direkt oder indirekt, die zunehmende Nutzung von Grundwasserressourcen verstärken. Um diesen Druck auf die Grundwasserleiter auszugleichen, werden verstärkt Maßnahmen zur gezielten Grundwasserneubildung (managed aquifer recharge = MAR) durchgeführt. Dies ermöglicht die unterirdische Speicherung von überschüssigem Wasser für Zeiten hohen Bedarfs. Die Komplexität von MAR-Anlagen macht ihre Planung und Umsetzung kompliziert und erfordert eine umfassende Bewertung der lokalen hydrogeologischen und hydrogeochemischen Bedingungen. Trotz der weltweiten Implementierung von MAR ist dessen Planung wenig reguliert. Umfassende Planungs- und Gestaltungsrichtlinien sind rar. Der Einsatz unterstützender Werkzeuge, wie numerischer und physikalischer Modelle oder Geoinformationssysteme (GIS), nimmt bei der MAR-Planung zu, aber ihre Einsatzmöglichkeiten und ihre Anforderungen an die Anwendung spiegeln sich selten in den verfügbaren MAR-Richtlinien wider. Um das Anwendungspotential und die Vor- und Nachteile der Werkzeuge für die MAR-Planung darzustellen, wurden für diese Arbeit Recherchen über den bisherigen Einsatz der Werkzeuge durchgeführt. Zusätzlich wurden Vorschläge zur Erhöhung ihrer Anwendbarkeit für die MAR Planung gemacht. Der Schwerpunkt lag dabei auf Oberflächeninfiltrationsverfahren. GIS wird in keiner MAR-Richtlinie als Planungsinstrument erwähnt, obwohl es zunehmend für die MAR-Kartierung eingesetzt wird. Eine Recherche über GIS-basierte MAR-Eignungsstudien zeigte, dass der MAR-Kartierungsprozess standardisiert werden kann mittels des oft genutzten Ansatzes: initiales Ausschneiden von Gebieten, welche Restriktionen unterliegen, dem folgend die Eignungskartierung mittels Paarvergleich für die Wichtung der GIS-Karten und der gewichteten Linearkombination als Entscheidungsregel, sowie eine abschließende Sensitivitätsanalyse. Die Standardisierung der Methodik könnte die Zuverlässigkeit und Vergleichbarkeit von MAR-Karten aufgrund des gemeinsamen methodischen Ansatzes erhöhen. Daher wurde die standardisierte Methodik in ein Web-GIS integriert, das über einen definierten Workflow die MAR-Kartierung vereinfacht. Numerische Modelle werden häufig für die Beurteilung von MAR-Systemen verwendet und sind in einigen MAR-Planungsrichtlinien ausgewiesen. Es wurden jedoch nur wenige Studien gefunden, die die Modelle der ungesättigten Zone für die Planung und Gestaltung von MAR Standorten verwendeten. Die in dieser Arbeit durchgeführte Literaturrecherche und eine darauf aufbauende Fallstudie zeigen, dass die numerische Modellierung viele Vorteile bietet, wie z. B. beim Design eines Monitoring-Netzwerkes oder bei der Planung von Infiltrationsszenarien. Physikalische Modelle, die für die MAR-Planung verwendet werden, sind meist Feld- oder Pilotversuche, da einige MAR-Gesetzgebungen Pilotstandorte im Rahmen der Vorabbewertung verlangen. Laborexperimente werden seltener eingesetzt und beschränken sich meist auf die Analyse sehr spezifischer Fragestellungen, wie z.B. der Kolmatierung. Diese Arbeit beschäftigt sich mit der Skalierbarkeit von Laborergebnissen auf die Feldskale, indem sie Ergebnisse aus drei physikalischen Modellen verschiedener Maßstäbe und Dimensionen vergleicht. Die Ergebnisse deuten darauf hin, dass Makroporen, Lufteinschlüsse und der Einfluss der Randbedingungen die quantitative Aussagekraft von Laborversuchen einschränken. Der Einsatz von 3D-Tanks anstelle von 1D-Bodensäulen oder von statistischen Indikatoren ist ein Mittel zur Erhöhung der Repräsentativität von Labormessungen. Nichtsdestotrotz hat die Anwendung physikalischerModelle das Potenzial, die MAR-Planung in Bezug auf detaillierte Prozessbewertung, Szenarien und Sensitivitätsanalysen zu unterstützen. Alle beschriebenen Instrumente haben ihre Vorzüge bei der Bewertung von MAR-Anlagen und sollten in MAR-Richtlinien detaillierter berücksichtigt werden, indem ihr Anwendungspotenzial, ihre Vor- und ihre Nachteile dargestellt werden. "The Vancouver Principles pushes the international community boldly forward on issues related to peacekeeping and protecting children used as weapons of war and ensure peacekeepers are prepared."LGen Roméo DallaireThe Vancouver Principles on Peacekeeping and the Prevention of the Recruitment and Use of Child Soldiers has rapidly been endorsed by states around the world, demonstrating the global desire for peacekeeping to address the use of children as soldiers. Co-created by the Roméo Dallaire Child Soldiers Initiative and the Canadian Government, the Vancouver Principles are a set of political commitments endorsed by states and are focused on "the need to prioritize and operationalize the prevention of the recruitment and use of child soldiers in areas of operations of peacekeeping, noting the critical role of such prevention to the achievement of peace and security, and taking into account the differential impact of conflict on girls and boys." The seventeen Principles range from the prevention-oriented planning, to the inclusion of child protection focal points in command structures, to the commitment to share best practices among states.For this volume of Allons-y, we asked contributors to focus on the implementation of the Vancouver Principles, with each article focussing on practical aspects of one of the principles and the current status of research in the area of focus. The contributors are researchers and practitioners in the field of Children, Peace and Security and offer their perspectives on current literature as well as field-based experiences in relation to the goals of the Vancouver Principles. The preface, commentary and six papers in this volume illustrate the complexities of contemporary armed conflict, the increasing and evolving impacts on children, and the importance of prevention.The volume opens with Commentary by Valentina Falco and Alec Wargo who provide an overview of the UN protection framework for children affected by armed conflict and the accompanying monitoring mechanisms. Shelly Whitman and Catherine Baillie Abidi lay the foundation of the concept of prevention and the increasing need to take preventative actions to break cycles of armed conflict. Victoria Bryce and Dustin Johnson build on this foundation and further explore how prevention can be operationalized within security sector structures to dually enhance the protection of children and improve operational effectiveness. In relation to improving protection, Dustin Johnson and Allyssa Walsh explore the role of gender in recruitment prevention, emphasizing the gendered dimensions of recruitment and use of children, and the need to better understand how the gender of peacekeepers affects child protection. Laura Cleave and William Watkins offer insights into early warning systems and the opportunities to embed child-specific indicators as a measure to prevent or disrupt recruitment processes. Marion Laurence explores the function of monitoring and reporting grave violations of children's rights in light of the move towards better and more coordinated use of data in UN peacekeeping. And finally, Jo Becker examines the alarming trend of the detention, rather than the release and rehabilitation, of children associated with armed groups.Given the unprecedented and increasing number of children living in conflict-affected areas, it is vital that the international community prioritizes children's rights and rallies toward building peaceful communities. Laura Cleave et William Watkins donnent un aperçu des systèmes d'alerte rapide et des possibilités d'intégrer des indicateurs propres aux enfants comme mesure de prévention ou de perturbation des processus de recrutement. Marion Laurence analyse la fonction de surveillance et de communication de l'information sur les violations graves des droits de l'enfant à la lumière de l'évolution vers une utilisation plus efficace et coordonnée des données dans le cadre du maintien de la paix des Nations Unies. Et enfin, Jo Becker examine la tendance alarmante de la détention, plutôt que de la libération et de la réadaptation, des enfants associés aux groupes armés.Compte tenu du nombre sans précédent et croissant d'enfants vivant dans des zones touchées par des conflits, il est essentiel que la communauté internationale donne la priorité aux droits de l'enfant et se mobilise pour édifier des communautés pacifiques. En aras de dilucidar si ¿las instituciones creadas con ocasión de la reforma administrativa de Ley 1444 de 2011 (por medio de la cual se escinden unos Ministerios, se otorgan precisas facultades extraordinarias al Presidente de la República para modificar la estructura de la Administración Pública y la planta de personal de la Fiscalía General de la Nación y se dictan otras disposiciones) con la denominación de agencias corresponden a la institucionalidad requerida para realizar el tránsito del Estado intervencionista al Estado regulador en Colombia? Al no contarse en Colombia con bibliografía suficiente para poder dar una respuesta directa a ese interrogante, se justifica llevar a cabo la presente investigación, con el objeto de determinar si la adecuación que se le ha dado el ordenamiento jurídico colombiano con las recientemente creadas agencias se ajusta a los requerimientos institucionales para transitar hacia el modelo de Estado Regulatorio. Para lograr este objetivo es necesario conocer previamente las características que identifican el modelo de Estado Regulatorio y estudiar el origen de las agencias, sus características y organización funcional. Ello permitirá luego estar en capacidad de establecer comparaciones con el modelo colombiano y determinar si es adecuada la normativa para su operatividad. Para alcanzar ese objetivo, el método utilizado será el deductivo, de manera que las conclusiones serán una consecuencia necesaria de la comparación con la premisa mayor (características de las agencias como estructuras organizacionales funcionales a la implantación del modelo regulatorio de Estado), de la premisa menor (régimen jurídico de las entidades creadas con la naturaleza de agencias en Colombia), para establecer si en el derecho colombiano las agencias son funcionales a la transición del modelo intervencionista al regulador. Al aproximarse al diseño institucional de las entidades creadas como agencias en Colombia, el legislador, muchas veces extraordinario, porque la inmensa mayoría de ellas se crearon por decretos expedidos con base en el otorgamiento de facultades extraordinarias al ejecutivo, les dio el carácter de unidades administrativas especiales o de entidades descentralizadas de naturaleza especial. De esta manera, al estar contempladas estas tipologías como parte del sector descentralizado de la rama ejecutiva del orden nacional, la hipótesis de la presente investigación es que hay una alta probabilidad de que esas figuras no respondan al diseño esperado para una entidad que se encarga de ejercer función regulatoria en un modelo de Estado regulador. La metodología para el desarrollo de la investigación consistirá en la revisión documental de la doctrina nacional y extranjera y la normatividad interna y la realización del análisis de la funcionalidad de las