'Der ärztliche Blick auf den Patienten wird heute stark bestimmt durch Forschungsparadigmata, die mit operationalisierenden Methoden weitgehend generalisierbare und vom Einzelfall abstrahierte Hypothesen prüfen und anwenden lassen. Dadurch entstand eine eigentümliche Leibferne der modernen Medizin, die traditionelle Auseinandersetzungen mit Leib und Krankheit in Vergessenheit geraten ließ. Der menschliche Leibbezug war aber im 20. Jahrhundert Gegenstand einer Reihe von phänomenologischen, anthropologischen, existenzialistischen sowie spezifisch medizinischen Ansätzen, von denen im Folgenden einige in ihren Grundzügen skizziert und zur Diskussion gestellt werden sollen. Prescriptions for specific active therapies such as chemotherapy increase at the end of life. However, some studies show that concomitant palliative management and often leading to oncological therapeutic abstention increases the survival time and quality of life of patients. The concept of end-of-life, which is widely used in the literature and is entitled to several laws, is not clearly defined at the legislative level. What is the end of life in Oncology? The main objective of our work was to define the notion of end of life through the study of the representations of physicians oncologists and practicing palliative care and its implementation in the exercise of the medical function. A qualitative study was conducted in the Pays de la Loire region using 14 semi-structured interviews from January 2019 to March 2019. The interviews were recorded and analyzed according to a so-called thematic content analysis. The end of life is a complex notion that could not be defined in a consensual and unique way. Two end-of-life concepts have been established: the end of life "objective" and the end of life "subjective". These two concepts made it possible to establish two decision models for stopping active oncology therapeutics. The "objective" end of life was not precisely defined but referred to the clinico-biological approach to death, the accumulation of arguments or evidence in favor of the inevitability of death and decreasing the prevailing medical uncertainty. The end of life "subjective", meant the understanding of the near death and by these multiple representations it shaped the sense of the medical actions. This work sheds additional light on the notion of end-of-life, highlighting the integration of a subjective personal component subject to numerous representations to rational medical practice based on validated knowledge and skills. ; Les prescriptions de thérapeutiques actives spécifiques comme les chimiothérapies augmentent en fin de vie. Abstract. The Department of Medical Psychology and Sociology at the University of Leipzig deals with a wide scope of scientific working fields. First of all, the conduction of representative surveys for the validation of psychometric instruments, the exploration of multiple research topics and the supply of reliable information about public opinion is a central part of our work. These surveys basically contribute to an empirical foundation of the following research fields of our department: (1) epidemiologic research on mental disorders and bodily complaints, (2) development and validation of psychometric instruments, (3) health status and life conditions of care giving relatives, and (4) migration and health.

L'objectif principal de notre travail était de définir la notion de fin de vie grâce à l'étude des représentations chez les médecins oncologues et exerçant les soins palliatifs et de sa mise en pratique dans l'exercice de la fonction médicale. Une étude qualitative a été menée dans la région Pays de la Loire à l'aide de 14 entretiens semi-dirigés de Janvier 2019 à Mars 2019. Les entretiens ont été enregistrés puis analysés selon une analyse de contenu dite thématique. La fin de vie est une notion complexe qui n'a pas pu être définie de manière consensuelle et unique. Deux concepts de fin de vie ont pu être établis: la fin de vie « objective » et la fin de vie « subjective». Ces deux concepts ont permis d'établir deux modèles décisionnels d'arrêt des thérapeutiques actives oncologiques. La fin de vie « objective » n'était pas définie précisément mais désignait l'approche clinico-biologique de la mort, l'accumulation d'arguments ou de preuves en faveur de l'inéluctabilité de la mort en diminuant l'incertitude médicale régnante. La fin de vie « subjective », désignait la compréhension de la mort proche et par ces multiples représentations façonnait le sens des actions médicales. Ce travail apporte un éclairage supplémentaire sur la notion de fin de vie mettant en évidence l'intégration d'une composante personnelle subjective soumise à de nombreuses représentations à la pratique médicale rationnelle fondées sur des connaissances et des compétences validées.

Aus phänomenologisch-empirischer Sicht wird der Wandel der Einstellung zur Technik am Beispiel des Lebens und Gewöhnens von Patienten an eine Dialysestation in einem Krankenhaus analysiert. Bei dieser Population stellt sich die Frage nach der Einstellung radikaler als bei anderen, denn hier ist "Technik" äquivok mit "Leben", ihre Verneinung wäre Selbstmord. Der Autor konzipiert Patientenarbeit als Arbeit der Patienten im Sinne einer Verknüpfungsleistung biographischer und trajektoraler Erfahrungs- und Handlungsketten zur Wiederherstellung lebensweltlicher Idealisierungen in einem Rahmen, der durch die medizinische Technologie gesetzt wird.
