In 2019, U.S. President Donald Trump was impeached for his ongoing conduct toward the Government of Ukraine. While much has been written about the domestic implications, few have raised questions about the potential violation of standing international agreements, especially those between the U.S. and Ukraine. Based upon a close textual analysis of available government documents and the agreement itself, we conclude that President Trump's 2019 actions toward the Government of Ukraine violated the 1994 Budapest Memorandum.Based upon the available government documents and a close textual analysis of relevant sections of the agreement, we conclude that President Trump's 2019 conduct toward the Government of Ukraine violated the 1994 Budapest Memorandum.Through his conduct, President Trump and his associates used "an act of military, or of political, economic or other coercion" (CSCE Final Act, 1975, p. 4, Section VI), to "subordinate" or influence the internal affairs, "political, social, economic [or] cultural systems," or "laws and regulations" of Ukraine (CSCE Final Act, 1975, p. 4, Section VI). (Budapest Memorandum, 1994, p. 4) in order to "[serve] their own interests" or to "secure advantages of any kind" (Budapest Memorandum, 1994, p. 4).This raises critical questions going forward: what are the implications of such a violation by a president of the United States? And how far will the Trump administration go?Although it would directly contradict Ukraine's long-held strategic culture and security interests (Sinovets, Budjeryn, 2017, p. 1), had Ukraine deemed this violation "extraordinary circumstances that jeopardize its supreme interests" (Budjeryn, n.d., p. 4), the international ramifications, damage to the nonproliferation paradigm and disarmament efforts could have been immense. A more aggressive nation might be inclined to strike back at such a violation.Additionally: even though this instance of President Trump's conduct was unique in that it directly violated an agreement, detrimental foreign policy and the degradation of the nonproliferation and arms control paradigm are nothing new for the Trump administration. From the JCPOA, to declining the INF treaty, to distancing and publicly degrading long-standing NATO allies (Fisher, 2016), to growing increasingly close with and complimenting authoritarian leaders such as Vladimir Putin – who also violated the Budapest Memorandum – reckless foreign policy is a consistent, destructive practice by the Trump administration. And as demonstrated by President Trump's decision to – wittingly or unwittingly – abuse the power of his office to violate an international agreement with a strategic partner for his own political gain, there is no telling what foreign policy damage his administration could do from here. ; В 2019 году президент США Дональд Трамп был обвинен в постоянном вмешательстве в внутреннюю политику Украины. Teachers today are faced with myriad expectations. Whether preparing children for school learning goals, state assessments, or building social skills, it seems that teachers are being asked to do more every year. Perhaps the most palpable effect of the standards movement has been the academic "trickle down," whereby our youngest learners have been exposed to a more academic, teacher-directed curriculum in many of today's kindergarten classrooms. The evidence is strong; kindergarten has become the new first grade. This article explores the apparent conflicts and tensions evident in many of today's kindergarten classrooms. In addition, recommendations are made to preservice teachers and school administrators in response to the challenges teachers face today in the current educational climate and the supports needed to ensure developmentally appropriate teaching. In this qualitative study four conversant and experienced kindergarten teachers were investigated over the course of a school year as they lived within the context of a changing curriculum, increased demands, and new expectations. This study looked to capture the lives of teachers and their abilities to develop and integrate strategies that consider the developmental nature of children, challenges of classroom diversity, and the imposition of standards and mandates.

Itemized deduction for medical expenses has existed in one form or another for more than 60 years. One justification for this tax deduction is that it reduces the burden for taxpayers with catastrophic expenses. Currently it shields more out-of-pocket spending on health care from taxes than any other tax provision. Based on a repeated cross section from 1999 to 2005 and a 5-year panel of tax returns, we find that filers aged 65 years or older claim the deduction more often, deduct larger amounts, and do so more persistently than younger taxpayers. Augmenting tax data with information from the MEPS, we find that no more than 41% of all tax units with non—tax preferred medical expenditures exceeding 10% of income claimed the deduction in 2005. Even for those claiming the deduction, the average tax benefit of the deduction offsets less than 10% of medical expenditures.