agencias al modelo regulatorio mediante la aplicación de una matriz de diez variables construida a partir de la caracterización de tales instituciones. Teniendo en cuenta el anterior contexto, el trabajo está dividido en tres capítulos: en el primer capítulo se estudia la metamorfosis de la transformación del Estado moderno, desde sus orígenes liberales, pasando por el intervencionismo hasta llegar al Estado regulador. Seguidamente en el capítulo dos, será explicada esa misma transformación en el caso colombiano haciendo especial énfasis en tres períodos importantes: el de la hegemonía liberal (1930-1946), época en que se realizó la reforma constitucional en que se consolidó el Estado intervencionista. Luego se describirán los aspectos históricos que influyeron en el declive del Estado intervencionista y, para finalizar este capítulo, se hará referencia a la Constitución de 1991 y su tránsito al Estado regulador, exponiendo la manera en que se ha pretendido ajustar a este su diseño institucional. Por último, en el capítulo 3º se define una matriz de caracterización de las agencias como autoridades administrativas independientes y se aplica al régimen jurídico de las catorce entidades creadas con esa naturaleza en la administración pública colombiana, con la finalidad de establecer si tales entidades corresponden a la institucionalidad requerida para la implementación del Estado regulador. ; In order to elucidate whether the institutions created on the occasion of the administrative reform of Law 1444 of 2011 (by means of which some Ministries are divided, precise extraordinary powers are granted to the President of the Republic to modify the structure of the Public Administration and the staff of the Office of the Attorney General of the Nation and other provisions are issued) with the denomination of agencies correspond to the institutional framework required to carry out the transition from the interventionist State to the regulatory State in Colombia? As there is not enough bibliography in Colombia to be able to give a direct answer to this question, it is justified to carry out the present investigation, in order to determine if the adaptation that the Colombian legal system has been given with the recently created agencies adjusts to the institutional requirements to move towards the Regulatory State model. To achieve this objective, it is necessary to previously know the characteristics that identify the Regulatory State model and study the origin of the agencies, their characteristics and functional organization. This will then allow us to be able to establish comparisons with the Colombian model and determine if the regulations are adequate for its operation. To achieve this objective, the method used will be the deductive one, so that the conclusions will be a necessary consequence of the comparison with the major premise (characteristics of the agencies as functional organizational structures to the implementation of the State regulatory model), of the premise minor (legal regime of entities created with the nature of agencies in Colombia), to establish whether in Colombian law the agencies are functional to the transition from the interventionist to the regulatory model. When approaching the institutional design of the entities created as agencies in Colombia, the legislator, often extraordinary, because the vast majority of them were created by decrees issued based on the granting of extraordinary powers to the executive, gave them the character of administrative units special or decentralized entities of a special nature. In this way, since these typologies are considered as part of the decentralized sector of the executive branch of the national order, the hypothesis of this research is that there is a high probability that these figures do not respond to the expected design for an entity that is in charge of exercise regulatory function in a regulatory state model. The methodology for the development of the research will consist of the documentary review of the national and foreign doctrine and the internal regulations and the analysis of the functionality of the agencies to the regulatory model by applying a matrix of ten variables constructed from the characterization of such institutions. Taking into account the above context, the work is divided into three chapters: the first chapter studies the metamorphosis of the transformation of the modern state, from its liberal origins, through interventionism to the regulatory state. Then, in chapter two, this same transformation will be explained in the Colombian case with special emphasis on three important periods: that of liberal hegemony (1930-1946), the time when the constitutional reform was carried out in which the interventionist State was consolidated. Then the historical aspects that influenced the decline of the interventionist State will be described and, to end this chapter, reference will be made to the 1991 Constitution and its transition to the regulatory State, exposing the way in which its institutional design has been adjusted to this. . Finally, in Chapter 3 a matrix of characterization of the agencies as independent administrative authorities is defined and it is applied to the legal regime of the fourteen entities created with that nature in the Colombian public administration, in order to establish whether such entities correspond to the institutional framework required for the implementation of the regulatory State.

The present training report was developed to obtain the integrated master's degree in pharmaceutical sciences. It is subdivided in three chapters that address the three main activities enclosed in the curricular unit "Internship" of the Integrated Master in Pharmaceutical Sciences. The first chapter detailed the laboratorial research component, developed at the Health Sciences Research Center from University of Beira Interior in the area of pharmaceutical chemistry. The ubiquitin proteasome system has been defined as potential target in the treatment of a range of clinical conditions, such as inflammation, neurodegenerative diseases and cancer, particularly, haematological malignancies. A variety of chemical synthesized and natural products have exhibited proteasome inhibitory activity from which three were approved for use in multiple myeloma treatment. 2-Thioxoimidazolidin-4-one arylaldehyde derivatives were described as novel noncovalent proteasome inhibitors in a recent study. Considering the structural similarity of thiobarbituric acid and 2-thioxoimidazolidin-4-one systems, 5-substituted (thio)barbiturate derivatives where designed and efficiently synthesized in this work as a potential novel class of proteasome inhibitors with anticancer interest. In this context, several barbiturate derivatives demonstrated promising antiproliferative effects in different cancer cell lines. Stability study of the 5-benzilidene(thio)barbiturates derivatives in solution was performed and led to the exclusion of 5-benzilidene thiobarbiturates due to their instability. Then, a xanthine oxidase and proteasome inhibition assay were performed. None of the compounds showed relevant inhibition of xanthine oxidase enzyme. However, 5-[1-[2-(4-nitrophenyl)hydrazinyl]ethylidene]barbiturate, 9b, showed interesting inhibitory activity in the proteasome inhibition assay. Cytotoxicity of the assayed compounds was evaluated by the MTT assay in healthy (NHDF) and antiproliferative effect of 9b in cancer cell lines (Caco-2 and PC-3) was assessed and compared with the effect of bortezomib, an approved proteasome inhibitor. Although 9b showed cytotoxicity against healthy and cancer cell lines, it was less potent than bortezomib. The second chapter describes the activities accomplished during the internship in Community Pharmacy that took place in the Pharmacy Holon Covilhã under the supervision of Dr. Patrícia Pais. It is divided to present generally the operations performed at the pharmacy, the legislation that regulates the sector, and the tasks and activities perform. The third chapter describes the hospital pharmacy internship at The Barts Heart Centre in St Bartholomew's Hospital in London, UK that was guided by my main supervisor and contact person, Paul Wright, the Lead Cardiac Pharmacist at Barts. It is organized based on the activity I observed or performed myself. It is divided into four main subchapters, The Barts Heart Centre, Oncology, Clinical trials, and Dispensary. ; O presente relatório de estágio foi elaborado com o objectivo de obter o grau de Mestre em Ciências Farmacêuticas. Encontra-se subdividida em três capítulos que abordam cada um dos períodos de aprendizagem inseridos na unidade curricular "Estágio" do Mestrado Integrado em Ciências Farmacêuticas. No primeiro capítulo é descrito todo o trabalho realizado no âmbito da área de investigação desenvolvida no Centro de Investigação em Ciências da Saúde da Universidade da Beira Interior, na área da química farmacêutica, sendo este intitulado "Synthesis and evaluation of 5-substituted (thio)barbiturates as proteasomal inhibitors for cancer therapy". O sistema ubiquitina-proteassoma foi definido como um potencial alvo terapêutico no tratamento de uma série de condições clínicas, tais como inflamação, doenças neurodegenerativas e cancro, particularmente, malignidades hematológicas. Uma variedade de produtos sintéticos e naturais têm sido associados a atividade inibitória do proteassoma, dos quais três compostos, bortezomib, carfilzomib, e ixazomib foram aprovados para uso clínico no tratamento do mieloma múltiplo. Os derivados de arilaldeídos de 2-tioxoimidazolidin-4-ona foram descritos como novos inibidores não covalentes de proteassoma num estudo recente. Face a isso, e considerando a similaridade estrutural dos últimos com os ácidos (tio)barbituricos, hipotetizou-se que derivados 5-substituídos de ácidos (tio)barbitúricos também poderiam ter atividade inibitória do proteassoma. Neste contexto, é também conhecido que vários derivados de barbituratos demonstraram efeitos antiproliferativos promissores em diferentes linhas celulares de cancro. No presente trabalho, foram desenvolvidos varios derivados 5-benzilideno(tio)barbitúricos utilizando como precursores ácidos (tio)barbitúricos e benzaldeídos e água como solvente. Os compostos obtidos foram devidamente caracterizados por determinação dos seus pontos de fusão, e espectroscopia de ressonância magnética nuclear de protão e carbono-13. Verificou-se, durante o estudo, a degreadação dos derivados 5-benzilideno tiobarbitúricos e a formação de bis-(tio)barbiturates em soluções aquosas, levando à exclusão destes compostos da avaliação biológica. Assim, tentou-se sintetizar seletivamente os bis-(tio)barbiturates puros, embora sem sucesso. Alternativamente, realizou-se a redução da dupla ligação exocíclica de 5-benzilidenotiobarbituratos como uma tentativa de aumentar a estabilidade dos compostos sintetizados. Os compostos obtidos foram incluídos nos estudos biológicos. Nos estudos da avaliação biológica foram incluídos os derivados 5-benzilidenobarbitúricos e os 5-benzyl(thio)barbituratos, determinado-se a sua atividade como inibidores do proteassoma e da xantina oxidase, tendo sido também analisando os seus efeitos citotóxicos em linhas celulares saudáveis (NHDF) e cancerígenas (Caco-2 e PC-3). A atividade inibitória do proteasoma dos compostos foi determinada mediante o bioluminescent Proteasome-Glo™ Assay. Num screening inicial, incubaram-se os compostos com a enzima em duas concentrações (10 e 100 µM), tendo sido utilizado o bortezomib como controlo positivo. Para o 5-[1-[2-(4-nitrofenil)hidrazinil]etilideno]-barbiturato 9b, composto que demonstrou a melhor atividade inibitória, procedeu-se a ensaio posterior para determinar a curva concentração-resposta. Os valores de IC50 foram calculados por um ajuste sigmoidal dos resultados obtidos, considerando-se um intervalo de confiança de 95%. Adicionalmente, os compostos foram avaliados quanto à capacidade para inibir a xantina oxidase, não se verificando atividade inibitória da mesma. A similaridade estrutural de 9b e bortezomib foi verificada visualmente, sendo realizados estudos in silico de superposição bidimensional e tridimensional. Os estudos de viabilidade celular foram realizados em fibroblastos normais da derme humana em células de cancro da próstata, e em células de cancro de cólon. Num screening inicial, as células de NHDF foram expostas aos compostos, em concentração de 10 e 100 µM durante 72 horas, tendo sido utilizado o 5-fluorouracilo como controlo positivo. A determinação da proliferação celular foi realizada mediante o ensaio do brometo de [3-(4,5-dimetiltiazol-2-yl)-2,5-difenil tetrazólio]. Para o composto 9b, que apresentou a maior atividade inibitória no ensaio do proteassoma e para bortezomib, efectuaram-se estudos de concentração-resposta. Para tal, os compostos foram incubados com as células, em seis concentrações distintas de cada composto no mesmo período de 72 horas, e os valores de IC50 foram calculados por um ajuste sigmoidal, considerando-se um intervalo de confiança de 95%. Adicionalmente, as células PC-3 e Caco-2 foram expostas a 9b e a bortezomib em diferentes concentrações, igualmente durante um período de 72 h. O composto 9b demonstrou citotoxicidade nas linhas celulares saudáveis e cancerígenas mas, contudo, esta foi inferior à citotoxicidade apresentada pelo bortezomibe. Como trabalho futuro, propõe-se estudos de viabilidade celular linha celular Jurkat, linha de células T leucémicas. Adicionalmente, propõe-se realizar estudos do docking molecular para prever o modo de ligação e afinidade de ligação do 9b com o proteassoma. Por outro lado, será igualmente importante sintesar mais derivados destes compostos, variando os substituintes no seu esqueleto molecular de modo a otimizar os resultados biológicos obtidos. O segundo capítulo é dedicado ao estágio em Farmácia Comunitária orientado pela Dr.ª Patrícia Pais e realizado na Farmácia Holon Covilhã. O terceiro capítulo refere-se ao estágio em Farmácia Hospitalar, que foi realizado no Barts Heart Centre, no Hospital St Bartholomew, em Londres, Reino Unido orientado pelo Dr.Paul Wright, Lead Cardiac Pharmacist em Barts Health NHS Trust. Em ambos os relatórios são abordados as atividades desenvolvidas, os conhecimentos adquiridos ao longo dos meus estágios em farmácia comunitária e em farmácia hospitalar.

The research project we are presenting focuses on the study and the reconstruction of the personality and artworks of Giulio Benso, a Genoese painter, draftsman and architect between Mannerism and Baroque, a specialist in illusionist decoration. The dispersion of a third of his works, in particular altar paintings and some fresco decorations, combined with the large number of graphic projects of different quality which have been attributed to him in recent decades have generated a bad reputation for his work, too often considered as a second-rate one compared to his contemporaries. In this study, his artistic activity will therefore be analyzed and contextualized using archival documents and ancient sources, from his training at the Academy of Giovanni Battista Paggi, a Genoese cultural environment, to the numerous Ligurian and foreign orders - German and French - assigned to him during his life, in order to highlight the contacts, the relationships with certain artists such as Giovanni Andrea Ansaldo and Giovanni Battista Carlone, the inspirations according to the Lombard, Venetian, Bolognese and Roman schools. The experiences he had and that allowed him to evolve in his work and to communicate his style to the successive generations of painters will also be taken into account. This analysis has been conducted in a precise and rigorous manner, by highlighting the existing documents on the artist, on his life and his works. The in-depth study of all the modern contributions published so far on him, on Genoa and on the Genoese school of the 16th and 17th centuries, has been enriched by documentary research in the various archives establishments of the Liguria region and by the examination of ancient sources, such as guides and manuscripts on the artistic production of the territory, with the aim of establishing the complete artistic activity of the painter, including destroyed paintings and frescoes, in order to highlight its evolution and the relationships with its contemporaries. This work thus presents an introductory part concerning the critical fortune lived by the painter during his life and after his death. This is followed by the second, which is focused on the study of his training at the Paggi Academy and on commissions for paintings and trompe l'oeil decorations received in Liguria. Then, a third chapter is devoted to missions abroad: in Germany, in Weingarten, described through the epistolary relationship between the artist and the sponsor Gabriele Bucellino, thanks to the old documents that were found and the letters transcribed by the heir of Benso, father Lorenzo Sertorio, and in France, in Cagnes-sur-Mer, in Provence, for which the vast iconographic and iconological program of the decor, chosen by the family, with a strong political reference has been studied. This study then presents two catalogs. One devoted to painting and the other to graphic production. The first follows a chronological order by mission, including lost or destroyed orders, thereby seeking to offer a reading of the painter's artistic investment in the social and cultural framework of Liguria, Germany and France. Long-term orders such as those from the Annunziata del Guastato in Genoa from 1638 to 1647 or those for Weingarten from 1627 to 1667 are processed in their unit to facilitate the analysis of their development. This is followed by the catalog of drawings presented chronologically. The study of Giulio Benso's graphic production requires a rigorous and scientific analysis based on stylistic as well as technical and documentary checks. Studies conducted in the past on the artist's drawings have revealed many attribution problems, which involve reordering. It is therefore proposed to try to establish the complete corpus of his graphic work by re-examining all the sheets attributed to him and those of the Genoese school kept in public and private collections. ; Le projet de recherche que nous présentons porte sur l'étude et la reconstruction de la personnalité et de l'œuvre de Giulio Benso, peintre, dessinateur et architecte génois entre le maniérisme et baroque, spécialiste dans le décor à trompe l'œil. La dispersion d'un tiers de ses œuvres, en particulier de retables et des quelques décors à fresque, unie au grand nombre de projets graphiques de diverses qualités qui lui ont été attribués dans les dernières décennies ont engendré une mauvaise réputation de son travail, trop souvent considéré de second ordre par rapport à ses contemporains. Dans le cadre de cette thèse, son activité artistique sera donc analysée et contextualisée à l'aide de documents d'archives et des sources anciennes, à partir de sa formation auprès de l'Académie de Giovanni Battista Paggi, dans un environnement culturel génois, en passant par les nombreuses commandes liguriennes et étrangères –allemandes et françaises– lui assignées au cours de la vie. Cela nous permettra de mettre en évidence les contacts, les relations avec certains artistes tels que Giovanni Andrea Ansaldo et Giovanni Battista Carlone, les inspirations d'après les écoles lombardes, vénitiennes, bolonaises et romaines et les expériences qu'il vécut et qui lui ont permis de faire évoluer son travail ainsi que de diffuser son style aux générations suivantes de peintres. Notre analyse sera conduite d'une manière rigoureuse, en mettant en avant les documents existants sur l'artiste, sur Gênes et sur l'École génoise du XVIe et XVIIe siècles. Par ailleurs, notre étude sera enrichie par les recherches documentaires dans les différents établissements d'archives et par le dépouillement des sources anciennes, dans le but d'établir la complète activité artistique du peintre, tableaux et fresques détruits compris, pour en mettre en valeur son évolution et les relations avec ses contemporains. Notre travail présente donc une partie introductive concernant la fortune critique que Benso connut au cours de sa vie et après sa mort. A cela, fait suite l'étude de sa formation à l'Académie de Paggi et des commandes de peintures et de décorations en trompe l'œil reçues à Gênes, dans le Ponente et à Pieve de Teco. Puis, un troisième chapitre est consacré aux missions à l'étranger : en Allemagne, plus précisément à Weingarten, pour lequel nous publions dans le détail la relation épistolaire entre l'artiste et le commanditaire Gabriele Bucellino, grâce aux documents anciens retrouvés et des lettres transcrites par l'héritier de Benso, père Lorenzo Sertorio. En France, à Cagnes-sur-Mer (Provence), où nous étudions le vaste programme iconographique et iconologique du décor, choisi par la famille, avec une forte référence politique. Puis, nous présentons deux catalogues. L'un consacré à la peinture et l'autre à la production graphique. Le premier suit un ordre chronologique par mission, incluant les commandes perdues ou détruites, en cherchant, ainsi, à offrir une lecture de l'investissement artistique du peintre, du cadre social et culturel de la Ligurie, de l'Allemagne et de la France. Les commandes de longues durées telles que celles de l'Annunziata del Guastato à Gênes de 1638 à 1647 et Weingarten, de 1627 à 1667 ont été traitées dans leur unité pour favoriser l'analyse de leur évolution. A cela, fait suite le catalogue des dessins présenté selon l'ordre chronologique d'exécution. L'étude de la production graphique de Benso nécessite une analyse rigoureuse et scientifique basée sur des vérifications stylistiques de même que documentaires, c'est donc ainsi que nous avons procédé. En effet, les analyses conduites dans le passé sur les dessins ont révélé beaucoup de problèmes d'attribution, qu'impliquaient une remise en ordre. Nous nous sommes proposés donc, de tenter d'établir le corpus complet de son œuvre graphique en réexaminant toutes les feuilles à lui attribuées et celles de l'école génoise conservées dans collections publiques et privées du monde.