BACKGROUND: Colorectal cancer is one of the leading causes of cancer morbidity and mortality in Europe. We aimed to ascertain the economic burden of colorectal cancer across Europe using a population-based cost-of-illness approach. METHODS: In this population-based cost-of-illness study, we obtained 2015 activity and costing data for colorectal cancer in 33 European countries (EUR-33) from global and national sources. Country-specific aggregate data were acquired for health-care, mortality, morbidity, and informal care costs. We calculated primary, outpatient, emergency, and hospital care, and systemic anti-cancer therapy (SACT) costs, as well as the costs of premature death, temporary and permanent absence from work, and unpaid informal care due to colorectal cancer. Colorectal cancer health-care costs per case were compared with colorectal cancer survival and colorectal cancer personnel, equipment, and resources across EUR-33 using univariable and multivariable regression. We also compared hospital care and SACT costs against 2009 data for the 27 EU countries. FINDINGS: The economic burden of colorectal cancer across Europe in 2015 was €19·1 billion. The total non-health-care cost of €11·6 billion (60·6% of total economic burden) consisted of loss of productivity due to disability (€6·3 billion [33·0%]), premature death (€3·0 billion [15·9%]), and opportunity costs for informal carers (€2·2 billion [11·6%]). The €7·5 billion (39·4% of total economic burden) of direct health-care costs consisted of hospital care (€3·3 billion [43·4%] of health-care costs), SACT (€1·9 billion [25·6%]), and outpatient care (€1·3 billion [17·7%]), primary care (€0·7 billion [9·3%]), and emergency care (€0·3 billion [3·9%]). The mean cost for managing a patient with colorectal cancer varied widely between countries (€259-36 295). Hospital-care costs as a proportion of health-care costs varied considerably (24·1-84·8%), with a decrease of 21·2% from 2009 to 2015 in the EU. Overall, hospital care was the largest proportion (43·4%) of health-care expenditure, but pharmaceutical expenditure was far higher than hospital-care expenditure in some countries. Countries with similar gross domestic product per capita had widely varying health-care costs. In the EU, overall expenditure on pharmaceuticals increased by 213·7% from 2009 to 2015. INTERPRETATION: Although the data analysed include non-homogenous sources from some countries and should be interpreted with caution, this study is the most comprehensive analysis to date of the economic burden of colorectal cancer in Europe. Overall spend on health care in some countries did not seem to correspond with patient outcomes. Spending on improving outcomes must be appropriately matched to the challenges in each country, to ensure tangible benefits. Our results have major implications for guiding policy and improving outcomes for this common malignancy. PURPOSE: With better access to early diagnosis and appropriate treatment, cervical cancer (CC) burden decreased in several European countries. In Eastern European (EE) countries, which accessed European Union in 2004, CC survival was worse than in the rest of Europe. The present study investigates CC survival differences across five European regions, considering stage at diagnosis (local, regional and metastatic), morphology (mainly squamous versus glandular tumours) and patients' age. METHODS: We analysed 101,714 CC women diagnosed in 2000-2007 and followed-up to December 2008. Age-standardised 5-year relative survival (RS) and the excess risks of cancer death in the 5 years after diagnosis were computed. RESULTS: EE women were older and less commonly diagnosed with glandular tumours. Proportions of local stage cancers were similar across Europe, while morphology- and stage-specific RS (especially for non-metastatic disease) were lower in Eastern Europe. Adjusting for age and morphology, excess risk of local stage CC death for EE patients remained higher than that for other European women. CONCLUSION: Stage, age and morphology alone do not explain worse survival in Eastern Europe: less effective care may play a role, probably partly due to fewer or inadequate resources being allocated to health care in this area, compared to the rest of Europe.

This study aimed to identify immediate areas for system strengthening by comparing the delivery of elective cancer surgery during the COVID-19 pandemic in periods of lockdown versus light restriction. Methods This international, prospective, cohort study enrolled 20 006 adult (≥18 years) patients from 466 hospitals in 61 countries with 15 cancer types, who had a decision for curative surgery during the COVID-19 pandemic and were followed up until the point of surgery or cessation of follow-up (Aug 31, 2020). Average national Oxford COVID-19 Stringency Index scores were calculated to define the government response to COVID-19 for each patient for the period they awaited surgery, and classified into light restrictions (index 60). The primary outcome was the non-operation rate (defined as the proportion of patients who did not undergo planned surgery). Cox proportional-hazards regression models were used to explore the associations between lockdowns and non-operation. Intervals from diagnosis to surgery were compared across COVID-19 government response index groups. This study was registered at ClinicalTrials.gov, NCT04384926. Findings Of eligible patients awaiting surgery, 2003 (10·0%) of 20 006 did not receive surgery after a median follow-up of 23 weeks (IQR 16–30), all of whom had a COVID-19-related reason given for non-operation. Light restrictions were associated with a 0·6% non-operation rate (26 of 4521), moderate lockdowns with a 5·5% rate (201 of 3646; adjusted hazard ratio [HR] 0·81, 95% CI 0·77–0·84; p<0·0001), and full lockdowns with a 15·0% rate (1775 of 11 827; HR 0·51, 0·50–0·53; p<0·0001). In sensitivity analyses, including adjustment for SARS-CoV-2 case notification rates, moderate lockdowns (HR 0·84, 95% CI 0·80–0·88; p<0·001), and full lockdowns (0·57, 0·54–0·60; p<0·001), remained independently associated with non-operation. Surgery beyond 12 weeks from diagnosis in patients without neoadjuvant therapy increased during lockdowns (374 [9·1%] of 4521 in light restrictions, 317 [10·4%] of 3646 in moderate lockdowns, 2001 [23·8%] of 11 827 in full lockdowns), although there were no differences in resectability rates observed with longer delays. Interpretation Cancer surgery systems worldwide were fragile to lockdowns, with one in seven patients who were in regions with full lockdowns not undergoing planned surgery and experiencing longer preoperative delays. Although short-term oncological outcomes were not compromised in those selected for surgery, delays and non-operations might lead to long-term reductions in survival. During current and future periods of societal restriction, the resilience of elective surgery systems requires strengthening, which might include protected elective surgical pathways and long-term investment in surge capacity for acute care during public health emergencies to protect elective staff and services.