Kanadas geographischer Norden liegt in dem Territorium Nunavut. Von hier aus ist es ebenso weit zum geographischen Nordpol wie zur US-kanadischen Grenze. Nunavut nimmt etwa 1/5 der kanadischen Landmasse ein, hat mit circa 38.000 Einwohnern jedoch bei weitem die kleinste Bevoelkerungsdichte des Landes. 85% der Einwohner sind Inuit deren Kultur sich in den vergangenen 70 Jahren radikal geaendert hat. Daher hat das Territorium heute mit mehreren Generationen von Inuit zu kaempfen die entweder traumatisiert sind oder zumindest schwer von den kulturellen und wirtschaftlichen Veraenderungen seit Ende des zweiten Weltkrieges gepraegt sind. Zur damaligen Zeit startete die Gruendung von festen Siedlungen in Nunavut und die Landflucht aus den temporaeren traditionellen Doerfern in die neuen Gemeinden. Egal ob wir von Senioren, Erwachsenen im mittleren Alter oder Jugendlichen sprechen, jede Generation hat eine Vielzahl von Veraenderungen erfahren, und erfaehrt sie immer noch. Diese beinhalten die Infragestellung persoenlicher und kultureller Identitaet, finanzielle Sicherheit, Wohnformen, Nahrungsmangel, Alkohol- und Drogenmisbrauch, Bildungssytem und der Wandel sozialer Werte wie inter-generationeller Wandel, Aufkommen neuer Geschlechterrollen, oder die Einfuehrung eines fremden politischen Systems und einer neuen Rechtsprechung. Andererseits werden heute auch weiterhin innerhalb von Inuitfamilien viele der traditionellen sozialen Werte praktiziert. Neben der Tragoedie, die mehrere aufeinanderfolgende Inuitgenerationen erleben mussten, bemueht sich die Gesellschaft das starke indigene Selbstbewusstsein wiederzubeleben, das sowohl dem Individuum als auch der Gesellschaft erlaubt ihre Kultur zu bewahren. Insbesondere Landrechtsorganisationen der Inuit oder indigene Nichtregierungsorganisationen treten fuer den Kulturerhalt ein, indem sie fuer mehr Anerkennung von und Stolz auf historische und rezente Errungenschaften der eigenen Kultur kaempfen. Die sozialen Probleme sowie die inner- und intra-kulturellen Prozesse die ich in meiner Arbeit beschreibe treffen nicht nur ausschliesslich auf Nunavut oder Inuitkultur zu. Stattdessen zeigen Studien aus anderen Regionen Kanadas und der Welt (LaPrairie 1987; Jensen 1986; Nunatsiaq News 6/30/2010) strukturell viele Aehnlichkeiten zur Situation in Nunavut auf. Obowhl durch den ueberregionalen Vergleich viele strukturelle Gemeinsamkeiten erkannt werden koennen, z.B. Marginalisierung der lokalen indigenen Bevoelkerung, Kolonialismus, Paternalisms und daraus entstehende Probleme wie persoenlicher und kultureller Identitaetsverlust, ist es ebenso wichtig einen tiefgruendigeren Blick auf die jeweiligen Besonderheiten der einzelnen regionalen Fallbeispiele zu werfen. Meiner Meinung nach, kann eine nachhaltige Verbesserung der Situation einer Gruppe, eines Dorfes oder Region nur dann erfolgen, wenn man kulturelle Besonderheiten in der Sozialisation, Kommunikation und Philisophie beruecksichtigt. Daher wird sich meine Dissertation ausschliesslich auf Nunavut und einige ausgewaehlte Fallbeispiele aus der Region konzentrieren. Die Fallbeispiele sollen lokale Unterschiede in der historischen Entwicklung und Gegenwartssituation illustrieren was zu einem besseren Verstaendnis der aktuellen Lage in den jeweiligen Doerfern sowie Nunavut als uebergordnete Einheit beitragen kann. Die Arbeit untersucht sowohl historische als auch rezente Ursachen die zu den vielen Problemen Nunavuts beitragen. Meine zeitgeschichtliche Einteilung in "Fruehkontaktphase", "Kontaktphase", "1. Generation" und "2. Generation" lehnt sich an Damas (2002: 7, 17) Terminologie von "Fruehkontaktphase", "Kontkat-Traditionsphase" und "Umsiedlungsphase" an. Er beschreibt damit drei Zeitperioden die sich deutlich in Bezug auf den Einfluss von Nicht-Inuit auf Inuit abheben. Kapitel zwei soll dabei die Kernaspekte der gegenwaertigen Sozialproblematik beschreiben. Diesbezueglich schaue ich auf die 4 Hauptfaktoren die meiner Meinung nach die Gesellschaft in Nunavut praegen: 1) Gewalt und andere soziale Probleme, 2) Die entsprechenden Organisationen und Programme die auf diese Probleme reagieren, 3) Bildung, 4) Inuitkulturspezifische Besonderheiten bezueglich Kommunikation und Sozialisation Diese vier Bereiche bilden die Grundlage fuer meine weiter Arbeit. Die darauffolgenden Kapitel fuehren den Leser durch den Wandlungsprozess einer vorkolonialen, halbnomadischen Gesellschaft zu einer sesshaften Gemeinschaft die sehr stark von einer Euro-kanadischen Lebensweise beeinflusst ist. Die einzelnen Kapitel behandeln jeweils eine neue Phase des kulturellen Wandels den ich in "Vor-Siedlungsphase, Erste, Zweite und Dritte Generation" unterteilt habe. Dabei untersuche ich die Formen und Relevanz von Gewalt und sozialer Probleme fuer die jeweilige Phase, wie sie mit der gesamtgeschichtlichen Entwicklung der Region zusammenhaengen, und wie die jeweiligen Generationen mit den Veraenderungen und Problemen umgegangen sind. . ; Canada's geographic centre lies in the Territory Nunavut. From here the distance to the geographic North Pole is as far as to the US border. Nunavut takes up about 1/5 of the Canadian land mass but has by far the smallest population with currently about 38,000 residents. 85% of its population are Inuit whose culture dramatically changed within the last 70 years. As a result, the territory is dealing with several generations of Inuit that are traumatized or at least severely affected by cultural and economic changes that started after World War 2 with the resettlement from the land into permanent communities. No matter if we are talking about the actual elders, mid-age adults or pre-teenagers, each of this generation experienced and still experiences various personal and cultural challenges of identity, financial and housing insecurity, food insecurity, substance abuse education, change of social values ranging from inter-generational and gender relationships to the introduction of a foreign political and legal system. On the other side, a lot of the traditional societal values are still being practiced in Inuit families. Despite all the tragedies that several generations of Inuit have experienced by now, the society keeps generating the strength and cultural pride that allows many Inuit both, as individuals and as a collective under the umbrella of either Inuit Land Claims or not for profit organizations to advocate on behalf of Inuit culture, to fight for more acknowledgement of Inuit culture and to enhance pride in the historic and present day cultural achievements of Nunavut's indigenous population. The social issues, inter- and intra-cultural processes described in my thesis are not exclusive to the situation in Nunavut or to Inuit. Studies from other regions, in Canada or from around the world (LaPrairie 1987; Jensen 1986; Nunatsiaq News 6/30/2010) reveal similar challenges. Though many structural similarities can be identified by comparing these studies with each other, e.g. marginalization of the indigenous local population, colonization, paternalism and resulting issues like personal and cultural identity loss, it is important to have a more in depth look into the single cases to determine which individual events and developments causes and maybe still cause such a devastating social situation as it is found among many indigenous peoples across the world. From my perspective effective improvements of the situation of a group, a respective community or region can only happen when particularities of socialization, communication and philosophy in the single cultural entities are being considered. That is why my thesis will exclusively focus on developments in Nunavut and use various case studies of communities. The case studies shall help to identify local differences in historic and recent developments and thus provide starting points for explanations of different developments in different Nunavut communities. The thesis is looking at both, historic and recent root causes for the many issues in Nunavut. The data that my my thesis is based on are a combination of literature and about 60 formal and informal interviews that I conducted in three Nunavut communities (Iqaluit, Whale Cove, Kugluktuk) during my 18 months of field work between October 2008 and March 2010. Many more spontaneous unstructured conversations between me and community members added to the pool of first-hand information that I gathered. Since my field work is limited to those three communities it has a very strong qualitative character. The quantitative side, which allows me to confidently apply my research analyses to entire Nunavut, comes from literature research as well as many informal conversations and a few formal interviews that I conducted with people who had some experience in other communities than Iqaluit, Kugluktuk and Whale Cove. Furthermore, while I was living at the old residence of the Nunavut Arctic College in Iqaluit, I spend time with college students from across Nunavut. Through them, I obtained "case studies "from following communities: Iqaluit, Qikiqtarjuaq, Kimmirut, Pangnirtung, Clyde River, Pond Inlet, Igloolik, Repulse Bay, Cape Dorset, Chesterfield Inlet, Baker Lake, Rankin Inlet, Whale Cove, Arviat, Taloyoak, Kugluktuk. My general categorization of "early contact period", "contact", "1st generation" and "2nd generation" is very similar to Damas' terms of "early contact phase", "contact – traditional", "resettlement" that he uses to create a timeline that describes the major phases of impact for Inuit society (Damas 2002: 7, 17). Chapters 2 is meant to provide an inventory of the key aspects of current social issues in Nunavut. In this context I am looking at the four major aspects that in my opinion shape Nunavut's society: 1) violence and other forms of social dysfunctions 2) the associated services and delivering agencies that try to address those matters 3) Education 4) Inuit cultural particularities in communication and socialization Those four areas are forming the foundation for the rest of my work. The following chapters will guide the reader through the historic transformation process of Inuit pre-colonial semi-nomadic society to a society that is living in permanent settlements, strongly influenced if not in many ways dominated by Euro-Canadian culture. Each of those chapters will be referring to the social and cultural changes that happened in the different time periods that I labeled with "Pre-settlement, First, Second, and Third Generation". The relevance of violence and other social dysfunctions, their context and strategies how each generation dealt with those matters will be analyzed while I will be also referring to the impacts that non-Inuit, primarily Euro-Canadians and Euro-Americans had and have on Inuit society. .

Актуальность. Неблагоприятная медико-демографическая ситуация в Украине определяет необходимость обоснования, создания и развития доступной, качественной и эффективной системы паллиативной и хосписной помощи (ПХП), что требует привлечения к оказанию ПХП медицинских работников учреждений здравоохранения (УЗ) первичной медицинской помощи (ПМП) и специализированных УЗ, координации работы учреждений, подчиненных различным министерствам и ведомствам, различных форм собственности. Цель работы: обосновать современную концептуальную модель оптимальной системы оказания ПХП населению Украины на уровне ПМП. Материалы и методы. В работе были использованы данные медицинской статистики, национальные и международные литературные источники, результаты социологического исследования, библиосемантические, статистические и социологические методы исследования, методы системного и структурно-функционального анализа. Результаты. Обоснована концептуальная модель оптимальной системы оказания ПХП на уровне ПМП, в частности, определены учреждения, которых привлекают к оказанию ПХП: 1) УЗ ПМП; 2) Выездные мультидисциплинарные бригады ПХП; 3) Лицензированные аптечные учреждения; 4) Учреждения социальной защиты населения. На уровне ПТП обеспечивается: 1) Выявление, организация определения статуса и учет паллиативных пациентов (ПП); 2) Оказание паллиативной медицинской помощи, организация госпитализации ПП в стационарные УЗ ПХП, по показаниям; 3) Организация психологической, социальной и религиозной/духовной поддержки и сопровождения ПП и членов их семей; 4) Консультирование, информирование и обучение ПП и членов их семей; 5) Координация и сотрудничество с выездной мультидисциплинарной бригадой ПХП или стационарными УЗ ПХП, врачами-специалистами, соответственно нозологии ПП, учреждениями социальной защиты населения, НПО, благотворительными фондами, волонтерами. Выводы. Медико-социальный анализ демографических показателей, заболеваемости и смертности в Украине свидетельствует о быстром сокращении численности и старении населения, высоком уровне смертности от злокачественных новообразований (ЗН) и тяжелых осложнений хронических неинфекционных заболеваний (ХНИЗ), определяющих высокую потребность населения в ПХП и требуют создания и развития доступной, качественной и эффективной системы ПХП. Существующая сеть и мощность стационарных УЗ ПХП – крайне недостаточна, что обусловливает значительную потребность оказания ПХП и паллиативного ухода на уровне ПМП с привлечением медицинских работников УЗ ПМП. Обоснована, разработана и апробирована на уровне отдельных инновационных составляющих концептуальная модель системы ПХП, основой которой является пациент-семья-ориентированный, мультидисциплинарный, межведомственный и межсекторальный подход на всех уровнях оказания медицинской помощи, с акцентом на ПМП, что соответствует современным международным стандартам и нормам, с целью интеграции ПХП в систему здравоохранения, которая обеспечит ее непрерывность и преемственность и обратную связь. Предложенная концептуальная модель оптимальной системы ПХП представляет собой высокопрофессиональную составляющую здравоохранения, требует современного высокотехнологичного оборудования и эффективных лекарственных средств, специальной профессиональной подготовки и мотивации кадров, соответствующей нормативно-правовой базы и, безусловно, надлежащего финансового обеспечения и политической поддержке Правительства Украина, региональных и местных органов власти, заинтересованности и поддержки всего общества. ; Background. The unfavorable medical and demographic situation in Ukraine requires an affordable, high-quality and efficient Palliative and Hospice Care (PHC) System substantiation, creation and development that requiring of Primary Health Care (PMC) providers (GPs) and health specialists involvement in PHC, such as Health Facilities (HF) of various Ministries and Departments, different forms of ownership Health Facilities (HF) work coordination. The purpose was to substantiate the optimal PHC System in Ukraine at the PMC level the modern conceptual model. Materials and methods. Medical statistics data, national and international literary sources, results of sociological research, bibliosemantic, statistical and sociological research methods, methods of systemic and structural-functional analysis were used in this work. Results. The optimal PHC System in Ukraine at the PMC level conceptual model is substantiated, in particular, the PHC involved institutions are substantiated: 1) PMC HF; 2) Multi-disciplinary PHC teams; 3) Licensed Pharmacy Facilities; 4) Social Care Facilities. At the PMC level it provides: 1) Identification, organization of status determination and registration of palliative patients (PP); 2) Providing palliative medical care, organization of PP hospitalization to stationary PHC HF, according to testimony; 3) PP and their families members of psychological, social and religious/spiritual support organization; 4) PP and their families members counseling, information and education; 5) Co-ordination and cooperation with a PHC multi-disciplinary team, or PHC HF stationary, health specialist according to the PP's nosology, social care institutions, NGOs, charitable foundations, volunteers. Conclusions. The medical and social analysis of demographic indicators, morbidity and mortality in Ukraine indicates a rapid decline in population and population aging, a high mortality rate from malignant neoplasms and chronic non-communicable diseases (HNCD) severe complications, which determine the high need of the population in PHC and require the affordable, high-quality, and efficient PHC System creation and development. The existing stationary PHC HF network and capacity is extremely inadequate, which results in a signi­ficant need for PHC at the PMC level with the PMC HF specialists involvement. The PHC System conceptual model that based on a patient-family-oriented, multidisciplinary, inter-departmental and multisectoral approach at all health care levels, with an emphasis on PMC, which is consistent with modern international standards and norms, in order to integrate PHC into the Health Care System is substantiated. The proposed optimal PHC System conceptual model is a highly professional component of Health Care System that requires modern high-tech equipment and effective medicines, personnel special training and motivation, a relevant regulatory framework and, of course, adequate the Government of Ukraine, regional and local authorities financial and political support and the interest and support of the whole society. ; Актуальність. Несприятлива медико-демографічна ситуація в Україні вимагає обґрунтування, створення і розвитку доступної, якісної і ефективної системи паліативної та хоспісної допомоги (ПХД), що потребує залучення до надання ПХД медичних працівників закладів охорони здоров'я (ЗОЗ) первинної медичної допомоги (ПМД) і спеціалізованих ЗОЗ, координації роботи закладів і установ, підпорядкованих різним міністерствам і відомствам, різних форм власності. Мета роботи: обґрунтувати сучасну концептуальну модель оптимальної системи надання ПХД населенню України на рівні ПМД. Матеріали та методи. У роботі були використані дані медичної статистики, національні та міжнародні літературні джерела, результати соціологічного дослідження, бібліосемантичні, статистичні і соціологічні методи дослідження, методи системного та структурно-функціонального аналізу. Результати. Обґрунтована концептуальна модель оптимальної системи надання ПХД на рівні ПМД, зокрема визначені заклади, що залучаються до надання ПХД: 1) ЗОЗ ПМД; 2) Виїзні мультидисциплінарні бригади ПХД; 3) Ліцензовані аптечні заклади; 4) Заклади соціального захисту населення. На рівні ПМД забезпечується: 1) Виявлення, організація визначення статусу та облік паліативних пацієнтів (ПП); 2) Надання паліативної медичної допомоги, організація госпіталізації ПП до стаціонарних ЗОЗ ПХД, за показаннями; 3) Організація психологічної, соціальної і релігійної/духовної підтримки і супроводу ПП і членів його сім'ї; 4) Консультування, інформування та навчання ПП і членів його сім'ї; 5) Координація та співпраця з виїзною мультидисциплінарною бригадою ПХД або стаціонарним ЗОЗ ПХД, лікарями-спеціалістами відповідно нозології ПП, закладами соціального захисту населення, НДО, благодійними фондами, волонтерами. Висновки. Медико-соціальний аналіз демографічних показників, захворюваності і смертності в Україні свідчить про швидке скорочення чисельності та постаріння населення, високий рівень смертності від злоякісних новоутворень (ЗН) і тяжких ускладнень хронічних неінфекційних захворювання (ХНІЗ), що визначають високу потребу населення у ПХД та вимагають створення і розвитку доступної, якісної і ефективної системи ПХД. Існуюча мережа та потужність стаціонарних ЗОЗ ПХД – вкрай недостатня, що зумовлює значну потребу надання ПХД та паліативного догляду на рівні ПМД із залученням медичних працівників ЗОЗ ПМД. Обґрунтовано, розроблено та апробовано на рівні окремих інноваційних складових концептуальну модель системи ПХД, основою якої є пацієнт-сім'я-орієнтований, мультидисциплінарний, міжвідомчий і міжсекторальний підхід на усіх рівнях надання медичної допомоги, з акцентом на ПМД, що відповідає сучасним міжнародним стандартам і нормам, з метою інтеграції ПХД в систему охорони здоров'я, що забезпечить безперервність та наступність її надання і зворотний зв'язок. Запропонована концептуальна модель оптимальної системи ПХД являє собою високопрофесійну складову охорони здоров'я, що потребує сучасного високотехнологічного обладнання та ефективних лікарських засобів, спеціальної професійної підготовки і мотивації кадрів, відповідної нормативно-правової бази і, безумовно, належного фінансового забезпечення і політичної підтримки Уряду України, регіональних і місцевих органів влади та зацікавленості і підтримки усього суспільства.

Contexte. L'augmentation du nombre de personnes âgées inuit est un phénomène récent dans les communautés nordiques et crée de nouveaux défis socio sanitaires. Cependant, les connaissances sur les déterminants sociaux (DSS) du vieillissement en bonne santé des populations inuit sont limitées. La définition inuit de la santé est holistique et géographique : la santé est créée par les interactions entre les individus et leur environnement. Cette dimension géographique est centrale pour comprendre comment les DSS favorisent un vieillissement en bonne santé, mais est rarement incluse dans les modèles de DSS inuit. Ma thèse de doctorat a pour but de conceptualiser et d'opérationnaliser un modèle des déterminants sociogéographiques du vieillissement en bonne santé dans les communautés inuit. Trois objectifs de recherche sont poursuivis : 1) conceptualiser, opérationnaliser et valider le concept de santé pour les Inuit âgés de 50 et plus ; 2) identifier les DSS à l'échelle individuelle, du logement et de la communauté associés à un vieillissement en bonne santé ; et 3) explorer la dimension géographique dans les mécanismes connectant les DSS au vieillissement en bonne santé. Méthodes. Cette thèse a été réalisée avec une méthodologie mixte et un devis exploratoire et explicatif. Les données qualitatives utilisées pour conceptualiser la définition de la santé proviennent d'ateliers organisés dans deux communautés du Nunavik en 2016. Des analyses de classes latentes ont été réalisées avec les données de l'Enquête auprès des peuples autochtones de 2006 (n = 850 Inuit ≥ 50 ans) pour opérationnaliser la définition de la santé en un indicateur holistique de santé. Les associations entre l'indicateur de santé holistique et les DSS mesurés aux échelles, individuelle, du logement et de la communauté ont été modélisées avec des régressions multinomiales. Une collecte de données a été réalisée à Baker Lake, Nunavut, à l'aide d'entretiens approfondis et d'un groupe de discussion auprès de 20 participants âgés de 50 à 86 ans pour explorer la dimension géographique des DSS et comprendre les mécanismes les connectant à la santé. Des analyses thématiques ont été réalisées sur ces données. Résultats. La santé était définie avec huit concepts : la santé générale, les limitations physiques, la santé mentale, la spiritualité, les comportements de santé, parler inuktitut, avoir des relations positives et de l'affection. Un indicateur de santé holistique à trois catégories a été créé avec les analyses de classes latentes : les participants en « bonne santé » pour tous les indicateurs, ceux ayant un profil de « santé intermédiaire » et les participants en « mauvaise santé » pour la plupart des indicateurs. Les associations entre l'indicateur de santé holistique et les DSS aux échelles individuelle, du logement et de la communauté étaient différentes pour les trois profils de santé. Par rapport à ceux ayant un profil de mauvaise santé, les personnes âgées inuit en bonne santé avaient plus de chance d'avoir de forts liens familiaux dans la communauté et de participer à des activités sociales. Celles ayant un profil de santé intermédiaire avaient plus de chances de vivre dans une communauté avec un statut socio-économique élevé et dans des logements en meilleures conditions. Les participants de ces deux profils étaient plus susceptibles que ceux en mauvaise santé d'avoir participé à des activités sur le territoire au cours de la dernière année. Les mécanismes géographiques liant les DSS au vieillissement en bonne santé ont été identifiés lors des entrevues et d'un groupe de discussion avec des aînés à Baker Lake. Des activités familiales et communautaires favorisaient les relations sociales positives et les connections symboliques au territoire, notamment le partage de nourriture traditionnelle. Des conditions de logement et des services communautaires adaptés à la santé des personnes âgées permettaient aux ainés de vieillir dans leur communauté. Des moyens de transports et d'informations adaptés étaient des facteurs facilitant l'accès aux ressources favorables à la santé entre les échelles du logement, de la communauté et du territoire. Conclusion. La dimension géographique des DSS conceptualisée dans cette thèse est centrale pour comprendre comment ces déterminants sociogéographiques influencent le vieillissement en bonne santé. Ces connaissances sont nécessaires pour guider l'élaboration et l'implémentation des politiques et des programmes sociaux et de santé adaptés au contexte de vieillissement en bonne santé dans les communautés inuit. ; Background: The aging of the Inuit population is a new phenomenon creating new and unique social and health challenges across the Arctic. There is relatively limited evidence about the health profile and the social determinants of health (SDH) for Inuit elders. The definition of Inuit health is holistic, and is anchored in a geographical dimension such that health is created through interactions between people and the land. This geographic dimension is central to understand the role of SDH in supporting healthy aging, yet are seldom included in Inuit SDH models. The overall aim of my doctoral thesis is to conceptualize and operationalize a model of the sociogeographic determinants supporting healthy aging in Inuit communities. This model is built in coherence with Inuit's definition of health which is The thesis follows three research objectives: 1) to conceptualize, operationalize and validate the concept of health for older Inuit aged 50 years and over; 2) to identify social determinants of health (SDH) at the individual, housing, and community levels associated with healthy aging; and 3) to explore the geographical dimension of the mechanisms connecting the social determinants to healthy aging. Methods: This thesis employs a mixed-methods exploratory and explanatory research design. Qualitative data used to conceptualize the definition of health for Inuit elders were retrieved from workshops conducted in two Nunavik communities in 2016. Using quantitative data from the 2006 Aboriginal Peoples Survey (APS) (n = 850 Inuit ≥ 50 years), latent class analyses was then used to operationalize the definition of health into a holistic indicator health. Associations between this indicator and selected SDH at the individual, housing and community scales were modelled using multinomial regressions. To further explore the geographical dimension of the SDH, and to understand the mechanisms linking the SDH to health, in-depth interviews and one focus groups with 20 participants aged 50 to 86 were conducted in Baker Lake, Nunavut. Thematic analyses were conducted on the data. Results: Health was defined along eight concepts: overall health, physical limitations, mental health, spirituality, health behaviours, speaking Inuktitut, having positive relationships, and affection. A three-category holistic health indicator was created from the latent class analysis: participants in "good health" for all indicators, those with an "intermediate health" profile, and participants in "poor health" for most indicators. Associations between the holistic health indicator and SDH at individual, housing and community scales were different for the three health profiles. Compared to those with a poorer health profile, older adults with a good health profile were more likely to have strong family ties in the community and to participate in social activities. Older adults with an intermediate health profile were more likely to live in a community with a higher socio-economic status, to live in better housing conditions. In comparison to older adults with a poorer health profile, those with good or intermediate health profiles were more likely to have engaged in land-based activities in the past year. The geographic mechanisms linking the SDH to healthy aging emerged through interviews and focus group with elders in Baker Lake. Family and community activities promoted positive social relationships and symbolic connections to the land, including the sharing of traditional food. Housing conditions and community services adapted for elders' health offered the possibility to age in place, i.e., in one's home and community. Adapted transportation and information systems were identified as facilitating factors to access resources supportive for health at the residential, community and environmental levels. Conclusion. The geographic dimension of SDH conceptualized and operationalized in this thesis is important to understand how sociogeographic determinants influence the health of Inuit elders. This type of information is needed to guide the formulation and implementation of social and public health policies and programs to support healthy aging across Inuit communities

Esta tesis tiene como objetivo proporcionar respuestas a preguntas sin resolver en nuestra comprensión del GC, así como proporcionar nuevas restricciones en la estructura del MWNSC mediante el uso de imágenes de alta resolución y la mejora del análisis. Examino las imágenes en un amplio rango de distancias desde el centro de la Galaxia: desde distancias muy grandes (~860 pc x 280 pc) hasta los arco-segundos más profundos donde habita el agujero negro supermasivo. En primer lugar, mi objetivo es estudiar el tamaño y la estructura espacial del MWNSC [1]. Utilizo datos y métodos que abordan posibles deficiencias en trabajos anteriores sobre el tema. Utilizo 0.2'' resolución angular Ks datos para crear un mapa de densidad estelar en el GC en los 86.4 pc x 21 pc centrales. Incluyo datos de imágenes asistidas por AO obtenidas para los parsecs internos. Además, utilizo imágenes del medio infrarrojo de IRAC/Spitzer. Modelo el bulbo galáctico y el disco estelar nuclear para subtraerlos del MWNSC. Finalmente, ajusto un modelo Sérsic a el MWNSC e investigo su simetría. Encuentro que el MWNSC está aplanado con una relación de eje de q=0.71± 0.06, un radio efectivo de Re = (5.1± 0.6) pc, y un índice Sérsic de n = 2.2 ± 0.4. Su eje mayor se puede inclinar desde el plano galáctico hasta -10 grados. La distribución de las estrellas gigantes más brillantes que el grupo rojo (o RC, del inglés red clump) es significativamente más plano que la distribución de las estrellas débiles. El MWNSC comparte sus principales propiedades con otros NSCs extragalácticos encontrados en galaxias espirales. Las diferencias en la estructura entre gigantes brillantes y las estrellas RC podrían estar relacionadas con la existencia de poblaciones de diferentes edades no completamente mezcladas. Esto puede indicar un crecimiento reciente del MWNSC a través de la formación de estrellas o la acreción de cúmulos. También reviso el problema de inferir la estructura más interna del MWNSC, para aclarar si muestra un corazón o una cúspide alrededor del agujero negro central. La existencia de una cúspide de densidad estelar dinámicamente relajada en grupos densos alrededor de agujeros negros masivos es una antigua predicción de la dinámica estelar, pero hasta ahora ha escapado de la confirmación observacional sin ambigüedades. Para estudiar la distribución de estrellas alrededor de SgrA*, uso los datos obtenidos con el instrumento NACO en el telescopio ESO/Very Large Telescope (VLT) y me centro en dos métodos diferentes para analizar tres rangos de brillo estelar diferentes [2, 3]. Encuentro que una ley de Nuker proporciona una descripción adecuada de la forma intrínseca del cúmulo nuclear (asumiendo simetría esférica). Encuentro que la densidad estelar disminuye con un índice 3D de ley de potencia dentro del rango gamma=1.1-1.4 para distancias más pequeñas que el radio de influencia de SgrA* (~3 pc). Podemos descartar un corazón plano con alta confianza. La cúspide es menos profunda que la pronosticada por la teoría, pero puede explicarse si se tiene en cuenta la historia de formación estelar del NSC [4]. Finalmente, me asomo al mismo centro de la Galaxia mejorando la reducción y al análisis de los datos existentes. Gracias a la técnica holográfica y el análisis bootstrapping, detectamos la emisión de NIR de SgrA* en épocas anteriores a 2003, cuando se tomaron las primeras mediciones debido al desarrollo de AO. Esto permite a [5] estudiar la actividad en el NIR a largo plazo y explorar el flujo de acreción en el agujero negro en épocas donde no se había detectado hasta el momento. ; This thesis aims at providing answers to unresolved questions in our understanding of the GC, as well as provide new constraints on the structure the MWNSC by using high resolution images and by improving the analysis. I examine images in a broad span of distances from the center of the Galaxy: from very large distances (~860 pc x 280 pc) to the innermost arcseconds where the supermassive black hole inhabits. Firstly, I aim to study the size and spatial structure of the MWNSC. I use data and methods that address potential shortcomings in previous work on the topic. I use 0:200 angular resolution Ks data to create a stellar density map in the central 86.4pc x 21 pc at the GC. I include data from selected AO assisted images obtained for the inner parsecs. In addition, I use IRAC/Spitzer MIR images. I model the Galactic bulge and the nuclear stellar disk in order to subtract them from the MWNSC. Finally, I fit a Sérsic model to the MWNSC and investigate its symmetry. I find that the MWNSC is flattened with an axis ratio of q = 0:71 _ 0:06, an effective radius of Re = (5:1 _ 0:6) pc, and a Sérsic index of n = 2:2 _ 0:4. Its major axis may be tilted out of the Galactic plane by up to -10 degree. The distribution of the giants brighter than the Red Clump (RC) is found to be significantly flatter than the distribution of the faint stars. The MWNSC shares its main properties with other extragalactic NSCs found in spiral galaxies. The differences in the structure between bright giants and RC stars might be related to the existence of not completely mixed populations of diferent ages. This may hint at recent growth of the MWNSC through star formation or cluster accretion. I also revisit the problem of inferring the innermost structure of the MWNSC, to clarify whether it displays a core or a cusp around the central black hole. The existence of dynamically relaxed stellar density cusp in dense clusters around massive black holes is a long-standing prediction of stellar dynamics, but it has so far escaped unambiguous observational confirmation. In order to study the distribution of stars around Sgr A*, I use data obtained with the NACO instrument at the ESO/Very Large Telescope (VLT) and I focus on two different methods to analyze three different stellar brightness ranges. I find that a Nuker law provides an adequate description of the nuclear cluster's intrinsic shape (assuming spherical symmetry). I find that the stellar density decreases with a 3D power-law index inside the range ϒ =1:1 - 1:4 for distances smaller than the influence radius of Sgr A* (~ 3 pc). We can rule out a at core with high confidence. The cusp is shallower than the predicted one by theory, but it can be explained if the star formation history of the NSC is taken into acount (Baumgardt et al., 2018). Finally, I peek at the very center of the Galaxy by improving the reduction and analysis of existing data. I use holographic technique and bootstrapping analysis. This has led to the detection of NIR emission of Sgr A* in epochs prior to 2003, when the first measurements were taken due to the development of AO. That allows Chen et al. (in preparation) to study the long-term NIR activity and explore the accretion flow onto the black hole in epochs where it had not been detected so far. ; Tesis Univ. Актуальность. Неблагоприятная медико-демографическая ситуация в Украине определяет необходимость обоснования, создания и развития доступной, качественной и эффективной системы паллиативной и хосписной помощи (ПХП), что требует привлечения к оказанию ПХП медицинских работников учреждений здравоохранения (УЗ) первичной медицинской помощи (ПМП) и специализированных УЗ, координации работы учреждений, подчиненных различным министерствам и ведомствам, различных форм собственности. Цель работы: обосновать современную концептуальную модель оптимальной системы оказания ПХП населению Украины на уровне ПМП. Материалы и методы. В работе были использованы данные медицинской статистики, национальные и международные литературные источники, результаты социологического исследования, библиосемантические, статистические и социологические методы исследования, методы системного и структурно-функционального анализа. Результаты. Обоснована концептуальная модель оптимальной системы оказания ПХП на уровне ПМП, в частности, определены учреждения, которых привлекают к оказанию ПХП: 1) УЗ ПМП; 2) Выездные мультидисциплинарные бригады ПХП; 3) Лицензированные аптечные учреждения; 4) Учреждения социальной защиты населения. На уровне ПТП обеспечивается: 1) Выявление, организация определения статуса и учет паллиативных пациентов (ПП); 2) Оказание паллиативной медицинской помощи, организация госпитализации ПП в стационарные УЗ ПХП, по показаниям; 3) Организация психологической, социальной и религиозной/духовной поддержки и сопровождения ПП и членов их семей; 4) Консультирование, информирование и обучение ПП и членов их семей; 5) Координация и сотрудничество с выездной мультидисциплинарной бригадой ПХП или стационарными УЗ ПХП, врачами-специалистами, соответственно нозологии ПП, учреждениями социальной защиты населения, НПО, благотворительными фондами, волонтерами. Выводы. Медико-социальный анализ демографических показателей, заболеваемости и смертности в Украине свидетельствует о быстром сокращении численности и старении населения, высоком уровне смертности от злокачественных новообразований (ЗН) и тяжелых осложнений хронических неинфекционных заболеваний (ХНИЗ), определяющих высокую потребность населения в ПХП и требуют создания и развития доступной, качественной и эффективной системы ПХП. Существующая сеть и мощность стационарных УЗ ПХП – крайне недостаточна, что обусловливает значительную потребность оказания ПХП и паллиативного ухода на уровне ПМП с привлечением медицинских работников УЗ ПМП. Обоснована, разработана и апробирована на уровне отдельных инновационных составляющих концептуальная модель системы ПХП, основой которой является пациент-семья-ориентированный, мультидисциплинарный, межведомственный и межсекторальный подход на всех уровнях оказания медицинской помощи, с акцентом на ПМП, что соответствует современным международным стандартам и нормам, с целью интеграции ПХП в систему здравоохранения, которая обеспечит ее непрерывность и преемственность и обратную связь. Предложенная концептуальная модель оптимальной системы ПХП представляет собой высокопрофессиональную составляющую здравоохранения, требует современного высокотехнологичного оборудования и эффективных лекарственных средств, специальной профессиональной подготовки и мотивации кадров, соответствующей нормативно-правовой базы и, безусловно, надлежащего финансового обеспечения и политической поддержке Правительства Украина, региональных и местных органов власти, заинтересованности и поддержки всего общества. ; Background. The unfavorable medical and demographic situation in Ukraine requires an affordable, high-quality and efficient Palliative and Hospice Care (PHC) System substantiation, creation and development that requiring of Primary Health Care (PMC) providers (GPs) and health specialists involvement in PHC, such as Health Facilities (HF) of various Ministries and Departments, different forms of ownership Health Facilities (HF) work coordination. The purpose was to substantiate the optimal PHC System in Ukraine at the PMC level the modern conceptual model. Materials and methods. Medical statistics data, national and international literary sources, results of sociological research, bibliosemantic, statistical and sociological research methods, methods of systemic and structural-functional analysis were used in this work. Results. The optimal PHC System in Ukraine at the PMC level conceptual model is substantiated, in particular, the PHC involved institutions are substantiated: 1) PMC HF; 2) Multi-disciplinary PHC teams; 3) Licensed Pharmacy Facilities; 4) Social Care Facilities. At the PMC level it provides: 1) Identification, organization of status determination and registration of palliative patients (PP); 2) Providing palliative medical care, organization of PP hospitalization to stationary PHC HF, according to testimony; 3) PP and their families members of psychological, social and religious/spiritual support organization; 4) PP and their families members counseling, information and education; 5) Co-ordination and cooperation with a PHC multi-disciplinary team, or PHC HF stationary, health specialist according to the PP's nosology, social care institutions, NGOs, charitable foundations, volunteers. Conclusions. The medical and social analysis of demographic indicators, morbidity and mortality in Ukraine indicates a rapid decline in population and population aging, a high mortality rate from malignant neoplasms and chronic non-communicable diseases (HNCD) severe complications, which determine the high need of the population in PHC and require the affordable, high-quality, and efficient PHC System creation and development. The existing stationary PHC HF network and capacity is extremely inadequate, which results in a signi­ficant need for PHC at the PMC level with the PMC HF specialists involvement. The PHC System conceptual model that based on a patient-family-oriented, multidisciplinary, inter-departmental and multisectoral approach at all health care levels, with an emphasis on PMC, which is consistent with modern international standards and norms, in order to integrate PHC into the Health Care System is substantiated. The proposed optimal PHC System conceptual model is a highly professional component of Health Care System that requires modern high-tech equipment and effective medicines, personnel special training and motivation, a relevant regulatory framework and, of course, adequate the Government of Ukraine, regional and local authorities financial and political support and the interest and support of the whole society. ; Актуальність. Несприятлива медико-демографічна ситуація в Україні вимагає обґрунтування, створення і розвитку доступної, якісної і ефективної системи паліативної та хоспісної допомоги (ПХД), що потребує залучення до надання ПХД медичних працівників закладів охорони здоров'я (ЗОЗ) первинної медичної допомоги (ПМД) і спеціалізованих ЗОЗ, координації роботи закладів і установ, підпорядкованих різним міністерствам і відомствам, різних форм власності. Мета роботи: обґрунтувати сучасну концептуальну модель оптимальної системи надання ПХД населенню України на рівні ПМД. Матеріали та методи. У роботі були використані дані медичної статистики, національні та міжнародні літературні джерела, результати соціологічного дослідження, бібліосемантичні, статистичні і соціологічні методи дослідження, методи системного та структурно-функціонального аналізу. Результати. Обґрунтована концептуальна модель оптимальної системи надання ПХД на рівні ПМД, зокрема визначені заклади, що залучаються до надання ПХД: 1) ЗОЗ ПМД; 2) Виїзні мультидисциплінарні бригади ПХД; 3) Ліцензовані аптечні заклади; 4) Заклади соціального захисту населення. На рівні ПМД забезпечується: 1) Виявлення, організація визначення статусу та облік паліативних пацієнтів (ПП); 2) Надання паліативної медичної допомоги, організація госпіталізації ПП до стаціонарних ЗОЗ ПХД, за показаннями; 3) Організація психологічної, соціальної і релігійної/духовної підтримки і супроводу ПП і членів його сім'ї; 4) Консультування, інформування та навчання ПП і членів його сім'ї; 5) Координація та співпраця з виїзною мультидисциплінарною бригадою ПХД або стаціонарним ЗОЗ ПХД, лікарями-спеціалістами відповідно нозології ПП, закладами соціального захисту населення, НДО, благодійними фондами, волонтерами. Висновки. Медико-соціальний аналіз демографічних показників, захворюваності і смертності в Україні свідчить про швидке скорочення чисельності та постаріння населення, високий рівень смертності від злоякісних новоутворень (ЗН) і тяжких ускладнень хронічних неінфекційних захворювання (ХНІЗ), що визначають високу потребу населення у ПХД та вимагають створення і розвитку доступної, якісної і ефективної системи ПХД. Існуюча мережа та потужність стаціонарних ЗОЗ ПХД – вкрай недостатня, що зумовлює значну потребу надання ПХД та паліативного догляду на рівні ПМД із залученням медичних працівників ЗОЗ ПМД. Обґрунтовано, розроблено та апробовано на рівні окремих інноваційних складових концептуальну модель системи ПХД, основою якої є пацієнт-сім'я-орієнтований, мультидисциплінарний, міжвідомчий і міжсекторальний підхід на усіх рівнях надання медичної допомоги, з акцентом на ПМД, що відповідає сучасним міжнародним стандартам і нормам, з метою інтеграції ПХД в систему охорони здоров'я, що забезпечить безперервність та наступність її надання і зворотний зв'язок. Запропонована концептуальна модель оптимальної системи ПХД являє собою високопрофесійну складову охорони здоров'я, що потребує сучасного високотехнологічного обладнання та ефективних лікарських засобів, спеціальної професійної підготовки і мотивації кадрів, відповідної нормативно-правової бази і, безумовно, належного фінансового забезпечення і політичної підтримки Уряду України, регіональних і місцевих органів влади та зацікавленості і підтримки усього суспільства.

The article reviews the specific field of activity of the staff and graduates of the Chernivtsi Medical Institute and their participation in helping the countries of the "third world" at the initial stage of the existence of an educational institution. Among the doctors who had been sent abroad were mentioned Osipov B.K., Kantor O.A., Zakryvydoroga S.P., Petrosyan N.G., Mokhun I.K., Lipkovsky V.P., Cherevko S.A. , Anipko AP, Shkvaruk MS, Khodorovsky G.I., Sinchuk O.M., Haidychuk S.T., Trefanenko AG, Trefanenko SP, Shcherbak V.V., Hayko GV, Zabolotniy MS, Vasiliev O.G., Kravets AA etc. The purpose of the study is to integrate the description of this historical period of BSMI in the chronological framework of the first 30th anniversary of its activities. Its relevance is dictated by the approaching 75th anniversary of the BSMU, in which the main role belongs to its employees of different generations. And the choice of the chronological framework of the article was made by the previous articles basis of the authors, in which they tried to highlight the least studied period of the existence of the CHESMI. In addition, the authors of the article suppose that the intellectual potential of Ukraine is one of the country's greatest wealth. The method of work is based on the analysis of archival, bibliographic materials, memoirs, synthesis of data from various sources. Conclusions. In line with the foreign policy of the USSR, which included, along with other forms of assistance to the "Third World" countries and assistance to highly qualified medical stuff, a number of staff and graduates of the Chernivtsi Medical Institute were sent abroad. All of them were specialists of high quality in their medical areas, most of them had scientific degrees of candidates of medical sciences, they possessed foreign languages. They also became selfmade doctors and teachers, who honestly performed the Hippocratic oath in the extreme weather conditions for Europeans. They continued research work, counseled and treated people, participated in the training of local staff, in advocating for medical knowledge among the population of the "Third World" countries. The results of research conducted in the countries of their business trip, were published materials in foreign publications. This activity promoted intellectual self-enrichment, increased professional skills and pedagogical skills. They duly represented their homeland abroad. ; L'article passe en revue le domaine d'activité spécifique du personnel et des diplômés de l'Institut médical de Tchernivtsi et leur participation à l'aide apportée aux pays du "tiers monde" au stade initial de l'existence d'un établissement d'enseignement. Sa pertinence est dictée par le 75e anniversaire du BSMU, dont le rôle principal appartient à ses employés de différentes générations. Et le choix du cadre chronologique de l'article a été fait par les articles de base des auteurs précédents, dans lesquels ils essayaient de mettre en évidence la période la moins étudiée de l'existence du CHESMI. En outre, les auteurs de l'article supposent que le potentiel intellectuel de l'Ukraine constitue l'une des plus grandes richesses du pays. La méthode de travail repose sur l'analyse de documents d'archives, de documents bibliographiques, de mémoires, de synthèses de données provenant de diverses sources. Conclusions. Conformément à la politique étrangère de l'URSS, qui comprenait, outre d'autres formes d'assistance aux pays du «tiers monde» et d'assistance à des produits médicaux hautement qualifiés, un certain nombre d'employés et de diplômés de l'Institut médical de Tchernivtsi ont été envoyés à l'étranger. Tous étaient des spécialistes de haute qualité dans leur domaine médical, la plupart d'entre eux étaient titulaires de diplômes scientifiques de candidats en sciences médicales et possédaient des langues étrangères. Ils sont également devenus des docteurs et des enseignants autonomes, qui ont honnêtement prêté le serment d'Hippocrate dans des conditions climatiques extrêmes pour les Européens. Ils ont poursuivi leurs travaux de recherche, conseillé et soigné des personnes, participé à la formation du personnel local et défendu les connaissances médicales au sein de la population des pays du "tiers monde". Esta investigación ten como obxectivo desentrañar as estratexias mediante as cales se leva a cabo o proceso de ensino e a aprendizaxe dos universos democráticos (ideas, experiencias e práticas democráticas) do alumnado de Educación Secundaria Obrigatoria na Provincia da Coruña, Galicia. O corpus de análise está integrado por 58 libros de texto das materias Educación Ético-Cívica, Educación para la Ciudadanía y los Derechos Humanos, Historia, Latin, Lingua castelá e Literatura, Lingua galega e Literatura, Relixión e Xeografía) de Educación Secundaria Obrigatoria (ESO) das editoriais Anaya, Edebé, Rodeira, Santillana e SM dos cuatro niveis da ESO. Empregouse unha metodoloxía de análise cualitativa de datos, cun itinerario analítico dedutivo-indutivo. As categorías dedutivas construironse a partires duhna síntese das categorías empregadas, por unha banda, no estudo comparativo internacional da International Association for the Evaluation of Educational Achievement (IEA), titulado International Report: Civic knowledge, attitudes, and engagement among lower-secondary school students in 38 countries (ICCS 2009); e por outra, naquelas categorías construidas por investigadores no campo da democracia e contenda política como David Held, Pierre Rosanvallon e Boaventura de Sousa Santos. De igual modo, mediante a realización de entrevistas semiestruturadas e grupos de discusión do alumnado e entrevistas ao profesorado, levouse a cabo unha aproximación a 11 Institutos de Educación Secundaria (IES) para establecer o modelo de prácticas e experiencias democráticas que son proporcionadas e promovidas na vida escolar e desde as institucións escolares. Os principais resultados aos que se chegaron nesta investigación son dous. O discurso curricular (textos e imaxes) orienta a idea de democracia cara a un deseño estritamente electoralista, representativo e elitista en detrimento doutros deseños institucionais democráticos como a democracia participativa, a democracia de alta intensidade ou, en xeral, aqueles deseños nos que teñen cabida outros vectores de expresión sociopolítica diferentes ao ritual electoral e que se vinculan coas acción sociais e coa participación política cidadá. This research aims to unravel the strategies for the teaching and learning of democratic universes (knowledges, ideas, experiences and democratic practices) of compulsory secondary education students in the Province of A Coruña, Galicia. The corpus examined is composed of Citizenship Education, Classic Culture, Ethic-Civics Education, Geography, History, Language and Literature, Latin and Religion textbooks published by Anaya, Edebé, Rodeira, Santillana and SM for the four year levels of Obligatory Secondary Education (ESO). A qualitative data analysis methodology has been used, with a deductive-inductive analytical itinerary. The deductive categories have been constructed from a synthesis of the categories used in the international comparative study of the International Association for the Evaluation of Educational Achievement (IEA), entitled International Report: Civic knowledge, attitudes, and engagement among lower-secondary school students in 38 countries (ICCS 2009). To these we have added categories developed by researchers in the field of democracy such as David Held, Pierre Rosanvallon and Boaventura de Sousa Santos. Through semi-structured interviews and discussion groups of students and interviews with teachers, we have consulted with 11 Secondary Education Institutes (IES) to establish the model of democratic practices and experiences that are provided and promoted in the school life and from school institutions. The main results of this study include, in the first place, that the curricular discourse (texts and images) guides the idea of democracy towards an eminently electoral, representative and elitist design to the detriment of other democratic institutional designs such as participatory democracy and high intensity democracy. In general, there is a tendency to exclude or undermine those designs involving other vectors of sociopolitical expression different from the electoral ritual, which are linked to social actions and citizen political participation. In the second place, the interviews, discussion groups and observations show that the experiences and democratic practices in the classroom and school reinforce a procedural, electoral and elitist idea of democracy, at the same time that they do not favor or allow participatory learning of democracy.

The article reviews the specific field of activity of the staff and graduates of the Chernivtsi Medical Institute and their participation in helping the countries of the "third world" at the initial stage of the existence of an educational institution. In line with the foreign policy of the USSR, which included, along with other forms of assistance to the "Third World" countries and assistance to highly qualified medical stuff, a number of staff and graduates of the Chernivtsi Medical Institute were sent abroad. All of them were specialists of high quality in their medical areas, most of them had scientific degrees of candidates of medical sciences, they possessed foreign languages. They also became selfmade doctors and teachers, who honestly performed the Hippocratic oath in the extreme weather conditions for Europeans. They continued research work, counseled and treated people, participated in the training of local staff, in advocating for medical knowledge among the population of the "Third World" countries. The results of research conducted in the countries of their business trip, were published materials in foreign publications. This activity promoted intellectual self-enrichment, increased professional skills and pedagogical skills.
